1.Comparison of short-term efficacy of drug-coated balloon angioplasty and plain old balloon angioplasty in the treatment of isolated popliteal artery chronic total occlusion and the influencing factors of primary patency rate

Objective:To explore the short-term effect of drug-coated balloon angioplasty (DCBA) and common plain old balloon angioplasty (POBA) in the isolated popliteal artery chronic total occlusion (CTO), and to analyze the factors affecting the postoperative primary patency rate.Methods:A retrospective cohort study approach was used in this study. A total of 42 isolated popliteal CTO patients admitted to the First Affiliated Hospital of Bengbu Medical University from January 2020 to June 2022 were divided into two groups according to their different balloons: 24 as POBA group and 18 as DCBA group. The primary patency rate of target lesions, the clinically-driven target lesion revascularization(CD-TLR) rate, amputation and toe amputation rate, and the improvement of ankle-brachial index (ABI) and Rutherford grade at 6 and 12 months after surgery were compared.Measurement data with normal distribution was expressed as xˉ± s and means between two groups were compared using independent samples t-test. The percentage of counting data was calculated, and the rate between groups was compared by χ2 test or Fisher's exact probability method. Kaplan-Meier survival curve method was used to plot the survival curves of primary patency rate and CD-TLR free rate at 12 months after surgery, and Log rank test was used to compare the differences between groups . Univariate log rank test and multivariate Cox regression was used to analyze the factors affecting the primary patency rate at 12 months in patients with isolated popliteal CTO. Results:12 months after surgery, 4 patients in the DCBA group experienced lumen restenosis or occlusion while 12 patients in the POBA group experienced lumen restenosis or occlusion. The cumulative primary patency rate of target lesions in the DCBA group was higher than that in the POBA group (Log-rank χ2=4.03, P=0.045). ABI in the DCBA group at 6 and 12 months was greater than that in the POBA group [(0.91±0.11) vs (0.83±0.09), (0.84±0.11) vs (0.70±0.12), t=2.40, P=0.021, t=3.64 and P=0.001].There were no significant difference in cumulative CD-TLR exemption, amputation and amputation at 12 months, and Rutherford grade at 6 and 12 months for both groups(all P>0.05). The results of univariate analysis showed that DCBA as surgical method, hypertension and coronary heart disease were the influencing factors of the primary patency rate after chronic occlusion of the isolated popliteal artery (all P<0.05). The results of multivariate Cox regression analysis indicated that DCBA as surgical method was a protective factor for primary patency at 12 months (odds ratio =0.31,95% confidence interval: 0.10~0.870., P=0.038), while hypertension was an independent risk factor( OR=5.63,95% confidence interval: 1.54~20.56, P=0.009). Conclusions:The cumulative primary patency rate of target lesions 12 months after isolated popliteal CTO was higher than that of POBA. DCBA as surgical method was a protective factor for primary patency rate 12 months in patients with isolated popliteal CTO, while hypertension was an independent risk factor.

3.Large- scale prospective clinical study on prophylactic intervention of COVID-19 in community population using Huoxiang Zhengqi Oral Liquid and Jinhao Jiere Granules.

To scientifically evaluate the intervention effect of Chinese medicine preventive administration(combined use of Huo-xiang Zhengqi Oral Liquid and Jinhao Jiere Granules) on community population in the case of coronavirus disease 2019(COVID-19), a large cohort, prospective, randomized, and parallel-controlled clinical study was conducted. Total 22 065 subjects were included and randomly divided into 2 groups. The non-intervention group was given health guidance only, while the traditional Chinese medicine(TCM) intervention group was given two coordinated TCM in addition to health guidance. The medical instructions were as follows. Huoxiang Zhengqi Oral Liquid: oral before meals, 10 mL/time, 2 times/day, a course of 5 days. Jinhao Jiere Granules: dissolve in boiling water and take after meals, 8 g/time, 2 times/day, a course of 5 days, followed up for 14 days, respectively. The study found that with the intake of medication, the incidence rate of TCM intervention group was basically maintained at a low and continuous stable level(0.01%-0.02%), while the non-intervention group showed an overall trend of continuous growth(0.02%-0.18%) from 3 to 14 days. No suspected or confirmed COVID-19 case occurred in either group. There were 2 cases of colds in the TCM intervention group and 26 cases in the non-intervention group. The incidence of colds in the TCM intervention group was significantly lower(P<0.05) than that in the non-intervention group. In the population of 16-60 years old, the incidence rate of non-intervention and intervention groups were 0.01% and 0.25%, respectively. The difference of colds incidence between the two groups was statistically significant(P<0.05). In the population older than 60 years old, they were 0.04% and 0.21%, respectively. The incidence of colds in the non-intervention group was higher than that in the intervention group, but not reaching statistical difference. The protection rate of TCM for the whole population was 91.8%, especially for the population of age 16-60(95.0%). It was suggested that TCM intervention(combined use of Huoxiang Zhengqi Oral Liquid and Jinhao Jiere Granules) could effectively protect community residents against respiratory diseases, such as colds, which was worthy of promotion in the community. In addition, in terms of safety, the incidence of adverse events and adverse reactions in the TCM intervention group was relatively low, which was basically consistent with the drug instructions.

4.Epidemiological and clinical characteristics of 46 corona virus disease 2019 cases in Beijing City

Objective:To investigate and analyze the epidemiological and clinical characteristics of 46 patients with corona virus disease 2019 (COVID-19) in Beijing City.Methods:A retrospective study was conducted to analyze the data of 46 patients with COVID-19 in Beijing from 20th January 2020 to 8th February 2020 at the Fifth Medical Center of the PLA General Hospital in Beijing City. Twelve, 23 and 11 patients were assigned to the mild group, common group and severe group, respectively. The epidemiological history, clinical characteristics, laboratory tests and imaging inspections were analyzed. Statistical analysis used Fisher exact test. If P<0.05, post- hoc test was used for pairwise comparison, and the statistics were corrected by Bonferroni test. Results:Among the 46 patients included in this study, 27 were male and 19 were female. The age range was between 3-79 years old, and the age was (41.8±16.3) years old. The average incubation period was (4.85±3.00) days. A total of 26 cases (56.5%) were clustered patients, and 26 cases had a history of staying in Wuhan, 10 cases had contact with Wuhan personnel. Fever (39 cases, 84.8%), cough (27 cases, 58.7%), and fatigue (25 cases, 54.3%) were the main clinical symptoms for these patients. The decrease in white blood cell counts occurred in 12 patients, four had the decrease in T lymphocyte percentage, 17 had the decrease in CD4 + T lymphocyte counts, seven had the decrease in CD8 + T lymphocyte counts, 21 had the increase level of C reactive protein (45.7%), and interleukin-6 (IL-6) level increased in 32 cases (69.6%), erythrocyte sedimentation rate (ESR) increased in 23 cases (50.0%), serum ferritin level increased in 26 cases (56.5%), and blood lactic acid level increased in nine cases. There were statistically significant differences in the proportion of cases with decreased absolute value of CD8 + T lymphocytes and T lymphocytes counts among the mild, common and severe groups (all P<0.05). Comparing the proportion of cases in the three groups with elevated C reactive protein, IL-6, ESR, serum ferritin and blood lactic acid levels, the differences were statistically significant (all P<0.05). The proportion of cases with elevated C reactive protein levels in severe group was higher than those in mild and common groups. The proportion of cases with elevated IL-6, ESR, and serum ferritin levels in severe and common group were higher than those in mild group. The proportion of cases with elevated blood lactic acid levels in severe group was higher than those in mild group. The differences between the above groups were statistically significant (all adjusted P<0.017). Analysis of chest X-rays results showed that 34 patients (73.9%) had inflammation in the lungs. Conclusions:The epidemiological characteristics of patients with COVID-19 in Beijing City are mainly imported cases and clustered cases. The clinical manifestations are mainly fever, fatigue and cough. C reactive protein, IL-6, ESR, serum ferritin and blood lactic acid levels are higher in severe patients.
5.Acute liver injury induced by Yinaoning tablets (益脑宁片)

A 52-year-old male patient received self-purchased Yinaoning tablets (益脑宁片) (3 tablets, trice daily) orally for health care. On day 18 of medication, the patient developed jaundiced skin and sclera and dark urine. On day 24, he developed fatigue and anorexia. Laboratory tests showed alanine aminotransferase (ALT) 1 132 U/L, aspartate aminotransferase (AST) 953 U/L, total bilirubin (TBil) 80.0 μmol/L, direct bilirubin (DBil) 73.4 μmol/L, alkaline phosphatase (ALP) 203 U/L, and gamma-glutamyltransferase (γ-GT) 155 U/L. Acute liver injury induced by Yinanning tablets was considered. Then the drug was discontinued and liver-protective therapy and drugs for anti-jaundice were given. On day 6 of drug withdrawal, the laboratory tests showed ALT 192 U/L, AST 158 U/L, TBil 51.0 μmol/L, DBil 39.9 μmol/L, ALP 178 U/L, and γ-GT 149 U/L, his appetite recovered, and the yellowish skin and sclera improved markedly. On day 13 of drug withdrawal, his yellowish skin and sclera disappeared, and the laboratory tests showed ALT 56 U/L, AST 62 U/L, TBil 30.7 μmol/L, DBil 22.4 μmol/L, ALP 164 U/L, and γ-GT 142 U/L. At 6 months of follow-up, there were no abnormalities in his liver function.
