1.Mechanism of Traditional Chinese Medicine in Treatment of Metabolic Dysfunction-associated Fatty Liver Disease: A Review

Metabolic dysfunction-associated fatty liver disease (MAFLD) is a chronic liver disease closely related to metabolic dysfunction. Its global prevalence is increasing year by year and shows a trend toward younger onset. It is closely associated with cardiovascular and cerebrovascular diseases, chronic kidney disease, and extrahepatic malignant tumors, and has become a major public health problem affecting the health of the Chinese population. At present, the treatment of MAFLD mainly focuses on lifestyle intervention. When metabolic cardiovascular risk factors and liver injury are present, pharmacological intervention is required. However, there is still no specific drug therapy for MAFLD. In recent years, many studies have found that the pathogenesis of MAFLD involves interactions among multiple factors, including insulin resistance, oxidative stress, endoplasmic reticulum stress, programmed cell death, immune dysregulation, inflammasomes, gut microbiota imbalance, and bile acid metabolism. Traditional Chinese medicine (TCM) has advantages in integrated regulation with multiple components and multiple targets, and shows significant efficacy in the treatment of MAFLD. Active components of Chinese medicinal herbs and Chinese medicine compound formulas exert definite therapeutic effects by regulating multiple signaling pathways to improve lipid metabolism disorders, inhibit inflammatory responses, and regulate the gut microbiota, thereby effectively improving clinical symptoms and delaying the occurrence and development of MAFLD. Therefore, strengthening research on the mechanisms of action of TCM in MAFLD is of crucial importance and significance for its treatment. This article summarizes the mechanisms by which Chinese medicine monomers and compound formulas intervene in MAFLD through the regulation of related signaling pathways, based on the retrieval and analysis of relevant literature from multiple databases, with the aim of providing theoretical support and a reference basis for the clinical application of TCM in the treatment of MAFLD.

Gastric cancer (GC) has an insidious onset and is mostly diagnosed in the middle and late stages after clinical detection. It is one of the malignant tumors with high incidence and mortality rates in the world. At present, the treatment plans are optimized mainly in terms of surgery, radiotherapy, and intervention, while the endpoints of clinical trials, such as patients' overall survival, progression-free survival, and disease-free survival, are still unsatisfactory. Therefore, effectively delaying the dynamic inflammation-cancer transformation has become an urgent bottleneck in the prevention and treatment of GC. In 1920s, Professor Otto Warburg discovered the phenomenon that tumor cells can accelerate glycolysis. Since then, the abnormal metabolic network inside tumor cells has gradually entered into researchers' view, and the hot academic research topic of metabolic reprogramming has been proposed. Tumor cells can meet their own energy consumption and adapt to external changes by adjusting their metabolic pathways to achieve rapid proliferation. In recent years, traditional Chinese medicine (TCM) is resolutely pursuing innovation in inheritance and the continuous refinement of research has led to the precision-oriented transition of TCM theories. Therefore, linking TCM with the treatment of tumors and precancerous diseases has certain research connotations. The searching and review of the publications in this field revealed that the number of publications in tumor-related metabolism increased dramatically, while there were only a few studies using TCM as a therapeutic solution. The research group has long been committed to the study of precancerous lesions of gastric cancer (PLGC) in Chinese and Western medicine. This article explained the dynamic process of inflammation-cancer transformation from the perspective of spleen deficiency-Qi stagnation-collateral stasis. The molecules of hypoxia-inducible factor (HIF)-1α, cancer-Myc (c-Myc), apolipoprotein E (APOE) and pyruvate kinase M2 (PKM2) were selected to reflect the biological connotation of inflammation-cancer transformation. The current achievements of TCM in regulating the metabolic reprogramming to intervene in inflammation-cancer transformation were summarized, with a view to providing more information for TCM to intervene in the inflammation-cancer transformation of gastric mucosa.

4.Mechanism of Tongxie yaofang reducing colon hypermotility in IBS-D rats by regulating gut microbiota-ENS-MM crosstalk

OBJECTIVE To study the effects of Tongxie yaofang reducing colon hypermotility in irritable bowel syndrome with diarrhea (IBS-D) rats by regulating gut microbiota-enteric nervous system (ENS)-muscularis macrophages (MM) crosstalk. METHODS Forty newborn male SD rats were randomly divided into control group, model group, TCM group [Tongxie yaofang 2.68 g/(kg·d), calculated by raw material], and positive control group [Live combined bifidobacterium and lactobacillus tablets 0.27 g/(kg·d)], with 10 rats in each group. Except for the control group, the IBS-D model of liver stagnation and spleen deficiency syndrome was induced in the other 3 groups with the method of mother-child separation+chronic restraint+Folium Sennae- induced diarrhea. After modeling, the administration groups were given relevant drug liquid intragastrically, once a day, for consecutive 2 weeks. At the end of modeling and after administration, the fecal properties (the incidence and the rate of loose stools), colonic motility (colon emptying time), and visceral sensitivity [abdominal withdrawal reflex (AWR) scores under different pressures] of rats were observed in each group. The concentration of lipopolysaccharide (LPS) in serum was detected after medication, and the expressions and distribution of bone morphogenetic protein 2 (BMP2), colony stimulating factor 1 (CSF1) and Toll-like receptor 4 (TLR4) in colon tissue were detected; the diversity, species composition and differences of gut microbiota were also determined. RESULTS At the end of modeling, compared with the control group, all rats of the other three groups suffered from loose stools (100%), the rate of loose stools and AWR scores at different pressures increased significantly, and colon emptying time was shortened significantly (P<0.01 or P<0.05). The incidences of loose stools were 20% in TCM group and 80% in the positive control group; the rate of loose stools and AWR scores at different pressures, serum concentration of LPS and protein expressions of CSF1 and TLR4 in muscle layer of colon tissue in TCM group significantly decreased, compared with the model group; colon emptying time, the average optical density of BMP2 protein in muscle layer of colon tissue, and the indexes of Chao 1, Shannon and Faith’s PD and Simpson E-mail:772699670@qq.com index of rats in TCM group were all prolonged or increased significantly, compared with the model group (P<0.01 or P< E-mail:aiwangzi312@163.com 0.05). The relative abundance ratio of Firmicutes/Bacteroidetes, from low to high, was in the order of TCM group, control group, positive control group and model group; the species composition of gut microbiota in TCM group was closer to control group, with dominant bacterial genera including Prevotella and Blautia. CONCLUSIONS Tongxie yaofang can regulate the expressions of BMP2 and CSF1, the key proteins of gut microbiota-ENS-MM crosstalk, by changing the gut microbiota, thus alleviating the abnormal hyperfunction of colon motility in IBS-D rats.
5.Mechanism of lncRNA-miRNA-related Signaling Pathways in Gastric Cancer and Chinese Medicine Intervention: A Review

Gastric cancer (GC) is one of the most common malignant tumors in the digestive system, with high morbidity and mortality. Early clinical symptoms of GC are not obvious, and most of them have entered the advanced stage after discovery, which greatly reduces the clinical cure rate and affects the quality of life of patients, and the prognosis is very poor. In recent years, with the continuous exploration in the field of bioinformatics, it has been found that micro-RNA (miRNA) and long non-coding RNA (lncRNA) exist as non-coding RNA (ncRNA) without translation ability, and regulate the expression levels of related signal proteins by acting on a certain target, thereby activating or inhibiting a certain signaling pathway, which plays an important role in assisting diagnosis, guiding clinical medication, and judging prognosis in the progress of GC. Chinese medicine is easily accepted by patients because of its good curative effect and less side effects. In the present basic studies, with the interaction mechanism between miRNA, lncRNA and signaling pathways as the breakthrough point, various studies on the regulation of related signaling molecules and signaling pathways by Chinese medicine have been carried out. A large number of experimental data have proved that the development of GC is closely related to the interaction of miRNA, lncRNA, and related signaling pathways, and Chinese medicine, with multi-target, multi-mechanism, and multi-pathway characteristics, affects various signaling molecules and signaling pathways and intervenes in the progress of GC cells. This paper reviewed the basic research on lncRNA, miRNA molecules, and main signaling pathways involved in the occurrence and development of GC, and summarized specific molecular mechanisms of Chinese medicine in the regulation of each signaling pathway, hoping to provide references for modern research of Chinese medicine in the intervention of GC progress at the molecular level.
6.Construction of a predictive model for pathological grading of rectal neuroendocrine tumors based on MRI features

Objective:To explore the value of MRI features in predicting the pathological grade of rectal neuroendocrine tumors and to develop a predicting model.Methods:A retrospective analysis was performed on 30 cases of rectal neuroendocrine tumors confirmed by surgery and pathology between 2013 and 2019. All of them underwent plain rectal MRI, DWI and dynamic contrast-enhanced MRI. The clinical features and MRI characteristics (ie. tumor location, maximum tumor diameter, boundary, growth pattern, enhancement of three-staged lesions, and the lymph node metastasis) were analyzed by statistical methods to evaluate the difference between different tumor pathologic grades (G1, G2 and G3). Characteristics with statistical significance were analyzed by collinearity diagnostics, and stepwise regression method was used to select independent predictors. Ordinal logistic regression analysis was then conducted to develop the predicting model.Results:Maximum tumor diameter, tumor boundary, growth pattern, mr-T, mr-N, EMVI, MRF, T2WI signal intensity, tumor enhancement degree in venous phase and distant metastasis were closely correlated with the pathological grade of rectal neuroendocrine tumors ( P<0.001, 0.001, 0.001, <0.001, 0.001, 0.004, 0.024, 0.015, 0.001, and <0.001, respectively). The mr-T and tumor enhancement degree in venous phase were identified as the independent predictors to construct the prediction model. The model got ideal performance in predicting the grades, with the areas under the receiver operating characteristic (ROC) curves (AUCs) of 0.945, 0.624 and 0.896, the sensitivities were 75.0%, 85.7%, and 90.9% and corresponding specificities were 88.9%, 52.6% and 93.3% for G1, G2 and G3 rectal neuroendocrine tumors, respectively. Conclusion:The model based on mr-T and tumor enhancement degree in venous phase can serve as a clinical tool for predicting the pathological grade of rectal neuroendocrine tumors.

8.MRI associated biomarker analysis for diagnosis of lymph node metastasis in T1-2 stage rectal cancer

Objective:To explore the diagnostic accuracy improved by magnetic resonance imaging (MRI) biomarkers for lymph node metastasis in T1-2 stage rectal cancer before treatment.Methods:Medical records of 327 patients with T1-2 rectal cancer who underwent pretreatment MRI and rectal tumor resection between January 2015 and November 2019 were retrospectively analyzed. Fifty-seven cases were divided into the lymph node metastasis group (N+ group) while other 270 cases in the non-lymph node metastasis group (N-group) according to the pathologic diagnosis. Two radiologist evaluated the tumor characteristics of MRI images. The relationship of the clinical and imaging characteristics of lymph node metastasis was assessed by using univariate analysis and multivariable logistic regression analysis. Receiver operating characteristic (ROC) curve was used to evaluate the diagnostic abilities for the differentiation of N- from N+ tumors.Results:Among the 327 patients, MR-N evaluation was positive in 67 cases, which was statistically different from the pathological diagnosis ( P<0.001). The sensitivity, specificity and accuracy of MRI for lymph node metastasis were 45.6%, 84.8% and 78.0%, respectively. Multivariate regression analysis showed that tumor morphology ( P=0.002), including mucus or not ( P<0.001), and MR-N evaluation ( P<0.001) were independent influencing factors for stage T1-2 rectal cancer with lymph node metastasis. The area under the ROC curve of rectal cancer with lymph node metastasis analyzed by the logistic regression model was 0.786 (95% CI: 0.720~0.852). Conclusions:Tumor morphology, including mucus or not, and MR-N evaluation can serve as independent biomarkers for differentiation of N- and N+ tumors. The model combined with these biomarkers facilitates to improve the diagnostic accuracy of lymph node metastasis in T1-2 rectal cancers by using MRI.

10.Effects of mistral-air inflatable blanket on neonatal blood coagulation function after surgery for general anesthesia

Objective To evaluate the value of mistral-air inflatable blanket in neonatal temperature,postoperative complications,and the influence of the blood coagulation function after surgery undergoing general anesthesia.Methods A total of 60 newborns with laparotomy operation undergoing general anesthesia were randomly divided into two groups,the control group and the observation group,30 persons each group.The newborns were given mistral-air inflatable blanket in the observation group,and conventional methods in the control group,two groups were transferred to Postanesthesia Care Unit by incubator at 37℃.The introperative nasopharyngeal temprature,HR,MAP and SpO2 were recorded before induction (T0),30 minutes during operation (T1),extubated instantly (T2),15 minutes during PACU (T3),30 min during PACU (T4).The extubation time and the incidence of apnoea and hyoxemia were recorded after operation.PT,APTT,TT and Fib were tested before induction and extubated instantly.Results The temperature of intraoperative and postoperative period were higher in the observation group than the control group,the difference was significant(P < 0.05).The heart rates of intraoperative and postoperative period is significantly higher in the control group than the observation group,the difference was significant(P < 0.05);the mean arterial pressure were no significant statistical difference(P > 0.05).The observation group extubation time were less than the control group,the difference was significant (P < 0.05).There was no significant difference about the incidence of hypoxemia and apnea in two groups(P > 0.05).The PT,APTT,TT and Fib in the observation group were better than the control group,the difference between the groups was significant(P < 0.05).Conclusion Mistral-air inflatable blanket can effectively prevent the decrease of the body temperature during operation and PACU,reduce the exduction time and postoperative complications,and protect blood coagulation function.
