1.Clinical features of the right-side non-thrombotic iliac vein compression syndrome and the efficacy of endovascular treatment

Objective To investigate the clinical and imaging features of right-side non-thrombotic iliac vein compression syndrome(NIVCS),and to evaluate the efficacy of endovascular treatment.Methods The clinical data and imaging materials of lower extremity venography from January 2019 to February 2024 in Affiliated Nanjing Gaochun People's Hospital,Jiangsu University were retrospectively analyzed.The patients with clinically diagnosed right-side NIVCS who received endovascular treatment were selected for this study.The clinical features and imaging findings,technical success rate,postoperative clinical improvement,and stent patency rate were analyzed.Results The median follow-up time was 30.2 months(3-58 months).A total of 24 patients with right-side NIVCS(aged 42-73 years)were enrolled in this study.Of the 24 patients,22(91.7%)were male and only 2(8.3%)were female,the proportion of male was significantly higher than that of female(P<0.001).According to Clinical Etiologic Anatomic Pathophysiologic(CEAP)grading,14 patients were of grade C3,9 patients were of grade C,and one patient was of grade C.Imaging examination showed that the most frequent site of compression was the proximal end of the external iliac vein(15/24,62.5%),the second frequent site of compression was the proximal part of the common iliac vein bifurcation(5/24,20.8%).The most common external pressure structure was the internal and external iliac arteries(14/24,58.3%),the second common external pressure structure was external iliac artery and sacrum(5/24,20.8%).The technical success rate was 100%,and only one stent was implanted in each patient(diameter of 12-14 mm and length of 80-90 mm).The most commonly used stent was the Smart control(19/24,79.2%).During follow-up period,the remission rate of edema was 85.7%(18/21),the ulcer healing rate was 100%(1/1),and all stents remained patent.Conclusion Right-side NIVCS is more commonly seen in male patients.Because the compression site is usually located at the distal end of iliac vein,thus,the implanted stent may rarely affect blood flow in the contralateral iliac vein.Therefore,endovascular treatment is a safe and effective method for right-side NIVCS.
2.Analysis of therapeutic efficacy of endovascular embolization using Embosphere microsphere in the treatment of massive hemoptysis

Objective To investigate the safety and efficacy of Embosphere microsphere in the treatment of massive hemoptysis during bronchial arterial embolization(BAE).To analyze the factors influencing the recurrence by Cox regression model.Methods Ninety patients with massive hemoptysis who underwent BAE using Embosphere microsphere were included.The immediate hemo-stasis rate within 24 hours,clinical success rate and prognostic factors were statistically analyzed.Results The immediate hemosta-sis rate within 24 hours was 94.4%(85/90).The clinical success rate was 56.7%(51/90).Cumulative hemoptysis-free recurrence con-trol rates at 6 months,1 year and 2 years postoperative were 81%,78%and 57%,respectively.Cox regression model analysis showed that the variables associated with recurrence were long-term hemoptysis,lung cancer,tuberculosis and lung destruction.Conclusion Embosphere microsphere are safe and effective embolic particles in the treatment of massive hemoptysis during BAE.Risk factors for hemoptysis recurrence include long-term hemoptysis,lung cancer,tuberculosis and lung destruction.

Objective To investigate the current situation of emergency rescue patient retention in Jianyang People's Hospital and explore its causes.Methods A retrospective analysis was conducted on the data of 4 834 patients rescued in the emergency room of Jianyang People's Hospital in 2022.They were divided into retention group and non retention group based on whether the emergency detention time exceeded 6 hours.With detention as the dependent variable and factors that may lead to emergency detention as the independent variable,binary logistic regression method was applied to screen for factors related to emergency detention.Results The median retention time for emergency rescue patients was 4.84 hours,with 3 721 cases(76.98%)having a retention time≤6 hours and 1 113 cases(23.02%)having a retention time>6 hours;the main influencing factors for emergency detention included age,visit time,visit season,admission method,involving two or more departments,diversion direction,and treatment compliance(P<0.05).Conclusion Emergency patients aged≥41 years old,treated from 17:00 to<08:00 the next day,visited in winter,admitted on their own,involving two or more departments,diverted to hospitalization,and with average or poor treatment compliance are prone to emergency detention.Therefore,the emergency department of the hospital should develop appropriate management measures based on the above factors to reduce emergency detention time.
