Objective To investigate the clinical and imaging features of right-side non-thrombotic iliac vein compression syndrome(NIVCS),and to evaluate the efficacy of endovascular treatment.Methods The clinical data and imaging materials of lower extremity venography from January 2019 to February 2024 in Affiliated Nanjing Gaochun People's Hospital,Jiangsu University were retrospectively analyzed.The patients with clinically diagnosed right-side NIVCS who received endovascular treatment were selected for this study.The clinical features and imaging findings,technical success rate,postoperative clinical improvement,and stent patency rate were analyzed.Results The median follow-up time was 30.2 months(3-58 months).A total of 24 patients with right-side NIVCS(aged 42-73 years)were enrolled in this study.Of the 24 patients,22(91.7％)were male and only 2(8.3％)were female,the proportion of male was significantly higher than that of female(P＜0.001).According to Clinical Etiologic Anatomic Pathophysiologic(CEAP)grading,14 patients were of grade C3,9 patients were of grade C,and one patient was of grade C.Imaging examination showed that the most frequent site of compression was the proximal end of the external iliac vein(15/24,62.5％),the second frequent site of compression was the proximal part of the common iliac vein bifurcation(5/24,20.8％).The most common external pressure structure was the internal and external iliac arteries(14/24,58.3％),the second common external pressure structure was external iliac artery and sacrum(5/24,20.8％).The technical success rate was 100％,and only one stent was implanted in each patient(diameter of 12-14 mm and length of 80-90 mm).The most commonly used stent was the Smart control(19/24,79.2％).During follow-up period,the remission rate of edema was 85.7％(18/21),the ulcer healing rate was 100％(1/1),and all stents remained patent.Conclusion Right-side NIVCS is more commonly seen in male patients.Because the compression site is usually located at the distal end of iliac vein,thus,the implanted stent may rarely affect blood flow in the contralateral iliac vein.Therefore,endovascular treatment is a safe and effective method for right-side NIVCS.

Objective To investigate the clinical application value of contrast-enhanced chest CT in the detection of responsible vessels for hemoptysis before selective arterial embolization(SAE).Methods The clinical data of 74 patients with hemoptysis trea-ted with interventional therapy and preoperative contrast-enhanced chest CT and digital subtraction angiography(DSA)were ana-lyzed retrospectively.The responsible vessels were identified and then embolized via angiography.The detection of the responsible vessels via preoperative contrast-enhanced chest CT was analyzed.The patients were followed up to observe the efficacy and compli-cations,and the influencing factors of interventional efficacy and recurrence were analyzed.Results A total of 245 responsible ves-sels were detected by preoperative contrast-enhanced chest CT,including bronchial arteries(n=178),ectopic bronchial arteries(n=10)and non-bronchial systemic artery(NBSA)(n=57),which could accurately show the anatomical information of responsible vessels.A total of 4 posterior intercostal arteries were missed.The diagnostic accuracy was 98.4％(245/249).All patients were followed up for 12 to 25.6 months.The immediate hemostasis rate was 93.2％(69/74)and the effective rate was 79.7％(59/74),respectively.The factors affecting the efficacy were bronchial artery to pulmonary circulation fistula,pleural thickening at the bleeding site,and underly-ing lung disease.Among the 59 patients with effective treatment,underlying lung disease was the influencing factor for postoperative recurrence.Conclusion Contrast-enhanced chest CT can provide anatomical information about the responsible vessels for interven-tional therapy of hemoptysis,improving surgical efficiency and reducing the recurrence rate of hemoptysis.

Exosomes are double phospholipid membrane vesicles that are synthesized and secreted by a variety of cells, including T cells, B cells, dendritic cells, immune cells, are extracellular vesicles. Recent studies have revealed that exosomes can play a significant role in under both physiological and pathological conditions. They have been implicated in regulation of inflammatory responses, immune response, angiogenesis, tissue repair, and antioxidant activities, particularly in modulating immunity in autoimmune diseases (AIDs). Moreover, variations in the expression of exosome-related substances, such as miRNA and proteins, may not only offer valuable perspectives for the early warning, and prognostic assessment of various AIDs, but may also serve as novel markers for disease diagnosis. This article examines the impact of exosomes on the development of AIDs and explores their potential for therapeutic application.

The rapid and accurate authentication of traditional Chinese medicines(TCMs)has always been a key scientific and technical problem in the field of pharmaceutical analysis.Herein,a novel heating online extraction electrospray ionization mass spectrometry(H-oEESI-MS)was developed for the rapid and direct analysis of extremely complex substances without the requirement for any sample pretreatment or pre-separation steps.The overall molecular profile and fragment structure features of various herbal medicines could be completely captured within 10-15 s,with minimal sample(＜0.5 mg)and solvent consumption(＜20 μL for one sample).Furthermore,a rapid differentiation and authentication strategy for TCMs based on H-oEESI-MS was proposed,including metabolic profile characterization,characteristic marker screening and identification,and multivariate statistical analysis model validation.In an analysis of 52 batches of seven types of Aconitum medicinal materials,20 and 21 key compounds were screened out as the characteristic markers of raw and processed Aconitum herbal medicines,respectively,and the possible structures of all the characteristic markers were comprehensively identified based on Com-pound Discoverer databases.Finally,multivariate statistical analysis showed that all the different types of herbal medicines were well differentiated and identified(R2X＞0.87,R2Y＞0.91,and Q2＞0.72),which further verified the feasibility and reliability of this comprehensive strategy for the rapid authentication of different TCMs based on H-oEESI-MS.In summary,this rapid authentication strategy realized the ultra-high-throughput,low-cost,and standardized detection of various complex TCMs for the first time,thereby demonstrating wide applicability and value for the development of quality standards for TCMs.

Objective:To investigate the prognosis and its influencing factors of elderly patients with diffuse large B-cell lymphoma (DLBCL), and to provide references for clinical treatment.Methods:The clinical data of 152 patients with DLBCL aged over 60 years old from January 2013 to June 2017 in Shanxi Provincial Cancer Hospital were retrospectively analyzed. Kaplan-Meier method was used for survival analysis, and the log-rank test was used for univariate analysis of factors affecting the prognosis of patients, and the Cox proportional hazard regression model was used for multivariate analysis.Results:The median overall survival (OS) time of 152 elderly patients with DLBCL was 36 months (32-40 months), and the 1, 2, and 3-year OS rates were 80.26%, 61.84%, and 57.24%, respectively. Univariate analysis showed that the differences in the 3-year OS rates of elderly DLBCL patients with different gender, clinical staging, lactate dehydrogenase (LDH), Ki-67, β 2-microglobulin (β 2-MG) levels, smoke history, use of rituximab and CHOP regimens were statistically significant (all P < 0.1). The results of multivariate Cox regression analysis showed that male, late clinical staging, elevated LDH, and elevated β 2-MG were risk factors for the OS of elderly DLBCL patients (all P < 0.05). The use of rituximab and CHOP regimens were the protective factors for the OS of elderly patients with DLBCL (all P < 0.05). Conclusions:The elderly male DLBCL patients with late clinical staging, elevated LDH and elevated β 2-MG have a poor prognosis, and the elderly DLBCL patients treated with CHOP regimen and rituximab have a better prognosis.

Based on the reported IDO1 inhibitor U-3i,11 phenylsulfonamide derivatives were designed and syn-thesized by adopting bioisosterism and molecular docking technology.The inhibitory activities of the target compounds against IDO1 were determined by the HeLa cell-based kynurenine assay.The results demonstrated that most compounds showed different degrees of inhibitory effects on IDO1.Among them,compounds 3b and 3e displayed the most potent activity and could reverse IDO1-mediated immune suppression,which might be worth of further investigation.

Objective To observe the dynamic changes of hepatic ischemia reperfusion injury ( IRI) by contrast enhanced ultrasound(CEUS) in rabbit model and investigate its clinical value . Methods Sixty rabbits were randomly assigned into seven groups :sham operation group and 6 hepatic IRI groups ( 0 .5 h ,2 h ,6 h ,12 h ,24 h ,72 h) . The rabbits of hepatic IRI group established as follows :the hepatic artery , portalvein and bile duct in the portal triad to the left liver lobes were interrupted for 60 min with a vascular clip and then reflow the blood . The perfusion imaging of left liver lobe was observed and then the peak signal Intensinty ( PI) ,time to peak intensinty ( Tp) ,area under the curve ( AUC) ,and mean transit time ( MTT ) were calculated . The rabbits were sacrificed and blood samples were withdrawn for the measurement of liver enzymes aspartate aminotransferase ( AST) and alanine aminotransferase ( ALT) ,liver samples from the left lateral were collected for the measurement of myeloperoxidase ( MPO) and histologic examination . The correlation between parameters of CEUS and the test results above were analyzed . Results There were significant differences in the parameters of CEUS between hepatic IRI group and sham group ( P < 0 .001) . PI and AUC showed a decreased tendency and the lowest values were measured in the 24 h group .Tp and MTT showed a upward tendency and the maximum values were measured in the 24 h group . The parameters of CEUS showed correlation with ALT ,AST and MPO ( P < 0 .001 ) . AUC showed significant correlation with ALT ,AST and MPO ( r = - 0 .702 , - 0 .802 , - 0 .846 ;all P < 0 .001 ) . Conclusions CEUS can assess liver functional and pathological changes in the process of hepatic IRI in real-time ,dynamically ,rapidly ,noninvasively and objectively . It is expected to replace the invasive pathological inspection . AUC is the best index for evaluating hepatic IRI in parameters of CEUS .

Objective To evaluate the hemostasis effect of 915 MHz microwave in the hepatic artery and portal vein injury in vivo dog liver.Methods After manufactured model of the hepatic artery and portal vein injury,915MHz microwave percutaneous coagulation therapy and drug injections were performed under the guidance of contrast-enhanced ultrasound (CEUS),the hemostatic effect,amount of bleeding and histologic findings were compared between the two groups.Results 915 MHz microwave coagulation had better hemostatic effect than drug injection,the amount of bleeding was significantly less than the injection method,and the pathological results showed that the vessel wall necrosis and surrounding tissue coagulation were more obvious than the injection.Conclusions CEUS guided 915 MHz microwave has a significant role in hemostasis,so the 915 MHz microwave percutaneous coagulation therapy of the hepatic artery and portal vein injury in vivo dog liver has important clinical auxiliary significance.

Objective To investigate the feasibility and promising applications of percutaneous transhepatic lymphosonography in detecting sentinel lymph nodes(SLNs).Methods Twenty five rabbits with VX2 tumor were included in this study.0.05 ml SonoVue was injected into the liver parenchyma at 12,3,6,9 points around the VX2 tumor.The situation of contrast-enhanced lymph-vessel emited from injected point and lymph nodes in hepatic portal or around tumor was observed,and then the position of the lymph nodes were detected with the help of the mark on the surface of the portal vein,caput pancreatis,collum vesicae biliaris.Methylene blue (MB) was injected in the same way as above.The injected points were massaged for five minutes,and then executed the experimental rabbits.The lymph nodes enhanced and all the lymph node dyed or not were taked out for recorded and pathologic examination.Results 34 SLNs were conformed by operation and pathological diagnosis in all the rabbits.All SLNs were confirmed pathologically,28 lymph nodes which were checked out by percutaneous transhepatic lymphosonography were all SLNs.In all the 31 lymph nodes which were checked out by MB,25 lymph nodes were SLNs and the rest were the second degree lymph nodes.The detection rate of percutaneous transhepatic lymphosonography (82.4％) and the MB (91.2％) showed no significant difference(P =0.169).There were 6 SLNs enhanced uniformitily in which 2 SLNs encroach by cancer cell and 22 enhanced asymmetrically in which 21 SLNs encroach by cancer cell.The sensitivity,specificity and accuracy of percutaneous transhepatic lymphosonography to detcect the SLNs benign or malignancy was 95.5％ (21/28),66.7％(4/6) and 89.3 ％ (25/28).Conclusions Percutaneous transhepatic lymphosonography is a reliable and noninvasive method to detect and estimate the SLNs of hepatic cancer.

Objective To evaluate the effect of 915 MHz microwave percutaneous coagulation of splenic artery injury in vivo experimental.Methods The manufacture model of the splenic artery injury were made and divided into two groups:group A (peripheral splenic artery injury) and group B (central type of splenic artery injury),915 MHz,2450 MHz microwave coagulation therapy and drug injections were performed under the guidance of ultrasound contrast.The hemostatic effect,operating time,total therapy time,amount of bleeding,amount of fluid infusion and histologic findings were compared.Results The treatment time of 915 MHz microwave[group A(4.65 ± 5.45),group B (9.38 ± 5.62) s] was less than those of 2450 MHz microwave[group A(86.36 ± 6.34) s,group B(127.72 ± 5.34) s] and drug injection [group A(128 ± 4.85)s,group B(182 ± 7.28)s],on the other hand,the amount of bleeding and fluid volume was less than that of rest methods.Conclusions Contrast-enhanced ultrasound guided 915 MHz microwave has significant hemostatic effect compared with the other two methods,915 MHz microwave percutaneous coagulation therapy has important significance to splenic artery injury in clinical.
