1.Analysis on Characteristics and Prescriptions of Chinese Patent Medicines for Functional Diarrhea on Market

ObjectiveThis study focused on the marketed Chinese patent medicines for the treatment of Functional Diarrhea (FDr) in China and their prescription characteristics, in order to provide support for the clinical application and research and development of anti-FDr Chinese patent medicines. MethodsCollect the information of Chinese patent medicines that have been marketed to treat FDr, use Microsoft Excel 2021 software to conduct preliminary data collation and statistical analysis, and use the ancient and modern medical record cloud platform (V2.3.9) to analyze the standardized Chinese patent medicine prescriptions from the aspects of drug nature and taste, medication characteristics and prescription rules. Results147 kinds of FDr Chinese patent medicines were included in this study. There are a total of 40 varieties of FDr Chinese patent medicines suitable for children; The distribution of dosage forms is mainly pills, tablets, and capsules. 110 prescriptions were screened, among which the proportion of Chinese patent medicines for the treatment of spleen deficiency syndrome was the highest; The top three drug use frequency were licorice, Atractylodes macrocephala, and Poria cocos; The medicinal properties are mainly warm and flat, and the medicinal taste is mostly pungent, sweet and bitter, and most of them belong to the two meridians of the spleen and stomach; The association rules analysis obtains 20 strong association pairing sets; Three drug combinations were obtained by cluster analysis. ConclusionFDr Chinese patent medicine shows unique value in clinical application, especially in the field of children. However, there are still problems such as strong professionalism in the indication expression of drug instructions, limited coverage of the medical insurance catalog, and lack of high-level evidence-based medicine and pharmacoeconomic evidence. To this end, in the future, efforts should be made to build a multi-level evidence-based evidence system, improve medication compliance, and deepen research on syndrome-based medication laws, so as to enhance the clinical application value and scientific connotation of FDr Chinese patent medicines.
2.Genetic testing and prenatal diagnosis of 671 Chinese pedigrees affected with Duchenne/Becker muscular dystrophy.

OBJECTIVE:
To summarize the genetic characteristics of 671 Chinese pedigrees affected with Duchenne/Becker muscular dystrophy (DMD/BMD).
METHODS:
Clinical data of the pedigrees were collected. Multiplex PCR, multiple ligation dependent probe amplification (MLPA), next generation sequencing (NGS), Sanger sequencing and long read sequencing were used to detect the variant of DMD gene in the probands and their mothers, and prenatal diagnosis was provided for high risk pregnant women.
RESULTS:
Among 178 pedigrees analyzed by multiplex PCR, 44 variants of the DMD gene were detected, with the genetic diagnosis attained in 110 pedigrees. Among 493 pedigrees analyzed by MLPA in combination with NGS or Sanger sequencing, 294 pathogenic/possible pathogenic variants were identified, among which 45 were unreported previously, and the genetic diagnosis attained in 484 pedigrees. Structural variants of the DMD gene were identified in two pedigrees by long-read sequencing. Among 444 probands, 341 have inherited the DMD gene variant from their mothers (76.8%). Among 390 women with a high-risk, 339 have opted to have natural pregnancy and 51 chose preimplantation genetic testing for monogenetic disease (PGT-M). The detection rate of neonatal patients and carriers following natural pregnancy was significantly higher than that for PGT-M.
CONCLUSION
Combined application of MLPA, NGS, Sanger sequencing and long-read sequencing is an effective strategy to detect DMD/BMD. PGT-M can effectively reduce the risk of fetuses. Above finding has expanded the spectrum of DMD gene variants and provided a basis for reproductive intervention for pregnancies with a high risk for DMD/BMD.

3.A model analysis on the knowledge-attitude-practice of children guardians in Jiangxi, Shanghai and Qinghai

Objective:To understand the knowledge attitude and practice (KAP) on vaccination among children's parents in Jiangxi, Shanghai, and Qinghai and explore the factors influencing KAP.Methods:The study selected two counties/districts in Jiangxi, Shanghai, and Qinghai, respectively, by stratified sampling and used a unified questionnaire to investigate the parental KAP of vaccination. A structural equation model (SEM) was used to explore factors influencing parental KAP, as well as the relationship between knowledge and behavior.Results:Of the 760 valid questionnaires, the knowledge of vaccination among children's parents was better, and the vaccination knowledge of parents in Qinghai and Shanghai were slightly better than those in Jiangxi. Parents mainly obtained vaccination knowledge through medical staff and vaccination manuals. The fitting degree of SEM was relatively good; the root mean square error of approximation of the model is 0.033. The higher the parents' education level, the better their knowledge of vaccination ( β?=0.082). Parental vaccination knowledge could influence whether the vaccinated children stay for half an hour in the clinics ( β?=0.541). It could also impact whether parents giving up vaccinating their children in the face of media reports about the adverse effects of vaccinations ( β?=0.515). Conclusions:The knowledge of vaccination among the parents in Jiangxi, Shanghai, and Qinghai was quite good. Moreover, we should pay more attention to the mass media programs and vaccination knowledge among parents with low or middle education backgrounds. Vaccination knowledge can be disseminated through medical staff, vaccination manuals, or mobile applications.
4.Six sigma quality management method on improvement of nurses compliance for standard blood culture specimens in intensive care unit

Objective To improve the compliance of nurses in ICU standard specimen collection of blood culture by six sigma quality management method, so as to improve the quality of blood culture sample submission, to ensure the accuracy of inspection results. Methods A total of 158 nurses in ICU from February 2016 to January 2017 were selected as the research objects. The six sigma quality management method was used to analyze the operation of blood culture samples collected by ICU nurses,and the improvement plan was worked out and implemented. The compliance rate of blood culture samples collected by nurses before and after implementation and the feedback information of laboratory about blood culture specimen quality were compared. Results Before the application of six sigma quality management method, the compliance rate of blood culture specimens collected by ICU nurses was 54.1%(152/281), and the compliance rate after improvement was 88.6%(271/306), and the difference was statistically significant (χ2=86.45, P<0.01). Among the factors influencing nurses' blood culture,the importance of blood culture was not enough, the operation process was not mastered, the relevant knowledge of blood culture was not mastered, and the allocation of related consumables was unreasonable. After implementation,the composition ratio was reduced from 50.4%(65/129), 24.0%(31/129), 45.7%(59/129), 21.7%(28/129) to 17.1%(6/35), 8.6%(3/35), 25.7%(9/35), 2.9%(1/35), and the difference was statistically significant (χ2=4.00- 12.39, P<0.01 or 0.05). The contamination rate of blood culture specimens after six sigma management was 6.2%(19/306) and significantly lower than that of 16. 4%(46/281) before implementation,and the difference was statistically significant (χ2=15.36, P<0.01). Conclusions The six sigma quality management method can improve the nurses' attention to the correct collection of blood culture samples, reveal the defects in the operation, and reduce the irregular behavior of blood culture process. Therefore, the compliance of ICU nurses with standardized blood culture samples is improved, and the contamination rate of blood culture specimens is effectively reduced.
