1.Comprehensive analysis of an m6A regulator-based prognostic model and its associations with immune infiltration, drug sensitivity, and intercellular communication in cervical cancer

[摘 要] 目的:探讨m6A调控因子在宫颈癌预后评估、免疫微环境特征及治疗反应中的作用,构建基于m6A调控因子的预后风险模型,并分析其与免疫浸润、药物敏感性及细胞间通信的关系。方法:基于TCGA和GEO数据库中宫颈鳞癌及腺癌(CESC)转录组数据,分析22个m6A调控因子的表达与突变情况;采用LASSO‑Cox回归构建m6A相关预后风险模型,并依据风险评分将患者分为高、低风险组。利用CIBERSORT和ESTIMATE算法评估两组免疫细胞浸润及微环境特征;通过oncoPredict预测抗肿瘤药物敏感性。整合单细胞转录组数据(E-MTAB-12305),采用CellChat分析细胞间通信网络。结果:共鉴定出12个在宫颈癌中差异表达的m6A调控因子。高风险组患者总生存期缩短(P < 0.05),免疫评分降低,且对紫杉醇、氟尿嘧啶、多柔比星等抗肿瘤药物敏感性较差(IC50较高)。单细胞分析显示宫颈癌组织中细胞间通信网络发生改变,其中T细胞与内皮/上皮细胞间的相互作用可能主要由CCL5‑ACKR1和MIF‑(CD74 + CXCR4)配体‑受体介导。在临床样本中,对关键的m6A调控因子进行mRNA和蛋白水平的验证,结果显示部分调控因子在宫颈癌组织中的表达水平高于正常宫颈组织(P < 0.05)。结论:m6A调控因子特征可有效预测宫颈癌患者预后,其风险评分与肿瘤免疫浸润、药物敏感性及细胞通信密切相关,为宫颈癌的预后分层及个体化治疗提供了新的分子依据。
2.Different cap thicknesses on corneal densitometry changes after SMILE surgery

AIM: To evaluate the effects of different corneal cap thicknesses during small-incision lenticule extraction(SMILE)on corneal densitometry(CD), visual quality, and corneal biomechanics during the first postoperative year, and to explore its safety.
METHODS: A prospective randomized contralateral controlled design was employed. Thin cap(110-120 μm)and thick cap(130-140 μm)SMILE procedures were performed on the two eyes of the same myopic patients undergoing surgery, respectively. Postoperative evaluations at 1 wk, 1, 3, 6 mo, and 1 y included CD, uncorrected distance visual acuity(UDVA), corrected distance visual acuity(CDVA), contrast sensitivity, wavefront aberrations, and corneal biomechanics. A linear mixed-effects model was used for statistical analysis.
RESULTS:A total of 60 patients(120 eyes, 60 eyes in the thin-cap group and 60 eyes in the thick-cap group)completed the surgery successfully and attended scheduled follow-up visits. The mean age was 26.3±3.8 y, including 34 males(57%)and 26 females(43%).The intended cap thickness was 115.24±3.10 μm in the thin-cap group and 135.28±3.45 μm in the thick-cap group. There were significant intergroup differences in lenticule thickness and ablation depth(P<0.001), whereas the residual stromal bed(RSB)thickness was comparable between groups(P=0.301). Central 0-6 mm CD significantly decreased over time(P<0.001), and the thin-cap group had overall higher densitometry values than the thick-cap group(P<0.001), with significant differences persisting from 1 wk to 1 y postoperatively(Bonferroni-corrected P<0.05). At 1 y, significant CD differences were most pronounced in the anterior and central layers(both P<0.05), with no significant differences in posterior layers. The thick-cap group showed a significant improvement in UDVA(P=0.002)and better contrast sensitivity(corrected P<0.05), while the thin-cap group had higher values of total root mean square of higher-order aberrations(HOA-RMS), spherical aberration, and coma(all P<0.05). No statistical differences were observed between groups in corneal biomechanical parameters(DA, CBI, SSI)(all P >0.05), and the incidence of safety events was also comparable between the two groups(P≥0.5).
CONCLUSION: Given an equivalent RSB thickness, thicker corneal caps can reduce anterior-layer CD, improve contrast sensitivity and higher-order aberration outcomes, without obvious increases in biomechanical risks or safety concerns.Therefore, it warrants consideration as a preferred intraoperative strategy for SMILE.
3.Exploring the Collateral Damage Mechanism and Sequential Treatment Strategy of Children IgA Vasculitis from the Perspective of "Wind Agitating Blood Turbidity"

As one of the common systemic vasculitides in children, IgA vasculitis is closely associated with the interaction of "Wind pathogen" and "turbid stasis" in its pathogenesis. Based on the blood turbidity theory, this paper proposes "wind agitating blood turbidity and collateral damage" as the core pathogenesis. Regarding treatment, the sequential therapeutic principle of "stanch bleeding, activating blood circulation, and nourishing blood" is adopted, and combined with the dynamic application of wind-dispelling medicinals to "expel, search, and guard against wind", turbidity-removing method is applied throughout all stages. In the acute stage,it is recommended to clear heat and purify turbidity, stanch bleeding and secure the collaterals. In the prolonged stage, the method of dissipating stasis and clearing up turbidity should be used, as well as activating blood circulation and unblocking the collaterals. In the recovery stage, it is advised to nourish blood to flourish the collaterals, reinforce healthy qi and resolve turbidity. This therapeutic system integrates the elimination of pathogens and the reinforcement of healthy qi to clear and resolve blood turbidity, ultimately achieving holistic collateral repair through securing, unblocking, and nourishing the collaterals, which provides new insights into the treatment of this disease in traditional Chinese medicine.
4.Treatment of Glaucoma Based on "Jueyin (厥阴) as the Closing Phase" from the Perspective of Spatiotemporal Theory

This paper explores the therapeutic approach for glaucoma based on the concept of "jueyin (厥阴) as the closing phase" from the perspectives of time and space. In traditional Chinese medicine, jueyin governs inward, converging aspect of qi, representing the crucial turning point between the end of yin and the emergence of yang, as well as the transformation between yin and yang. When the closing and descending function of jueyin operates smoothly, it promotes the inward convergence and smooth descent of qi, enabling the internal retention of blood, spirit, and emotions, which nourishes the internal organs and moistens the meridian-sinews. Conversely, dysfunction of this "closing" mechanism results in a disturbance of yin and yang, a mixture of cold and heat, and disharmony of qi and blood. It is proposed that "failure of jueyin to properly close and descend" is a core pathomechanism of glaucoma. From the perspective of spatiotemporal theory, clinical treatment should focus on "regulating the closing function of jueyin and harmonizing yin and yang". The modified Wumei Pill (乌梅丸) is recommended to adjust the ascending-descending and entering-exiting dynamics of jueyin qi transformation, thereby restoring its free flow, achieving yin and yang balance, and ensuring nourishment to the ocular system.
5.Expression and functional study of FKBP10 in oral squamous cell carcinoma

Objective:
To investigate the expression and functional role of FK506 binding protein 10 (FKBP10) in oral squamous cell carcinoma (OSCC), and to provide a research basis for the estimated prognosis and targeted therapy of OSCC.
Methods:
A total of 284 OSCC samples and 19 normal samples were selected from the Cancer Genome Atlas (TCGA) database, and diagnostic analysis was performed to determine mRNA expression. Survival analysis for FKBP10 and OSCC was conducted on a gene expression profile interaction analysis website. Real-time fluorescence quantitative PCR and Western Blot were used to detect the mRNA and protein expression of FKBP10 in four OSCC cell lines and SAS and SCC9 cells transfected with siRNA. The cell proliferation ability of FKBP10-silenced cells was detected using the CCK8 method, and the cell cycle distribution and apoptosis were detected by flow cytometry. Cell migration and invasion ability were detected through wound healing and invasion experiments. The expression changes of total protein and phosphatidylinositol 3-kinase (PI3K)-serine/threonine kinase (AKT) after FKBP10 silencing were analyzed by proteomics and Western Blot.
Results:
According to the analysis of gene expression levels, the mRNA expression level of FKBP10 in OSCC was significantly higher than that in normal tissues (P < 0.001). In terms of diagnosis, the expression level of FKBP10 has unique diagnostic value for OSCC (P < 0.05). The survival analysis of FKBP10 and OSCC showed that a high expression of FKBP10 led to a decrease in patient survival and poor prognosis (P < 0.05). The expression of FKBP10 mRNA and protein in OSCC cell lines was higher than that in normal oral keratinocytes (P < 0.001). Silencing FKBP10 can reduce the proliferation, invasion, and migration ability of SAS and SCC9 (P < 0.001), and also block their cell cycle in the G0/G1 phase (P < 0.001), with a significant increase in apoptosis (P < 0.05). Protein mass spectrometry and Western blot analysis revealed that FKBP10 silencing significantly downregulated the expression of multiple proteins in the RAP1 signaling pathway, mainly RAP guanine nucleotide exchange factor 1 (RAPGEF1) (P < 0.05) and the phosphorylation of PI3K-AKT proteins (P < 0.05).
Conclusion
FKBP10 is highly expressed in OSCC, leading to poor prognosis for patients. Downregulated FKBP10 expression can inhibit the proliferation, migration, and invasion ability of OSCC cells, hinder cell cycle progression, and promote apoptosis via the RAP1-PI3K-AKT axis. FKBP10 is a potential therapeutic target and prognostic biomarker for OSCC.
6.Synthetic MRI Combined With Clinicopathological Characteristics for Pretreatment Prediction of Chemoradiotherapy Response in Advanced Nasopharyngeal Carcinoma

Objective:
To explore the feasibility of synthetic magnetic resonance imaging (syMRI) combined with clinicopathological characteristics for the pre-treatment prediction of chemoradiotherapy (CRT) response in advanced nasopharyngeal carcinoma (ANPC).
Materials and Methods:
Patients with ANPC treated with CRT between September 2020 and June 2022 were retrospectively enrolled and categorized into response group (RG, n = 95) and non RGs (NRG, n = 32) based on the Response Evaluation Criteria in Solid Tumors (RECIST) 1.1. The quantitative parameters from pre-treatment syMRI (longitudinal [T1] and transverse [T2] relaxation times and proton density [PD]), diffusion-weighted imaging (apparent diffusion coefficient [ADC]), and clinicopathological characteristics were compared between RG and NRG. Logistic regression analysis was applied to identify parameters independently associated with CRT response and to construct a multivariable model. The areas under the receiveroperating characteristic curve (AUC) for various diagnostic approaches were compared using the DeLong test.
Results:
The T1, T2, and PD values in the NRG were significantly lower than those in the RG (all P < 0.05), whereas no significant difference was observed in the ADC values between these two groups. Clinicopathological characteristics (Epstein–Barr virus [EBV]-DNA level, lymph node extranodal extension, clinical stage, and Ki-67 expression) exhibited significant differences between the two groups. Logistic regression analysis showed that T1, PD, EBV-DNA level, clinical stage, and Ki-67 expression had significant independent relationships with CRT response (all P < 0.05). The multivariable model incorporating these five variables yielded AUC, sensitivity, and specificity values of 0.974, 93.8% (30/32), and 91.6% (87/95), respectively.
Conclusion
SyMRI may be used for the pretreatment prediction of CRT response in ANPC. The multivariable model incorporating syMRI quantitative parameters and clinicopathological characteristics, which were independently associated with CRT response, may be a new tool for the pretreatment prediction of CRT response.
7.Mechanism of Colquhounia Root Tablets against diabetic kidney disease via RAGE-ROS-PI3K-AKT-NF-κB-NLRP3 signaling axis.

This study aimed to explore the therapeutic mechanisms of Colquhounia Root Tablets(CRT) in treating diabetic kidney disease(DKD) by integrating biomolecular network mining with animal model verification. By analyzing clinical transcriptomics data, an interaction network was constructed between candidate targets of CRT and DKD-related genes. Based on the topological eigenvalues of network nodes, 101 core network targets of CRT against DKD were identified. These targets were found to be closely related to multiple pathways associated with type 2 diabetes, immune response, and metabolic reprogramming. Given that immune-inflammatory imbalance driven by metabolic reprogramming is one of the key pathogenic mechanisms of DKD, and that many core network targets of CRT are involved in this pathological process, receptor for advanced glycation end products(RAGE)-reactive oxygen species(ROS)-phosphatidylinositol 3-kinase(PI3K)-protein kinase B(AKT)-nuclear factor-κB(NF-κB)-NOD-like receptor family pyrin domain containing 3(NLRP3) signaling axis was selected as a candidate target for in-depth research. Further, a rat model of DKD induced by a high-sugar, high-fat diet and streptozotocin was established to evaluate the pharmacological effects of CRT and verify the expression of related targets. The experimental results showed that CRT could effectively correct metabolic disturbances in DKD, restore immune-inflammatory balance, and improve renal function and its pathological changes by inhibiting the activation of the RAGE-ROS-PI3K-AKT-NF-κB-NLRP3 signaling axis. In conclusion, this study reveals that CRT alleviates the progression of DKD through dual regulation of metabolic reprogramming and immune-inflammatory responses, providing strong experimental evidence for its clinical application in DKD.

8.Forty years of construction and innovative development of scientific regulation system of traditional Chinese medicine in China.

Since the promulgation of the first Drug Administration Law of the People's Republic of China 40 years ago in 1984, China has undergone four main stages in the traditional Chinese medicine(TCM) regulation: the initial establishment of TCM regulation rules(1984-1997), the formation of a modern TCM regulatory system(1998-2014), the reform of the review and approval system for new TCM drugs(2015-2018), and the construction of a scientific regulation system for TCM(2019-2024). Over the past five years, a series of milestone achievements of TCM regulation in China have been achieved in the six aspects, including its strategic objectives and the establishment of a science-based regulatory system, the reform of the review and approval system for new TCM drugs, the optimization and improvement of the TCM standard system and its formation mechanism, comprehensive enhancement of regulatory capabilities for TCM safety, international harmonization of TCM regulation and its role in promoting innovation. Looking ahead, centered on advancing TCMRS to establish a sound regulatory framework tailored to the unique characteristics of TCM, TCM regulation will evolve into new reform patterns, advancing and extending across eight critical fronts, including the legal framework and policy architecture, the review and approval system for new TCM drugs, the quality standard and management system of TCM, the comprehensive quality & safety regulation and traceability system, the research and transformation system for TCMRS, AI-driven innovations in TCM regulation, the coordination between high-quality industrial development and high-level regulation, and the leadership in international cooperation and regulatory harmonization. In this way, a unique path for the development of modern TCM regulation with Chinese characteristics will be pioneered.

9.Clinical and genetic characteristics of osteopetrosis in children.

OBJECTIVES:
To study the clinical and genetic characteristics of osteopetrosis (OPT) in children.
METHODS:
A retrospective analysis was performed on the clinical data of 14 children with OPT. Whole-exome sequencing was used to detect pathogenic genes, and clinical phenotypes and genotypic features were summarized.
RESULTS:
Among the 14 children (10 males and 4 females), the median age at diagnosis was 8 months. Clinical manifestations included systemic osteosclerosis (14 cases, 100%), anemia (12 cases, 86%), infections (10 cases, 71%), thrombocytopenia (9 cases, 64%), hepatosplenomegaly (8 cases, 57%), and developmental delay (5 cases, 36%). Malignant osteopetrosis (MOP) cases had lower platelet counts, creatine kinase isoenzyme, and serum calcium levels, but higher white blood cell counts, lactate dehydrogenase, and alkaline phosphatase levels compared to non-MOP cases (P<0.05). Genetic testing identified 15 variants in 12 patients, including 8 variants in the CLCN7 gene (53%), 6 in the TCIRG1 gene (40%), and 1 in the TNFRSF11A gene (7%). Three novel CLCN7 variants were identified: c.2351G>C, c.1215-43C>T, and c.1534G>A. All four patients with TCIRG1 variants exhibited MOP clinical phenotypes. Of the seven patients with CLCN7 variants, 4 presented with intermediate OPT, 2 with benign OPT, and 1 with MOP.
CONCLUSIONS
Clinical phenotypes of OPT in children are heterogeneous, predominantly involving CLCN7 and TCIRG1 gene variants, with a correlation between clinical phenotypes and genotypes.

10.Impact of Spinal Manipulative Therapy on Brain Function and Pain Alleviation in Lumbar Disc Herniation: A Resting-State fMRI Study.

OBJECTIVE:
To elucidate how spinal manipulative therapy (SMT) exerts its analgesic effects through regulating brain function in lumbar disc herniation (LDH) patients by utilizing resting-state functional magnetic resonance imaging (rs-fMRI).
METHODS:
From September 2021 to September 2023, we enrolled LDH patients (LDH group, n=31) and age- and sex-matched healthy controls (HCs, n=28). LDH group underwent rs-fMRI at 2 distinct time points (TPs): prior to the initiation of SMT (TP1) and subsequent to the completion of the SMT sessions (TP2). SMT was administered once every other day for 30 min per session, totally 14 treatment sessions over a span of 4 weeks. HCs did not receive SMT treatment and underwent only one fMRI scan. Additionally, participants in LDH group completed clinical questionnaires on pain using the Visual Analog Scale (VAS) and the Japanese Orthopedic Association (JOA) score, whereas HCs did not undergo clinical scale assessments. The effects on the brain were jointly characterized using the amplitude of low-frequency fluctuations (ALFF) and regional homogeneity (ReHo). Correlation analyses were conducted between specific brain regions and clinical scales.
RESULTS:
Following SMT treatment, pain symptoms in LDH patients were notably alleviated and accompanied by evident activation of effects in the brain. In comparison to TP1, TP2 exhibited the most significant increase in ALFF values for Temporal_Sup_R and the most notable decrease in ALFF values for Paracentral_Lobule_L (voxelwise P<0.005; clusters >30; FDR correction). Additionally, the most substantial enhancement in ReHo values was observed for the Cuneus_R, while the most prominent reduction was noted for the Olfactory_R (voxelwise P<0.005; clusters >30; FDR correction). Moreover, a comparative analysis revealed that, in contrast to HCs, LDH patients at TP1 exhibited the most significant increase in ALFF values for Temporal_Pole_Sup_L and the most notable decrease in ALFF values for Frontal_Mid_L (voxelwise P<0.005; clusters >30; FDR correction). Furthermore, the most significant enhancement in ReHo values was observed for Postcentral_L, while the most prominent reduction was identified for ParaHippocampal_L (voxelwise P<0.005; clusters >30; FDR correction). Notably, correlation analysis with clinical scales revealed a robust positive correlation between the Cuneus_R score and the rate of change in the VAS score (r=0.9333, P<0.0001).
CONCLUSIONS
Long-term chronic lower back pain in patients with LDH manifests significant activation of the "AUN-DMN-S1-SAN" neural circuitry. The visual network, represented by the Cuneus_R, is highly likely to be a key brain network in which the analgesic efficacy of SMT becomes effective in treating LDH patients.
