1.Rapid health technology assessment of efficacy,safety and cost-effectiveness of vonoprazan in the treatment of gastroesophageal reflux disease

OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of vonoprazan (VPZ) in the treatment of gastroesophageal reflux disease (GERD) by rapid health technology assessment, and provide evidence for clinical decision-making. METHODS English and Chinese databases including PubMed, Medline, Cochrane Library, CNKI, VIP, and Wanfang data as well as the official websites of domestic and international health technology assessment agencies were searched by computer from the database establishment to August 2024. After literature screening, data extraction and quality evaluation of included studies were conducted by two researchers, the results were described and analyzed qualitatively. RESULTS A total of 21 pieces of literature were included, involving 15 systematic reviews/meta-analyses and 6 pharmacoeconomic studies. In terms of efficacy, compared with the control regimen (different doses of VPZ, placebo, other positive controls or combination therapy), VPZ (mainly 20 mg/d VPZ) significantly improved the total response rate, mucosal healing rate at 2nd week, symptom remission rate, and maintenance rate at 12th and 24th week after mucosal healing (P<0.05); when the endoscopic Los Angeles classification was C/D, the effective rate of VPZ was significantly higher than that of the control regimen (P<0.05). In terms of safety, there was no significant difference in the incidence of adverse events between VPZ and the control regimen for GERD treatment (P>0.05), but the risk of elevated serum gastrin and abnormal liver function caused by long-term use of VPZ was higher than that of the control regimen (P<0.05). In terms of cost-effectiveness, compared with rabeprazole, lansoprazole and esomeprazole, VPZ had a cost- effectiveness/cost-utility advantage. CONCLUSIONS VPZ is effective, safe and cost-effective in the treatment of GERD.
2.Research progress of transosseous tunnel technique in the treatment of inferior pole fracture of the patella

Due to the small and numerous fracture fragments,achieving precise reduction and reliable fixation is a difficult point in orthopedic surgery for inferior pole fracture of the patella.With the exploration and verification of numerous surgeons,transosseous tunnel technique has shown important clinical value and application prospects in the treatment of inferior pole fracture of the patella.This technique provides fixed fulcrums by constructing transosseous tunnels,and has derived various surgery,including transosseous tunnel with high tension wire or steel wire fixation,combined titanium/tension band wiring,patellar concentrator,suture anchor,edge steel plate and so on.Clinical practice has proved that these surgeries can effectively maintain the stability of fracture reduction and have good clinical effects.This article reviews the research progress of transosseous tunnel technique in the treatment of inferior pole fracture of the patella,which provides a reference for the selection of clinical surgeries.
3.Research progress on breast milk improving brain damage and neurodevelopment in preterm infants

With the continuous development of perinatal medicine and neonatology,although the survival rate of infants with low gestational age and low birth weight has improved,the incidence of neurodevelopmental morbidities remains high.Breast milk is the preferred source of enteral nutrition for preterm infants,playing an important role in growth,development,immunity,metabolism,and other functions.Of particular note is its effect on the nervous system,not only promoting brain development and maturation in preterm infants,but also effectively reducing the incidence and severity of brain injury,thereby improving long-term neurological outcomes.This review aims to explore the effects of breast milk on brain injury and repair,brain structural maturation,and neurodevelopmental outcomes in preterm infants,in order to further enhance the understanding of the importance of breast milk for the neurodevelopment of preterm infants.

Neurogenic bladder(NB)is one of the most challenging urinary system disorders,with spinal cord injury(SCI)being an important etiological factor.Animal models provide crucial tools for investigating the pathogenesis,therapeutic strategies,and novel drug screening for NB subsequent to SCI.We reviewed and synthesized recent literature on NB animal models after SCI from both domestic and international sources.This review summarizes and analyzes research advancements using these models in terms of animal species,SCI segments,modeling techniques,and evaluation indicators,with the aim of offering insights and guidance for future experimental research based on animal models of NB following SCI.

8.Construction of damage control operation simulation training platform for traumatic brain injury of wartime based on mixed reality

Objective To develop a damage control operation(DCO)simulation training platform for traumatic brain injury(TBI)in wartime based on mixed reality to open up a new path for surgical skills training of military surgeons.Methods The platform mainly consisted of wartime TBI DCO simulation training software,a surgical manikin and a HoloLens 2 MR device.The simulating training software was developed with C# language and the technologies of MR,basic gestures,spatial scanning positioning and etc on the basis of constructed surgical decision-making training system,virtual surgical environment and functional modules.The surgical manikin was customized with reference to the standard body type of an adult male with a height of 180 cm,and an electronic chip was developed and placed inside the head of the manikin to execute data matching with the simulation training software.The simulation training software was installed and run in the HoloLens 2 MR device to realize TBI DCO simulation training on the virtual reality interactive model.Results The platform developed implemented the functions of virtual reality interactive model reset positioning,operation simulation training,examination and on-site demonstration,which gained advantages in stimulating learning interest and facilitating risk-free,time-and space-indepen-dent,immersive and interactive learning and was generally recognized by the trainees.Conclusion The simulation training platform can be a supplementary to other training means to improve the ability of military surgeons in damage control operation.[Chinese Medical Equipment Journal,2024,45(2):17-21]
9.Study on the influencing factors and risk prediction model for proteinuria in patients with malignant tumors induced by apatinib

OBJECTIVE To study the influencing factors for proteinuria in patients with malignant tumors treated with apatinib, then establish and evaluate a risk prediction model based on it. METHODS A total of 120 patients with malignant tumors treated with apatinib in our hospital from January 2020 to December 2022 were selected as the training set, and the clinical data was collected. Univariate analysis and multivariate Logistic regression analysis were used to identify independent risk factors for proteinuria associated with apatinib and then construct a risk prediction model. The predictive value of the model was evaluated by using the receiver operator characteristic (ROC) curve. A total of 34 patients with malignant tumors treated with apatinib from January to December 2023 in our hospital were selected as the validation set, and their clinical data were obtained to cross-validate the accuracy of the prediction model. RESULTS The incidence of proteinuria in the training set of 120 patients was 26.67%. The proportions of patients with smoking history, combined hypertension, apatinib daily dose of ≥500 mg, and alanine aminotransferase level were significantly higher in proteinuria group than those in non-proteinuria group. At the same time,the neutrophilic granulocyte count was significantly lower than that in non-proteinuria group (P<0.05). Patients with smoking history and combined hypertension were the independent risk factors for apatinib-induced proteinuria (odds ratios were 5.005 and 5.342, respectively; with 95% confidence intervals of 1.806- 13.872 and 1.227-9.602, respectively; P<0.05). The binary Logistic regression model equation for the probability (P) of apatinib- induced proteinuria is expressed as LogitP=1.610XMH+1.233XSH-1.483 (MH for combined hypertension, SH for the smoking history), with a model accuracy of 80.0%. ROC curve analysis demonstrated the area under the ROC curve of 0.771, the maximum Youden’s index of 0.474, and the optimal cut-off value for LogitP was 0.159 9, with a sensitivity of 90.6% and specificity of 56.8%. Cross-validation results indicated an overall prediction accuracy of 88.24% for the 34 patients. CONCLUSIONS Combined hypertension and smoking history are independent risk factors for apatinib-induced proteinuria. The constructed risk prediction model has moderate predictive value and can be used to predict the risk of proteinuria in patients with malignant tumors induced by apatinib.

Hypertension is a common chronic disease,and its incidence is increasing year by year worldwide.Its pathological mechanism is complex,involving genetics,environment,lifestyle and other factors.In recent years,the role of immune-inflammatory mechanism in the pathogenesis of hypertension has gradually attracted attention.Especially T helper cell 17(Th17)and regulatory T cells(Treg),as important components of the immune system,the effect of their balance on hypertension has become a research hotspot.This article reviews the research progress of the correlation between Th17/Treg and hypertension,in order to provide new ideas and methods for the immunotherapy and prevention of hypertension.
