1.Research progress of transosseous tunnel technique in the treatment of inferior pole fracture of the patella

Due to the small and numerous fracture fragments,achieving precise reduction and reliable fixation is a difficult point in orthopedic surgery for inferior pole fracture of the patella.With the exploration and verification of numerous surgeons,transosseous tunnel technique has shown important clinical value and application prospects in the treatment of inferior pole fracture of the patella.This technique provides fixed fulcrums by constructing transosseous tunnels,and has derived various surgery,including transosseous tunnel with high tension wire or steel wire fixation,combined titanium/tension band wiring,patellar concentrator,suture anchor,edge steel plate and so on.Clinical practice has proved that these surgeries can effectively maintain the stability of fracture reduction and have good clinical effects.This article reviews the research progress of transosseous tunnel technique in the treatment of inferior pole fracture of the patella,which provides a reference for the selection of clinical surgeries.
2.Hemolytic disease of the fetus and newborn caused by Rh system anti-c antibodies: a case report and literature review

Objective: To summarize the laboratory findings of a case of hemolytic disease of the fetus and newborn (HDFN) caused by Rh system anti-c antibodies and to review the literature, so as to explore the characteristics of anti-c HDFN. Methods: The ABO blood type, Rh blood type, direct antiglobulin test (DAT) results, and the presence of unexpected antibodies and their titers were determined by serological methods. The cases of anti-c HDFN in our laboratory in China and abroad were statistically analyzed, and the incidence of severe HDFN caused by anti-c, anti-D and anti-E was compared. Results: The blood type of the child was B (Rh CcDee) with a positive DAT. Anti-c antibody was detected in both serum and eluate, with a serum antibody titer of 4. The mother’s blood type was AB (Rh CCDee) with a negative DAT, and anti-c antibody was detected in the serum with a titer of 128. Among 20 cases of anti-c HDFN, 17 were DAT positive, and 9 (45%, 9/20) underwent blood transfusion or exchange transfusion. The incidence of severe HDFN was 47.60% (10/21) for anti-c, 47.60% (10/21) for anti-D and 31.30% (5/16) for anti-E. Conclusion: Maternal pregnancy and/or blood transfusion are the main reasons for the production of Rh alloantibodies such as anti-c. The prevention and management of anti-c should be similar to that of anti-D. Rh antigen-matched (five antigens of Rh blood group) transfusion is necessary for women of childbearing age to avoid antibody production, and Rh typing and antibody screening during prenatal examination is recommended to ensure early detection, intervention and treatment.

4.Effect analysis of arthroscopic acromioplasty in the treatment of subacromial impingement syndrome

Objective To analyze the effect of arthroscopic acromioplasty on subacromial impingement syndrome.Methods 100 SIS patients admitted to our joint surgery department from August 2018 to August 2022 were selected as the observation group,all of whom underwent shoulder arthroscopic acromioplasty.Additionally,100 patients who underwent conservative treatment during the same period were selected as the control group.Compare the University of California Shoulder Score(UCLA),American Association of Shoulder and Elbow Surgeons Score(ASES),Fudan University Shoulder Function Score(FUSS),incidence of complications,and short-term efficacy between the control and observation groups at different times before and after surgery in the observation group.Results All patients will undergo a follow-up period of 12 months.The incidence of postoperative complications in the observation group was relatively low;The short-term efficacy of the observation group was significantly higher than that of the control group(P<0.05);At 1 month,6 months,and 12 months after surgery,the UCLA,ASES,and FUSS scores of the observation group patients were significantly higher than the preoperative scores of the above indicators(P<0.05).Conclusion Arthroscopic acromioplasty in the treatment of subacromial impingement syndrome has a good short-term effect.

6.Value of direct immunohistochemical staining in assisting intraoperative frozen diagnosis of bronchiolar adenoma.

Objective: To explore the feasibility and application value of intraoperative direct immunohistochemical (IHC) staining in improving the diagnosis accuracy in difficult cases of bronchiolar adenoma (BA). Methods: Nineteen cases with single or multiple pulmonary ground-glass nodules or solid nodules indicated by imaging in Cancer Hospital of Chinese Academy of Medical Sciences from January to July 2021 and with difficulty in differential diagnosis at frozen HE sections were selected. In the experimental group, direct IHC staining of cytokeratin 5/6 (CK5/6) and p63 was performed on frozen sections to assist the differentiation of BA from in situ/micro-invasive adenocarcinoma/adenocarcinoma/invasive mucinous adenocarcinoma. In the control group, two pathologists performed routine frozen HE section diagnosis on these 19 cases. The diagnostic results of paraffin sections were used as the gold standard. The sensitivity and specificity of BA diagnosis, consistency with paraffin diagnosis and time used for frozen diagnosis were compared between the experimental group and the control group. Results: The basal cells of BA were highlighted by CK5/6 and p63 staining. There were no basal cells in the in situ/microinvasive adenocarcinoma/adenocarcinoma/invasive mucinous adenocarcinoma. In the experimental group, the sensitivity and specificity with aid of direct IHC staining for BA were 100% and 86.7%, respectively, and the Kappa value of frozen and paraffin diagnosis was 0.732, and these were significantly higher than those in the control group (P<0.05). The average time consumption in the experimental group (32.4 min) was only 7 min longer than that in the control group (25.4 min). Conclusions: Direct IHC staining can improve the accuracy of BA diagnosis intraoperatively and reduce the risk of misdiagnosis, but require significantly longer time. Thus frozen direct IHC staining should be restricted to cases with difficulty in differentiating benign from malignant diseases, especially when the surgical modalities differ based on the frozen diagnosis.

Objective:To study the intervention effect and underlying mechanism of Fufang Huangbaiye Tuji (FFHBY) on skin with deep Ⅱ degree burn wound. Method:Patients with deep Ⅱ degree burn of fire-toxin injuring fluid syndrome diagnosed in the Affiliated Hospital of Chengde Medical University from June 2019 to June 2020 were randomly divided into a control group (iodophor solution, 35 mL per 1% body surface area), a low-dose treatment group (FFHBY, 17.5 mL per 1% body surface area), and a high-dose treatment group (FFHBY, 35 mL per 1% body surface area), 40 cases in each group. The patients in each group were treated correspondingly with dressing chance once per day. The pathological changes of the wound were observed on the 14th day after treatment. Wound symptoms and signs in each group before treatment and on the 7th, 14th, and 21st days after treatment were quantified, and the clinical efficacy on the 21st day after treatment was evaluated.
