As a complex autoimmune disease, rheumatoid arthritis (RA) involves immune microenvironment dysregulation resulting from excessive activation of pyroptosis, which is a crucial factor in disease progression. Based on the theory of ying-wei in traditional Chinese medicine, "ying decline and wei attack" is considered the fundamental pathogenesis of RA. Pyroptosis serves as a microscopic manifestation of this concept, suggesting a potential correlation between "ying decline and wei attack" and pyroptosis nd immune microenvironment dysregulation in RA. Accordingly, treatment principles based on this theory are proposed: in the early stage of the disease, boosting wei to consolidate the exterior, and regulating ying to dispel pathogens; in the middle and late stages, harmonizing ying to remove stagnation, and nourishing its transformational source.
2.Mechanism of Chaijin JieYu Anshen formula regulating synaptic plasticity of hippocampal neurons in insomnia-concomitant depression rats based on HDAC5/MEF2C pathway

Aim To investigate the mechanisms of Chaijin JieYu Anshen formula modulating the depres-sive behaviors and the synaptic plasticity of hippocam-pal neurons in insomnia-concomitant depression rats based on the histone deacetylase 5(HDAC5)/myocyte enhancer factor 2C(MEF2C)pathway.Methods A rat model of insomnia-concomitant depression was es-tablished by PCPA injection combined with chronic un-predictable mild stress(CUMS),and the experiment was divided into the control group,the model group,the high,medium and low dose group of Chaijin JieYu Anshen formula,and the positive drug group.The de-pression of rats was evaluated by sugar-water prefer-ence test,open field test and morris water maze.The levels of 5-hydroxytryptamine(5-HT)and dopamine(DA)in serum were measured by enzyme linked im-munosorbent assay(ELISA).The pathological damage of hippocampal neurons was observed by HE staining and Nissl staining.The damage of dendritic spines of hippocampal neurons was observed by Golgi staining,and the levels of HDAC5,MEF2C,postsynaptic densi-ty-95(PSD-95)and synaptophysin 1(SYN1)in hip-pocampus were measured by Western blot,immunohis-tochemistry and immunofluorescence.Results Com-pared with the model group,the Chaijin JieYu Anshen formula could increase the sugar-water preference rate of the model rats,reduce the immobility time in the open field experiment,increase the total activity dis-tance,shorten the evasion latency in the localization navigation experiment,and prolong the residence time in the quadrant where the platform was located in the space exploration experiment(P<0.05,P<0.01).Moreover,the Chaijin JieYu Anshen formula improved the hippocampal neuron and dendritic spine damage and increase the dendritic branch length and dendritic spine density of hippocampal neurons(P<0.01,P<0.01),restore the serum levels of 5-HT and DA in insomnia-concomitant depression rats(P<0.05,P<0.01),down-regulate the HDAC5 protein,and up-regulate the expression of MEF2C,PSD-95,and SYN1 protein(P<0.05,P<0.01 or P<0.001).Conclusions Chaijin JieYu Anshen formula may alle-viate the depression-like behavior of model rats by re-ducing the expression of HDAC5 protein,thus deregu-lating the inhibition of transcription factor MEF2C,promoting the expression of PSD-95 and SNY1 protein,and exerting a protective effect on hippocampal neurons and synapses.
3.Clinical Features and Prognosis of Acute T-cell Lymphoblastic Leukemia in Children——Multi-Center Data Analysis in Fujian

Objective:To evaluate the efficacy of acute T-cell lymphoblastic leukemia(T-ALL)in children and explore the prognostic risk factors.Methods:The clinical data of 127 newly diagnosed children with T-ALL admitted to five hospitals in Fujian province from April 2011 to December 2020 were retrospectively analyzed,and compared with children with newly diagnosed acute precursor B-cell lymphoblastic leukemia(B-ALL)in the same period.Kaplan-Meier analysis was used to evaluate the overall survival(OS)and event-free survival(EFS),and COX proportional hazard regression model was used to evaluate the prognostic factors.Among 116 children with T-ALL who received standard treatment,78 cases received the Chinese Childhood Leukemia Collaborative Group(CCLG)-ALL 2008 protocol(CCLG-ALL 2008 group),and 38 cases received the China Childhood Cancer Collaborative Group(CCCG)-ALL 2015 protocol(CCCG-ALL 2015 group).The efficacy and serious adverse event(SAE)incidence of the two groups were compared.Results:Proportion of male,age ≥ 10 years old,white blood cell count(WBC)≥ 50 × 109/L,central nervous system leukemia,minimal residual disease(MRD)≥ 1%during induction therapy,and MRD ≥ 0.01%at the end of induction in T-ALL children were significantly higher than those in B-ALL children(P<0.05).The expected 10-year EFS and OS of T-ALL were 59.7%and 66.0%,respectively,which were significantly lower than those of B-ALL(P<0.001).COX analysis showed that WBC ≥ 100 x 109/L at initial diagnosis and failure to achieve complete remission(CR)after induction were independent risk factors for poor prognosis.Compared with CCLG-ALL 2008 group,CCCG-ALL 2015 group had lower incidence of infection-related SAE(15.8%vs 34.6%,P=0.042),but higher EFS and OS(73.9%vs 57.2%,PEFS=0.090;86.5%vs 62.3%,PoS=0.023).Conclusions:The prognosis of children with T-ALL is worse than children with B-ALL.WBC ≥ 100 × 109/L at initial diagnosis and non-CR after induction(especially mediastinal mass has not disappeared)are the risk factors for poor prognosis.CCCG-ALL 2015 regimen may reduce infection-related SAE and improve efficacy.
4.Effect of CD8+CD28-T Cells on Acute Graft-Versus-Host Disease after Haploidentical Hematopoietic Stem Cell Transplantation

Objective:To investigate the effect of CD8+CD28-T cells on acute graft-versus-host disease(aGVHD)after haploidentical hematopoietic stem cell transplantation(haplo-HSCT).Methods:The relationship between absolute count of CD8+CD28-T cells and aGVHD in 60 patients with malignant hematological diseases was retrospectively analyzed after haplo-HSCT,and the differences in the incidence rate of chronic graft-versus host disease(cGVHD),infection and prognosis between different CD8+CD28-T absolute cells count groups were compared.Results:aGVHD occurred in 40 of 60 patients after haplo-HSCT,with an incidence rate of 66.67%.The median occurrence time of aGVHD was 32.5(20-100)days.At 30 days after the transplantation,the absolute count of CD8+CD28-T cells of aGVHD group was significantly lower than that of non-aGVHD group(P=0.03).Thus the absolute count of CD8+CD28-T cells at 30 days after transplantation can be used to predict the occurrence of aGVHD to some extent.At 30 days after transplantation,the incidence rate of aGVHD in the low cell count group(CD8+CD28-T cells absolute count<0.06/μl)was significantly higher than that in the high cell count group(CD8+CD28-T cells absolute count ≥0.06/μl,P=0.011).Multivariate Cox regression analysis further confirmed that the absolute count of CD8+CD28-T cells at 30 days after transplantation was an independent risk factor for aGVHD,and the risk of aGVHD in the low cell count group was 2.222 times higher than that in the high cell count group(P=0.015).The incidence of cGVHD,fungal infection,EBV infection and CMV infection were not significantly different between the two groups with different CD8+CD28-T cells absolute count.The overall survival,non-recurrent mortality and relapse rates were not significantly different between different CD8+CD28-T cells absolute count groups.Conclusion:Patients with delayed CD8+CD28-T cells reconstitution after haplo-HSCT are more likely to develop aGVHD,and the absolute count of CD8+CD28-T cells can be used to predict the incidence of aGVHD to some extent.The absolute count of CD8+CD28-T cells after haplo-HSCT was not associated with cGVHD,fungal infection,EBV infection,and CMV infection,and was also not significantly associated with the prognosis after transplantation.
5.The Value of REG3α,sST2,and TNFR1 in Risk Stratification and Prognostic Evaluation of Acute Graft-versus-Host Disease after Allogeneic Hematopoietic Stem Cell Transplantation in Children

Objective:To explore the value of REG3α,sST2 and TNFR1 in peripheral blood for risk stratification and prognostic evaluation of acute graft-versus-host disease(aGVHD)after allogeneic hematopoietic stem cell transplantation(allo-HSCT)in children.Methods:From January 2020 to March 2022,70 children with aGVHD after allo-HSCT in the First Affiliated Hospital of Zhengzhou University were selected as the research objects,of which 50 cases were mild aGVHD(grade Ⅰ-Ⅱ)and 20 cases were severe aGVHD(grade Ⅲ-Ⅳ).30 healthy children who underwent physical examinations in our hospital during the same period were selected as the control group.Luminex platform was used to detect the protein expression levels of REG3α,sST2 and TNFR1 during aGVHD occurrence,and the differences between the three groups were analyzed by one-way ANOVA.According to the outcome of aGVHD treatment within 28 days,the patients were divided into a good prognosis group of 58 cases and a poor prognosis group of 12 cases.The ROC curve was used to analyze the value of REG3α,sST2 and TNFR1 in predicting the prognosis of children with aGVHD.Results:The peripheral blood levels of REG3α,sST2 and TNFR1 in the mild aGVHD and severe aGVHD groups were significantly higher than those in the control group(P<0.05),and those in the severe aGVHD group were significantly higher than those in the mild aGVHD group(P<0.05).Compared with the good prognosis group,the peripheral blood levels of REG3α,sST2 and TNFR1 in the poor prognosis group were significantly higher(t=9.27,3.33,2.97;P<0.01).ROC curve analysis showed that the area under the curve(AUC),sensitivity and specificity of the combined detection of REG3α,sST2 and TNFR1 in predicting the prognosis of children with aGVHD were higher than those of the above indicators detected alone or in pairs.Conclusion:The expression levels of REG3α,sST2 and TNFR1 were related to the severity of aGVHD.The combination of REG3α,sST2 and TNFR1 has a high clinical value in predicting the prognosis of children with aGVHD,which is expected to provide a reliable reference for clinical evaluation of the prognosis of children with aGVHD.
6.The value of BISAP score,MLR,and NAR in the assessment and prognosis of acute pancreatitis condition

Objective To investigate the value of acute pancreatitis severity bedside index(BISAP)score,monocyte to lymphocyte ratio(MLR),and neutrophil to albumin ratio(NAR)in the severity and prognosis of acute pancreatitis(AP).Methods The clinical data of 208 patients with acute pancreatitis admitted to the Affiliated Hospital of Southwest Medical University from January 2022 to December 2023 were retrospectively collected and divided into mild(MAP,n=131 cases)and severe(SAP,n=77 cases)groups according to the severity of the disease.Spearman correlation test was used to analyze the relationship between BISAP score,MLR,NAR and severity of disease and hospitalization days,and subject operating curve(ROC curve)analyzes the clinical significance of the above indicators in the prognosis of AP patients.Results The number of hospitalization days,BISAP score,MLR,NAR,WBC,PCT,FPG,TC,and Ca+were greater in the SAP group than in the MAP group,differences were statistically significant(P<0.05);while the differences in Comorbidities,etiology,history of smoking,history of alcohol consumption,TC、TG、Ceea、GFR were not statistically significant(P>0.05).By correlation analysis,the BISAP score,MLR and NAR were positively correlated with the severity of the disease(r=0.474、0.542、0.519)and the number of hospitalization days(r=0.401、0.370、0.396),differences were statistically significant(P<0.05).Binary Loginstic regression showed that BISAP score,MLR,NAR,and FPG were all risk factors affecting patient prognosis.ROC curve analysis of BISAP score,MLR,and NAR were predictive of poor prognosis in AP patients(AUC=0.766、0.825、0.808),the combined predictive effect of the three was greater than that of a single index(AUC=0.863).Conclusion The number of hospitalization days,BISAP score,MLR,NAR,WBC,PCT,FPG,TC,and Ca+were greater in the SAP group than in the MAP group,differences were statistically significant(P<0.05);while the differences in Comorbidities,etiology,history of smoking,history of alcohol consumption,TC、TG、Ceea、GFR were not statistically significant(P>0.05).By correlation analysis,the BISAP score,MLR and NAR were positively correlated with the severity of the disease(r=0.474、0.542、0.519)and the number of hospitalization days(r=0.401、0.370、0.396),differences were statistically significant(P<0.05).Binary Loginstic regression showed that BISAP score,MLR,NAR,and FPG were all risk factors affecting patient prognosis.ROC curve analysis of BISAP score,MLR,and NAR were predictive of poor prognosis in AP patients(AUC=0.766、0.825、0.808),the combined predictive effect of the three was greater than that of a single index(AUC=0.863).
7.Design of punching and positioning device for training Da Vinci surgical robots

Objective To design a punching and positioning device for training Da Vinci surgical robots to improve the efficiency and accuracy of operation training.Methods The punching and positioning device mainly consisted of an abdominal girth,a connecting plate,a punching plate,a positioning disk and a punching cartridge.The abdominal girth had a long-strip shape and was made of stretch nylon elastic band;the connecting plate had a rectangular shape with its length direction perpendicular to the abdominal girth;the punching plate also had a rectangular shape and was made of hard silicone;the positioning plate was made of hard silicone and privided with a plated through hole;the punching cartridge were equipped with anti-fog components composed of heating wires and support strips,which had scale bars inside.Ten physicians were randomly divided into a control group and an experimental group,with 5 ones in each group.The two groups carried out positioning and punching respectively,with the traditional method for the control group and the device designed for the experimental group,which were compared in terms of the time consumption,ratio for lens fogging and number of collisions with the robotic arm.Results The experimental group had the time consumption and ratio for lens fogging lower significantly when compared with the control group(P<0.05),while had no obvious difference in number of collisions with the robotic arm(P>0.05),and the requirements of the training specifications were met.Conclusion The punching and positioning device has high efficiency and accuracy,and thus is worthy promoting for the operation training of surgical robots.[Chinese Medical Equipment Journal,2024,45(7):35-39]
8.Design of experimental pig anesthesia device for Da Vinci surgical robot training

Objective To design an experimental pig anesthesia device for training da Vinci surgical robot in order to achieve precise control of anesthesia depth.Methods The experimental pig anesthesia device for Da Vinci surgical robot training was composed of an animal anesthesia table,an oral opening and closing mechanism,a nasal catheter assembly and an anesthesia gas detection system.The animal anesthesia table consisted of an animal bearing surface made of high-quality stainless steel and a protective cover made of transparent acrylic material;the oral opening and closing mechanism was made up of an electric digital tensioner,a support rod,a rotating socket and binding straps;the nasal catheter assembly included an anesthesia gas ventilating catheter and a set of nasal fixed airbag;the anesthesia gas detecting system involved in a SS316 one-way inlet valve,a GT-SKSG-A-D isoflurane detector,a gas pathway,an exhaust gas collection container,an extractor fan and several vents.Results The anesthesia device achieved precise control of anesthesia depth for experimental pigs,improving anesthesia induction and preventing anesthesia gas from spilling out.Conclusion The anesthesia device enhances the success rate for experimental pig anesthesia and the efficacy for Da Vinci surgical robot training effectively.[Chinese Medical Equipment Journal,2024,45(11):27-31]

Objective To investigate the clinical characteristics and prognosis of pneumococcal meningitis(PM),and drug sensitivity of Streptococcus pneumoniae(SP)isolates in Chinese children.Methods A retrospective analysis was conducted on clinical information,laboratory data,and microbiological data of 160 hospitalized children under 15 years old with PM from January 2019 to December 2020 in 33 tertiary hospitals across the country.Results Among the 160 children with PM,there were 103 males and 57 females.The age ranged from 15 days to 15 years,with 109 cases(68.1% )aged 3 months to under 3 years.SP strains were isolated from 95 cases(59.4% )in cerebrospinal fluid cultures and from 57 cases(35.6% )in blood cultures.The positive rates of SP detection by cerebrospinal fluid metagenomic next-generation sequencing and cerebrospinal fluid SP antigen testing were 40% (35/87)and 27% (21/78),respectively.Fifty-five cases(34.4% )had one or more risk factors for purulent meningitis,113 cases(70.6% )had one or more extra-cranial infectious foci,and 18 cases(11.3% )had underlying diseases.The most common clinical symptoms were fever(147 cases,91.9% ),followed by lethargy(98 cases,61.3% )and vomiting(61 cases,38.1% ).Sixty-nine cases(43.1% )experienced intracranial complications during hospitalization,with subdural effusion and/or empyema being the most common complication[43 cases(26.9% )],followed by hydrocephalus in 24 cases(15.0% ),brain abscess in 23 cases(14.4% ),and cerebral hemorrhage in 8 cases(5.0% ).Subdural effusion and/or empyema and hydrocephalus mainly occurred in children under 1 year old,with rates of 91% (39/43)and 83% (20/24),respectively.SP strains exhibited complete sensitivity to vancomycin(100% ,75/75),linezolid(100% ,56/56),and meropenem(100% ,6/6).High sensitivity rates were also observed for levofloxacin(81% ,22/27),moxifloxacin(82% ,14/17),rifampicin(96% ,25/26),and chloramphenicol(91% ,21/23).However,low sensitivity rates were found for penicillin(16% ,11/68)and clindamycin(6% ,1/17),and SP strains were completely resistant to erythromycin(100% ,31/31).The rates of discharge with cure and improvement were 22.5% (36/160)and 66.2% (106/160),respectively,while 18 cases(11.3% )had adverse outcomes.Conclusions Pediatric PM is more common in children aged 3 months to under 3 years.Intracranial complications are more frequently observed in children under 1 year old.Fever is the most common clinical manifestation of PM,and subdural effusion/emphysema and hydrocephalus are the most frequent complications.Non-culture detection methods for cerebrospinal fluid can improve pathogen detection rates.Adverse outcomes can be noted in more than 10% of PM cases.SP strains are high sensitivity to vancomycin,linezolid,meropenem,levofloxacin,moxifloxacin,rifampicin,and chloramphenicol.[Chinese Journal of Contemporary Pediatrics,2024,26(2):131-138]
10.Repair effect of different doses of human umbilical cord mesenchymal stem cells on white matter injury in neonatal rats

Objective To compare the repair effects of different doses of human umbilical cord mesenchymal stem cells(hUC-MSCs)on white matter injury(WMI)in neonatal rats.Methods Two-day-old Sprague-Dawley neonatal rats were randomly divided into five groups:sham operation group,WMI group,and hUC-MSCs groups(low dose,medium dose,and high dose),with 24 rats in each group.Twenty-four hours after successful establishment of the neonatal rat white matter injury model,the WMI group was injected with sterile PBS via the lateral ventricle,while the hUC-MSCs groups received injections of hUC-MSCs at different doses.At 14 and 21 days post-modeling,hematoxylin and eosin staining was used to observe pathological changes in the tissues around the lateral ventricles.Real-time quantitative polymerase chain reaction was used to detect the quantitative expression of myelin basic protein(MBP)and glial fibrillary acidic protein(GFAP)mRNA in the brain tissue.Immunohistochemistry was employed to observe the expression levels of GFAP and neuron-specific nuclear protein(NeuN)in the tissues around the lateral ventricles.TUNEL staining was used to observe cell apoptosis in the tissues around the lateral ventricles.At 21 days post-modeling,the Morris water maze test was used to observe the spatial learning and memory capabilities of the neonatal rats.Results At 14 and 21 days post-modeling,numerous cells with nuclear shrinkage and rupture,as well as disordered arrangement of nerve fibers,were observed in the tissues around the lateral ventricles of the WMI group and the low dose group.Compared with the WMI group,the medium and high dose groups showed alleviated pathological changes;the arrangement of nerve fibers in the medium dose group was relatively more orderly compared with the high dose group.Compared with the WMI group,there was no significant difference in the expression levels of MBP and GFAP mRNA in the low dose group(P>0.05),while the expression levels of MBP mRNA increased and GFAP mRNA decreased in the medium and high dose groups.The expression level of MBP mRNA in the medium dose group was higher than that in the high dose group,and the expression level of GFAP mRNA in the medium dose group was lower than that in the high dose group(P<0.05).Compared with the WMI group,there was no significant difference in the protein expression of GFAP and NeuN in the low dose group(P>0.05),while the expression of NeuN protein increased and GFAP protein decreased in the medium and high dose groups.The expression of NeuN protein in the medium dose group was higher than that in the high dose group,and the expression of GFAP protein in the medium dose group was lower than that in the high dose group(P<0.05).Compared with the WMI group,there was no significant difference in the number of apoptotic cells in the low dose group(P>0.05),while the number of apoptotic cells in the medium and high dose groups was less than that in the WMI group,and the number of apoptotic cells in the medium dose group was less than that in the high dose group(P<0.05).Compared with the WMI group,there was no significant difference in the escape latency time in the low dose group(P>0.05);starting from the third day of the latency period,the escape latency time in the medium dose group was less than that in the WMI group(P<0.05).The medium and high dose groups crossed the platform more times than the WMI group(P<0.05).Conclusions Low dose hUC-MSCs may yield unsatisfactory repair effects on WMI in neonatal rats,while medium and high doses of hUC-MSCs have significant repair effects,with the medium dose demonstrating superior efficacy.[Chinese Journal of Contemporary Pediatrics,2024,26(4):394-402]
