1.Mechanism of action of Sterculiae Lychnophorae Semen against PM2.5-induced acute lung injury based on network pharmacology,molecular docking and experimental validation

Aim To investigate the anti-acute lung injury(ALI)effect of Sterculiae Lychnophorae Semen(SLS)and its mechanism.Methods The main ac-tive components of SLS and their core targets and path-ways of action against ALI were obtained by network pharmacology methods.Subsequently,molecular doc-king technology and in vitro cellular experiments were applied for validation.Results A total of 19 core tar-gets were obtained,including HSP90AA1,CASP3,TNF,MAPK8 and MAPK14.The mechanisms may in-volve signaling pathways such as cancer,PI3K/Akt and MAPK.Molecular docking confirmed that the key targets of SLS formed a better binding activity with the relevant active ingredients.The in vitro results showed that SLS was able to protect the PM2.5-contaminated BEAS-2B cells,inhibit their NO,IL-1β and TNF-αlevels,and reduce the expression of p-p38 MAPK and p-JNK proteins.Conclusions The study successfully predicts the active ingredients,targets and signaling pathways of SLS against ALI,and in vitro experiments demonstrate that SLS might protect BEAS-2B cells from PM2.5 stimulus-induced inflammation and apoptosis by inhibiting the over-activation of p38 MAPK and JNK signaling pathways.
2.Changes of cardiovascular function and quality of life in patients with chronic heart failure after medi-cal community homogenization management

Objective:To investigate the effect of medical community homogenization management(MCHM)on self-care ability,heart function,vascular endothelial function and quality of life in patients with chronic heart failure(CHF).Methods:This randomized controlled study enrolled 110 CHF patients admitted in Panzhihua Central Hos-pital between April 2021 and April 2023.They were divided into intervention group(n=55)and routine group(n=55).The control group received routine management,while intervention group received additional MCHM,both groups were intervened for 3 months.Self-care ability,heart function,vascular endothelial function,quality of life and incidence of adverse events were compared between two groups.Results:Compared with patients in control group,those in intervention group had significant higher total score of self-care of heart failure index(SCHFI)[(82.30±2.98)points vs.(59.46±3.19)points],stroke volume(SV)[(73.30±2.31)ml vs.(54.66±1.96)ml],left ventricular ejection fraction(LVEF)[(58.25±2.90)% vs.(52.41±2.52)%],early diastolic peak flow velocity/late diastolic peak flow velocity(E/A)[(1.95±0.18)vs.(1.30±0.16)]and the level of nitric oxide(NO)[(106.70±4.44)μmol/L vs.(82.36±4.66)μmol/L](P<0.001 all),and significant lower left ventricular end-diastolic diameter(LVEDd)[(49.79±1.58)mm vs.(56.49±2.17)mm],endothelin-1(ET-1)[(36.66±2.65)ng/L vs.(46.88±2.66)ng/L],Minnesota living with heart failure questionnaire(MLHFQ)total score[(36.12±3.23)points vs.(54.67±3.35)points](P<0.001 all).Patients in intervention group had significant lower incidence of adverse events comparing to those in control group(7.27% vs.25.45%,P=0.010).Conclu-sion:Medical community homogenization management could significantly improve self-care ability,heart func-tion,vascular endothelial function and quality of life with good safety in CHF patients.
3.Premature mortality and disease burden attributable to major chronic diseases in Huangyan District, Taizhou City, Zhejiang Province, 2015‒2024

ObjectiveTo analyze the premature mortality and disease burden attributable to major chronic diseases in Huangyan District, Taizhou City, Zhejiang Province from 2015 to 2024, and to provide data support for the efforts of local chronic disease prevention and control. MethodsBased on the mortality surveillance data of registered residents in Huangyan District from 2015 to 2024, mortality rates and premature mortality rates of malignant tumors, diabetes mellitus, cardiovascular and cerebrovascular diseases, and chronic respiratory diseases were calculated. Indicators such as potential years of life lost (PYLL), average years of life lost (AYLL), and potential years of life lost rate (PYLLR) were also estimated. ResultsFrom 2015 to 2024, the crude mortality rate of major chronic diseases among registered residents in Huangyan District was 514.27/100 000, with an age-standardized mortality rate of 377.17/100 000. The crude (standardized) mortality rate was 588.00/100 000 (422.00/100 000) in males and 440.50/100 000 (328.08 /100 000) in females, indicating higher mortality among males. The fluctuation ranges of premature mortality rate of malignant tumors, diabetes mellitus, cardiovascular and cerebrovascular diseases and chronic respiratory diseases were 4.53%‒8.38%, 0.15%‒0.72%, 2.00%‒4.03% and 0.23%‒0.47%, respectively. The premature mortality rates of malignant tumors and cardiovascular and cerebrovascular diseases showed a decreasing trend (all P<0.05), while that of diabetes mellitus increased statistically significant (P<0.05). Malignant tumors accounted for the highest PYLL and PYLLR (5 107.15 person-years and 9.47‰, respectively), whereas chronic respiratory diseases contributed the lowest (213.25 person-years and 0.40‰, respectively). Cardiovascular and cerebrovascular diseases resulted in the highest AYLL (10.62 years), while chronic respiratory diseases had the lowest (7.64 years). The PYLL, AYLL, and PYLLR for major chronic diseases were 5 432.20 person-years, 10.24 years, and 19.99‰ in males, and 2 617.15 person-years, 10.49 years, and 9.78‰ in females, respectively, indicating greater loss of life expectancy in males than in females. ConclusionFrom 2015 to 2024, both age-standardized mortality and premature mortality attributable to major chronic diseases of registered residents declined in Huangyan District, reflecting notable achievements in chronic disease control. However, malignant tumors and cardiovascular and cerebrovascular diseases remained the main causes of death, while the burden of diabetes mellitus continued to rise. Males experienced higher premature mortality rate and life expectancy loss caused by major chronic diseases than females. Targeted chronic disease screening, risk factor surveillance, and interventions among high-risk populations should be further strengthened by local health authorities.

5.Design and application of foot pad for arch support after plantar skin extraction

Objective To develop a foot pad for arch support after plantar skin grafting and evaluate its rehabilitation effect on scar tissue in the arch area of postoperative patients.Methods The foot pad for arch support after plantar skin grafting was fabricated from medical-grade silicone and comprised a pad body and an arch support component.The pad body featured shock-absorbing convex patterns on its surface,precision-cut grooves in the metatarsal region and an upwardly convex arch section;the arch support component consisted of an arch-supporting portion with heel reinforcement and a heel-supporting portion,both peripherally integrated with vibration-damping through-holes.Totally 82 burn patients undergoing grafting using plantar skin were selected and divided equally into a control group and an experimental group with the random number table method.In the control group,anti-scarring care such as applying silicone gel and pressure therapy by rehabilitation nurses was carried out immediately after the healing of the plantar skin removal site.In the experimental group,the foot pad was used for rehabilitation care besides the routine treatment in the control group.The two groups were compared in terms of scarring and adverse reactions at the plantar skin removal site.SPSS 25.0 software was used for statistical analysis.Results Within 1 week after healing,the number of patients in the experimental group who had subcutaneous bruising and rupture was less than that in the control group,and the difference was statistically significant(P<0.05).At 3 months after healing,the experimental group behaved better than the control group in scarring,and the difference was statistically significant(P<0.05).Conclusion The foot pad developed with simple structure and easy operation can be used for rehabilitation exercise of patients after plantar skinning.[Chinese Medical Equipment Journal,2025,46(3):115-117]
6.Dual Gain Effect of Hyperbaric Oxygen Combined with Vestibular Rehabilitation Training on Patients with Tinnitus and Dizziness

Objective:To analyze the dual gain effect of hyperbaric oxygen combined with vestibular rehabilitation training on patients with tinnitus and dizziness.Methods:This study was a retrospective analysis,and 90 patients with tinnitus and dizziness who visited the outpatient department of our hospital from January 2022 to January 2025 were selected to carry out the study,and were divided into observation group and control group according to the treatment methods,with 45 cases in each group.The matched group patients were treated with conventional medication,while the observation group received hyperbaric oxygen combined with vestibular rehabilitation training on the basis of the matched group.The clinical efficacy of the two groups was compared,including the concentration of oxygenated hemoglobin in the cerebral cortex,vestibular function,severity of tinnitus,and severity of dizziness before and after treatment.Results:The total effective rate of the observation group was higher than the matched group(P<0.05);Post-treatment,there was no significant change in the concentration of oxygenated hemoglobin in the parietal and occipital cortex between the two groups(P>0.05).However,the concentration of oxygenated hemoglobin in the temporal lobe,dorsolateral prefrontal cortex,and frontal polar cortex increased,and the observation group was higher than the matched group(P<0.05);Post-treatment,the values of hemiparesis,total area of center of gravity movement(CA),and total length of center of gravity movement(PL)in both groups decreased,and the observation group was lower than the matched group(P<0.05);Post-treatment,the severity score of tinnitus in both groups decreased,and the observation group was lower than that in the matched group(P<0.05);Post-treatment,the degree score of vertigo in both groups decreased,and the observation group was lower than that in the matched group(P<0.05).Conclusion:Hyperbaric oxygen combined with vestibular rehabilitation training has a significant dual gain effect on patients with tinnitus and dizziness,which can improve clinical efficacy,promote cortical neural activity,improve vestibular function,and thereby alleviate the severity of tinnitus and dizziness.

Anticipatory anxiety is a negative emotion that arises when individuals encounter potential threats or uncertainties in the future. It is the core symptom of a variety of anxiety disorders, and is closely associated with the occurrence, severity, treatment outcome, and prognosis of anxiety disorders, which has garnered a growing amount of focus in clinical practice. Nevertheless, scientific research on anticipatory anxiety continues to face obstacles such as unclear pathological mechanisms, the absence of simple and consistent self-assessment tools, and effective interventions. To improve understanding of the role of anticipatory anxiety in the diagnosis and treatment of anxiety disorders, this study reviews pertinent domestic and international literature, and briefly introduces the concept, assessment and measurement, activation paradigm, pathological mechanisms, and interventions of anticipatory anxiety.
9.Mechanistic of Yueju Wan volatile oil in inhibiting inflammation for antidepressant effects by regulating AGE/PI3K/Akt pathway.

The antidepressant activity and molecular mechanisms of Yueju Wan volatile oil were investigated. The Yueju Wan volatile oil was extracted by using supercritical CO_2. Gas chromatography-mass spectrometry(GC-MS) combined with network pharmacology identified 28 chemical constituents in Yueju Wan volatile oil, primarily terpenes and lactones. A total of 123 overlapping targets were associated with depression, including core targets of interleukin-1β(IL-1β), signal transducer and activator of transcription 3(STAT3), and caspase-3(CASP3). These targets were mainly involved in the prolactin, advanced glycation end products/receptor(AGE/RAGE), and phosphoinositide 3-kinase/protein kinase B(PI3K/Akt) signaling pathways. A reserpine-induced depression mouse model was established to evaluate the therapeutic effects and mechanisms of Yueju Wan volatile oil. The effects of Yueju Wan volatile oil on depression-like behavior in mice were evaluated by analyzing body mass, body temperature index, tail suspension immobility time, forced swimming immobility time, and sucrose preference. Hematoxylin-eosin(HE) staining revealed neuronal protection of Yueju Wan volatile oil in the brain of mice. Enzyme-linked immunosorbent assay(ELISA) and Western blot were employed to detect the protein expression of AGEs, IL-1β, phosphorylated PI3K(p-PI3K), Akt, phosphorylated Akt(p-Akt), nuclear factor κB(NF-κB), and brain-derived neurotrophic factor(BDNF). Behavioral evaluation showed that Yueju Wan volatile oil could effectively control the decline of body mass and body temperature of depressed mice, reduce tail suspension and swimming immobility time, and enhance their preference for sucrose. Histopathological examination showed that Yueju Wan volatile oil could alleviate the neuronal damage in CA1 and dentate gyrus(DG) of the hippocampus of mice. ELISA and Western blot results showed that Yueju Wan volatile oil could significantly increase the protein expression levels of PI3K, Akt, and BDNF and significantly decrease the protein expression levels of AGEs, IL-1β, p-PI3K, p-Akt, and NF-κB in the hippocampus of mice. Furthermore, the p-PI3K/PI3K and p-Akt/Akt ratios were significantly decreased at medium and high doses. These findings suggest that the aromatherapy of Yueju Wan volatile oil can significantly improve reserpine-induced depression-like behavior in mice, which may be related to reducing the expression of neuronal membrane protein AGEs, reducing the phosphorylation levels of PI3K and Akt, inhibiting NF-κB entry into the nucleus, and alleviating the release of pro-inflammatory factors and nerve injury.

10.Effects of MTHFR and GGH gene polymorphisms on plasma concentrations and toxicity following high-dose methotrexate therapy in children with acute lymphoblastic leukemia.

OBJECTIVES:
To investigate the effects of methylenetetrahydrofolate reductase (MTHFR) rs1801133 and γ-glutamyl hydrolase (GGH) rs11545078 gene polymorphisms on plasma concentrations and toxicity following high-dose methotrexate (MTX) therapy in children with acute lymphoblastic leukemia (ALL).
METHODS:
Children with ALL treated at the Xuzhou Children's Hospital of Xuzhou Medical University from January 2021 to April 2024 were selected for this study. Genotypes of MTHFR rs1801133 and GGH rs11545078 were determined using multiplex polymerase chain reaction. MTX plasma concentrations were measured by enzyme-multiplied immunoassay technique, and toxicity was graded according to the Common Terminology Criteria for Adverse Events version 5.0. The relationships between MTHFR rs1801133 and GGH rs11545078 genotypes and both MTX plasma concentrations and associated toxicities were analyzed.
RESULTS:
In the low-risk ALL group, the MTHFR rs1801133 genotype was associated with increased MTX plasma concentrations at 72 hours (P<0.05). In the intermediate- to high-risk group, the MTHFR rs1801133 genotype was associated with increased MTX plasma concentrations at 48 hours (P<0.05), and the GGH rs11545078 genotype was associated with increased MTX plasma concentrations at 48 hours (P<0.05). In the intermediate- to high-risk group, the MTHFR rs1801133 genotype was associated with the occurrence of reduced hemoglobin (P<0.05), and the GGH rs11545078 genotype was associated with the occurrence of thrombocytopenia (P<0.05).
CONCLUSIONS
Detection of MTHFR rs1801133 and GGH rs11545078 genotypes can be used to predict increased MTX plasma concentrations and the occurrence of toxic reactions in high-dose MTX treatment of ALL, enabling timely interventions to enhance safety.
