1.Early-onset Alzheimer's disease comorbid with anti-N-methyl-D-aspartate receptor encephalitis: a case report

This article reports a case of a male patient with anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis comorbid with early-onset Alzheimer's disease (AD). The patient presented with depression, hallucinations and delusions syndrome, and cognitive impairment, and was admitted to a psychiatric hospital. Through multidisciplinary consultation, the diagnosis was confirmed based on positive biological and pathological biomarkers. The patient's symptoms improved after receiving a therapeutic regimen comprising high-dose glucocorticoid therapy and psychiatric-related treatments. Based on relevant current research findings domestically and internationally, this article reviews the pathogenesis and clinical manifestations of early-onset AD and anti-NMDAR encephalitis, in order to enhance the discrimination ability of psychiatric specialists for such patients and improve the diagnosis and treatment level. 2.Effect of Electroacupuncture on Metabolism-Inflammation Network in Patients with Simple Obesity:A Randomized Controlled Trial

ObjectiveTo observe the clinical efficacy of electroacupuncture in treatment of simple obesity patients and its effects on the metabolism-inflammation network. MethodsSeventy-two patients with simple obesity were randomly divided into a electroacupuncture group and a sham electroacupuncture group, with 36 patients in each group. In the electroacupuncture group, conventional acupuncture was applied at Zhongwan (CV 12), bilateral Tianshu (ST 25), bilateral Daheng (SP15), Qihai (CV 6), Guanyuan (CV 4), bilateral Zusanli (ST 36) and Fenglong (ST 40). The sham electroacupuncture group used placebo needle at same acupoints as the electroacupuncture group. Both groups were given sparse-dense waves with a frequency of 2/10 Hz, 30 minutes per session, 3 times a week, for a total of 8 weeks of treatment. The changes of human body parameters including body weight, body mass index (BMI), waist circumference, and body fat percentage (BFP), metabolic factors including serum fasting blood glucose (FBG), triglyceride (TG), total cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL), leptin, and ghrelin, and inflammatory factors including interleukin-6 (IL-6) and tumor necrosis factor α (TNF-α) were compared before and after treatment. Correlations between changes in leptin, ghrelin and inflammatory factors were further analyzed. ResultsWaist circumference was significantly reduced after treatment in both groups compared with baseline (P<0.05), and the post-treatment waist circumference was lower in the electroacupuncture group than in the sham electroacupuncture group (P<0.05); body weight, BMI and BFP decreased significantly after treatment compared to pretreatment levels in the electroacupuncture group (P<0.05). Serum leptin, IL-6 and TNF-α declined significantly after treatment in both groups versus baseline (P<0.05); the electroacupuncture group had lower post-treatment levels of leptin, IL-6 and TNF-α as well as larger pre-post changes than the sham group (P<0.05); ghrelin increased markedly after treatment in both groups (P<0.05), with higher post-treatment ghrelin and greater pre-post variation observed in the electroacupuncture group (P<0.05). In the electroacupuncture group, changes in ghrelin were negatively correlated with IL-6 and positively correlated with TNF-α (P<0.01); in the sham electroacupuncture group, ghrelin changes were negatively correlated with both IL-6 and TNF-α, whereas leptin changes were positively correlated with the two inflammatory markers (P<0.01). ConclusionElectroacupuncture can effectively improve clinical manifestations, as well as regulate serum metabolic and pro-inflammatory factors in patients with simple obesity, and discrepant correlation patterns of metabolism-inflammation are observed between the two groups.
3.Additional role of low-density lipoprotein cholesterol on the risk of osteoporosis in men with or without coronary heart disease: a real-world longitudinal study.

BACKGROUND:
Early control of low-density lipoprotein cholesterol (LDL-C) is crucial for reducing the progress of cardiovascular disease. However, its additional role to the risk of primary osteoporosis in men with coronary heart disease was inconclusive. Our study aims to determine the association of LDL-C and its trajectories for osteoporosis risk in the middle-aged and aged men of China.
METHODS:
The retrospective cohort study of 1546 men aged 69.74 ± 11.30 years conducted in Beijing, China from 2015 to 2022. And the incidence of primary osteoporosis was annually recorded. LDL-C trajectories were further identified by latent class growth model using repeated measurements of LDL-C. The association of baseline LDL-C for osteoporosis was estimated using hazard ratio (HR) with 95% CI in Cox proportional hazard model, while mean level and trajectories of LDL-C for osteoporosis were evaluated using odds ratio (OR) with 95% CI in logistic regression model.
RESULTS:
During the median 6.2-year follow-up period, 70 men developed primary osteoporosis. The higher level of baseline LDL-C (HR = 1.539, 95% CI: 1.012-2.342) and mean LDL-C (OR = 2.190, 95% CI: 1.443-3.324) were associated with higher risk of osteoporosis in men with coronary heart disease after adjusted for covariates. Compared with those in the LDL-C trajectory of low-stable decrease, participants with medium-fluctuant trajectory, whose longitudinal LDL-C started with a medium LDL-C level and appeared an increase and then decrease, were negatively associated with osteoporosis risk (OR = 2.451, 95% CI: 1.152-5.216). And participants with initially high LDL-C level and then a rapid decrease demonstrated a tendency towards reduced risk (OR = 0.718, 95% CI: 0.212-2.437).
CONCLUSIONS
Elevated LDL-C level and its long-term fluctuation may increase the risk of primary osteoporosis in men. Early controlling a stable level of LDL-C is also essential for bone health.

5.Clinical Characteristics and Prognostic Analysis of Newly Diagnosed Acute Myeloid Leukemia Patients with NRAS and KRAS Gene Mutations.

OBJECTIVE:
To retrospectively analyze the clinical characteristics, co-mutated genes in newly diagnosed acute myeloid leukemia (AML) patients with NRAS and KRAS gene mutations, and the impact of NRAS and KRAS mutations on prognosis.
METHODS:
The clinical data and next-generation sequencing results of 80 newly diagnosed AML patients treated at our hospital from December 2018 to December 2023 were collected. The clinical characteristics, co-mutated genes of NRAS and KRAS , and the impact of NRAS and KRAS mutations on prognosis in newly diagnosed AML patients were analyzed.
RESULTS:
Among 80 newly diagnosed AML patients, NRAS mutations were detected in 20 cases(25.0%), and KRAS mutations were detected in 9 cases(11.3%). NRAS mutations predominantly occurred at codons 12 and 13 of exon 2, as well as codon 61 of exon 3, while KRAS mutations were most commonly occurred at codons 12 and 13 of exon 2, all of which were missense mutations. There were no statistically significant differences observed in terms of age, sex, white blood cell count(WBC), hemoglobin(Hb), platelet count(PLT), bone marrow blasts, first induction chemotherapy regimen, CR1/CRi1 rates, chromosome karyotype, 2022 ELN risk classification and allogeneic hematopoietic stem cell transplantation(allo-HSCT) among the NRAS mutation group, KRAS mutation group and NRAS/KRAS wild-type group (P >0.05). KRAS mutations were significantly correlated with PTPN11 mutations (r =0.344), whereas no genes significantly associated with NRAS mutations were found. Survival analysis showed that compared to the NRAS/KRAS wild-type group, patients with NRAS mutation had a relatively higher 5-year overall survival (OS) rate and relapse-free survival (RFS) rate, though the differences were not statistically significant (P =0.097, P =0.249). Compared to the NRAS/KRAS wild-type group, patients with KRAS mutation had a lower 5-year OS rate and RFS rate, with no significant differences observed (P =0.275, P =0.442). There was no significant difference in the 5-year RFS rate between the KRAS mutation group and NRAS mutation group (P =0.157), but the 5-year OS rate of patients with KRAS mutation was significantly lower than that of patients with NRAS mutation (P =0.037).
CONCLUSION
In newly diagnosed AML patients, KRAS mutation was significantly correlated with PTPN11 mutation. Compared to patients with NRAS/KRAS wild-type, those with NRAS mutation showed a more favorable prognosis, while patients with KRAS mutation showed a poorer prognosis; however, these differences did not reach statistical significance. Notably, the prognosis of AML patients with KRAS mutation was significantly inferior compared to those with NRAS mutation.

Since the promulgation of the first Drug Administration Law of the People's Republic of China 40 years ago in 1984, China has undergone four main stages in the traditional Chinese medicine(TCM) regulation: the initial establishment of TCM regulation rules(1984-1997), the formation of a modern TCM regulatory system(1998-2014), the reform of the review and approval system for new TCM drugs(2015-2018), and the construction of a scientific regulation system for TCM(2019-2024). Over the past five years, a series of milestone achievements of TCM regulation in China have been achieved in the six aspects, including its strategic objectives and the establishment of a science-based regulatory system, the reform of the review and approval system for new TCM drugs, the optimization and improvement of the TCM standard system and its formation mechanism, comprehensive enhancement of regulatory capabilities for TCM safety, international harmonization of TCM regulation and its role in promoting innovation. Looking ahead, centered on advancing TCMRS to establish a sound regulatory framework tailored to the unique characteristics of TCM, TCM regulation will evolve into new reform patterns, advancing and extending across eight critical fronts, including the legal framework and policy architecture, the review and approval system for new TCM drugs, the quality standard and management system of TCM, the comprehensive quality & safety regulation and traceability system, the research and transformation system for TCMRS, AI-driven innovations in TCM regulation, the coordination between high-quality industrial development and high-level regulation, and the leadership in international cooperation and regulatory harmonization. In this way, a unique path for the development of modern TCM regulation with Chinese characteristics will be pioneered.

8.Determination of docusate sodium by ion-pair high-performance liquid chromatography

To reduce the dependency on high-carbon-load chromatographic columns,a new method has been established for the determination of the content of docusate sodium using ion-pair high-performance liquid chromatography(IP-HPLC).Tetrapropylammonium chloride was used as the ion-pair reagent with a mobile phase,composition of acetonitrile:10 mmol/L tetrapropylammonium chloride solution=66∶34,adjusting pH to 6.5 with 0.1%phosphoric acid solution,flow rate of 1.5 mL/min,detection wavelength of 214 nm,column temperature of 35℃,and an injection volume of 25 μL,and quantified by an external standard method.The main peak of docusate sodium exhibited a tailing factor of 1.34.The method showed good linearity within the range of 0.02 mg/mL to 0.40 mg/mL,with a correlation coefficient(r)of 0.999 9.It also demonstrated good repeatability,with recovery ranging from 97.0%to 98.2%(n=6).The quantification limit was 3.31 μg/mL,and the detection limit was 2.76 μg/mL.In summary,the new method shows good durability,a wide linear range,and high sensitivity,it is suitable for the determination of docusate sodium.
9.Research status and prospect of universal influenza vaccine

Influenza has high morbidity worldwide, affecting people of all ages, with the greatest impact on children and the elderly. Vaccination is one of the most cost-effective means to reduce the burden of influenza. However, due to the antigenic drift and shift of influenza viruses, the protection rate of the current seasonal influenza vaccines is not ideal, and the protection they provide against potential influenza pandemics is very limited. Therefore, it is of great significance to develop a universal influenza vaccine that has cross-immune protection against different subtypes of influenza virus. This article discusses several key factors of universal influenza vaccines and the current research progress, and prospects the future development trend of universal influenza vaccines.
10.Analysis of latent profiles and influencing factors of sleep in first-trimester pregnant women

BackgroundSleep disorder in the first trimester is a fairly common health problem, and previous studies have mainly reflected the overall sleep quality through scale assessments, which may not accurately capture the differences among various subtypes. ObjectiveTo explore the latent profiles of sleep quality in first-trimester pregnant women and identify the physiological, psychological and social factors, in order to provide practical references for the development of personalized interventions for sleep disorders in first-trimester pregnant women. MethodsA total of 1 066 first-trimester pregnant women who visited the obstetric outpatient clinic of a tertiary A hospital in Shenzhen from October 2021 to October 2022 were investigated using the general information questionnaire, Pittsburgh Sleep Quality Index (PSQI), Edinburgh Postnatal Depression Scale (EPDS), Chinese version of short International Physical Activity Questionnaire (IPAQ-S-C) and Social Capital Assessment Tool in Pregnancy for Maternal Health (SCAT-MH). Then the sleep profiles were identified through latent profile analysis, and the robust mixture regression model was employed to determine the influencing factors of sleep profiles. ResultsA 3-profile solution showed the best fit: 732 cases (68.67%) of good sleep quality group, 87 cases (8.16%) of low sleep efficiency group, and 247 cases (23.17%) of daytime dysfunction group. In comparison with subjects in good sleep quality group, first-trimester pregnant women in low sleep efficiency group were at a younger age (OR=0.951, 95% CI: 0.922~0.980), held a Bachelor's degree or above (OR=1.869, 95% CI: 1.260~2.773) and exhibited lower levels of social capital (OR=0.962, 95% CI: 0.951~0.973), while those in daytime dysfunction group were at an older age (OR=1.072, 95% CI: 1.027~1.120) and had higher levels of depression (OR=1.166, 95% CI: 1.115~1.218). Pregnant women who were workers (OR=0.507, 95% CI: 0.293~0.876) were less likely to report daytime dysfunction. ConclusionThree latent profiles with significant heterogeneity are derived from the sleep quality of first-trimester pregnant women, and levels of depression and social capital are the main influencing factors of sleep quality.
