1.Clinical Efficacy of Shenqi Yangxin Decoction in Treatment of Patients with Ischemic Cardiomyopathy and Its Effect on Serum H2S and Ca2+

ObjectiveTo evaluate the clinical efficacy of Shenqi Yangxin decoction in the treatment of ischemic cardiomyopathy (ICM) with Qi and Yin deficiency and blood stasis syndrome and its effect on serum hydrogen sulfide (H2S) and calcium ion (Ca2+). MethodsA total of 64 ICM patients with Qi and Yin deficiency and blood stasis syndrome who met the inclusion criteria were randomly divided into a control group (n=32) and a treatment group (n=32). All patients received conventional Western medicine treatment. The treatment group was additionally given Shenqi Yangxin decoction. The TCM syndrome score, Minnesota Living with Heart Failure Questionnaire (MLHFQ) score, left ventricular ejection fraction (LVEF), N-terminal pro-B-type natriuretic peptide (NT-proBNP), 6-minute walk test (6MWT), New York Heart Association (NYHA) cardiac function classification, and serum H2S and Ca2+ levels were compared between the two groups pre- and post-treatment. ResultsTwo cases dropped out from each group during the study. Finally, 30 patients in each group were included in the analysis. There were no significant differences in age, gender, course of coronary heart disease, underlying diseases, and laboratory tests between the two groups. Compared with baseline, the TCM syndrome score, MLHFQ score, and NT-proBNP in both treatment group and control group decreased significantly (P<0.01), LVEF, 6MWT, and H2S increased significantly (P<0.01), and serum Ca2+ increased (P<0.05). Compared with the control group after treatment, the MLHFQ score and NT-proBNP in the treatment group decreased (P<0.05), the TCM syndrome score decreased significantly (P<0.01), LVEF, 6MWT, and serum Ca2+ increased (P<0.05), and H2S increased significantly (P<0.01). The improvement degree of the NYHA cardiac function classification in the treatment group was higher than that in the control group, but there was no significant difference. ConclusionShenqi Yangxin decoction is effective in treating ICM patients with Qi and Yin deficiency and blood stasis, which could significantly improve cardiac function and quality of life, and its therapeutic effect may be related to the regulation of serum H2S and Ca2+ levels.
2.Study on the current status and pathway optimization of traditional Chinese medicine preparations record-filing management in medical institutions across different provinces

OBJECTIVE To provide scientific reference for perfecting the record-filing management policy of traditional Chinese medicine preparations formulated via traditional processes for medical applications (hereinafter referred to as “TCM preparations”) in medical institutions, and further promote the sound development of the research and record-filing work of such preparations. METHODS A literature review combined with cross-sectional research was carried out. We collected policy documents and preparation information related to the record-filing of TCM preparations issued by national and provincial drug administration authorities on official websites during 2018-2025. A comparative analysis was conducted on the implementation and revision of core policy documents for the registration management of TCM preparations across provinces, the scope of record-filing dosage forms, conditions for rejection of record-filing, and exemptions for registration materials. Statistical analysis on the quantity and dosage forms of record-filing preparations in each province was conducted by means of data induction and trend chart plotting, and targeted suggestions were put forward to optimize the management pathway of record-filing. RESULTS CONCLUSIONS There was obvious heterogeneity in record-filing policies among provinces. Twelve provinces refined the applicable dosage forms for record-filing, fifteen provinces supplemented conditions for rejection of record-filing (16 items in total), and nine provinces expanded the scope of material exemptions. The number of recorded preparations presented a striking regional imbalance:Qinghai had the largest quantity of 4 269 varieties, while Hainan only had 14 varieties, with a gap of more than 300 times. The recorded preparations were mainly traditional dosage forms including pills (6 928 varieties) and powders (4 360 varieties). Only 12 provinces formulated the provincial catalog for TCM preparations allocation. In most regions, preparation circulation was only permitted within medical consortia, leading to a low transformation rate of scientific and technological achievements of preparations. It is recommended to optimize the record-filing management pathway from three aspects: strengthening policy standardization while maintaining flexibility, streamlining record-filing procedures and documentary requirements, and reinforcing technical support and achievement transformation. These measures can reduce regional policy differences, improve work efficiency of record-filing, and advance the standardized development and clinical value transformation of hospital-made TCM preparations.
3.Effects of aerobic exercise on cognitive function in elderly patients with bipolar disorder

BackgroundBipolar disorder is a severe mental disorder characterized by alternating episodes of mania and depression. It has a high recurrence rate and a high rate of disability. Elderly patients often suffer from more severe cognitive impairment due to neurodegenerative changes. The clinical intervention for bipolar disorder mainly relies on drug treatment, but drugs have limited effects on improving cognitive function, and some drugs may even aggravate cognitive impairment. Aerobic exercise, as a non-drug intervention method, has shown positive effects in improving cognitive function in patients with mental disorders. However, there are still insufficient studies on elderly patients with bipolar disorder. ObjectiveTo explore the impact of aerobic exercise on the cognitive function of elderly patients with bipolar disorder, so as to provide references for the formulation of strategies to improve the cognitive function of this population. MethodsA randomized controlled trial was conducted. A total of 60 elderly patients who were hospitalized at the Third People's Hospital of Lanzhou from January 2024 to March 2025 and met the diagnostic criteria for bipolar disorder as defined in the International Classification of Diseases,tenth edition (ICD-10) were selected. They were divided into the study group and the control group, with 30 cases in each group, based on the random number table method. Both groups were given quetiapine fumarate tablets and magnesium valproate sustained-release tablets. The study group additionally received aerobic exercise (including slow walking, doing exercises, and running), three times a week, for 30 minutes each time. At the baseline and at the end of the 3rd, 6th, 9th, and 12th week of the intervention, the Hamilton Depression Scale -24 item (HAMD-24), the Young Manic Rating Scale (YMRS), the Montreal Cognitive Assessment (MoCA), and Wechsler Adult Intelligence Scale Revised in China (WMS-RC) were used for assessment. ResultsThe time effect of the HAMD-24 score was statistically significant (Wald χ2=124.564, P<0.01). The time effect of the YMRS score was also statistically significant (Wald χ2=145.608, P<0.01), and the interaction effect between time and group was statistically significant (Wald χ2=14.968, P<0.05). At the 3rd, 9th, and 12th week of the intervention, the YMRS score of the study group was lower than that of the control group in the same period, and the differences were statistically significant (P<0.05). The time effect and group effect of the MoCA score were both statistically significant (F=99.347, 16.623, P<0.01). The time effect, group effect, and interaction effect between time and group of the WMS-RC memory quotient were all statistically significant (F=64.678, 10.197, 12.472, P<0.01). At the 6th, 9th, and 12th weeks of the intervention, the MoCA score and WMS-RC memory quotient of the study group were higher than those of the control group, and the differences were statistically significant (P<0.05). ConclusionAerobic exercise may help improve the manic symptoms and cognitive functions (especially memory ability) of elderly patients with bipolar disorder. 5.Clinical effects of Supplemented Jiao'ai Decoction combined with warm acupuncture and moxibustion on patients with endometriosis due to Congealing Cold with Blood Stasis

AIM To investigate the clinical effects of Supplemented Jiao'ai Decoction combined with warm acupuncture and moxibustion on patients with endometriosis due to Congealing Cold with Blood Stasis.METHODS Eighty-six patients were randomly assigned into control group(43 cases)for 3-menstrual cycle intervention of warm acupuncture and moxibustion,and observation group(43 cases)for 3-menstrual cycle intervention of both Supplemented Jiao'ai Decoction and warm acupuncture and moxibustion.The changes in clinical effects,TCM syndrome scores,dysmenorrhea degree indices(VAS score,PGE2,PGF2α),hemodynamic indices(RI,PI),sexhormones(E2,FSH,LH),inflammatory factors(IL-1β,IL-6,TNF-α)and incidence of adverse reactions were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome scores,dysmenorrhea degree indices,hemodynamic indices,sexhormones and inflammatory factors(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with endometriosis due to Congealing Cold with Blood Stasis,Supplemented Jiao'ai Decoction combined with warm acupuncture and moxibustion can safely and effectively regulate sexhormone levels,endometrial hemodynamics,inhibit inflammatory responses,and alleviate dysmenorrhea symptoms.
6.Factors influencing the incidence and duration of delirium in ICU patients

The data collected included gender, age, anesthesia method, surgical duration, infection duration, extubation time, duration of mechanical ventilation and hypoxia, Acute Physiology and Chronic Health EvaluationⅡ (APACHEⅡ) score, Confusion Assessment Method for the Intensive Care Unit (CAM-ICU) score, duration of sedative medication, and ICU length of stay. Binary Logistic regression and multiple linear regression analysis were employed to investigate the factors influencing the incidence and duration of delirium in ICU patients.Results:The incidence of delirium among 1 500 ICU patients was 38.4% (576/1 500), with a duration of 1.10 (1.00, 3.20) d. Binary Logistic regression analysis revealed that infection duration, APACHEⅡ score, mechanical ventilation duration, and sedative medication duration were statistically significant factors influencing delirium incidence in ICU patients ( P<0.05). Multiple linear regression analysis revealed that delirium type, duration of hypoxia, use of sedative medications, and length of ICU stay were statistically significant factors influencing the duration of delirium in ICU patients ( P<0.05) . Conclusions:ICU patients have a relatively high incidence of delirium, and numerous factors influence the incidence and duration of delirium. Clinical nursing professionals should identify risk factors early and develop corresponding intervention measures to reduce the incidence of delirium in ICU patients and promote their recovery.

The number of patients with eye diseases in China is enormous,and the negative effects of these conditions,such as impaired visual function,psychological burdens,and restricted social participation,are becoming increasingly severe.Due to the limited and unevenly distributed ophthalmic resources,and the significant limitations of traditional diagnostic and therapeutic approaches in terms of accuracy and efficiency,there is an urgent need for more sensitive and efficient modalities.With the rapid advancement of artificial intelligence technology,ophthalmic diagnosis has entered a new stage of intelligent transformation.Facial photos,as a noninvasive and convenient medium,show unique advantages in eye disease diagnosis.Artificial intelligence systems based on facial photo analysis have been applied to the screening and diagnosis of conditions such as myopia,strabismus,ptosis,and thyroid eye disease,showing promising results.This review introduces the workflow of intelligent diagnosis for ocular diseases based on facial photographs,with a focus on recapitulating relevant research findings both domestically and internationally in recent years.It summarizes the innovative features and application advantages of intelligent diagnosis systems for eye diseases based on facial photos,analyzes the current technical bottlenecks and challenges in application,proposes corresponding countermeasures,and discusses future development directions,aiming to provide references and new insights for the intelligent screening and diagnosis of eye diseases.
9.Clinical characteristics of patients with pregnancy-related chronic pain visiting pain clinic

Objective:To analyze the clinical characteristics of patients with pregnancy-related chronic pain visiting the pain clinic.Methods:The number of pregnant patients who completed a pregnancy registration at the Obstetrics and Gynecology Hospital Affiliated to Nanjing Medical University from 2022 to 2024 was collected. The medical records were reviewed to identify the patients who visited the department of pain of our hospital due to chronic pain related to pregnancy. The clinical characteristics such as the visiting situation, gestational weeks, age and types of pain were analyzed.Results:From 2022 to 2024, the total number of registered pregnant patients was 64, 818, of which, 2, 224 cases visited the pain clinic, and the annual proportions of pregnancy-related chronic pain visits were 2.540%, 3.836% and 3.889% respectively. Among the patients who attended the clinic, 77.97% were pregnant (5.82% in early pregnancy, 41.93% in mid-pregnancy, and 52.25% in late pregnancy), and 21.03% were postpartum patients. A total of 83.72% were aged 20-34 yr. The types of pain were pelvic girdle pain (40.96%), limb joint pain (28.82%), low back pain (14.16%), cervical spondylosis (3.64%), peripheral nerve entrapment syndrome (3.42%), headache (2.97%), chest and back pain (2.79%), pelvic and perineal pain (1.66%), neuralgia (0.94%) and other pains (0.63%).Conclusions:From 2022 to 2024, the proportion of registered pregnant women at our hospital who visited to the pain clinic due to pregnancy-related chronic pain increases year by year. The common types of pain are pelvic girdle pain, limb joint pain and low back pain.
10.Neuroimaging Mechanism of The Modified Electro-convulsive Therapy on The Anti-depressive Effects and Cognitive Impairment

Modified electro-convulsive therapy (MECT) is one of the most potent treatments for major depressive disorder (MDD). However, it remains a second-line option due to significant side effects, such as transient memory loss. The relationship between therapeutic efficacy and cognitive impairment warrants further investigation to develop improved treatment regimens. In this review, we examine recent evidence from magnetic resonance imaging (MRI) studies aiming to identify structural and functional brain changes specifically associated with both the antidepressant effects and the amnesic outcomes of MECT. MECT induces widespread alterations across multiple brain systems. Increases in gray matter volume (GMV) have been observed in the prefrontal, temporal, and parietal cortices, as well as in subcortical regions such as the hippocampus (HP), amygdala, and striatum. Strengthening of myelination has also been reported along the dorsolateral prefrontal-limbic pathways. Functional changes include increased spontaneous neural activity in prefrontal areas, reorganization of intrinsic connectivity within the default mode network (DMN), and altered functional connectivity (FC) among the DMN, salience network (SN), and central executive network (CEN). Correlational studies have identified structural and functional alterations linked to antidepressant efficacy, including right hippocampal volume enlargement, prefrontal cortical thickening, reduced iron deposition in the striatum, decreased FC within certain DMN nodes, and enhanced effective connectivity from the dorsolateral prefrontal cortex (DLPFC) to the right angular gyrus. In contrast, the amnesic effects have been associated with increased volumes in the left hippocampus and bilateral dentate gyrus; enhanced FC in the left angular gyrus and left posterior cingulate cortex (PCC); increased FC between the right ventral anterior insula and DLPFC; and reduced FC in the left thalamus and bilateral precuneus. Changes in the hippocampus appear to correlate with both antidepressant efficacy and memory impairment. Clinical studies have found no significant correlation between the severity of memory impairment and the reduction in depressive symptoms, suggesting that the therapeutic and adverse effects may arise from distinct regional or subregional mechanisms. Supporting this hypothesis, recent findings show that increased right hippocampal volume is significantly associated with reduced depression scores, whereas increased volume in the left dentate gyrus correlates with declines in delayed recall performance. Additionally, enhanced connectivity between the anterior hippocampus and middle occipital gyrus (MOG) has been linked to mood improvement, while decreased FC between the mid-hippocampus and angular gyrus has been associated with impairments in memory integration. In conclusion, current evidence suggests that the antidepressant and memory-impairing effects of MECT may localize to distinct hippocampal subregions. These effects likely result from differential modulation of local neural activity and functional connectivity, leading to divergent behavioral outcomes. Given that both effects may originate in deep and spatially constrained structures such as the hippocampus, small-sample studies and conventional methodologies may fail to differentiate them effectively. Future research should employ large-scale, longitudinal designs utilizing high-field MRI and multimodal neuroimaging to characterize MECT-induced structure-function coupling in the hippocampus and its integration at the network level. Additionally, multiscale analyses spanning molecular, circuit, and network dimensions would be beneficial.
