Objective:To investigate the efficacy of volar anatomical locking plates combined with dorsal mini-plates in treating com-plex distal radius fractures. Methods:We retrospectively included patients with complex distal radius fractures who had been treated with volar locking plates and dorsal mini-plates at Chongqing General Hospital from January 2019 to June 2023.The quality of articu-lar surface fragment reduction was assessed through image analysis.Follow-up data included the Mayo wrist score,Disabilities of the Arm,Shoulder, and Hand Questionnaire(DASH)score, wrist range of motion, time to fracture healing, and complications.Results:Twenty-eight patients[19 males,9 females;mean age,(45.4±11.9)years]were included, with a mean follow-up of 1(4.9±3.6)months.Their injury mechanisms included falls from height(11 cases),traffic accidents(9 cases),and simple falls(8 cases). After op-eration,the volar tilt was(5.1±4.9)°,and the ulnar inclination was(19.8±2.8)°;at the final follow-up,the angles were(5.2±4.8)° and(19.3±2.5)°,respectively,neither showing any significant change(P>0.05).At the final follow-up,all fractures healed, with a mean healing time of 3.6 months. At 1 year after operation, the wrist extension was(50.7±6.3)°;the wrist flexion was(49.4±6.1)°;the DASH score was 15.0±6.3;the Mayo wrist scores were excellent or good in 24 cases;the grip strength of the affected side averaged 84.4%of that of the contralateral side.No implant loosening occurred. Two cases of superficial wound infection and one case of exten-sor tendon irritation were reported. Conclusion:Locked volar plat-ing combined with dorsal mini-plating provides stable fixation for comminuted articular surface fragments in complex distal radius fractures, enabling early functional rehabilitation with favorable wrist recovery and low rates of dorsal tendon irritation.

Objective To explore the relationship between serum levels of glycoprotein fibroglycan(SRGN)and secretory CD146(sCD146)and clinicopathological characteristics and prognosis in elderly pa-tients with breast cancer(BC).Methods A total of 128 elderly BC patients treated in the hospital from April 2019 to April 2021 were retrospectively selected as the BC group, and another 70 healthy elderly women were selected as the control group. The serum SRGN and sCD146 levels in BC group and control group, and patients with different clinical and pathological characteristics in BC group were compared.Kaplan-Meier curve and COX regression were used to analyze the effect of serum levels of SRGN and sCD146 on the prognosis of eld-erly BC patients.Receiver operating characteristic curve was used to analyze the predictive value of serum lev-els of SRGN and sCD146 for the prognosis of elderly BC patients. Results The levels of serum SRGN and sCD146 were(13.17±3.35)μg/L,(118.23±20.51)ng/L in the BC group, which were higher than(2.52±0.41)μg/L,(20.03±4.16)ng/L in the control group(t=26.460,39.572,both P<0.001).Compared with elderly BC patients with TNM stage Ⅰ-Ⅱ and histological grade Ⅰ-Ⅱ,serum SRGN and sCD146 levels in elderly BC patients with TNM stage Ⅲ and histological grade Ⅲ were higher(P<0.05).The 3-year progres-sion free survival rates in SRGN high and low expression groups were 66.13%(41/62)and 92.42%(61/66),respectively,and the 3-year progression free survival rate in SRGN high expression group was lower than that in SRGN low expression group(Log Rank χ2=14.180,P<0.001).The 3-year progression free survival rates in sCD146 high and low expression groups were 68.85%(42/61)and 89.55%(60/67),respectively,and the 3-year progression free survival rate in sCD146 high expression group was lower than that in sCD146 low ex-pression group(Log Rank χ2=8.614,P=0.003).TNM stage Ⅲ,histological grade Ⅲ,serum SRGN and sCD146 were risk factors for the poor prognosis of elderly BC patients. The area under the curve(AUC)of the combination of serum SRGN and sCD146 for predicting the prognosis of elderly BC patients was 0.850,which was larger than that of SRGN(AUC=0.798)and sCD146(AUC=0.771)alone(Z=2.057,2.217,P=0.042,0.029).Conclusion Serum levels of SRGN and sCD146 are elevated in elderly BC patients, and both are in-volved in the disease progression of BC.The combination of the two factors has high predictive value for the prognosis of elderly BC patients.

Objective To discuss the effects of prophylactic chemotherapy on the outcomes and prognosis of invasive mole patients. Methods One hundred and fifteen invasive mole (IM) patients older than 40 years were registered in Peking Union Medical Collage Hospital.Eleven of them were treated with prophylactic chemotherapy before diagnosed as IM prophylactic chemotherapy group, while the other 104 cases received therapeutic chemotherapy after diagnosed as IM (non-prophylactic chemotherapy group). The general clinical data (including age, clinical stage, risk factor score),treatment, outcomes and relapse of patients were retrospectively compared between two groups. Results (1) The age of prophylactic chemotherapy group and non-prophylactic chemotherapy group were (47±5) versus (46±4) years old.Ratio of clinical stage Ⅰ-Ⅱ were 3/11 versus 29.8% (31/104),clinical stage Ⅲ-Ⅳ were 8/11 versus 70.2% (73/104).Ratio of risk factor score 0-6 were 11/11 versus 84.6% (88/104),risk factor score >6 were 0 versus 15.4%(16/104).There were no significant statistical differences between two groups in age, clinical stage or risk factor score (all P>0.05).(2) Treatment:the total chemotherapy courses between prophylactic chemotherapy group and non-prophylactic chemotherapy group (median 7 versus 5) were significantly different (Z=3.071,P=0.002).There were no significant statistical differences between two groups in the chemotherapy courses until negative conversion of β-hCG,consolidation chemotherapy courses, total therapeutic chemotherapy courses or ratio of hysterectomy (all P>0.05).(3) Outcomes and relapse:between the prophylactic chemotherapy group and the non-prophylactic chemotherapy group, the complete remission rate were 11/11 versus 98.1%(102/104),the relapse rate were 0 versus 1.0%(1/102).There were no significant difference between the two groups in outcomes or relapse rate (P>0.05).Conclusions Prophylactic chemotherapy does not substantially benefit the IM patients older than 40 years.Prophylactic chemotherapy may not significantly improve patients' prognosis,in which increased sample size is required in further study.
