1.Impact of Postoperative Reduction Quality on Biomechanics of the Femoral Head Following Internal Fixation of Femoral Neck Fractures

Objective To investigate the effect of postoperative reduction quality in femoral neck fracture internal fixation on mechanical properties of the femoral head from the perspective of trabecular bone biomechanics.Methods From patients who underwent hip replacement surgery for femoral neck fractures,a total of 26 femoral head slice specimens were obtained.The central axis of the primary compressive trabeculae was defined as the 0° group,with the intersection point of the primary compressive trabeculae and the femoral calcar serving as the center.By rotating the specimens to simulate different reduction angles,the cut femoral head slice specimens were randomly divided into five groups:-10°,-5°,0°,5°,and 10°,representing femoral heads with varying reduction qualities.The specimens were subjected to single compression load tests and fatigue load tests.The load was set from 70 N to 1 400 N,at a frequency of 1 Hz,with 10 000 cycles.Axial stiffness,displacement,and the number of collapse cycles were measured,to compare the biomechanical properties of femoral head specimens under different reduction qualities.Results There were differences in the axial stiffness,displacement,and number of collapse cycles among the femoral head specimens in different groups.Under 800 N load,the axial stiffness of 0° group was significantly greater than that of±10° groups(P<0.05).The axial stiffness of 0° group was also greater than that of the±5° groups,but the differences were not statistically significant(P>0.05).The axial stiffness of±5° groups was greater than that of±10° groups(P<0.05).0° group had a lower displacement than±5° groups and±10° groups.However,the differences in displacement between 0° group and±5° groups were not statistically significant(P>0.05),while the differences between the 0° group and±10° groups were statistically significant(P<0.05).The differences in displacement between±5° groups and±10° groups were also statistically significant(P<0.05).0° group had a significantly higher number of collapse cycles than±10° groups(P<0.05).The number of collapse cycles in 0° group was also higher than that in±5° groups,but the differences were not statistically significant(P>0.05).The number of collapse cycles in±5° groups was significantly higher than that±10° groups(P<0.05).Conclusions The quality of reduction after internal fixation of femoral neck fractures significantly affects the biomechanical properties of the femoral head.This study provides a scientific basis for optimizing treatment and postoperative management,aiming to improve clinical outcomes and patients' quality of life.

3.Application of egg yolk oil combined with Qingrejiedu decoction for external washing to promote wound healing after perianal abscess surgery

Objective To explore the promoting effect of egg yolk oil combined with heat-clearing and detoxi-fying traditional Chinese medicine external washing on postoperative wound healing in patients with perianal abscesses based on growth factors in the wound granu-lation tissue.Methods Eighty patients with perianal abscesses admitted to our hospital from January 2023 to May 2023 were randomly divided into a control group and an observation group,with 40 patients in each group.Three days after surgery,the control group received routine treatment,such as external washing with heat-clearing and detoxifying Chinese herbal medicine and dressing changes with vase-line gauze.The observation group received additional treatment with egg yolk oil,and both groups were treated continuously for 7 days.The clinical efficacy,improvement in clinical symptoms,and wound recovery at different time points;serum inflammatory cytokines;and expression levels of wound granulation tissue-related factors were compared between the two groups.Results The total effective rate was higher in the observation group than the control group(95%vs.75%,P<0.05).The time of pruritus disappearance,decaying flesh,emer-gence of new epithelium,and healing were shorter in the observation group than in the control group(P<0.05).The pain degree,wound secretion,granulation morphology and surrounding tissue edema score of the observation group were significantly decreased at 7 and 10 days after surgery compared with the control group(P<0.05).Serum levels of interleukin(IL)-1β,IL-8,and tumor necrosis factor-α were lower in the observation group than the control group at 7 and 10 days after surgery(P<0.05).The levels of basic fibroblast growth factor,vascular endothelial cell growth factor,and transforming growth factor-βin wound granulation tissue were higher in the observation group than the control group at 7 and 10 days after surgery(P<0.05).Conclusion The external application of egg yolk oil combined with heat-clearing and detoxifying Chinese herbs can significantly improve the clinical symptoms of patients with perianal abscesses and promote wound healing,with significant therapeutic effects.This may be related to the inhibition of inflammatory cytokine expression and the promotion of wound granulation tissue growth factor expression.
4.Diagnostic Value of Serum FGF22 and CXCL16 Levels in Patients with Parkinson's Disease for Cognitive Impairment

Objective To explore the diagnostic value of serum levels of fibroblast growth factor 22(FGF22)and CXC chemokine ligand 16(CXCL16)in patients with Parkinson's disease(PD)in cognitive impairment.Methods A total of 125 PD patients admitted to Handan Traditional Chinese Medicine Hospital from June 2022 to June 2023 were selected as the research subjects,and they were separated into a non cognitive impairment group(n=38)and a cognitive impairment group(n=87)based on whether they had cognitive impairment.Enzyme-linked immunosorbent assay(ELISA)was applied to determine the levels of serum FGF22 and CXCL16 in each group.Spearman correlation was applied to analyze the correlation among serum FGF22 and CXCL16 expression levels,Unified Parkinson's Disease Rating Scale(UPDRS)and Montreal Cognitive Assessment(MoCA)scores.Multivariate logistic regression was applied to analyze the factors affecting cognitive impairment in PD patients.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic efficacy of FGF22 and CXCL16 for cognitive impairment in PD patients.Results Compared with the non cognitive impairment group,the expression level of serum FGF22(184.16±14.62ng/ml vs 203.24±12.15ng/ml)in cognitive impairment group and the MoCA score(23.91±3.14 分 vs 26.54±2.31 分)were decreased(t=7.048,4.460,all P<0.05),while the expression levels of CXCL16(2.59±0.46ng/ml vs 2.06±0.34ng/ml)and the UPDRS score(41.43±5.62 score vs 32.46±4.28 score)were increased(t=6.376,8.782,all P<0.05),and the differences were statisaically significant.According to Spearman correlation analysis,the expression level of serum FGF22 was negatively correlated with UPDRS score(r=-0.435,P<0.05),but positively correlated with MoCA score(r=0.742,P<0.05).The expression level of CXCL16 in serum was positively correlated with UPDRS score(r=0.532,P<0.05),but negatively correlated with MoCA score(r=-0.623,P<0.05).Multivariate logistic analysis showed that FGF22 and MoCA scores were protective factors for cognitive impairment in PD patients(all P<0.05),while CXCL16 and UPDRS scores were risk factors for cognitive impairment in PD patients(all P<0.05).The combination of serum FGF22 and CXCL16 in the diagnosis of cognitive impairment in PD patients had better AUC than their respective alone diagnoses(Z=2.919,2.437,P=0.003,0.015),with a sensitivity and a specificity of 93.10%and 73.68%,respectively.Conclusion The levels of serum FGF22 and CXCL16 were closely related to cognitive impairment in PD patients,and the combination of the two could better diagnose whether PD patients have cognitive impairment.
5.Causes and treatments for congestion after pig-tail catheter drainage of liver abscess

Objective To investigate the causes and treatments for the congestion after pig-tail catheter drainage of liver abscess.Methods Clinical data of 3 patients with complicated liver abscess admitted to Tangdu Hospital of Air Force Military Medical University from April 2016 to January 2017 were retrospectively analyzed.The informed consents of all patients were obtained and the local ethical committee approval was received.Case 1,male,aged 84 years;Case 2,female,aged 57 years;Case 3,female,50 years.The pig-tail catheter was clogged after being placed in 3 patients for drainage and the drainage catheters were therefore rotated clockwise or counter clockwise.Results After multiple rotations,the head end of drainage catheter was loosened and unclogged and necrotic tissues was drained out.Mild or moderate fever was observed in cases 1 and 3 after rotating the pig-tail catheter,which was relieved spontaneously.3 patients presented with mild bleeding after rotating the catheter and no significant change was noted in Hb.After the drainage catheter was unobstructed,the fever was relieved and all were cured and discharged.No recurrence was observed at postoperative 1 year.Conclusions For the intractable congestion of pig-tail catheter in liver abscess patients,rotating and loosening the catheter can be a good appoach.
6.Survival of BCLC-B hepatocellular carcinoma patients after liver resection combined with microwave coagulation therapy vs transarterial chemoembolization

Objective To explore the prognosis of patients with BCLC-B hepatocellular carcinoma (HCC) receiving LR + MCT vs TACE.Methods BCLC-B HCC patients undergoing LR + MCT (107 cases) and TACE (65 cases) were retrospectively evaluated,in terms of 30-day mortality rate,the median survival,early and long term survival rate and complications.Results The hospital mortality rate was similar between the two groups (x2 =0.285,P =0.593).However,the LR + MCT group were with a significantly higher postoperative complication rate than the TACE group (x2 =4.694,P =0.03).At the same time,the LR + MCT group had significantly higher median Survival rates (x2 =9.135,P =0.003),but,there was no statistical difference for five-year survival (x2 =4.767,P =0.434).Conclusions LR + MCT are as safe as TACE for patients with BCLC-B HCC,and it provides longer median survival rates.
