1.Construction and empirical study of selection system for drug directory of county-level medical community based on multi-criteria decision analysis

OBJECTIVE To explore the construction of selection system for drug directory of the county-level medical community based on multi-criteria decision analysis, and provide decision-making basis for the selection of drug directory of medical community. METHODS Taking county-level medical community in Chongqing as an example,Delphi method and analytic hierarchy process were employed to construct the selection system for drug directory of the county-level medical community. Selected drugs were quantitatively scored based on the constructed index system, and the drug directory was selected according to the drug’s comprehensive score. The implementation effect of the directory was then evaluated through questionnaire surveys one year after the implementation of the directory. RESULTS The expert authority coefficients of the two rounds of consultation were> 0.8, with Kendall’s W values of 0.213 and 0.196, respectively (P<0.001). Finally, the selection system for drug directory of the medical community was determined to include five evaluation dimensions: safety, effectiveness, economy, accessibility, and innovation, along with eight evaluation indicators. In the drug directory selected according to the above method, the proportions of centrally procured drugs, medical insurance drugs, and essential drugs had all increased compared to before the selection; the comprehensive scores of chemical drugs ranged from 50.25 to 96.31 scores, and the proportion of drugs scoring between 70 and 100 scores had increased from 78.06% before selection to 85.82%. Among them, antiparasitic drugs had the highest comprehensive scores, while drugs for the digestive tract and metabolism were the most numerous. The evaluation scores of each indicator and the comprehensive scores of drugs in the drug directory after the selection process increased significantly than before selection (P< 0.05). CONCLUSIONS The selection system for drug directory of the county-level medical community constructed in this study is scientific, objective and operable. This process facilitates the promotion of standardized and unified management of drugs in the medical community.
2.The role of brevican regulation in the antidepressant effects of electroacupuncture in a chronic stress rat model

Objective:
To investigate the mechanism of electroacupuncture (EA) for treating depression and to explore the role of brevican in the medial prefrontal cortex (mPFC) in modulating stress susceptibility and the antidepressant effects of EA in rats.
Methods:
Twenty-four Sprague–Dawley (SD) rats were equally divided into three groups: green fluorescent protein (GFP) + control, GFP + chronic unpredicted mild stress (CUMS), and short-hairpin RNA targeting on brevican (shBcan) + CUMS. Another 24 SD rats were equally divided into CUMS + GFP, CUMS + GFP + EA, and CUMS + shBcan + EA groups. Behavioral tests were conducted to assess depression-like behavior. Western blot analysis was used to evaluate the expression of brevican, aggrecan, GLuA1, and PSD95 in mPFC subregions.
Results:
Behavioral parameter evaluation show that rats in the shBcan + CUMS group exhibited a significantly reduced sucrose preference (P = .0002) and increased immobility time (P = .0011) compared to those in rats in the GFP + CUMS group. Western blotting showed that brevican expression was significantly downregulated in the PrL of the shBcan + CUMS group compared with that in the GFP + CUMS group (P = .0192). Furthermore, compared to the CUMS + GFP + EA group, the CUMS + shBcan + EA group exhibited a significantly decreased sucrose preference (P = .0334), increased immobility time (P = .0465), and increased latency to food (P = .0261). In the CUMS + shBcan + EA group, the EA-induced brevican and PSD95 overexpression was reversed, compared with that in the CUMS + GFP + EA group (P = .0454 and P = .0198, respectively).
Conclusion
EA exerts its antidepressant effects through the modulation of brevican expression in rats. Our findings highlight the important role for brevican in stress susceptibility, which could be a potential target for treating depression.
3.Research progress of astrocyte phagocytosis in Alzheimer's disease

Astrocytes are heavily activated in Alzheimer's disease,engulfing damaged synapses,Aβ proteins,Tau proteins,apoptotic cells and other substrates.However,these substrates are difficult to degrade,accumulate as the disease progresses,and impair the phagocytosis of astrocytes.During phagocytosis,astrocytes recognize different substrates through a variety of phagocytosis receptors and partially degrade the substrates through degrad-ing enzymes and lysosomal pathways.The accumulation of Aβ and Tau proteins in astrocytes caused astrocyte im-mune and metabolic disorders,and Aβ toxicity changed after phagocytosis.In addition,astrocytes and microglia form a complementary pattern and cooperate to complete phagocytosis through interaction.Regulating the pathway of astrocyte phagocytosis and degradation is believed to be a potential novo therapeutic for clinical treatment of Alzheimer's disease.
4.Related risk factors analysis of pancreatic fistula after radical resection of gastric cancer and establishment of risk prediction scoring model

Objective:To investigate the risk factors of pancreatic fistula after radical resection of gastric cancer, and to establish a risk prediction scoring model for pancreatic fistula.Methods:The clinico-pathological data of 312 patients with gastric cancer admitted to the Fourth Hospital of Hebei Medical University from January 2019 to January 2020 were retrospectively analyzed. Multiple factor logistic regression model was used to analyze the risk factors of pancreatic fistula after radical resection of gastric cancer, and a risk prediction scoring model based on the risk factors was established. Hosmer-Lemeshow test was used to detect the goodness of fit of regression equation, and receiver operating characteristics (ROC) curve was used to evaluate the distinction degree of regression equation.Results:Among 312 patients with gastric cancer, 27 cases (8.65%) had pancreatic fistula after radical resection of gastric cancer. Multiple factor logistic regression analysis showed that male patients ( OR = 5.312, 95% CI 1.532-18.420, P = 0.008), age ≥ 60 years old ( OR = 4.928, 95% CI 1.493-16.250, P = 0.009), preoperative diabetes mellitus ( OR = 3.062, 95% CI 1.091-8.589, P = 0.034), lesion location in the gastric body-gastric antrum ( OR = 3.121, 95% CI 1.052-9.251, P = 0.040), intraoperative omental bursa resection ( OR = 6.209, 95% CI 2.084-18.478, P = 0.001), intraoperative lymph node dissection at D2+ station ( OR = 3.114, 95% CI 1.044-9.281, P = 0.042), intraoperative combined organ resection ( OR = 5.063, 95% CI 1.473-17.400, P = 0.010), preoperative TNM stage Ⅲ ( OR = 4.973, 95% CI 1.189-20.792, P = 0.028) were independent risk factors for pancreatic fistula after radical resection of gastric cancer. A risk prediction equation of pancreatic fistula after radical resection of patients with gastric cancer was established: P = -8.619+1.670X 1+1.595X 2+1.119X 3+1.138X 4+1.826X 5+1.136X 6+1.622X 7+1.604X 8; factor X was set as a binomial assignment (0 or 1); X1-X8 were listed as follows respectively: gender (the male was 1), age (≥60 years old was 1), preoperative diabetes history (yes was 1), lesion location (gastric body-gastric antrum was 1), intraoperative resection of omental bursa or not (yes was 1), intraoperative lymph node dissection at D2+ station or not (yes was 1), intraoperative combined organ resection or not (yes was 1), preoperative TNM stage (stage Ⅲ was 1). The goodness of fit of regression equation was high ( P = 0.395). The area under the curve of ROC by using risk prediction scoring model to judge pancreatic fistula was 0.916 (95% CI 0.872-0.960, P<0.01). The probability of pancreatic fistula in patients with score ≥ 5 was 40.90%, and the probability of pancreatic fistula in patients with score < 5 was 3.35%. Conclusions:The occurrence of pancreatic fistula after radical resection of gastric cancer is closely related to a variety of risk factors. By establishing a risk prediction scoring model for pancreatic fistula after radical resection of gastric cancer, it is helpful to effectively identify patients with high risk of pancreatic fistula after radical surgery during the perioperative period.
5.Effectiveness of sexuality education curriculum to improve the attitude towards homosexuality in primary school

Objective:
To analyze the influence of sexuality education on primary school students’ attitude towards homosexuality, and provide practical teaching evidence of intervening campus bullying based on sexual orientation.
Methods:
A primary school offering school-based sexuality education curriculums in Beijing was selected as an experimental school, another school with similar location, nature and scale, quality of teachers and teaching environment was selected as a control school. The graduates in experimental school received 6 years of sexuality education in grade 1 to grade 6 of primary school. A survey was conducted in Jun. 2018 among all the graduates in grade 6 of the two schools through an attitude scale on homosexuality adopting cluster sampling.
Results:
The scores of attitude towards homosexuality in experimental school and control school were (42.85±15.46) and (60.17±12.54) respectively, with significantly statistical difference(t=-9.47, P<0.01). There were interaction effects on attitudes of primary school students towards homosexuality between gender and experience of sexuality education (P<0.05). There was no significant difference in attitude towards homosexuality between male and female students in control school(t=0.58, P>0.05), but in experimental school, the attitude scores of male and female students respectively were (47.64±14.31) and (38.13±15.19), with significantly statistical difference(t=3.47, P<0.01). There was no significant difference in attitude towards different sexual orientation between experimental school students and control school students(t=0.10,1.53,P>0.05).
Conclusion
Sexuality education curriculum can help improve attitude towards homosexuality of primary school students, which has better intervention effects on female students than male students, attitudinal difference in gay and lesbian has not formed in primary school students.
6.A biomechanical study of patellar tension plating system for patellar fractures

Objective To investigate biomechanical properties of the patella tension plating system in order to provide a theoretical basis for its clinical application. Methods Thirty-six models of artificial patella were randomly divided into 3 groups ( n=12 ) . After transverse patellar fractures were created in the models, the 3 groups were subjected to fixation respectively with Kirschner wire tension band ( tension band group ) , patellar concentrator ( concentrator group ) and patellar tension plate ( tension plate group ) . Next, 6 specimens from each group were placed on a mechanical testing machine to measure the fracture displacements after 100 cycles of simulated knee flexion and extension movements. Tensile strength tests were performed on the remaining 6 specimens in each group to measure the maximum load at fixation failure. Results The fracture displacement in the tension plate group ( 0. 40 ± 0. 26 mm ) was significantly smaller than those in the tension band group ( 2. 58 ± 0. 72 mm ) and in the concentrator group ( 1. 25 ± 0. 74 mm ) ( P < 0. 05 );the maximum load at fixation failure in the tension plate group ( 1 , 709 ± 206 N ) was significantly greater than those in the tension band group ( 581 ± 122 N ) and in the concentrator group ( 1, 003 ± 211 N ) ( P <0. 05 ) . Conclusion As a new treatment for patellar fractures, the patellar tension plating system can perform better in biomechanical properties than Kirschner wire tension band and patellar concentrator.
7.A comparative analysis of the changes of transcriptome after fusion of esophageal cancer cells with human umbilical mesenchymal stem cells.

OBJECTIVETo compare the transcriptome of esophageal cancer cells (EC9706), human mesenchymal stem cells (MSCs), and after fusion of esophageal cancer cells with MSCs, and to further study their different expression profiles and the changes of their signaling pathways.
METHODSWe examined the gene expression profiles of these cells with transcriptome microarray using LIMMA package and several web-based applications, such as DAVID, ToppGene and MSigDB. The resulting sets of differentially expressed genes (DEGs) were comprehensively analyzed to identify the pathways and their changes after the cell fusion.
RESULTSA total of 4 548 significantly DEGs among the three cell lines were found by LIMMA. Three functional annotation web tools predicted that DNA damage repair, cell cycle arrest and apoptosis pathways were enriched. Total DEGs were mapped to the canonic pathways with KEGGanim which depicted that the core genes of DNA damage repair, cell cycle arrest and pro-apoptosis were up-regulated in fusion cells, and they mightbe combined to respond the fusion-induced damage stress. The up-regulation of suppressive factor DUSP6 might feedback inhibit the MAPK signaling pathway in the fusion cells, too.
CONCLUSIONSTranscriptome analysis suggests that hMSCs and EC9706 cell fusion may inhibit growth of EC cells by induction of pro-apoptotic signaling and DUSP6 negative feedback inhibition mechanism. In addition, the changes of immune regulation-related and differentiation-related genes indicate that the fusion cells inherited certain immune-suppressive function from the stem cells.

9.Effect of chest wall radiotherapy in different manners using tissue equivalent bolus on skin and lung of cavia cobayas

Objective To probe the influence of electron beam radiotherapy in different manners using different tissue equivalent boluses on skin and lung.Methods Adult female cavia cobayas were randomly divided into four groups as control group,fuU-time with bolus group,half-time with bolus group and without bolus group.Acute-irradiation animal models were established using electron beam in different manners with or without 0.5 cm tissue equivalent bolus.Pathological changes in lung,hair vesicle and fibroblast cell count were analyzed 40 clays after irradiation.Results The radiation dermatitis in the group with bolus was slighter than that of the group without bolus,but the radiation pneumonia was reverse.With bolus,the radiation dermatitis of haft-time group was slighter than that of full-time group.The injury repair of half-time group was more active than full-time group.Conclusions The treatment of haft-time bolus could protect lung without serious skin complications.
10.Research of gene chip detection and gene expression profile of nasal polyps.

OBJECTIVE:
To investigate the gene expression profile by using gene chip technology and probe into the role of corresponding gene in the pathogenesis of nasal polyps by analysing the difference of the gene depression.
METHOD:
The total RNAs were respectively extracted from 6 pairs of inferior turbinates and nasal polyps, and then were reversely transcribed to cDNAs with incorporation of fluorescent dUTP as the hybridization probes. The mixed probes were then hybridized with the BiostarH-40 s gene chips, it was scanned by laser scanner and the acquired image was analyzed by software.
RESULT:
1887 genes were differently expressed in gene profile of nasal polyps, among which 1099 were upregulated and 788 were down-regulated. Six genes were found in all gene chips, among which 4 genes were upregulated and 2 were down-regulated. The 6 genes encoded the protein of the transmembrane 4 superfamily, highly similar to GAMMA-interferon-inducible protein IP-30 precursor, highly similar to complement factor I precursor and insulin-like growth factor binding protein 3 (IGFBP3).
CONCLUSION
Detecting the differently expressed genes will provide clues and theoretical foundation for the pathogenesis of nasal polyps. The nasal polyps is a polygenic disease and the genes of GAMMA-interferon-inducible protein, insulin-like growth factor binding protein may play an important role in its pathogenesis.
