1.Research on RULA, REBA and OWAS based exposure risk assessment methods

Objective:To accurately identify occupational health and safety hazards in different industries with distinct characteristics, a comprehensive occupational ergonomics assessment scale was developed by integrating the Rapid Upper Limb Assessment (RULA) evaluation framework, the Rapid Entire Body Assessment (REBA) and the Ovako Working Posture Analysing System (OWAS) .Methods:Between May 2023 and May 2024, based on the proportional reasoning method and segmented function method, the algorithm logic of RULA and REBA was aligned, and the evaluation rules for the revision content of the innovation scale were inferred using the method of proportion reasoning. Twenty postures were randomly selected for correlation verification.Results:The results showed that at a significance level of α=0.01, the normalized score of the comprehensive ergonomic assessment scale had a correlation coefficient of r=0.845 with RULA and r=0.633 with REBA. The behavioral level score of the comprehensive ergonomic assessment scale had a correlation coefficient of r=0.911 with RULA and r=0.737 with REBA, indicating significant correlations. Conclusion:The innovative comprehensive ergonomic assessment scale has high reliability and can be used for work activities in different workplaces and work contents, promoting the standardization of ergonomic assessment of occupational exposure hazards.

3.Patients with breath test positive are necessary to be identified from irritable bowel syndrome: a clinical trial based on microbiomics and rifaximin sensitivity

Background::As a non-invasive and effective diagnostic method for small intestinal bacterial overgrowth (SIBO), wild-use of breath test (BT) has demonstrated a high comorbidity rate in patients with diarrhea-predominant irritable bowel syndrome (IBS-D) and SIBO. Patients overlapping with SIBO respond better to rifaximin therapy than those with IBS-D only. Gut microbiota plays a critical role in both of these two diseases. We aimed to determine the microbial difference between IBS-D overlapping with/without SIBO, and to study the underlying mechanism of its sensitivity to rifaximin.Methods::Patients with IBS-D were categorized as BT-negative (IBSN) and BT-positive (IBSP). Healthy volunteers (BT-negative) were enrolled as healthy control. The patients were clinically evaluated before and after rifaximin treatment (0.4 g bid, 4 weeks). Blood, intestine, and stool samples were collected for cytokine assessment and gut microbial analyses.Results::Clinical complaints and microbial abundance were significantly higher in IBSP than in IBSN. In contrast, severe systemic inflammation and more active bacterial invasion function that were associated with enrichment of opportunistic pathogens were seen in IBSN. The symptoms of IBSP patients were relieved in different degrees after therapy, but the symptoms of IBSN rarely changed. We also found that the presence of IBSN-enriched genera ( Enterobacter and Enterococcus) are unaffected by rifaximin therapy. Conclusions::IBS-D patients overlapping with SIBO showed noticeably different fecal microbial composition and function compared with IBS-D only. The better response to rifaximin in those comorbid patients might associate with their different gut microbiota, which suggests that BT is necessary before IBS-D diagnosis and use of rifaximin.Registration::Chinese Clinical Trial Registry, ChiCTR1800017911.
4.Effect of complete nursing order guidelines on improving the nursing quality in ICU

Objective To discuss the effect of the complete nursing order and guidelines on improving the nursing quality in ICU .Methods The nursing quality , patients ’ satisfaction and the hierarchical training examination scores before and after using the improved nursing order guidelines were compared , from 2012 to 2013 .Totals of 177 patients in 2012 were treated with conventional nursing order system , and 183 patients in 2013 were treated with the improved nursing order guidelines .Results In 2012, totals of 177 patients with 3 566 times nursing observation were included , and 183 patients, with 3 634 times nursing observation were included in 2013.In 2013, oral care failure rate was 0.28%, which was significantly lower than 1.18% in 2012, with a significant difference(χ2 =20.45, P=0.00).In 2013, the bed elevation unqualified rate was 0.33%, which was significantly lower than 1.54% in 2012, with a significant difference (χ2 =28.69,P=0.00).In 2013, the artificial airway ballonet pressure implementation rate was 99.56%,which was significantly higher than 98. 65%in 2012, with a significant difference (χ2 =16.76,P=0.00).In 2013, the patients’ satisfaction scores was (98.08 ±0.67), which was significantly higher than (94.58 ±2.47) in 2012, with a significant difference (t=4.91,P =0.00).The nurse training assessment scores was (89.33 ±9.64) in 2013, which was significantly higher than ( 82.17 ±4.04 ), with a significant difference ( t =2.62, P =0.02 ). Conclusions Nursing order guidelines applied in ICU bedside can promote the improvement of the nursing quality, ensure the safety of nursing care , and let patients receive continuous and homogeneous care .Improved nursing order guidelines not only effectively improve the whole nursing level of ICU , but also provide diversified platform for nurse training at all levels .
