1.Neuroprotective mechanism of autophagy in bradykinin postconditioning after cardiac arrest in rats

Objective:To explore the protective effects of bradykinin postconditioning on cardiopulmonary resuscitation (CPR) rats, and to assess the underlying mechanisms.Methods:Forty-eight adult male Sprague-Dawley (SD) rats were randomly divided into four groups according to random number table: Sham operation group, cardiac arrest (CA) group, bradykinin treatment (BK) group, and AMP-activated protein kinase (AMPK) inhibitor Compound C+ bradykinin treatment (CP+BK) group, finally, 8 rats in each group were taken for follow-up experiment. CA was induced by asphyxia. Rats in the Sham group received arteriovenous catheterization, endotracheal intubation, and mechanical ventilation, without CA. Compound C (250 μg/kg) was intraperitoneally injected in CP+BK group 30 minutes before CA, and the same volume of dimethyl sulfoxide (DMSO) was given in the remaining groups. Bradykinin (150 μg/kg) was intraperitoneally injected in BK group and CP+BK group 48 hours after restoration of spontaneous circulation (ROSC), and same volume of saline was given in the remaining groups. The neural function of rats in each group was evaluated with neurological deficit score (NDS) 72 hours after ROSC. Microtubule-associated protein light chain 3 (LC3) and p62 expressions were detected by immunohistochemistry, autophagosomes were observed by transmission electron microscopy, and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling method (TUNEL) assay was used to assess apoptosis.Results:Compared with the Sham group, the NDS was decreased (60.75±5.80 vs. 80.00±0.00, P < 0.01), the expression levels of LC3 and p62 elevated [LC3 ( A value): 1.04±0.64 vs. 0.40±0.14, p62 ( A value): 2.75±0.57 vs. 0.36±0.12, both P < 0.05], the number of autophagosomes and apoptotic cells increased in the CA group [(39.00±8.00)% vs. (3.87±1.90)%, P < 0.05]. Compared with the CA group, the NDS (67.75±6.32 vs. 60.75±5.80, P < 0.05), the expression of LC3 ( A value: 1.60±0.34 vs. 1.04±0.64, P < 0.05), and the number of autophagosomes increased in the BK group, while the expression of p62 and the rate of apoptotic cells reduced [p62 ( A value): 1.51±0.32 vs. 2.75±0.57, apoptotic cells rate: (23.03±1.91)% vs. (39.00±8.00)%, both P < 0.05]. Compared with the BK group, the NDS (59.00±8.19 vs. 67.75±6.32, P < 0.05), the expression of LC3 ( A value: 0.62±0.41 vs. 1.60±0.34, P < 0.05) and the number of autophagosomes declined in the CP+BK group, while the expression of p62 and the rate of apoptotic cells elevated [p62 ( A value): 3.50±0.47 vs. 1.51±0.32, apoptotic cells rate: (44.53±10.15)% vs. (23.03±1.91)%, both P < 0.05]. Conclusion:Bradykinin postconditioning played a neuroprotective role in CPR rats by activating autophagy and reducing apoptosis.

4.Clinical analysis of eosinophilic gastroenteritis in children

Objective To discuss the clinical manifestations, laboratory indexes, imageological ex-amination results,endoscopic and histopathological features of eosinophilic gastroenteritis( EG) in children. Methods Thirty-three children with EG were collected with clinical manifestations,allergic history,labora-tory examination,endoscopic and histopathological features and treatments,who were hospitalized at depart-ment of pediatric gastroenterology of Shengjing Hospital of China Medical University between January 2012 and December 2017. Results The main clinical manifestations of EG were abdominal pain(75. 8%),vomi-ting(30. 3%)and diarrhea(21. 2%). The hemoglobin level decreased in 7 cases(21. 2%). The eosinophil count increased in 13 cases ( 39. 4%) . Total serum IgE elevated in 18 cases ( 54. 5%) , while 23 cases were positive in serum food or respiratory specific IgE. The fecal occult blood test showed positive in 7 cases (21. 2%). Among 28 children conducted with whole abdominal CT,there were 13 cases of pelvis fluid, 12 cases of enlarged lymph nodes and 4 cases of thickened intestinal wall. All 33 children went through the endoscopic examination,the main histological change were erythema-exudation(31 cases,93. 9%),mucosal hyperemia edema(5 cases,15. 2%) and hemorrhage(4 cases,12. 1%). The main pathological change was mucosal inflammation with a large number of eosinophil infiltration(≥20 per HPF) . Among the 33 cases,17 cases relieved through treatment of avoiding allergen, antacids, anti-allergic agents and leukotriene receptor antagonist,with 1 case recurrence. The other 16 cases relieved after treatment of glucocorticoid and the above therapy,with 3 cases recurrence. Conclusion The clinical manifestations of children with EG are various and lack of specificity. In terms of diagnosis,the elevated level of eosinophil count and total serum IgE,or posi-tive in serum food specific IgE may be taken as reference. The definite diagnosis is based on pathological examination (eosinophil infiltration≥20 per HPF). For treatment,avoiding allergen,antacids,anti-allergic agents and leukotriene receptor antagonist are effective in several children. Glucocorticoid can obtain satisfac-tory curative effect in the uneffective children.
5.Clinical study of benazepril combined with amlodipine in the treatment of essential hypertension

Objective To investigate the clinical efficacy of benazepril combined with amlodipine in the treatment of essential hypertension.Methods From January 2016 to January 2017,90 patients with coronary heart disease in Taizhou Central Hospital were selected in the study,and they were randomly divided into three groups,with 30 cases in each group.The observation group(A group)was treated with benazepril combined with amlodipine,the control group (B group )was treated with benazepril,and the control group (C group)was treated with amlodipine. The clinical efficacy,blood pressure,blood lipid changes,blood glucose changes before and after treatment in the three groups were compared.Results The total effective rate of A group was 96.367%,which of B group was 66.67%, which of C group was 70.00%,and the clinical efficacy among the three groups had statistically significant difference (χ2=11.96,P<0.05).Before treatment,the systolic blood pressure(SBP)and diastolic blood pressure(DBP)of the three groups had no statistically significant differences(F=0.98,0.85,all P>0.05).After treatment,the SBP, DBP of the three groups were significantly lower than those before treatment(t=5.68,7.21,3.52,3.16,3.64,3.16, all P<0.05),the SBP and DBP of A group were significantly lower than those of B group and C group(F=6.24, 3.27,all P<0.05).After treatment,only the TC,TG,HDL-C levels of A group had statistically significant differ-ences compared with those before treatment(t=5.29,6.28,2.31,all P<0.05),and the TC,TG levels of A group were significantly lower than those of B group and C group(F=3.18,2.86,all P<0.05),while the HDL-C level of A group increased significantly compared with that of B group and C group(F=3.78,P<0.05 ).Before treatment, the FPG,FINS among the three groups had no statistically significant differences (F=0.85,0.95,all P>0.05 ). After treatment,only the FPG,FINS of A group had statistically significant differences compared with those before treatment(t=5.14,3.65,all P<0.05),and the FPG,FINS of A group were significantly lower than those of B group and C group(F=5.27,2.86,all P<0.05).The adverse reaction rate in A group was 6.67%,which in B group was 30.00%,which in group C was 26.67%,and there was no statistically significant difference among the three groups (χ2=1.72,P>0.05).Conclusion Benazepril combined with amlodipine in the treatment of essential hypertension is effective,safe,and has less adverse reactions.
6.Exclusive enteral nutrition's moderating effect on Th17 cell of juvenile rat with inflammatory bowel disease

Objective Through the TNBS-induced inflammatory bowel disease model in young rats,to observe the level of Th17 cell related cytokine IL-17A on the intestinal tissue of IBD young rats model after feeding EEN,Peptisorb,and investigate the enteral nutrition's regulative effects on Thl7 cells.Methods Thirty six four-week healthy male Sprague-Dswley rats were selected and divided into 4 groups,blank control group and EEN control group with 8 rats in each group,IBD group and IBD-EEN group with 10 rats in each group:blank control group and EEN control group were given normal saline enema,then fed with normal diet and enteral nutrition separately;IBD group and IBD-EEN group were given 5% TNBS enema,establish the IBD model,then fed with normal diet and enteral nutrition separately.The weights,defecate characters and occult blood situations of all rats were determined.Also,we carried out the activity index score of disease according to the Cooper score method.On the seventh day,all rats were sacrificed and the colon specimens were collected.We observed mucosal injury by HE staining and expression level of Thl7 cell related cytokine IL-17A that were detected by immunohistochemical staining.Results Compared with IBD group,IBD-EEN group was improved significantly in terms of defecate character and occult blood situation.Both IBD group and IBD-EEN group gained their body weights than before,(39.95 ± 2.929) g and (38.40 ± 4.545) g respectively after 7 days (P≥0.05).In IBD group,gross tissue and pathological section displayed significant intestinal inflammation change,showing typical inflammatory bowel disease change;under the microscope,intestinal mucosa showed a large amount of inflammatory cell infiltration,a part of section displayed inflammatory granuloma formation,normal glandular structure was damaged.After EEN treatment,DAI scores werr decreased significantly;intestinal inflammation changes were relieved;intestinal mucosa inflammatory cell infiltration were reduced.Immunohistochemical study showed that the expression of IL-17A cytokine in the intestinal mucosa of IBD group was higher than that of IBD-EEN group (P < 0.05).Conclusion The exclusive enteral nutrition treatment possessed the effect of reducing IBD young rat's intestinal mucosal inflammation,the inner mechanism may be related with the changing expression of Th17-cell cytokines,IL-17A.
7.Clinical analysis of preterm infants parenteral nutrition associated cholestasis

Objective To discuss the clinical feature of cholestasis preterm infants who have used parenteral nutrition early after birth,and analyze the relative factors and preventive meatures of parenteral nutrition associated cholestasis (PNAC).Methods We retrospectively analyzed the preterm infants who were born and hospitalized at Shengjing Hospital of China Medical University from January,2011 to April,2016.There were 89 cases in total that used at least 2 weeks of parenteral nutrition in the early stage after birth and were developed to cholestasis afterwards.We divided 89 cases into PNAC group and multifactor group:the former was corresponded to PNAC diagnosis standards,accounting for41 cases (46.07%);the latter with other etiology in addition to parenteral nutrition accounts for 48 cases.The clinical characteristics were studied and analyzed using case-control method.Results The sex ratio of premature cholestasis was 2.18:1,the mean gestational age was (31.05 ±2.15) weeks,the mean birth weight was (1360.55 ±421.14) g,and the mean using-time of parenteral nutrition was (26.22 ± 9.78) d.Futhermore,PNAC group was divided into hepatitis group and non-hepatitis group according to the alanine aminotransferase level,and they both had statistical significance in gestational age,starting time of parenteral nutrition,appearance time of jaundice,enteral feeding time,starting dose of amino acid,maximum dose of fat emulsion (P < 0.05).We divided them into long-term group (≥20 d) and short-term group (< 20 d) according to the using time of parenteral nutrition.The level of alanine aminotransferase,glutamic oxalacetic transaminase,total bilirubin,direct bilirubin and bile acid of longterm group was higher than those of short-term group,but with P≥0.05.The multifactor group was mostly accompanied with multiple infections in the order of bacterial infection (75.0%),fungal infection (20.83%),cytomegalovirus infection (8.33%) and syphilis infection (2.08%),etc.The other complication's incidence rate of PNAC group and multifactor group had no statistical significance.In terms of prognosis,the liver function of two group improved remarkably than before,with PNAC group having more significance (P <0.05).Conclusion PNAC is the major factor of preterm cholestasis,and the degree of liver damage of PNAC preterm was related to starting time of parenteral nutrition,using time of parenteral nutrition,enteral feeding time,starting dose of amino acid and maximum dose of fat emulsion.The liver function of PNAC group is recovered obviously through regulatory treatment,and it possesses a better prognosis than multifactorgroup.
8.Changes and clinical significance of ischemia modified albumin in patients with cardiopulmonary resuscitation

Objective To study the changes in the level of serum IMA in patients with cardiac arrest followed by CPR,and explore the value of serum IMA in predicting prognosis of these patients after CPR.Methods (1) A total of 36 patients (25 men and 11 women) after CPR following cardiac arrest were recruited into experimental group (group C).Of them,15 cases (11 men and 4 women) and 21 cases (14men and 7 women) were assigned into resuscitation group (group R) and non resuscitation group (group NR),respectively,according to the presence or absence of ROSC.Meanwhile,another 60 healthy subjects (39 men and 21 women) matched for age and gender were recruited into healthy control group (group H).(2) The determination of the serum IMA level and comparisons of changes in serum IMA levels among groups were carried out.Results (1) The level of serum IMA in group C was higher than that in group H (P < 0.05) ; The serum IMA level in group R was lower than that in group NR and higher than that in group H (P <0.05 for both).(2) The area under the ROC curve for serum IMA in predicting the prognosis of patients treated with CPR after cardiac arrest was 0.73.When the cut off value was 128.25 U/mL,the sensitivity and specificity were 57.1% and 93.3% respectively; the positive predictive value and negative predictive value were 92.3% and 60.9% ; positive likelihood ratio and negative likelihood ratio were 8.5 and 0.5 ; the correct diagnosis index was 72.2%,and Youden index was 50.5%.Conclusions Serum IMA has a certain value in prediction of prognosis in patients treated with CPR following cardiac arrest.
9.The research of specialized continuous education model in the department of Plastic and Aes-thetic surgery

Plastic and Aesthetic surgery is a science which creates beauty by combining know-ledge and art. Combined with the professional characteristics of plastic surgery, we reformed the cur-riculum content and teaching mode of continuous education, including a established interactive theoret-ical learning model based on Journal club, the strengthening of clinical practice integrated with multiple related disciplines, the expanding the knowledge of Sociology, Psychology and Ethics, and the construc-tion of a long-term platform of network resources. Therefore, a comprehensive and specialized continu-ous education model in the department of plastic and aesthetic surgery was ultimately formed, whose preliminary assessment was favorable, and could be helpful in the cultivation of high-quality plastic and aesthetic surgeons in the future.
10.Efficacy analysis of laparoscopic surgery for primary local gastric and intestinal gastrointestinal stromal tumors.

OBJECTIVETo investigate the feasibility and short-term efficacy of laparoscopic resection of primary local gastric and intestinal gastrointestinal stromal tumors(GIST).
METHODSClinicopathological data of 26 patients with GIST, 20 located at the stomach and 6 at the intestine, undergoing laparoscopic complete resection from October 2010 to April 2013 were retrospectively analyzed.
RESULTSHand-assisted laparoscopic surgery was performed in 3 patients with gastric GIST, while the other 23 underwent regular laparoscopic surgery. All the procedures were performed successfully without conversion to open operation. According to tumor location and growth types, gastric local resection was performed in 18 cases, distal gastrectomy in 2 cases and intestinal segmental resection in all 6 cases of intestinal GIST. The mean diameter of tumor was (4.5±1.6) cm. The mean operational time was(96.2±28.2) min, with a mean blood loss of (49.6±38.6) ml. Postoperative bowel function recovery time was (2.3±0.7) d and the length of postoperative hospital stay was (6.8±1.9) d. Bleeding from gastrointestinal tract developed in 1 patient after resection of intestinal GIST. Postoperative pathology indicated very low risk of GIST in 1(3.8%), low risk in 13(50.0%), intermediate in 9(34.6%) and high risk in 3(11.5%) patients, respectively. After a follow-up ranging form 3 to 32 months, no recurrence or death was found.
CONCLUSIONLaparoscopic surgery of primary local GISTs from stomach or intestine is safe and feasible in selected patients, with less invasiveness, rapid recovery, and favorable short-term outcomes.
