1.Xuefu Zhuyutang in Malignant Tumor Disease: A Review

Cancer has become a significant global public health issue, severely impacting public health and societal development. Despite advances in tumor treatment methods in recent years and a gradual decline in cancer mortality rates, drug-related adverse reactions and drug resistance remain substantial challenges. Traditional Chinese medicine (TCM) has demonstrated significant clinical efficacy in cancer treatment and small side effects, making it widely applied in the field of oncology. Xuefu Zhuyutang, derived from Yilin Gaicuo, is known for its abilities to invigorate blood circulation, dispel blood stasis, promote Qi flow, and alleviate pain. It was specifically formulated by the esteemed WANG Qingren of the Qing dynasty for the "blood stasis syndrome in the blood mansion" and is commonly used to treat Qi stagnation and blood stasis syndrome. Clinical studies have shown that Xuefu Zhuyutang, when combined with conventional Western medications, produces significant effects in the treatment of malignant tumors such as liver cancer, lung cancer, and cervical cancer. It substantially reduces the incidence of adverse reactions following Western treatments, including radiation esophagitis, radiation encephalopathy, radiation-induced oral mucositis, and edema. Additionally, it alleviates cancer-related pain and fever, blood hypercoagulability, and associated complications such as depression and anxiety, and also mitigates chemotherapy-induced side effects like hand-foot syndrome. Basic research has demonstrated its potential anti-tumor mechanisms, including the inhibition of Wnt/β-catenin signaling pathway activation, suppression of mitogen-activated protein kinase (MAPK) pathway activation, and anti-tumor angiogenesis. Pharmacological studies have revealed that its active components inhibit tumor cell proliferation and migration, induce tumor cell apoptosis, suppress tumor angiogenesis, enhance the cytotoxicity of natural killer cells against tumors, improve the tumor microenvironment, and regulate immune function. This paper reviewed the latest research progress on Xuefu Zhuyutang in the treatment of malignant tumors from four aspects: theoretical exploration, clinical studies, mechanisms of action, and pharmacological basis, aiming to provide insights and methods for the clinical diagnosis and treatment of malignant tumors.

3.The Effect of Saikosaponin D on the Growth of Esophageal Cancer Eca109 Cells by Regulating the HIF-2α/NF-κB p65 Pathway

Objective To investigate the effects of saikosaponin D(SSD)on the growth of esophageal squamous cell carcinoma(ESCC)Eca109 cells and the hypoxia-inducible transcription factor-2α(HIF-2α)/nuclear factor-κB p65(NF-κB p65)pathway.Methods Different concentrations of SSD(0,3,6,9,12,15,18,21,24 μmol·L-1)were used to treat Het-1a normal esophageal squamous epithelial cells and Eca109 ESCC cells to screen the experimental concentration of SSD.Eca109 cells were separated into control(NC)group,HIF-2α inhibitor(PT2385)group,SSD low and high concentration(SSD-L,SSD-H)groups,SSD-H+empty lentivirus(LV-NC)group,and SSD-H+HIF-2α recombinant lentivirus(LV-HIF-2α)group.MTT assay was applied to detect cell viability.Flow cytometry was applied to detect cell apoptosis.ELISA was applied to detect the levels of TNF-α,IL-1β,and IL-8 in cell culture media.Western blot was applied to detect cell proliferation,apoptosis,and protein expression related to the HIF-2α/NF-κB p65 pathway.The Eca109 cell tumor bearing nude mouse model was established to verify the effect of SSD on the in vivo growth of Eca109 cells.Results 6 and 12 μmol·L-1 SSD were selected as experimental concentrations.Compared with the NC group,the cell viability,TNF-α,IL-1β,IL-8 levels in cell culture medium,PCNA,Bcl-2,HIF-2α,p-NF-κB p65/NF-κB p65 protein levels,subcutaneous transplant tumor volume and weight in nude mice,serum TNF-α,IL-1β,IL-8 levels,and positive expression of HIF-2α and NF-κB p65 proteins in the PT2385 group,SSD-L group,and SSD-H group were lower,the apoptosis rate,Bax,and Cleaved-caspase3 protein levels were higher(P<0.05 or P<0.01).Overexpression of HIF-2α prominently weakened the inhibitory effects of SSD on the proliferation of Eca109 cells and the growth of subcutaneous transplanted tumors in nude mice(P<0.05 or P<0.01).Conclusion SSD can inhibit the proliferation of Eca109 cells,induce cell apoptosis,and suppress the growth of subcutaneous transplanted tumors of Eca109 cells in nude mice.Its effect may be related to the inhibition of the activation of the HIF-2α/NF-κB p65 pathway.
4.Effects and mechanisms of exosomal miRNA in treatment of multiple myeloma

BACKGROUND:Multiple myeloma is one of the most common hematological malignancies,which is difficult to treat,prone to relapse,and resistant to drugs.The endogenous transport system of exosomes plays a communication role by affecting the exchange of functional components between cells.Among them,miRNA is more easily packaged in exosomes due to its small size,so it affects cell function in many ways and over a wide range.OBJECTIVE:To summarize the mechanism of exosomal miRNA in multiple myeloma and to propose potential targets.METHODS:The terms"multiple myeloma,bone marrow mesenchyml stem cell,fibroblasts,peripheral blood,complication,progression,drug resistance,exosomal miRNA"were used as English search terms in the PubMed database.The time frame of the search was from the inception to June 2024.After reading the titles and abstracts,we excluded irrelevant or repetitive literature,and finally included 81 papers for review.RESULTS AND CONCLUSION:(1)Multiple myeloma cells participate in multiple myeloma bone lesions by regulating the balance of osteogenic and osteoclast functions through various miRNA mediated exosomes,regulating erythrocyte calcium ion efflux channels to participate in the occurrence of hypercalcemia,promoting renal epithelial mesenchymal transition leading to renal fibrosis and renal dysfunction,and upregulating bone marrow-derived mononuclear suppressor cells to promote suppression of the immune microenvironment.In addition,miRNAs in the exosomes of multiple myeloma cells are also involved in the occurrence and development of immunotherapy resistance,as well as pathological processes such as angiogenesis,cell proliferation,and cell aging.(2)Both bone marrow mesenchymal stem cells and tumor-associated fibroblasts can affect the proliferation,metastasis,and apoptosis of multiple myeloma cells through exosomal miRNA,interfere with angiogenesis,and regulate the resistance of chemotherapy drugs.(3)Peripheral blood circulating exosomal miRNAs,as cutting-edge noninvasive biomarkers,can play a key role in judging the stage and progression of multiple myeloma disease,predicting patient formation and prognosis,and evaluating drug sensitivity and drug resistance.(4)Multiple myeloma exosomes long non-coding RNA,circular RNA,miRNA,mRNA and target gene can form different combinations,and the flexible construction of new biological axes and biomolecular networks is in line with the development concept of holistic medicine.(5)Exosome miRNA has great development potential as a therapeutic target for predictive biomarkers and new drug research and development.How to maximize the use of exosome miRNA and how to accelerate the clinical transformation of related scientific research results deserve further investigation.
5.Research advances on the application of exosomal multi-omics analysis technology in warning and diagnosis of burn sepsis

As one of the primary causes of death in burn patients, sepsis presents challenges in early warning and diagnosis, mainly due to its nonspecific clinical manifestations and the limitations of traditional biomarker detection efficiency. As an important carrier of intercellular information transfer, exosomes and their contents (RNAs, proteins, and metabolites) can reflect the pathophysiological status of the body, thus attracting significant attention in the field of disease diagnosis. This review aims to summarize the research advances of exosomal multi-omics (transcriptomics, proteomics, metabolomics, etc.) analysis technologies in the warning and diagnosis of burn sepsis, and explore their application potential in revealing disease mechanisms, screening specific early biomarkers, and integrating emerging bioinformatics technologies. The goal is to provide new strategies and directions for achieving the precision diagnosis and treatment of burn sepsis.

Chemotherapy is an important treatment for tumors, but most patients experience varying degrees of chemotherapy- induced myelosuppression. Four properties theory of traditional Chinese medicine (TCM) has unique advantages in improving chemotherapy-induced myelosuppression. The monomers from TCM with different properties and flavors, such as cold-natured (e.g. Scutellaria baicalensis, Rhus chinensis), cool-natured (e.g. Ligustrum lucidum, Ophiopogon japonicus), warm-natured (e.g. Panax ginseng, Epimedium brevicornu, Curcuma longa, Angelica sinensis), hot-natured (e.g. Cinnamomum cassia, Aconitum carmichaeli), and neutral-natured (e. g. donkey-hide gelatin, Lycium barbarum, Rhodiola rosea, fungi), can exert anti- myelosuppressive effects by reducing damage to hematopoietic stem/progenitor cells, improving the bone marrow hematopoietic microenvironment, inhibiting the oxidative stress response, regulating signaling pathways, so as to ultimately repaire inflammatory damage and improve hematopoietic function, thereby playing an anti-myelosuppressive role.

Cervical spinal cord injury without fracture-dislocation (CSCIWFD) is referred to as a special type of cervical spinal cord injury characterized by traumatic spinal cord dysfunction and no significant bony structural abnormalities on imagines. Duo to the high risk of missed diagnosis during the initial consultation, CSCIWFD may lead to progressive neurological deterioration or even complete paralysis, severely impacting patients′ prognosis. Currently, there are no established consensuses over the diagnosis and treatment of CSCIWFD, such as the lack of evidence-based standards for indications of non-surgical treatment and risk of secondary neurological injury, as well as debates over the optimal timing for surgical intervention and indications for different surgical approaches. To address these issues, the Spine Trauma Group of the Orthopedic Branch of the Chinese Medical Doctor Association organized experts in the relevant fields to formulate Diagnosis and treatment guideline for acute cervical spinal cord injury without fracture- dislocation in adults ( version 2025) . Based on evidence-based medicine and the principles of scientific rigor and clinical applicability, the guidelines proposed 11 recommendations covering terminology, diagnosis, evaluation treatment, and rehabilitation, etc., aiming to standardize the management of CSCIWFD.

Vertebral refracture following percutaneous vertebral augmentation (PVA) is commonly seen in elderly patients with osteoporotic thoracolumbar compression fractures (OTLCF). It can lead to recurrent pain, loss of vertebral height, progression of kyphosis, and even neurological dysfunction, significantly impairing patients′ quality of life. Current diagnosis and treatment face multiple challenges, including high misdiagnosis rate, difficulty in choosing between surgical and non-surgical treatment options, lack of standardized surgical protocols, interference from intralesional bone cement during procedures, inadequate stability of internal fixation in osteoporotic bone, and suboptimal compliance of anti-osteoporotic therapy. Establishing a standardized diagnostic and therapeutic framework is urgently needed. To standardize the management process and improve outcomes for vertebral refractures after PVA in elderly OTLCF patients, Spinal Trauma Group of the Orthopedic Branch of Chinese Medical Doctor Association organized experts in the field to develop Guideline for the diagnosis and treatment of vertebral refracture after percutaneous vertebral augmentation in elderly patients with osteoporotic thoracolumbar compression fractures ( version 2025), based on current literature and clinical experience, and adhering to principles of scientific rigor and clinical applicability. A total of 11 recommendations were proposed, encompassing diagnosis, treatment, and rehabilitation of vertebral refracture after PVA in elderly patients with OTLCF, aiming to provide a foundation for a standardized management.
