Objective To investigate the clinical value of myocardial contrast echocardiography(MCE)quantitative analysis in diagnosing myocardial microcirculation disorders in coronary artery disease and evalua-ting outcomes after percutaneous coronary intervention(PCI).Methods Fifty patients diagnosed with severe stenosis of a single or more coronary arteries(stenosis rate of over 75%)through coronary CT angiography(CTA)or coronary angiography(CAG)from June 2022 to June 2024 were selected as the coronary heart dis-ease group.Thirty patients with coronary artery lesions complicated with myocardial infarction who under-went PCI treatment were selected as the post PCI group.Eighty patients with normal or only mild coronary artery stenosis(stenosis rate below 50%)were selected as the control group. All participants underwent MCE examination.Quantitative parameters, including peak intensity(A),upslope rate(β),time-to-peak(TTP),ar-ea under the curve(MCE-ACU),regional myocardial blood flow(MBF=A×β),and left ventricular ejection fraction(LVEF),were analyzed using ROI software in both normal and lesion-supplied myocardial regions.Receiver operating characteristic(ROC)was used to analyze the diagnostic value of various parameters of MCE for myocardial micro perfusion caused by coronary artery disease and myocardial perfusion after PCI.Re-sults Quantitative MCE parameters were significantly lower in the coronary heart disease group and post-PCI groups compared to the control group(P<0.05),with the post-PCI group showing the lowest values.Except for TTP,all parameters differed significantly between groups(P<0.05).Despite successful stent placement(TIMI grade 3 flow)in lesion vessels post-PCI,A,β,and MCE-ACU remained reduced in lesion-supplied re-gions compared to normal territories(P<0.05).The ROC curve showed that MCE parameters A,β,and MBF had independent diagnostic value for myocardial microcirculation disorders in coronary heart disease. The sensitivity and specificity of multi parameter combined diagnosis of myocardial microcirculation disorders in coronary heart disease were significantly improved, and MCE parameters were correlated with left ventricu-lar systolic function. Conclusion MCE quantitative analysis provides significant diagnostic value for myocardi-al microcirculation disorders in coronary artery disease patients and effectively evaluates PCI therapeutic effi-cacy and long-term prognosis.

Tumor cells can escape the surveillance and attack of the immune system, namely immune escape. In recent years, regulatory B (Breg) cell have attracted much attention in researches about tumor immunity. Breg cell plays an important role in tumor immune escape by secreted various cytokines. Mechanistic progress has been made in this field, which provides more targets and ideas for tumor clinical treatment. However, the phenotypic classification of Breg cells and its complex mechanism in tumor immunity still need to be systematically analyzed.

Background:
Developmental hypothyroidism impairs learning and memory in offspring, which depend on extensive neuronal circuits in the entorhinal cortex, together with the hippocampus and neocortex. The entorhinal-dentate gyrus pathway is the main entrance of memory circuits. We investigated whether developmental hypothyroidism impaired the morphological development of the entorhinal-dentate gyrus pathway.
Methods:
We examined the structure and function of the entorhinal-dentate gyrus pathway in response to developmental hypothyroidism induced using 2-mercapto-1-methylimidazole.
Results:
1,1´-Dioctadecyl-3,3,3´,3´-tetramethylindocarbocyanine perchlorate tract tracing indicated that entorhinal axons showed delayed growth in reaching the outer molecular layer of the dentate gyrus at postnatal days 2 and 4 in hypothyroid conditions. The proportion of fibers in the outer molecular layer was significantly smaller in the hypothyroid group than in the euthyroid group at postnatal day 4. At postnatal day 10, the pathway showed a layer-specific distribution in the outer molecular layer, similar to the euthyroid group. However, the projected area of entorhinal axons was smaller in the hypothyroid group than in the euthyroid group. An electrophysiological examination showed that hypothyroidism impaired the long-term potentiation of the perforant and the cornu ammonis 3–cornu ammonis 1 pathways. Many repulsive axon guidance molecules were involved in the formation of the entorhinaldentate gyrus pathway. The hypothyroid group had higher levels of erythropoietin-producing hepatocyte ligand A3 and semaphorin 3A than the euthyroid group.
Conclusion
We demonstrated that developmental hypothyroidism might influence the development of the entorhinal-dentate gyrus pathway, contributing to impaired long-term potentiation. These findings improve our understanding of neural mechanisms for memory function.

This article reported a patient with familial dysalbuminemic hyperthyroxinemia (FDH). A 38-year-old male occasionally found to have increased FT 4 level with normal FT 3 and TSH on routine physical examination was admitted to our hospital. Thyroid function was rechecked and found that the levels of TT 4, FT 4, rT 3 were increased, while TT 3, FT 3, TSH, TRAb, TPOAb and TGAb were normal. The patient had no other symptoms of thyrotoxicosis except occasional diarrhea. Gene sequencing revealed a heterozygous mutation of c. 653G>A (p.R218H) in exon 7 of albumin (ALB) gene, therefore the patient was confirmed as a case of FDH.

Journal of Clinical Hepatology 2019;35(5):1153-1156
Malignant biliary obstruction is often in an advanced stage at the time of diagnosis, and palliative relief of obstruction has become the main treatment option. Biliary stent implantation is currently the most widely used treatment for the relief of biliary obstruction, but stent reobstruction has become a common problem in clinical practice. Endoscopic retrograde cholangiopancreatography (ERCP) combined with intraductal radiofrequency ablation can solve the problem of direct obstruction in malignant biliary obstruction and improve stent patency rate, survival time, and survival rate, with few complications and a low surgery-related mortality rate, and therefore, it can be used as a safe and feasible palliative treatment method for malignant biliary obstruction.
