OBJECTIVE To explore the expression levels of microribonucleic acid-223(miR-223),nucleotide-bind-ing oligomerized domain-like receptor 3(NLRP3)and effect protein Caspase-1(Caspase-1)messenger ribonucleic acid(mRNA)in the thoracolumbar vertebral fracture patients with postoperative incision infections and analyze the diagnostic values.METHODS A total of 222 patients with thoracolumbar vertebral fracture who were treated in Jinan People's Hospital from Jan.2021 to Dec.2022 were enrolled in the study and were divided into the infection group with 25 cases and the no infection group with 197 cases according to the status of postoperative infections.The relative expression levels of peripheral blood miR-223,NLRP3 and Caspase-1 mRNA were detected by means of real-time fluorescent quantitative polymerase chain reaction(qPCR),the values of the three indexes in predic-tion of the postoperative incision infections were analyzed by means of receiver operating characteristic(ROC)curves,and Pearson correlation analysis was performed for the linear relationship between the miR-223,NLRP3 and Caspase-1 and the serum amyloid A(SAA)protein,monocyte chemoattractant protein-1(MCP-1)and Toll-like receptor-2(TLR-2).RESULTS There were significant differences in the levels of miR-223,NLRP3 and Caspase-1 mRNA between the infection group and the no infection group(P＜0.05);the levels of the above inde-xes of the infection group were 3.01±1.00,2.25±0.75 and 2.31±0.77,respectively.The area under the curve(AUC)of the joint detection of miR-223,NLRP3 and Caspase-1 was 0.935 in prediction of the postoperative inci-sion infections in the patients with thoracolumbar vertebral fracture,with the sensitivity 92.00％,the specificity 80.70％.There were significant differences in the levels of SAA,MCP-1 and TLR-2 between the infection group and the no infection group(P＜0.05);the levels of the above indexes of the infection group were(169.74±19.64)mg/L,(60.45±14.63)ng/L and(75.73±13.63)μg/L,respectively.The levels of miR-223,NLRP3 and Caspase-1 mRNA are positively correlated with SAA,MCP-1 and TLR-2(P＜0.05).CONCLUSIONS The chan-ges of miR-223,NLRP3 and Caspase-1 mRNA are associated with the postoperative incision infections in the pa-tients with thoracolumbar vertebral fracture.The joint detection of miR-223,NLRP3 and Caspase-1 mRNA may facilitate the diagnosis of postoperative incision infections,and the three indexes are positively correlated with SAA,MCP-1 and TLR-2.

Objective To investigate the plasma level of interleukin-31(IL-31)in patients with pulmonary arterial hypertension(PAH)and its clinical relevance.Methods The patients who were diagnosed as PAH in Zhongshan Hospital,Fudan University from January 1,2021 to December 30,2023(PAH group)and the healthy people in the same period(control group)were selected.The clinical data and follow-up records were collected.Plasma levels of IL-31,IL-1β,IL-6,IL-10,IL-12p70,monocyte chemoattractant protein-1(MCP-1),tumor necrosis factor-α(TNF-α)and transforming growth factor-β1(TGF-β1)were detected by enzyme-linked immunosorbent assay(ELISA).Pearson correlation test was used to evaluate the correlations between IL-31 and right cardiac catheterization parameters,echocardiography parameters and blood indices in patients with PAH.Cox proportional hazard model was used to analyze the prognostic factors of patients in PAH group.Results A total of 50 patients with PAH and 22 healthy controls were included.There was no significant difference in age,gender,body mass index and left ventricular ejection fraction between the two groups.Compared with the control group,the plasma level of IL-31 in the PAH group was significantly higher(168.82[149.14,177.26]pg/mL vs 152.76[145.58,159.41]pg/mL,P=0.001).Pearson correlation test showed that the plasma level of IL-31 in PAH patient was positively correlated with mean pulmonary artery pressure(r=0.652,P＜0.001)and pulmonary vascular resistance(r=0.651,P＜0.001),but was negatively correlated with tricuspid annular plane systolic excursion(r=-0.496,P＜0.001).Cox proportional hazard model showed that higher plasma level of IL-31 was an independent predictor of readmission for heart failure/all-cause mortality in patients with PAH(HR=1.130,95％CI 1.052-1.214,P=0.001).Conclusions Plasma level of IL-31 may be significantly increased in patients with PAH and be positively correlated with the severity of PAH,and elevated level of IL-31 is predictive of poor prognosis in PAH patients.

Objective To study the curative effect of psychological intervention combined with drug treatment of children addicted to the network game.Methods One hundred and twelve children addicted to the network game were selected,who were admitted to the outpatient department of the Second Affiliated Hospital of Xinxiang Medical University from Feb.2009 to Oct.2013 were selected,according to the admission time sequence,they were randomly divided into drug treatment group,psychological intervention group(as control group) and psychological intervention combined with drug therapy group(test group).Each of the control groups had 38 cases of patients,while treatment group had 36 cases.Drug treatment group was treated with oral Fluoxetine Hydrochloride Capsules 10 mg,1 time/d; the psychological intervention group was given the general psychological conversation,until discharged from the hospital; the test group adopted the cognitive restructuring,imagine the debate,self suggestion,remind yourself of cognitive therapy and behavior contract,behavior reinforcement,behavior extinction,aversive stimulus,self-regulation of behavior therapy combined with drug therapy.Respectively,before treatment,2 weeks,4 weeks and 8 weeks after the end of the treatment,Chinese Internet Addiction Scale,Coping Style Questionnaire (CSQ) and symptom checklist 90 (SCL-90) of each factor were evaluated.Results By the end of the 2 weeks and 8 weeks after the treatment,CIAS scores in the treatment group were significantly lower than that in drug therapy and psychological intervention group,the differences were statistically significant (t =2.49,2.03,all P ＜ 0.05 ; t =2.72,2.72,all P ＜ 0.01) ; scores of 3 factors of fantasy,self accusation and retreat in CSQ group in coping style questiennaire in the coping style questionnaire in the treatment group were lower than those in control groups,the differences were statistically significant (t =3.99,3.78,2.15,all P ＜ 0.05; t=2.64,4.20,3.72,all P ＜ 0.01); the problem solving,help seeking factor in the test group were higher than those in the control groups,the differences were statistically significant (t =2.69,2.43,5.31,2.25; all P ＜ 0.01) ; variation test combined physicochemical factors scores at different stages had on difference comparison of those in the 2 control groups (all P ＞ 0.05).Comparison of the SCL-90 score between the test group and the 2 control groups,the differences were statistically significant (t =5.05,4.31,all P ＜ 0.01).There was no significant difference of the scores between the 2 control groups (all P ＞ 0.05).Conclusions Psycholoical intervention therapy including cognitive therapy,behavior therapy combined with drug treatment have good effect on children's addiction to Web games.

[Objective]To retrospectively analyze the complications of 528 cases of scoliosis surgery,and evaluate its strategy.[Method]From January 1980 to January 2001,528 patients with scoliosis underwent correction.There were 336 females and 192 males.The average age at surgery was 14.2 years.The average preoperation Cobb's angle was 48.2?(ranged from 34? to 102?).[Result]The patients were followed up for 3~24 years,the average was 12.5 years.The postoperative Cobb's angle averaged 24?.The average correction was 50.2%.Complications included 1 case of spinal cord injury,151 cases of sympathetic nerve injury,12 cases of superior mesenteric artery syndrom,6 cases of screw fracture,13 cases of rod fracture,3 cases of steel cable breakage,25 cases of hook displacement,20 cases of wire breakage,30 cases of flatback syndrome,6 cases of crankshaft phenomenon and 9 pseudoarthroses.[Conclusion]Satisfactory results could be obtained to treat scoliosis.

[Objective]To evaluate the clinical results of posterior hemivertebra resection and correction with transpedicular instrumentation for congenital scoliosis.[Method]From July 2002 to December 2006,thirty-nine consecutive cases,including 22 males and 17 females,underwent posterior hemivertebra resection and correction using transpedicular instrumentation.The average age at surgery was 8.3 years ranging from 3 to 15 years.There were 30 cases with fully segmented hemivertebra and 9 cases with partial segmented hemivertebra.Instrument included 16 cases of ISOLA,10 cases of CD-Horizon,10 cases of Moss-Miami and 3 cases of TSRH.The pre-and post-operative standing radiographic and at final follow-up were measured,and the Cobb's angles in the coronal and sagittal plane were analyzed,respectively.[Result]The average follow-up was 2.8 years ranging from 1 to 4.5 years.The Cobb's angle in coronal plane was corrected from 48? to 13?postoperatively and 13.2? at final follow-up,representing 70.2% correction rate.The kyphosis was corrected from 39? to 12.1? postoperatively and 12.7? at final follow-up,representing 68.7% correction rate.All cases had bony fusion,there was no neurological complication,no implant failure or infection.One case had crankshaft phenomenon.[Conclusion]Posterior hemiveterbra resection with instrumentation is a safe and effective treatment option for congenital scoliosis.Congenital scoliosis should be treated in early age.
