Objective:To compare the efficacy of Kirschner wire fixation combined with Suture-Bridge technique and simple Kirschner wire fixation in the treatment of old lateral malleolar avulsion fractures under all-inside arthroscopy.Methods:A retrospective cohort study was conducted to analyze the clinical data of 56 patients with old lateral malleolar avulsion fractures, admitted at Xuzhou Central Hospital from January 2018 to June 2022, including 32 males (32 feet) and 24 females (24 feet), aged 19-57 years [(35.2±11.1)years]. Twenty-five patients were treated with Kirschner wire fixation under all-inside arthroscopy (K-wire group), while the other 31 patients were treated with Kirschner wire fixation combined with Suture-Bridge technique under all-inside arthroscopy (K-wire combined with suture group). The surgical time, incision healing at 2 weeks after surgery, and fracture healing rate at 6 months after surgery of the two groups were detected. The American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scores, and foot and ankle ability measure (FAAM) scores [including activities of daily living (ADL) scores and sports function (S) scores] of the two groups were assessed before surgery, at 3, 6 months after surgery, and at the last follow-up. The anterior drawer test was performed at 3, 6 months after surgery and at the last follow-up. The time of return to sports after surgery and complication rates were also recorded.Results:All the patients were followed up for 12-18 months [(14.2±2.2)months]. The surgical time in the K-wire combined with suture group was (56.3±12.5)minutes, longer than (41.1±8.2)minutes in the K-wire group ( P<0.01). The incisions in both groups were healed by first intention at 2 weeks after surgery. In the K-wire combined with suture group, all the fractures were healed at 6 months after surgery (100%), while in the K-wire group 22 patients (88%) obtained fracture healing ( P<0.05). There were no statistically significant differences in the AOFAS ankle-hindfoot scores, FAAM-ADL scores, or FAAM-S scores between the two groups preoperatively ( P>0.05). At 3, 6 months after surgery, and at the last follow-up, the AOFAS ankle-hindfoot scores in the K-wire combined with suture group were (89.7±3.4)points, (92.8±2.8)points, and (94.9±3.3)points respectively, higher than (87.4±4.4)points, (90.4±4.1)points, and (92.5±4.6)points in the K-wire group ( P<0.05); the FAAM-ADL scores and the FAAM-S scores in the K-wire combined with suture group were (90.1±3.5)points, (91.5±2.9)points, (92.8±3.0)points and (91.4±5.4)points, (92.8±5.0)points, (94.4±4.8)points respectively, higher than (86.8±5.0)points, (88.7±3.8)points, (90.3±3.7)points and (88.0±5.3)points, (89.5±4.8)points, (91.5±5.2)points ( P<0.05 or 0.01). The AOFAS ankle-hindfoot scores, FAAM-ADL scores, and FAAM-S scores at all postoperative time points were higher than those preoperatively and increased with the passage of the follow-up time ( P<0.01). The results of the anterior drawer test at all postoperative time points in the two groups were negative. The time of return to sports after surgery in the K-wire combined with suture group was (9.2±1.3)weeks, shorter than (10.3±1.5)weeks in the K-wire group ( P<0.01). One patient (3.2%) in the K-wire combined with suture group and 3 patients (12.0%) in the K-wire group had Kirschner wire loosening and withdrawal ( P>0.05). Neither of the two groups developed complications such as nerve or tendon injuries. Conclusion:Compared with simple Kirschner wire fixation under all-inside arthroscopy, Kirschner wire fixation combined with Suture-Bridge technique under all-inside arthroscopy achieves higher fracture healing rate, more obvious improvement in life quality and ankle function, as well as earlier return to sports in the treatment of old lateral malleolar avulsion fractures.

Objective:To compare the efficacy of different suture techniques under arthroscopy in rotator cuff injury.Methods:A retrospective analysis was performed on clinical data of 117 patients who received arthroscopic rotator cuff surgery from August 2020 to February 2023 in Department of Bone and Joint Surgery, Xuzhou Central Hospital, Jiangsu Province. According to different suture techniques, they were divided into single-row group (56 cases, single-row suture method) and double-row group (61 cases, double-row suture method). The perioperative indicators, surgical complications, shoulder range of motion, pain score (visual analogue scale, VAS), shoulder joint score (Constant-Murley score, CMS) and re-tear rate were compared between the two groups. Chi-square test was used for between-group comparison of enumeration data. Quantitative data that conforms to a normal distribution were represented as xˉ±s, and t-test or one-way ANOVA were used. LSD-t test was used for pairwise comparisons.Repeated measures analysis of variance was used to compare the range of motion, pain score, and CMS score of the shoulder joint before and after treatment. Results:The surgical time in single-row group (78.58±17.25) min was shorter than that in double-row group (88.14±18.51)min, and the difference was statistically significant ( t=2.88, P=0.005). After surgery, the ranges of motions of flexion, abduction and inward rotation of the body side of the two groups were increased gradually, and the ranges of motions in double-row group (anterior flexion: (89.67±6.93)° at 6 months postoperatively; (135.18±12.52)° at 12 months postoperatively), (abduction: (122.52±7.32)° at 6 months postoperatively; (160.87±8.31)° at 12 months postoperatively), (lateral internal rotation:(73.35±4.45)° at 6 months postoperatively; (80.82±6.27)° at 12 months postoperatively) were higher than those in single-row group (anterior flexion: (82.01±6.35)° at 6 months postoperatively; (125.21±10.41)° at 12 months postoperatively), (abduction: (115.23±6.84)° at 6 months postoperatively; (151.41±7.54)° at 12 months postoperatively), (lateral internal rotation: (68.32±4.21)° at 6 months postoperatively; (75.32±5.12)° at 12 months postoperatively), the differences were statistically significant (flexion: 6 months after surgery, t=6.37, 12 months after surgery, t=4.66; abduction: 6 months after surgery, t=5.68, 12 months after surgery, t=6.43; inward rotation: 6 months after surgery, t=6.41, 12 months after surgery, t=5.17; all P<0.001). The VAS score in the two groups was decreased significantly after surgery while the CMS was increased significantly, and there were statistically significant differences in double-row group (VAS: (3.53±0.74) points at 3 months postoperatively; (2.76±0.54) points at 6 months postoperatively; (1.87±0.38) points at 12 months postoperatively), (CMS: (66.06±5.52) points at 3 months postoperatively; (74.17±4.44) points at 6 months postoperatively; (83.87±4.38) points at 12 months postoperatively) at different time points after surgery compared to single-row group (VAS: (4.02±0.80) points at 3 months postoperatively; (2.99±0.58) points at 6 months postoperatively; (2.25±0.41) points at 12 months postoperatively), (CMS: (63.21±5.95) points at 3 months postoperatively; (71.69±4.63) points at 6 months postoperatively; (80.25±3.25) points at 12 months postoperatively), the differences were statistically significant (VAS: 3 months after surgery, t=3.44, 6 months after surgery, t=2.22, 12 months after surgery, t=5.20 ; CMS: 3 months after surgery, t=2.69, 6 months after surgery, t=2.96, 12 months after surgery, t=5.04; P values were <0.001, 0.028, <0.001,0.008, 0.004, and <0.001, respectively). The re-tear rate was significantly lower in double-row group (3.28%，2/61) than that in single-row group (14.29%，8/56), but there was no statistically significant difference between the two groups (adjusted χ2=3.23, P=0.070). Conclusion:Rotator cuff surgery can effectively restore shoulder function in patients with rotator cuff injury. Single-row suture method has simpler operation and shorter surgical time, but double-row suture method has a better fixation effect and is more conducive to prognosis.

With the growing demand for physical activity, an increasing number of individuals with chronic Achilles tendon ruptures are opting for surgical intervention. Surgical approaches encompass end-to end anastomosis, tendon flap techniques, tendon transfer procedures, and free tendon grafting, among others. When selecting the appropriate surgical method and determining the surgical indications, it is imperative to consider factors like the length of the Achilles tendon defect, patient age, aesthetic preferences, functional requirements, and local tissue conditions. As medical devices evolve and surgical techniques advance, the criteria for surgical intervention are also evolving. Drawing from existing literature evidence, it becomes crucial to define reasonable parameters for addressing Achilles tendon defects with each surgical technique, aligning more closely with clinical needs. Additionally, auxiliary technologies such as biologic therapy and innovative biomaterials have demonstrated promising results in laboratory or animal models. The focal point of advancing these auxiliary technologies lies in facilitating the translation of pertinent clinical outcomes in the future.

Objective:To compare the clinical efficacy of patients with chronic lateral rotational ankle instability(CLRAI) after all-inside arthroscopic lateral ligament augmentation procedure and Brostr?m procedure.Methods:This is a retrospective cohort study. The clinical and imaging data of 106 CLRAI patients were collected at the Xuzhou Central Hospital from January 2021 to December 2022. The patients included 55 males and 51 females with an age of (32.6±8.2) years (range: 16 to 50 years). All patients were treated under all-inside arthroscopic, and were divided into Brostr?m-Gould surgery group ( n=54) and Brostr?m surgery group ( n=52) according to different ligament repair methods. At 3, 6, and 12 months after surgery, ankle inversion stress tests and anterior drawer tests were used to examine the stability of the ankle joint and observe gait. The American Orthopedic Foot and Ankle Society ankle hindfoot scale (AOFAS-AH) and Karlsson ankle function score (KAFS) were used to assess ankle function; Tegner score was used to assess the patient′s level of exercise; the foot and ankle outcome score(FAOS)(including score of symptoms,pain,function, daily living,function, sports and recreational activities (sport); quality of life (QOL) was used to assess the patient′s daily activity ability. Comparisons of data were made using independent sample t test, repeated measures analysis of variance, LSD- t multiple comparison method, χ2 test or Mann-Whitney U test. Results:All operations were successfully accomplished. All incisions healed by first intention, without evidence of postoperative complications of implant rejection, ligation reaction, and nerve and vessel injury. All patients were followed up at 3, 6, and 12 months after surgery. Ankle varus stress test and anterior drawer test were negative. No evidence supporting lateral ankle instability was obtained. All patients eventually regained normal gait. No patients underwent revision surgery. Repeated measurement analysis of variance showed that AOFAS-AH, Tegner, KAFS and FAOS scores in the Brostrom-Gould group and the Brostrom group were significantly higher than those before surgery ( P<0.01). The change trends of Tegner score and FAOS-sport score were significantly different between the two groups ( F=18.839, P<0.01; F=8.169, P=0.005). Multiple comparisons revealed that at 3-, 6-and 12-month follow-up, the Tegner scores (3 months: 3.7±0.5 vs. 3.3±0.5, t=-3.980, P<0.01; 6 months: 4.4±0.6 vs. 3.8±0.7, t=-4.792, P<0.01; 12 months: 5.8±0.9 vs. 5.1±1.0, t=-3.889, P<0.01), sport scores (3 months: 82.5±3.7 vs. 79.3±3.8, LSD- t=-4.316, P<0.01; 6 months: 88.5±4.9 vs. 85.7±3.8, LSD- t=-3.312, P=0.001;12 months: 90.1±4.3 vs. 88.2±5.1, LSD- t=-2.112, P=0.037) in the Brostr?m-Gould surgery group were higher than those in the Brostr?m surgery group, with statistical significances. Conclusions:Both Brostr?m-Gould and Brostr?m procedures under all-inside arthroscopic can make ankle stability and improve ankle function in the treatment of CLRAI. However, the former maybe shorten the time to return to exercise and achieve higher motor function.

Objective To compare the clinical effects of arthroscopically-assisted reduction and internal fixation(ARIF)combined with enhanced recovery after surgery(ERAS)and open reduction and internal fixation surgery(ORIF)in the treatment of posterior lateral tibial plateau fractures.Methods Seventy patients with posterior lateral tibial plateau fractures in the Department of Orthopaedics,Xuzhou Central Hospital,from January 2020 to November 2022 were retrospectively selected and divided into ARIF group(with ERAS,n=32)and ORIF group(without ERAS,n=38)according to the treatment methods.All patients were evaluated for fracture type by imaging examination after admission.The operation time,length of hospital stay,early postoperative pain score(evaluated by visual analogue scale[VAS]),knee joint function(evaluated by hospital for special surgery[HSS]scale)at 3 months and thigh circumference difference at 6 months postoperatively were compared between the two groups.Results The operation time in the ARIF group was significantly shorter than that in the ORIF group([67.84±9.89]min vs[85.16±9.18]min,P＜0.001),and the length of hospital stay was significantly shorter in the ARIF group([7.13±1.41]d vs[8.74±1.84]d,P＜0.001).On the third day after operation,the VAS score in the ARIF group was significantly lower than that in the ORIF group([4.00±1.44]vs[5.39±1.24],P＜0.001).ARIF group had better joint function than ORIF group 3 months after operation,and the difference of 10 cm thigh circumference on patella in ARIF group was smaller than that in ORIF group 6 months after operation.Conclusions Compared to ORIF,patients with posterior lateral tibial plateau fractures treated with ARIF combined with ERAS showed faster postoperative recovery,shorter hospital stay,and more precise clinical efficacy.

Acute lung injury (ALI) is a prevalent and severe clinical condition characterized by inflammatory damage to the lung endothelial and epithelial barriers, resulting in high incidence and mortality rates. Currently, there is a lack of safe and effective drugs for the treatment of ALI. In a previous clinical study, we observed that Jinyinqingre oral liquid (JYQR), a Traditional Chinese Medicine formulation prepared by the Taihe Hospital, Affiliated Hospital of Hubei University of Medicine, exhibited notable efficacy in treating inflammation-related hepatitis and cholecystitis in clinical settings. However, the potential role of JYQR in ALI/acute respiratory distress syndrome (ARDS) and its anti-inflammatory mechanism remains unexplored. Thus, the present study aimed to investigate the therapeutic effects and underlying molecular mechanisms of JYQR in ALI using a mouse model of lipopolysaccharide (LPS)-induced ALI and an in vitro RAW264.7 cell model. JYQR yielded substantial improvements in LPS-induced histological alterations in lung tissues. Additionally, JYQR administration led to a noteworthy reduction in total protein levels within the BALF, a decrease in MPAP, and attenuation of pleural thickness. These findings collectively highlight the remarkable efficacy of JYQR in mitigating the deleterious effects of LPS-induced ALI. Mechanistic investigations revealed that JYQR pretreatment significantly inhibited NF-κB activation and downregulated the expressions of the downstream proteins, namely NLRP3 and GSDMD, as well as proinflammatory cytokine levels in mice and RAW2647 cells. Consequently, JYQR alleviated LPS-induced ALI by inhibiting the NF-κB/NLRP3/GSDMD pathway. JYQR exerts a protective effect against LPS-induced ALI in mice, and its mechanism of action involves the downregulation of the NF-κB/NLRP3/GSDMD inflammatory pathway.
