1.Carbon ion radiotherapy planning: a study of prescription dose conversion between microdosimetric kinetic model and local effect model

Objective:In carbon ion treatment planning of water phantom, establish a conversion factor calculation system and conversion factor curves for organs at risk (OAR) for microdosimetric kinetic models (MKM) and local effect models (LEM), and validate them in clinical patient planning.Methods:Using a uniform spherical water phantom as the research object, relative biological effectiveness-weighted doses (RWD) for the LEM were re-calculated based on the physical dose of RayStation-MKM. The median dose within the planning target volume (PTV) of LEM and MKM was regarded as the conversion factor. The impacts of single-fraction target prescription dose, spread-out Bragg peak (SOBP) width and depth, shape, and irradiation mode on the conversion factor were assessed, and a conversion factor calculation system was established. Additionally, the accuracy of the conversion factor calculation system was validated using both water phantoms and clinical patient cases. The conversion factor curves for OAR were computed based on clinical patient treatment plans.Results:The primary influencing factors for the conversion factors were the single-fraction prescription dose, target SOBP width and depth. The conversion factors were increased with the increase of SOBP width and target depth, whereas decreased with the increase of the single-fraction prescription dose. Under single-field irradiation, a conversion factor calculation system was established based on above 3 parameters. For the plans of 9 patients, the average difference between the calculated results and the conversion factor calculation system was 0.340% ± 0.203%, and the average difference in the conversion curves for OAR was 2.650% ± 2.399%.Conclusion:A dose conversion factor calculation system and conversion factor curves for OAR for carbon ion radiotherapy are established for MKM and LEM, and their accuracy meets the requirements for use in clinical patient treatment plans.

3.Analysis of the relationship between serum ATX,Adropin and inflammatory factors and poor prognosis in patients with dilated cardiomyopathy

Objective To investigate the relationship between serum autotaxin(ATX),energy balance re-lated protein(Adropin)and inflammatory factors and poor prognosis in patients with dilated cardiomyopathy(DCM).Methods A total of 105 DCM patients admitted to Qingdao Huangdao District Traditional Chinese Medicine Hospital from June 2017 to February 2020 were selected as the DCM group,and 100 healthy volun-teers who came to the hospital for physical examination during the same period were selected as the control group.The levels of serum ATX,Adropin and inflammatory factors[hypersensitive C-reactive protein(hs-CRP)and interleukin-6(IL-6)]in the two groups were detected,and the correlation between serum ATX,Adropin and inflammatory factors was analyzed by Pearson correlation analysis.Patients in DCM group were divided into good prognosis group and poor prognosis group according to the occurrence of endpoint events during follow-up.Logistic regression was used to analyze the risk factors of poor prognosis in DCM patients,and receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum ATX and Adropinfor poor prognosis in DCM patients.Results The serum levels of ATX,hs-CRP and IL-6 in DCM group were higher than those in control group,and the level of Adropin was lower than that in control group,with statistical significance(P<0.05).Pearson correlation analysis showed that serum ATX level was posi-tively correlated with hs-CRP and IL-6,and serum Adropin level was negatively correlated with hs-CRP and IL-6(P<0.05).New York Heart Association(NYHA)functional classification m to Ⅳ and serum ATX lev-el in the poor prognosis group were higher than those in the good prognosis group,the heart failure duration and left ventricular end-diastolic diameter were higher than those in the good prognosis group,the left ventric-ular ejection fraction(LVEF)and serum Adropin levels were lower than those in the good prognosis group,the difference was statistically significant(P<0.05).Logistic regression model analysis showed that NYHA functional classification Ⅲ to Ⅳ and high ATX were risk factors for poor prognosis in DCM patients,and high Adropin and high LVEF were protective factors for poor prognosis in DCM patients(P<0.05).ROC curve a-nalysis showed that the area under the curve(AUC)of serum ATX and Adropin in predicting poor prognosis of DCM patients was 0.841 and 0.793,respectively,and the AUC of combined prediction of poor prognosis of DCM patients was greater than that of single prediction.Conclusion Serum ATX is abnormally elevated and serum Adropin is abnormally decreased in DCM patients,both of which are closely related to inflammatory factors.Detection of serum ATX and Adropin levels can provide reference for prognosis assessment of DCM patients.
4.Expert consensuses on the application of nanopore sequencing technology in the detection of pathogenic micro-organisms

OBJECTIVE To improve the diagnosis and treatment level of critically ill infectious diseases, standardize the clinical application of nanopore sequencing and promote the sound development of the technology. METHODS Division of Therapeutic Drug Monitoring of Chinese Pharmacological Society and Expert Committee of Precision Medicine for Clinical Treatment of Guangdong Pharmaceutical Association initiated and organized multidisciplinary experts to discuss and determine the consensus writing outline by using the nominal group method, forming a preliminary consensus draft; expert consultation was performed by using Delphi method, and then experts’ opinions were analyzed and revised to form consensus. RESULTS & CONCLUSIONS Consensuses of Experts on the Application of Nanopore Sequencing Technology in the Detection of Pathogenic Microorganisms covers targeted sequencing, metagenomic sequencing and whole genome sequencing, and is standardized in terms of sample collection and storage, detection process, bioinformatics analysis and report interpretation; the recommendations are provided for the key issues.
5.Research and practice of entrepreneurial practice in cultivating innovative ability of medical laboratory technology students

Setting up the innovative and entrepreneurship education and teaching system is the important content of the current university education reform. Through the idea of leading innovation with entrepreneurial practice, we have implemented a series of education reforms in the students of medical laboratory technology, reformed the teaching methods and content, set up the tutors for undergraduates and improved the scientific research ability of students. The results show that the education reforms have enhanced the ability and sense of innovation, providing a good reference for the reform of innovation and entrepreneurship education in medical laboratory technology.
6.Therapeutic effect of the treatment combined with hyperbaric oxygen on 36 patients with herpes zoster

Objective:To observe the clinical effect of the treatment combined with hyperbaric oxygen (HBO) on patients with herpes zoster.Methods:A total of 72 patients with herpes zoster treated in the Department of Dermatology of Zhengzhou Second Hospital from January 2018 to December 2019 were selected and divided into HBO group and control group by random number table method, with 36 patients in each group. The control group received acyclovir and other conventional treatments, while the HBO group received HBO on the basis of the treatments of the control group. The improvement of the skin lesions, scores of numeric rating scales (NRS) on pain, and efficacy of the two groups were observed and evaluated after treatment. After a 3-month follow-up, the occurrence of postherpetic neuralgia (PHN) was statistically analyzed.Results:The time of blistering relieving, incrustation, and complete incrustation of the HBO group were all lower than those of the control group with statistically significant difference ( P<0.05). The NRS scores of both groups were all lower than those before treatment with significantly statistical difference ( P<0.01); and the NRS scores of the HBO group were significantly lower than those of the control group after treatment ( P<0.05). The total effective rate of the HBO group (91.67%) was significantly higher than that of the control group (83.33%). The difference was statistically significant ( χ2=4.31, P=0.04). After treatment, 3 cases of PHN occurred in the HBO group, while 7 cases in the control group; the PHN incidence of the HBO group was lower than that of the control group with significantly statistical difference ( P<0.05). Conclusion:In treating herpes zoster, the treatment combined with HBO can achieve a satisfactory therapeutic effect, which is worthy of popularization and application in clinic.
7.Effect of frailty phenotype on activities of daily living in maintenance hemodialysis patients

Objective:To investigate the status of frailty and activities of daily living (ADL) in maintenance hemodialysis (MHD) patients, and to explore the effect of frailty phenotype on ADL.Methods:The patients who underwent MHD in Kidney Disease Center of the First Affiliated Hospital from March 2019 to March 2020 were enrolled in this study. The demographic and laboratorial data were collected by cross-sectional survey method. Fried frailty phenotype scale and ADL scale were used to evaluate the frailty and ADL, respectively. The differences of basic data and different frailty phenotypes between the normal function group and the function decline group were compared in terms of ADL, physical self-maintenance ability and instrumental ADL ability. Pearson correlation analysis was used to analyze the correlation between frailty and ADL, and binary logistic regression analysis was used to analyze the influencing factors of ADL.Results:A total of 676 MHD patients were included in this study, including 434 males (64.2%) and 242 females (35.8%). The age was (59.2±19.4) years old, and the median dialysis age was 59.0 (25.3, 110.0) months. There were 159 frailty patients (23.5%), 230 pre-frailty patients (34.0%), and 287 non-frailty patients (42.5%). The ADL was decreased in 163 patients (24.1%), including 131 patients (19.4%) with decreased physical self-maintenance ability and 161 patients (23.8%) with decreased instrumental ADL ability. Pearson correlation analysis showed that the frailty score was positively correlated with total ADL score ( r=0.728, P<0.001), physical self-maintenance ability score ( r=0.669, P<0.001) and instrumental ADL ability score ( r=0.729, P<0.001). Binary logistic regression analysis results showed that older age and lower physical activity, fatigue, slowed steps and lower grip strength in the frailty phenotypes were the independent influencing factors of ADL, physical self-maintenance ability and instrumental ADL ability (all P<0.05). Conclusions:The prevalence of frailty is 23.5% in MHD patients, and 24.1% of MHD patients have decreased ADL. Elder age and lower physical activity, fatigue, reduced step counts, and lower grip strength in frailty phenotypes are the independent influencing factors for poor ADL, poor physical self-maintenance ability and poor instrumental ADL ability.

Objective:To observe and evaluate the clinical effect of hyperbaric oxygen combined therapy on patients with non-arteritic anterior ischemic optic neuropathy (NAION).Methods:A total of 110 patients with NAION admitted to the Department of Ophthalmology of Zhengzhou Second Hospital from January 2017 to January 2019 who met the inclusion criteria were selected and divided into control group (55 cases) and hyperbaric oxygen group (55 cases) by random number table method. The control group was given conventional therapies such as hormone shock, microcirculation improvement, and nerve nutrition, while the hyperbaric oxygen group was given conventional treatment and hyperbaric oxygen therapy, with chamber pressure of 2.0 ATA, once a day, 10 times for a course of treatment for 30 days. The visual acuity, mean sensitivity (MS), mean defect (MD), pattern visual evoked potential (PVEP) amplitude, and latency period of the two groups were observed and analyzed to evaluate the efficacy of treatment. The effect of hyperbaric oxygen combined therapy on serum human chondroglycoprotein-39 (chtinase-3-like-1 protein, YKL-40) in patients with NAION was also observed.Results:Before treatment, there were no statistically significant differences in visual acuity, mean sensitivity, mean defect, PVEP amplitude, latency period, and serum YKL-40 between the two groups ( P>0.05). The visual MD in the hyperbaric oxygen group after treatment was lower than that in the control group after treatment and also lower than that before treatment in the same group, and the differences were statistically significant ( P<0.05). The visual acuity and MS in the hyperbaric oxygen group after treatment were higher than those in the control group and also those before treatment in the same group, and the differences were statistically significant ( P<0.05). The PVEP amplitudes of the patients in the hyperbaric oxygen group were higher than those in the control group and also those before treatment in the same group, and the latency period was earlier than that in the control group and also that before treatment in the same group, with statistically significant differences ( P<0.05). The level of serum YKL-40 reached the peak after 7 days of treatment. After 3, 7, 15 and 30 days of treatment, the levels of serum YKL-40 were all lower than those of the control group on the same day, with statistically significant differences ( P<0.05). After treatment, the total effective rate of the hyperbaric oxygen group (83.63%) was higher than that of the control group (74.55%), but there was no statistically significant difference ( P>0.05). Conclusion:Hyperbaric oxygen combined therapy can significantly improve the vision acuity, MS, MD, PVEP amplitude, and latency period of the patients with NAION, and reduce inflammatory response, which is beneficial to improve clinical efficacy.
