Objective To enhance medical service efficiency and optimize healthcare resource utilization, our hospital developed a novel one-stop integrated medical service model combining pre-hospitalization with day surgery. Methods Starting in August 2022,a Class A Tertiary Hospital in Guangzhou implemented a multi-dimensional collaborative mechanism:1.Process reengineering:Standardized workflows shifted preoperative tests and anesthesia evaluations to pre-hospitalization.2.Resource in-tegration:Established a one-stop medical service center as a comprehensive service coordination hub, integrating deposit pay-ment,examination scheduling, testing, medical check-ups,bed allocation,and"one-click admission"into a unified diagnostic and treatment service chain.3.Closed-loop management:Streamlined workflow from outpatient evaluation to follow-up.Results By 2024 vs 2022:Increased annual discharges by 30,000+cases.Reduced preoperative hospitalization by 0.4 days.Im-proved bed occupancy(+10.72%)and turnover(+10.86%).Achieved 94%patient satisfaction. Conclusion This model enhances bed efficiency, reduces hospitalization delays, and offers a scalable framework for healthcare optimization,demonstra-ting both social and operational benefits.

Objective:To investigate the host and vector composition of pestis and the epidemic situation of pestis among animals in the border port areas of Longchuan County, Yunnan Province. Methods:In September 2022, a survey was conducted on rodents and their surface parasitic fleas in the border port areas of Longchuan County, according to three habitat types: residential areas, agricultural areas, and forest-shrubbery areas. Samples of murine animals organs and their surface parasitic fleas were collected for isolation and identification of Yersinia pestis. Blood samples of plague indicating animals, such as murine animals, dogs and cats were collected, and serum plague F1 antibody was detected by indirect hemagglutination assay. Meanwhile, a retrospective investigation was conducted on the occurrence of self dead rats, sick rats, and suspected cases in the local area from 2018 to 2022 through interviews with farmers. Results:A total of 168 murine animals belonging to 3 orders, 5 families, 10 genera, and 11 species were captured in three habitats in Longchuan County. Among them, the capture rate in residential areas was 5.00% (30/600), with Rattus tanezumi and Suncus murinus as dominant species, with a composition ratio of 50.00% (15/30). The capture rate in agricultural areas was 9.67% (122/1 262), with Rattus tanezumi and Suncus murinus as dominant species, with a composition ratio of 50.82% (62/122) and 44.26% (54/122), respectively. The capture rate in the forest-shrubbery areas was 6.25% (16/256), with Hylomys suillus and Eothenomys eleusis as dominant species, with a composition ratio of 37.50% (6/16) and 31.25% (5/16), respectively. Among the captured murine animals, 20 individuals carried 52 parasitic fleas, belonging to 2 species of 2 genera and 2 families. The total flea infection rate was 11.90% (20/168), and the total flea index was 0.31 (52/168). The dominant specie was Xenopsylla cheopis (90.38%, 47/52). The flea infection rate in residential areas was 33.33% (10/30), and the flea index was 1.23 (37/30). The flea infection rate in agricultural areas was 7.38% (9/122), and the flea index was 0.11 (14/122). The flea infection rate in the forest-shrubbery areas was 6.25% (1/16), and the flea index was 0.06 (1/16). The samples of murine animals and their parasitic fleas obtained were isolated and cultured by Yersinia pestis, and the results were negative. A total of 144 serum samples from murine animals, dogs and cats were separated, and no F1 antibody against pestis was detected. According to interviews and investigations, no abnormal situations such as a large number of self dead rats, sick rats, and suspected cases were found from 2018 to 2022. Conclusions:No plague epidemic has been found in the border port areas of Longchuan County recently. The main host of plague, Rattus tanezumi, and the main vector, Xenopsylla cheopis, remain the dominant species in this area.

Objective Construction and Practice of a Whole-Process Digitalized Hemodialysis Management System. Methods ① Integrate device data through the Internet of Things(IoT)to enable automatic collection of dialysis parameters and real-time validation of quality control rules.② Build an"intelligent error prevention system"based on Data Analysis, realizing the transformation from patient management to digital quality control management in hemodialysis treatment.③ Implement a closed-loop management system for the entire patient dialysis process. Results The system reduced clinicians' time spent manu-ally filling and verifying documentation by 75%,and increased the adequacy compliance rate for maintenance dialysis patients to 100%per quarter and other goals. Conclusion Traditional hemodialysis management relies on manual records, leading to issues such as fragmented data, delayed quality control, material waste, and high risks of hospital-acquired infections. The entire process, from patient appointment to post-dialysis summary management lacks coordination, and manual calculation of key indica-tors is prone to errors, with insufficient timeliness in quality surveillance warnings. The construction of an information system can break down operational silos and enable"data-driven decision-making",and provides a replicable model for the digital manage-ment of hemodialysis.

A 57-year-old female patient with small cell lung cancer was treated with serplulimab (100 mg by intravenous infusion on the first day, 21 days as a cycle). After the 10th cycle of treatment, the patient suddenly presented drowsiness, shortness of breath, dry mouth, nausea, vomiting, fatigue and other symptoms. Laboratory tests showed fasting blood glucose 38.9 mmol/L, glycosylated hemoglobin 7.9%, serum C peptide 0.2 μg/L, pH value 7.05, anion gap 31 mmol/L, anti-islet cell antibody 36 kU/L, anti-glutamic acid decarboxylase antibody 131 kU/L, urine ketone body (+++). The patient was diagnosed with fulminant type 1 diabetic ketoacidosis, which was considered to be related to serplulimab. The drug was stopped, and insulin, rehydration, correction of acid-base imbalance, and other treatments were given. After 3 days, the patient′s consciousness returned to normal. After 12 days, her breathing was stable, dry mouth, nausea, vomiting and other symptoms were relieved. Laboratory tests showed random blood glucose 13.6 mmol/L, pH value 7.44, anion gap 6 mmol/L, and urine ketone body negative. After 26 days, her random blood glucose was controlled at 10.0-12.0 mmol/L.

Hepatic ischemia-reperfusion injury (HIRI) is the main cause of liver damage and even multiple organ failure after complex liver surgery. When liver ischemia reperfusion occurs, the non-coding RNAs in the liver tissue is dysregulated and part of the non-coding RNAs with abnormal expression is involved in HIRI regulation.Non-coding RNAs to may be the intervention target for reducing HIRI.This article summarized the types and related functions of non-coding RNAs,the role of different non-coding RNAs in HIRI,and the interconnections between various non-coding RNAs in HIRI.

Journal of Clinical Hepatology 2019;35(8):1885-1888
Cholangiocarcinoma is a malignant tumor originating from the epithelial cells of the bile duct, and its incidence and mortality rates keep increasing year by year. Unknown pathogenesis of this disease has led to the difficulties in early diagnosis and treatment. With the development of genetic engineering and bioinformatics, more and more studies have found that long non-coding RNA (lncRNA) is closely associated with the pathophysiological process of cholangiocarcinoma. This article summarizes the research advances in the role of lncRNA in mediating the biological behaviors of cell proliferation, apoptosis, metastasis, invasion, epithelial-mesenchymal transition, and drug resistance in cholangiocarcinoma, as well as the association of lncRNA with clinical features and prognosis.
