1.Influence of COVID-19 prevention and control on the epidemic trend of notifiable infectious diseases in the first quarters, Zhejiang Province

ObjectiveTo determine the influence of COVID-19 prevention and control on the epidemic characteristics and dynamics of notifiable infectious diseases in the first quarters, Zhejiang Province, and to explore more effective countermeasures against infectious diseases. MethodsDescriptive epidemiology was conducted to determine the change in notifiable infectious diseases during the prevention and control of COVID-19 in Zhejiang Province by retrieving the data of notifiable infectious diseases from 2017 to 2022 in the Chinese information system for disease control and prevention. Cumulative reported new cases of notifiable infectious diseases in the first quarters of 2017‒2019 were compared with that of 2020‒2022. ResultsA total of 546 753 cases of notifiable infectious diseases were newly reported in the first quarters of 2017‒2019, with an average incidence of 321.92/105. In contrast, a total of 509 908 cases of notifiable infectious diseases were newly reported in the first quarters of 2020‒2022, during which the COVID-19 epidemic occurred, with an average incidence of 270.39/105. The incidence in 2020‒2022 significantly declined by 51.53/105, compared with that in 2017‒2019 (χ²=8 072.06, P<0.001). In the first quarters of 2020‒2022, the average incidence of zoonotic diseases and vector-borne diseases decreased by more than 50%. In addition, the incidence of respiratory, enteric, blood-borne, and sexually transmitted diseases declined to certain degree. ConclusionThe decline in the newly reported cases of non-COVID-19 notifiable infectious diseases in the first quarters of 2020‒2022 indicates that the countermeasures against COVID-19 epidemic, such as multi-disease co-prevention, multi-sectoral collaboration, societal mobilization and personal hygiene and protection, may also decrease the incidence of multiple infectious diseases. It suggests the countermeasures are effective, which would provide evidence for routine prevention and control of infectious diseases in future.
2.Research Progress of Androgen/Androgen Receptor Signaling Pathway in Hepatocellular Carcinoma

Hepatocellular carcinoma (HCC) is a kind of primary liver cancer with a high mortality rate. In China, the incidence ratio in males to females with HCC is 2:1–5:1. The difference in sex hormone pathways between males and females and the interaction between androgen/androgen receptors and HBV can lead to an incidence difference between males and females with HCC. Hence, the androgen/androgen receptor oncogenic pathway in hepatocellular carcinoma has received considerable attention. This review mainly summarizes the recent research progress on the androgen/androgen receptor oncogenic pathway in hepatocellular carcinoma.
3.Analysis of related factors of concomitant hypertension and diabetes among rural residents in Shanghai, China

ObjectiveTo investigate the related factors of hypertension and comorbid hypertension-diabetes mellitus (HT-DM) among rural residents in Shanghai, China. MethodsA total of 2 159 residents over 35 years old were randomly selected from a rural community in the suburbs of Shanghai through cluster random sampling. Their basic sociological characteristics and blood lipid levels were investigated. Chi-square test was used to analyze the relationship between hypertension (or HT-DM) and gender, age, education levels, marital status, original household registration and current household registration. The relationship between hypertension (or HT-DM) and blood lipid level was analyzed by a conditional logistic regression model. ResultsThe prevalence of hypertension was 26.35% and that of HT-DM was 4.68% in the study population. Chi-square test analysis showed that hypertension was significantly associated with age, marital status, educational level, and rural household registration, while HT-DM was significantly associated with age, educational level, and rural household registration. Chi-square trend analysis indicated that the prevalence of hypertension and HT-DM increased with age in both male and female populations (χ2=129.82, 193.04,both P<0.001, and χ2=9.29, 40.57, both P<0.01). The unconditional logistic regression analysis indicated that the lower level of high-density lipoprotein cholesterol (HDL-C) and higher level of triglyceride (TG) were associated with an increased the risk of hypertension by 2.79 times and 2.06 times, respectively. The lower level of HDL-C increased risk of HT-DM by 4.21 times. ConclusionThe prevalence of hypertension in the rural population in Shanghai is relatively high. Age, education level, rural household registration and HDL-C are common related factors of hypertension and HT-DM.
4.Short-term outcome of ascending aorta replacement combined with total aortic arch fenestration technique for acute type A aortic dissection

Objective To report our clinical experience and outcomes of thoracic endovascular aortic repair (TEVAR) for acute Stanford type A dissection using ascending aorta replacement combined with implantation of a fenestrated stent-graft of the entire aortic arch through a minimally invasive technique. Methods From 2016 to 2020 in our hospital, 24 patients (17 males and 7 females, aged 45-72 years) with complicated Stanford type A aortic dissection, underwent replacement of the proximal ascending aorta with TEVAR. None of the patients with dissection involved the three branches of the superior arch, and all patients were replaced with artificial blood vessels of the ascending aorta under non-hypothermic cardiopulmonary bypass, preserving the arch and the three branches above the arch, and individualized stent graft fenestration. Results Surgical technical success rate was 100.0%. There was no intraoperative complication or evidence of endo-leak in 1 month postoperatively. Hospital stay was 10±5 d. During postoperative follow-up, the stent was unobstructed without displacement, the preserved branch of the aortic arch was unobstructed, and the true lumen of the descending aorta was enlarged. Conclusion This hybrid technique by using TEVAR with fenestrated treatment is a minimally invasive and effective method to treat high-risk patients with acute Stanford type A aortic dissection.
5.Screening strategy on precision prevention strategies for three types of malignant tumors

Malignant tumors can be classified into three categories, rapidly progressing tumors, slowly progressing tumors, and "indolent" tumors. Rapidly progressing tumors (such as liver cancer, pancreatic cancer, and cholangiocarcinoma) have acute onset, shorter time duration from onset to death, and poorer treatment effects, which warrants primary prevention. Slowly progressing tumors (such as lung cancer, colorectal cancer, breast cancer, and gastric cancer) have slow onset, clear precancerous lesions, longer time duration from onset to death, and better therapeutic effects, which is accordingly suitable for secondary prevention. “Indolent” tumors (such as prostate and thyroid cancer) do not affect the life expectancy and are suitable for tertiary prevention. Early screening of “indolent” tumors may lead to overtreatment. Furthermore, early screening of rapidly progressing tumors is difficult to identify early cancers, which results in low cost-effectiveness. In contrast, for slowly progressing tumors suitable for secondary prevention, early screening may have cost-effectiveness, though there might be over-diagnosis. It is crucial to adopt appropriate prevention and treatment strategies for diverse types of tumors. Currently, large-scale cohort studies and randomized controlled clinical trials with complete follow-up may accurately evaluate the effect of cancer prevention strategies. This review discusses the significance of screening in precision prevention of tumors based on the characteristics of tumor progression and patients’ prognosis.
6.Expression of PD-1hiCXCR5-CD4+T cells in patients with systemic lupus erythematosus

Objective To investigate the expression of peripheral programmed death (PD)-1hiCXCR5-CD4+T cells and its clinical significance in systemic lupus erythematosus (SLE). Methods Peripheral blood PD-1hiCXCR5-CD4+ T cells from 21 SLE patients and 16 healthy controls were examined by flow cytometry. The levels of serum anti-double-stranded deoxyribonucleic acid (dsDNA) antibodies were determined using immunoradiometric as-say. Data were analyzed with t test and Pearson's correlation test. Results The per-centages of PD-1hiCXCR5- cells within CD4+ T cell were significantly higher in SLE patients [(2.1 ±2.0)%] compared to normal controls [(0.3±0.3)%] (t=2.959, P<0.01). The percentages of PD-1hiCXCR5-cells within CD4+T cells in moderate to severe active SLE patients (3.0 ±2.0)% was significantly increased compared to patients with mild or inactive (1.0±1.4)%(t=2.574, P<0.05) and normal controls (0.3±0.3)% (t=5.149, P<0.01). The percentages of PD-1hiCXCR5- cells within CD4+ T cells from SLE patients were positively related with systemic lupus erythematosus disease activity index (SLEDAI) (r=0.475, P=0.0297). SLE patients in serum anti-dsDNA antibodies positive group (2.7±2.1)%displayed a higher percentage of PD-1hiCXCR5-cells within CD4+T cells than patients in serum anti-dsDNA antibodies negative group (0.6 ±0.5)% (t=2.303, P<0.05). The percentages of PD-1hiCXCR5-cells within CD4+T cells from SLE patients were positively correlated with anti-dsDNA antibody titers. Conclusion The percentages of PD-1hiCXCR5- cells within CD4+ T cells from SLE patients are increased and are positively correlated with SLEDAI and anti-dsDNA antibody levels. Increased percentage of PD-1hiCXCR5-cells within CD4+T cells might play an important role in the pathogenesis of SLE.
7.Related risk factors for progressive occlusion of intracranial aneurysms treated with low-profile visualized intraluminal support device stent in mid-term follow up

Objective To analyze the angiographic results of intracranial aneurysms without complete embolization immediately after low-profile visualized intraluminal support device (LVIS) stent at mid-term follow up,and explore the risk factors for healing of partial occluded aneurysms.Methods One hundred and sixty-one patients with intracranial aneurysms treated by LVIS stent embolization in our hospital from December 2014 to December 2018 were selected;193 aneurysms in total,including 93 un-ruptured aneurysms and 100 ruptured aneurysms,were noted.DSA was performed immediately after operation to evaluate the degrees of aneurysm embolization according to Raymond grading criteria.The embolization degrees of aneurysms were compared at 8 months after surgery and immediately after surgery,and the healing of aneurysms (Raymond grading 1) was calculated.Univariate Logistic regression analysis and multivariate Logistic regression analysis (forward maximum likelihood ratio method) were used to screen the risk factors for healing of incomplete aneurysm embolization.Results The embolization degrees immediately after surgery were as follows:78 were with Raymond grading 1,54 with Raymond grading 2,and 61 with Raymond grading 3;complete aneurysm embolization were noted in 78 aneurysms (40.41%) and incomplete aneurysm embolization were noted in 115 (59.59%).Follow up (8 months after surgery) results indicated that,of the 193 aneurysms,171 were with Raymond grading 1,10 with Raymond grading 2,and 12 with Raymond grading 3;there were 171 aneurysms (88.60%) having complete aneurysm embolization and 22 (11.40%) having incomplete aneurysm embolization.In the aneurysms having incomplete aneurysm embolization immediately after surgery,the healing rate was 81.74% (94/115).Univariate Logistic regression analysis showed that hypertension,diabetes mellitus,posterior circulation aneurysm,dissecting aneurysm,body-neck ratio and embolization degrees immediately after surgery were risk factors for healing of aneurysms (P<0.05).Multivariate Logistic regression analysis revealed that posterior circulation aneurysm and diabetes mellitus were independent risk factors for healing of aneurysms (P<0.05).The area under the curve of receiver operating characteristic curve of the regression model was 0.755,indicating that the prediction efficiency of the regression model was moderate.Conclusion Treatment of intracranial aneurysms with LVIS stent is effective,and the complete occlusion rate is high,even in the aneurysms with incomplete embolization immediately after surgery;aneurysms located in the posterior circulation and associated with diabetes can affect the treatment of aneurysms.

It reviews the nurse-led follow-up visit from the perspective of continued nursing care, in order to provide a reference for China to further standardize the follow-up content for patients with enterostomy.
9.Curative effect of arthroscopic treatment for subacromial impingement syndrome

Objective To analyze the clinical curative effect of arthroscopic acromioplasty treatment for subacromial impingement syndrome(SAIS). Methods A total of 52 patients who had SAIS were studied in our hospital by using the Neer score evaluation, the results of the pre-operative and post-operative scores were statistically analyzed. Results All the 52 cases were followed up, the follow-up ranged from 3 to 24 months with an average of 9 months. According to the Neer scoring system, 34 cases less than 70 points in the pre-operation were poor, 14 cases with 71-79 points were fair, 4 cases with 80-89 points were good. And 23 cases with 71-79 points in the post-operation were fair, 24 cases with 80-89 points were good,5 cases with more than 90 points were excellent. The Neer score was(55.25±19.00)before operation, and(81.13±6.28)after operation. There was a significantly statistical difference (t=-9.33, P<0.05). Conclusion The pain and activity of the patients were improved by the arthro scopic acromioplasty. And it has many advantages,such as faster post-operative recovery,low risk of complications and so on.
10.Intensive care on one case of combined heart-liver transplantation

Objective To summarize the diagnosis and treatment of one case of combined heart liver transplantation. Methods On November 24, 2011, one case of combined heart-liver transplantation was performed on a patient with Ebstein's anomaly and tricuspid valve replacement after 5 years,complicated with congestive cirrhosis,liver failure dccompensation,preoperative heart failure Ⅲ degree and B grade of liver function Child-Pugh score. The operation was done with the graded cardiopulmonary bypass assisted mode:first creating the vena cava-aortic bypass to complete heart transplantation, second creating the femoral vein-ascending aorta bypass to complete liver transplantation,and third stopping and neutralizing.The aortic cross-clamping time was 54 min and the an hepatic phase was 38 min.The total time of three times of cardiopulmonary bypass was 199 min and the total time-consuming of operation was 517 min. The patient was given basiliximab +methylprednisolone for immune induction therapy, and tacrolimus + mycophenolate mofetil +prednisone solution for anti-rejection. After operation, liver protecting treatment, anti-infection therapy and nutrition support therapy were given.Results The recipient died of multiple organ failure after 78 days.The mechanical ventilation treatment duration for this recipient was 78 days and ECMO adjuvant therapy for postoperative hypoxemia time lasted 63 days.Conclusion The combined heart liver transplantation is an effective measures for treatment of heart and liver failure.
