Cholesterol is a crucial lipid in the human body.Elevated levels of cholesterol can result in condi-tions such as hypercholesterolemia.3-hydroxy-3-methylglutaryl coenzyme A reductase(HMGCR)serves as a pivotal en-zyme in the synthesis of cholesterol, acting as a rate-limiting factor. As a result,HMGCR plays a critical role in maintain-ing cholesterol balance, with the regulatory processes being intricate in vivo. This review outlines the advancements in un-derstanding the regulatory mechanisms of HMGCR,encompassing transcriptional control, degradation pathways, and en-zyme activity.

ObjectiveTo investigate the effects of Guizhi Fulingwan on oxidative stress and renal fibrosis in diabetic mice based on the nuclear factor E2-associated factor 2/antioxidant response element (Nrf2/ARE) pathway, and to explore its mechanism of action. MethodA diabetic mouse model was established by feeding high-fat diet combined with intraperitoneal injection of streptozotocin. The diabetic mice were divided into a model group, a metformin group (250 mg·kg-1), and Guizhi Fulingwan low, medium, and high dose groups (400.4, 800.8, 1 601.6 mg·kg-1), with 10 mice in each group. Another 10 mice were included in a normal group. The treatment was conducted continuously for 8 weeks. Body mass and blood glucose were recorded, and the renal index was calculated. Triglyceride (TG), total cholesterol (TC), urea nitrogen (BUN), serum creatinine (SCr), superoxide dismutase (SOD), and malondialdehyde (MDA) levels were measured. Pathological changes in renal tissues were observed using Hematoxylin-eosin, periodate-Schiff, and Masson staining. The protein levels of α-smooth muscle actin (α-SMA), fibronectin (FN), and transforming growth factor-β1 (TGF-β1) were detected by immunohistochemistry. The protein levels of Nrf2, Kelch-like ECH-associated protein 1 (Keap1), SOD1, NAD(P)H single-nucleotide oxidase 1 (NQO1), and heme oxygenase-1 (HO-1) were detected by Western blot. ResultCompared with the normal group, the model group showed elevated glucose, renal index, TG, TC, BUN, and SCr levels, thickened glomerular basement membrane, increased collagen fibers in renal tissues, decreased SOD activity, elevated MDA content, increased protein levels of FN, TGF-β1, α-SMA, and Keap1, and decreased protein levels of Nrf2, SOD1, NQO1, and HO-1 (P<0.01). Compared with the model group, the Guizhi Fulingwan and metformin groups showed reduced blood glucose, renal index, TG, TC, BUN, and SCr levels, improved renal histopathological injury, significantly increased SOD activity, significantly reduced MDA content, reduced protein levels of FN, TGF-β1, α-SMA, and Keap1, and increased protein levels of Nrf2, SOD1, NQO1, and HO-1 (P<0.05, P<0.01). ConclusionGuizhi Fulingwan can improve oxidative stress and inhibit renal fibrosis, and its mechanism may be related to the activation of the Nrf2/ARE signaling pathway.

Objective To observe the effect of core stability training combined with eye acupuncture on motor function in patients with hemiplegia after stroke. Methods Eighty patients with hemiplegia after stroke were randomly divided into a treatment group and a control group (40 cases in each). Both groups were given regular rehabilitation training, but the patients in the treatment group also were taught core stability training of their waist and abdominal muscle groups, their shoulder muscles, trunk and pelvis. They were also given eye acupuncture.A trunk control test (TCT) , Berg's balance scale (BBS), the modified Barthel index (MBI), functional ambulation categories (FACs) and the Fugl Meyer assessment scale (FMA) were used to assess motor function before and after treatment. Results At the end of 6 weeks of treatment, all of the measurements in both groups had significantly improved. The average TCT, BBS, MBI, FMA, and FAC scores of the treatment group significantly exceeded those of the control group. Conclusions Core stability training and eye acupuncture together can significantly improve the motor function of hemiplegic stroke survivors. The combination therapy is worthy of clinical popularization and application.

Objective To evaluate the therapeutic effetive, toxicity and survial rate of non-operation esophageal carcinoma patients treated by tegafur plus cisplatin combined with three-dimensional conformal radiotherapy (3D-CRT).Methods 184 cases of non-operation patients with esophageal carcinoma were divided into two groups. The cases of treatment group were treated by cisplatin and tegafur. The cases of control group were treated by cisplatin and 5-Fu.All patients received 3D-CRT,and the total radiation dose was 50-70 Gy.Each group had 14 cases treated with primary chemotherapy for 1 course,28 cases for 2 courses, and 50 cases without any chemotherapy. Results The recent effect rate was 69.57 % (64/92) in treatment group and 67.39 % (62/92) in control group, there was no statistical difference (x2 =0.101,P =0.874).Radioactive esophagitis and leukopenia were the main side effect of treatment. The treatment group was significantly lower than that of the control group, the difference had statistical significance. The 1-,2-and 3-year survival rates were 74.9 %,61.6 %,52.8 %,respectively in treatment group and 75.8 %,56.9 %,48.0 %,respectively in control group. The control group survival rates were significantly higher than the treatment group, the difference had statistical significance (x2 =7.325,P =0.007).Conclusions Cisplatin plus tegafur or 5-Fu combined with 3D-CRT is effective way of treatment for non-operation of esophageal carcinoma. The toxicity of cisplatin plus tegafur group is significantly lower than cisplatin plus 5-Fu group, therefor it can improve the quality of life of the patients obviously.
