1.Status of Clinical Practice Guideline Information Platforms

Clinical practice guidelines represent the best recommendations for patient care. They are developed through systematically reviewing currently available clinical evidence and weighing the relative benefits and risks of various interventions. However, clinical practice guidelines have to go through a long translation cycle from development and revision to clinical promotion and application, facing problems such as scattered distribution, high duplication rate, and low actual utilization. At present, the clinical practice guideline information platform can directly or indirectly solve the problems related to the lengthy revision cycles, decentralized dissemination and limited application of clinical practice guidelines. Therefore, this paper systematically examines different types of clinical practice guideline information platforms and investigates their corresponding challenges and emerging trends in platform design, data integration, and practical implementation, with the aim of clarifying the current status of this field and providing valuable reference for future research on clinical practice guideline information platforms.
2.Longitudinal association between compulsive behaviour and smartphone addiction in middle school students

Objective:
To explore the potential causal association between adolescent compulsive behaviour and smartphone addiction based on longitudinal data, so as to provide reference for the establishment of adolescent smartphone addiction interventions.
Methods:
A preliminary survey and follow-up were conducted on 8 907 middle and high school students in a district of Shenzhen in 2022 and 2023, respectively. Compulsive behaviours were measured by using the Mental Health Inventory for Middle School Students-60 Items (MMHI-60), smartphone addiction was assessed by using the Smartphone Addiction Scale-Short Version ( SAS- SV), and the associations between compulsive behaviours and smartphone addiction were analysed by using multilevel mixed-effects models and subgroup analyses.
Results:
Smartphone addiction detection rates among middle school students were significantly associated with genders, father s education level, mother s education level, study load subgroups, and whether or not they were single-parent families, and there were statistical differences ( χ 2=17.21-175.34, P <0.05). Students with compulsive behaviours were 2.98 times more likely to develop smartphone addiction than those without compulsive behaviours ( OR=2.98, 95%CI=2.77-3.22, P <0.05). Subgroup analysis of middle school students without smartphone addiction in the first year found that compulsive behaviours significantly predicted smartphone addiction ( OR= 1.76 , 95%CI=1.54-2.01, P <0.05).
Conclusion
There is a potential causal association between obsessive-compulsive behaviours and smartphone addiction in middle school students, and obsessive-compulsive behaviours in middle school students could significantly predicted the occurrence of smartphone addiction.

4.Molecular epidemiological characteristics and risk factors of HvKp infection in Hulunbuir area

Objective To investigate the molecular epidemiology of hypervirulent Klebsiella pneumoniae(HvKp)and its independent risk factors for infection,and to provide research basis for anti-infection treat-ment.Methods A total of 519 Klebsiella pneumoniae strains were collected from Inner Mongolia Forestry General Hospital from January 2020 to December 2022.String test was used to distinguish hypermyxoid strains(HMV-Kp)and non-HMV-KP.The rmpA,rmpA2,and iutA genes were detected by common PCR agarose gel electrophoresis to screen the HvKp strains.Multilocus sequence typing analysis was performed on 60 HvKp strains and the minimum spanning tree was drawn.Multivariate Logistic regression was used to ana-lyze the risk factors of HvKp infection.Results The positive rate of HMV-Kp was 39.69%,the positive rate of HvKp was 37.19%,and HMV-Kp accounted for 76.68%of HvKp.The detection rate of HvKp in general surgery department was the highest,and the detection rate of HvKp in pus specimens was the highest.By ST typing comparison,a total of 18 types of 60 HvKp strains were detected,ST23 type was the most common type(50.00%),followed by ST86 type(8.33%).Multivariate Logistic regression analysis showed that male,liver abscess,infection or suppuration of other tissues and organs,and use of macrolide antibiotics in the past 3 months were independent risk factors for HvKp infection(P<0.05).Conclusion There is a strong associa-tion between HvKp and HMV-Kp strains,and ST23 type is the dominant type in this study.Male,liver ab-scess,infection or suppuration of other tissues and organs,and use of macrolide antibiotics in the past 3 months are independent risk factors for HvKp infection.
5.A qualitative study on the psychological recovery pathway and support needs of the second victim of nurses

Objective:To explore the psychological recovery pathway and support needs of second victims of nurses who have experienced medical adverse events through qualitative research.Methods:From June to December 2022, a purposive sampling method was used to select 20 nurses who had experienced level Ⅲ or above adverse events and voluntarily participated in this study from 3 grade A tertiary hospitals in Hubei Province and 1 grade A tertiary hospital in Sichuan Province. Semi-structured interviews were conducted with the research subjects and the data was analyze by using thematic analysis.Results:The psychological recovery path of nurses who had experienced medical adverse events included three stages, named as the plateau period, stress response period, and coping and growth period. The second victim of nurses hoped to receive support from the department′s environmental culture, organizational atmosphere, and non punitive culture, and nurses in the platform stage had a stronger need for the first two types of support; the nurses preferred emotional support from superiors, colleagues, family/friends during the stress response period, and preferred knowledge learning and training support during the coping and growth period.Conclusions:The psychological recovery of the second victim of nurses will go through a plateau period, a stress response period, and a coping and growth period. There are differences in support needs during each period, and more targeted intervention plans should be developed.
6.Lipid metabolism distribution in patients with tick-borne encephalitis based on liquid chromatography-mass spectrometry

Objective:To analyze the metabolic status of forest encephalitis patients, research the effect of forest encephalitis on lipid metabolism, and clarify the possible pathogenesis.Methods:Based on liquid chromatography-mass spectrometry (LC-MS), lipidomics analysis was performed in 50 patients with tick-borne encephalitis and 39 healthy samples from Hulunbuir region. The patients′ serum samples were analyzed by utilizing the multivariate statistical analysis method such as principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA).Results:A total of 465 peaks were detected in the samples, and metabolites such as phospholipids and glycerides were identified. To identify the significant differential metabolites ( P<0.01, VIP>1, FC>2), a total of 26 biomarkers were screened, which phospholipids tend to be upregulated[log 2(Fold change)>0], such as phosphatidylcholine (PC) and phosphatidylethanolamine (PE); while glycerides tend to be decreased[log 2(Fold change)<0], such as diacylglycerol (DAG) and triacylglycerol (TAG), and those metabolites were closely related to anti-inflammatory and other metabolic pathways. The AUC value of potential biomarkers obtained was 0.999, which could be used for diagnosis of disease group and healthy group. Conclusions:The study showed that phospholipid metabolism and glyceride metabolism pathways would have some change after human infected by virus, and the differential metabolites could be used as potential markers for the diagnosis of tick-borne encephalitis, which provide a theoretical basis for disease research.
7.The impact of mindfulness meditation intervention on the occupational identity and turnover intention of the second victim of nurses

Objective:To explore the influence of mindfulness meditation intervention on the professional identity and turnover tendency of the second victim of nurses.Methods:The convenience sampling method was used to select the second victim of 46 nurses who experienced medical adverse events within six months from January to June 2019 in Zhongnan Hospital of Wuhan University, and then an 8 weeks of mindfulness meditation intervention was carried out for them. In the entire intervention process, a self-control study was conducted, the Second Victim Experience and Support Scale (SVSET) and the Nurse Occupational Identity Scale were used to assess nurses' turnover intentions, absence and professional identity before the intervention and at 2 hours, 4 weeks, and 8 weeks after the intervention.Results:By analyzing the data, the results showed that after the mindfulness meditation intervention, the second victim of the nurse before intervention, 2h, 4 weeks, and 8 weeks of intervention were 3.28±0.87, 3.20±0.83, 2.85±0.92, 2.71±0.87, the difference was statistically significant ( F time value was 6.609, P<0.05). Before and after the intervention of the sense of grasp in professional identity were 4.91±0.83, 5.47±0.81, 5.83±0.91, 5.94±0.80, the difference was statistically significant ( F time value was 20.973, P<0.01). Consensus before and after intervention were 4.61±0.78, 5.09±0.85, 5.54±1.12, 5.76±0.85, the difference was statistically significant ( F time value was 22.237, P<0.01). The meaningful sense before and after intervention were 4.38±1.03, 5.04±0.75, 5.44±0.86, 5.59±0.76, the difference was statistically significant ( F time value was 23.656, P<0.01). The self-efficacy before and after intervention were 4.71±0.82, 5.33±0.63, 5.83±0.84, 5.90±0.77, the difference was statistically significant ( F time value was 28.866, P<0.01). The self-determination before and after intervention were 4.43±0.97, 5.06±0.81, 5.59±0.91, 5.62±0.87, the difference was statistically significant ( F time value was 25.237, P<0.01). Before and after intervention, the influence of tissue was 4.17±0.96, 4.71±0.85, 4.96±0.99, 5.30±0.88, the difference was statistically significant ( F time value was 16.487, P<0.01). Before and after intervention, the patients' feelings were 4.59±0.96, 5.08±0.84, 5.70±0.83, 5.83±0.68, the difference was statistically significant ( F time value was 25.406, P<0.01). Conclusion:This article provides an effective method to help nurses' second victims out of their predicament which confirms that mindfulness meditation intervention can effectively reduce turnover tendency of nurses 'second victims and improve their professional identity.
8.Transgelin inhibits proliferation and invasion of human thyroid cancer cells by blocking the mitogen-activated protein kinase/extracellular signal-regulated kinase signaling pathway

Objective:To explore the role of transgelin(TAGLN) in the occurrence and development of papillary thyroid carcinoma (PTC) and its possible signal pathway.Methods:One hundred cases of PTC tissues and corresponding paracancerous normal thyroid tissues were collected. Realtime quantitative PCR (RT-qPCR), Western blotting, and immunohistochemistry were used to analyze the expression of TAGLN in PTC tissues and corresponding paracancerous normal thyroid tissues. PTC cells were transfected with plasmid and shRNA lentivirus vector respectively to up-regulate or down-regulate the expression of TAGLN in order to detect the effects of them on the proliferation, invasion, and migration by cell proliferation assay(cell counting kit-8, CCK-8)and cell invasion and migration assays (Transwell). The effects of TAGLN on mitogen-activated protein kinase (MAPK)/extracellular-signal regulating kinase (ERK) signal pathway was detected with Western blotting.Results:RT-qPCR showed that there was no difference in the expression of TAGLN mRNA between PTC and corresponding paracancerous normal thyroid tissues ( P>0.05); Western blotting demonstrated that the expression of TAGLN protein in PTC tissues was significantly lower than that in corresponding paracancerous normal thyroid tissues ( P<0.01). Immunohistochemical results revealed that the expression of TAGLN in PTC tissues was significantly lower than that in corresponding paracancerous normal thyroid tissues. Overexpression of TAGLN inhibited the proliferation, invasion, and migration of PTC cells ( P<0.01), but knockdown of TAGLN promoted the proliferation, invasion, and migration of PTC cells ( P<0.01). Overexpression of TAGLN decreased the expression of phosphorylated ERK ( P<0.05), whereas silencing TAGLN increased phosphorylated ERK level in PTC cells( P<0.01). Conclusion:The expression of TAGLN in PTC is significantly decreased. It is related to the occurrence and development of PTC, and its mechanism may be related to MAPK/ERK signal pathway.
9.Research progress on clinical manifestations and influencing factors of medical personnel as the second victims

This paper reviews the literatures on the second victim at home and abroad and introduces the definition of the second victim in patient safety incidents and the controversy over the terms of the second victim. In addition, the clinical manifestations and morbidity of medical staff as second victims, the influencing factors of second victim syndrome and the adverse consequences of second victim syndrome were reviewed. The paper aims to improve the attention of medical institutions to the second victim and the attention of the society to the physical and mental health of medical personnel, so as to provide reference for the follow-up research on medical personnel as the second victim.
