1.Application of active screening on carbapenem-resistant Enterobacterales monitoring in intensive care units:a multi-center study

Objective To evaluate the effectiveness of active screening in improving the detection rate of carbape-nem-resistant Enterobacterales(CRE)in the intensive care units(ICUs).Methods From July 2023 to June 2024,active screening of rectal swab CRE was conducted on ICU patients in 10 hospitals.ICU patients who underwent ac-tive screening from July 2023 to June 2024 were selected as the study group,while those who did not undergo active screening from July 2022 to June 2023 were selected as the control group.Difference in CRE detection rates between the two groups of patients was compared.Results A total of 7 803 ICU patients were included in the study group,744 CRE strains were detected,with a detection rate of 9.53%,out of which 304 CRE strains were detected through routine detection(detection rate 3.90%),3 707 patients underwent active screen,440 CRE strains were detected(detection rate 11.87%).7 561 ICU patients were included in the control group,out of which 250 CRE strains were detected through routine detection,with a detection rate of 3.31%.There was a statistically significant difference in the overall detection rate of CRE between two groups of patients(x2=246.18,P<0.001).In the study group,CRE detection rate of active screening(11.87%)was higher than that of routine detection(3.90%),with statistically significant difference(x2=264.26,P<0.001).A total of 17 CRE strains were detected from the study group.The proportions of Klebsiella pneumoniae(80.92%vs 73.41%)and Serratia marcescens(2.30%vs0.23%)in the routine detection group were both higher than in the active screening group,while the proportion of Escherichia coli in the routine detection group was lower(8.22%vs 19.55%),all with statistically significant differences(all P<0.05).Conclusion The prevalence of CRE in ICUs is relatively high,with a wide range of bac-terial species.Active screening can improve the detection rate of CRE.
2.Formation pathway of sleep deprivation in shift work nurses based on grounded theory

Objective:To explore the formation pathway of sleep deprivation in shift work nurses so as to provide a reference for improving their sleep.Methods:Purposive and theoretical sampling methods were used to select 18 shift work nurses from the clinical departments of six Class Ⅲ Grade A hospitals in Hangzhou City, Zhejiang Province, from January to April 2024 for semi-structured interviews. The interview data were coded and analyzed by searching literature and interviewing shift work nurses, applying Strauss procedural grounded theory to inductively develop relevant concepts and categories.Results:After three levels of coding, 26 initial categories and 10 main categories were refined. Three core categories, namely, doctor-patient tension, institutional mechanism imbalance, and work pressure overload, were finally summarized to construct a formation pathway of sleep deprivation in shift work nurses.Conclusions:External doctor-patient tension, misaligned institutional mechanisms in healthcare organizations, and work pressure overload among shift work nurses are important factors in the formation of sleep deprivation, with work pressure overload being a core factor. It is recommended that sleep deprivation be alleviated through impartial reporting of medical incidents, rational handling of medical disputes, optimization of scheduling arrangements, provision of psychological counselling and enhanced staff training.
3.Amplification effect of hearing mechanics in unilateral hearing loss.

Objective:To evaluate the effectiveness of amplification intervention with hearing aids for restoring binaural auditory function in patients with unilateral moderate to severe sensorineural hearing loss. Methods:This study selected 30 patients with normal hearing in one ear and moderate to severe sensorineural hearing loss in the other ear. They were fitted with hearing aids for the worse ear and underwent more than half a year and one year of adaptation training. The Chinese translation of the Twelve-item version of SSQ(C-SSQ12), angle identification test, speech recognition score(SRS) at different signal-to-noise ratios(SNR=5 and SNR=10) and audiometric thresholds were used to compare the results before and after hearing aid use to evaluate the effectiveness of the unilateral hearing loss intervention. Results:The results of the audiometric thresholds, C-SSQ12 scores, angle identification test, and SRS at SNR=5 and SNR=10 in the worse ear of the unilateral hearing loss patients after hearing aid use were all statistically significant compared to before hearing aid use(P<0.01). Conclusion:Amplification intervention with hearing aids has significant effects on restoring binaural auditory function in patients with unilateral moderate to severe sensorineural hearing loss.

6.Effects of multidisciplinary rehabilitation nursing combined with "3C" intervention in patients after knee surgery with manipulation under anesthesia

Objective:To explore the effects of multidisciplinary rehabilitation nursing combined with "3C" intervention in the nursing of patients after knee surgery with manipulation under anesthesia (MUA) .Methods:From March 2018 to March 2020, convenience sampling was used to select 171 patients after knee surgery with MUA in a rehabilitation center in Shanghai, and patients were divided into experimental group (87 cases) and control group (84 cases) according to admission time. Experimental group implemented multidisciplinary rehabilitation nursing combined with "3C" intervention, and control group implemented conventional rehabilitation. The intervention effects of knee flexion angle, Hospital for Special Surgery (HSS) , rehabilitation knowledge, rehabilitation compliance were compared between the two groups of patients.Results:The passive knee flexion angle of experimental group in the first week of intervention and the active knee flexion angle in the second and tenth weeks of intervention were greater than those of control group, and the differences were statistically significant ( P<0.01) . The HSS score of experimental group after intervention was higher than that of control group, and the difference was statistically significant ( P<0.01) . The experimental group's mastery of rehabilitation knowledge and rehabilitation compliance were higher than those of control group, and the differences were statistically significant ( P<0.01) . Conclusions:The implementation of nurse-led multidisciplinary rehabilitation nursing combined with "3C" intervention for patients after knee surgery with MUA is conducive to improving the patient's grasp of rehabilitation knowledge, compliance with rehabilitation interventions, the patient's knee flexion angle and knee joint function, as well as increasing the patient's satisfaction with nursing work.

8.MRI late gadolinium enhancement of left ventricular apical aneurysms in hypertrophic cardiomyopathy

Objectives To discuss the characteristics of late gadolinium enhancement (LGE) magnetic resonance (MR) imaging in patients with hypertrophic cardiomyopathy (HCM) and left ventricular apical aneurysm (LVAA) and its related prognostic value. Methods Thirty HCM patients with LVAA were collected from August 2004 to August 2013. All cases with coronary artery diseases were ruled out, and all patients underwent LGE derived by cardiac MR (CMR). Five cases of LVAA were pathological confirmed. Atrial and ventricular diameters, apical aneurysm diameters and left ventricular ejection fraction were measured, and apical aneurysm LGE was evaluated. All patients were then followed up. Comparisons in continuous parameters between patients with or without LGE were performed by independent t test. A Cox proportional hazard model was used to estimate the hazard rate for adverse cardiovascular events. Results LGE was identified in 21 LVAAs and non-LGE in 9 LVAAs. Between two groups, there was no significant difference in the size of LVAA [(16.4 ± 11.5) mm vs. (20.3 ± 9.8) mm, P=0.63]. In particular, the complete transition from non-LGE to LGE LVAA was recorded during follow-up in one patient. Pathological findings confirmed that LGE indicated fibrous tissue in LVAA, and LVAA without LGE indicated viable myocardium. The follow-up suggested that the patterns and the size of LVAA were associated with the adverse events in these patients (LGE of LVAA, HR=7.94, P=0.064; the size of LVAA, HR=1.08, P=0.009). Conclusions LGE-MR had important clinical significance in HCM patients with LVAA. LGE in LVAA corresponded with the fibrous tissue and was associated with the prognosis.
9.Relationship Between Right Ventricular Glucose Metabolism and Right Heart Function in Patients With Dilated Cardiomyopathy

Objective: To investigate the relationship between right ventricular (RV) glucose metabolism by18F-fludeoxyglucose positron emission tomography (18F-FDG PET) and right heart function in patients with dilated cardiomyopathy (DCM).
Methods: The18F-FDG PET imaging was performed in 34 consecutive DCM patients, with the reference of Herrero method, the corrected RV standard uptake value (cRVSUV), corrected left ventricular standard uptake value (cLVSUV) and the ratio of RV to LV SUV (cR/L) were obtained. And all 34 patients received cardiac magnetic resonance imaging (cMRI) examination within 7 days, the left ventricular ejection fraction (LVEF) and RVEF were automatically calculated with Simpson’s principle. The pulmonary arterial systolic pressure was measured by echocardiography.
Results: The corrected cRVSUV and cR/L by18F-FDG PET were negatively related to RVEF by cMRI, (r=-0.513, P<0.01) and (r=-0.463,P<0.01) respectively, and meanwhile, the corrected cRVSUV and cR/L were also negatively related to LVEF, (r=-0.387,P<0.01 andr=-0.362,P<0.01) respectively.
Conclusion: Increased RV glucose metabolism closely related to RV dysfunction, the18F-FDG uptake value by PET might be used as an index for assessing the right heart function and prognosis in DCM patients.
10.Relationship between myocardial glucose metabolism in right heart and cardiac function in patients with idiopathic dilated cardiomyopathy

Objective To explore the relationship between myocardial glucose metabolism in the right heart and cardiac function in patients with idiopathic dilated cardiomyopathy (IDCM).Methods From October 2010 to May 2013,64 consecutive patients (45 males,19 females; mean age:(53 ± 12) years) with IDCM were enrolled in this study.All patients underwent 18F-FDG myocardial PET imaging,late-gadolinium enhancement cardiac MRI (cMRI-LGE) and UCG.TrueD software was used to determine the SUVmax in right (SUV-R) and left (SUV-L) ventricular free wall,and the ratio of SUV-R to SUV-L (SUV-R/L) was calculated.The cMRI-LGE was used to evaluate LVEF,RVEF and LGE in left heart.Pulmonary artery systolic pressure (PASP) was measured by UCG.Two-sample t test and Pearson correlation analysis were used to analyze the data.Results Both SUV-R and SUV-R/L had negative correlation with LVEF (r=-0.48,-0.33; both P<0.01) and also with RVEF (r=-0.45,-0.44; both P<0.01).SUV-R/L positively correlated with PASP (r =0.58,P<0.01).SUV-R/L of patients with and without LGE were statistically different (0.45±0.18 vs 0.36±0.12; t=2.50,P<0.05).SUV-R and SUV-L of patients with LGE were 2.0±0.9 and 4.7±1.6,and those of patients without LGE were 1.7±0.9 and 5.0±2.8.Both differences of SUV-R and SUV-L between the two groups were not statistically significant (t=0.54 and 0.63,both P>0.05).Conclusions Myocardial glucose metabolism in right heart is closely correlated with left and right ventricular function.The assessment of 18F-FDG uptake in right heart by PET imaging is potentially useful for evaluation of the severity and prognosis in IDCM patients.
