1.Effect of β-amyloid on calcium homeostasis and endoplasmic reticulum calcium storage of hippocampal neurons in rat

Objective:To study the effect of β-amyloid(Aβ) on calcium homeostasis and endoplasmic reticulum calcium storage of hippocampal neurons in rats.Methods:A total of 60 adult male SD rats were randomly divided into six groups by body mass matching method, with 10 rats in each group.Three groups were injected Aβ 25-35 into the hippocampus(2 μL per side), and divided into low dose group(2.5 μg/μL), medium dose group(5.0 μg/μL) and high dose group(7.5 μg/μL) respectively, and the other 3 groups were set up as the normal saline group(2 μL 0.9% sodium chloride solution), sham-operated group(rats craniotomy without injection) and normal control group(normal feeding without any treatment). The rats were fed until 14 days after operation, and the behavior and state of the rats were observed and recorded, as well as the body weight and total food intake ratio.And the rat hippocampal cells endoplasmic reticulum pathological change were observed by using the electron microscope and light microscope, meanwhile the concentration of intracellular free Ca 2+ ions was detected by the laser scanning confocal microscope and the expression level of PS, SERCA and RyR mRNA and protein by real-time PCR and Western blot methods respectively.The experimental results were analyzed using SPSS 25.0 software for statistical analysis. Repeated measurement ANOVA and one-way ANOVA were used for multi-group comparison, and Dunnett test and Tukey test were used for further pairwise comparison. Results:(1) The body weight of rats in each group was analyzed by repeated measurement ANOVA, and the difference of time effect was statistically significant( F=153.15, P<0.001), but there was no significant difference between the intergroup effects and interaction effects( F=1.547, P=0.374; F=1.598, P=0.113). The body weight of high, medium and low dose Aβ 25-35 groups at 7, 14 days after injection had no significant difference compared with the control group(all P>0.05). The food utilization rates of the high, medium and low doses of Aβ 25-35 groups were(22.9±4.0)%, (23.0±4.2)% and(22.6±3.2)%, respectively, and there was no significant difference compared with the control group((23.7±5.0)%, P>0.05). Within 14 days after injection, listlessness and lethargy were observed in rats in the high dose Aβ 25-35 group.(2) Pathological observation results showed that the endoplasmic reticulum of rat hippocampal cells in the high dose and medium dose groups of Aβ 25-35 was expanded and swelled, and the mitochondria were swollen and deformed.(3) 14 days after Aβ 25-35 injection, the fluorescence intensity of free calcium in hippocampus of rats in high, medium and low dose groups were(820.43±6.89), (720.12±4.30) and (680.50±4.32), respectively, which were all higher than that in the control group(592.17±3.97)(all P<0.001). (4) RT-PCR and Western blot results showed that compared with the control group, high dose and medium dose Aβ 25-35 injection could up-regulate the expression of PS and SERCA mRNA and protein in hippocampal cells(all P<0.05), while down-regulate the expression of RyR mRNA and protein in hippocampal cells(all P<0.05). Conclusion:The deposition of Aβ 25-35 in hippocampal tissues can disrupt the homeostasis of calcium ions in hippocampal tissues, and then cause the increase of free calcium and its related proteins, thus playing the neurotoxic role.
2.CAS-R-2 frameless and Leksell frame stereotaxic devices used in trepanation and drainage for patients with hypertensive cerebral hemorrhage: a comparative analysis

Objective:To compare the simplicity, safety, efficacy, prognoses and economic burden of CAS-R-2 frameless stereotactic device and Leksell frame stereotactic device in assisting surgery for patients with hypertensive cerebral hemorrhage (ICH, hematoma volume: 20-40 mL).Methods:The clinical data of 120 patients with supratentorial ICH, admitted to our hospital from December 2012 to December 2019, were retrospectively analyzed; trepanation and drainage assisted by CAS-R-2 frameless stereotactic device was performed in 65 patients (frameless group), and trepanation and drainage assisted by Leksell frame stereotactic device was performed in 55 patients (frame group). The differences of surgery time, hematoma evacuation rate 7 d after surgery, incidences of recurrent hemorrhage and intracranial infection during hospitalization, length and expense of hospitalization, and modified Rankin scale (mRs) scores 6 months after surgery were compared between the two groups.Results:As compared with those in the frame group, patients from the frameless group had significantly shorter surgery time ([0.5±0.1] h vs. [2.2±0.5] h), significantly lower incidence of recurrent hemorrhage (0% vs. 9.1%) and significantly lower incidence of intracranial infection (1.5% vs. 9.1%) during hospitalization ( P<0.05). The hospitalization expense of patients from the frame group was significantly lower than that in the frameless group ( P<0.05). There were no significant differences in hematoma evacuation rate 7 d after surgery, length of hospital stays, and mortality and mRs scores 6 months after treatment between the two groups ( P>0.05). Conclusion:For patients with supratentorial ICH, trepanation and drainage assisted by CAS-R-2 frameless stereotactic device has the same curative effect and prognoses as Leksell frame stereotactic one; the former has higher simplicity and clinical safety, and the latter has lower economic burden.
3.Clinical analysis of 27 neurobrucellosis patients

Objective To analyze the clinical characteristics and prognosis of neurobrucellosis(NB).Methods Twenty-seven cases of NB patients who received treatment in Disease Prevention and Control Center of Inner Mongolia Autonomous Region and Inner Mongolia Medical University Hospital from January to December in 2016 were collected.The clinical data of these patients were recorded and systematically analyzed.Results Twenty-four cases(88.89%)had a history of exposure to cattle and sheep.Twenty-four cases(88.89%)were admitted with fever,18(66.67%)cases with sweat,16 cases(59.26%)with headache,14 cases(51.85%)with neurological symptoms of meningitis and meningoencephalitis,4 cases(14.81%)with auditory nerve damage and 3 cases(11.11%)with spinal cord damage.Elevated white blood cells were found in 2 cases(7.41%),elevated serum C-reactive protein in 3 cases(11.11%),elevated procalcitonin in 2 cases(7.41%)and elevated erythrocyte sedimentation rate in 15 cases(55.56%).All the 27 patients underwent lumbar puncture and the cerebrospinal fluid test results were abnormal,of which increased protein levels in 17 cases(62.96%),increased mononuclear cell ratio 14 cases(51.85%),increased pressure in 14 cases(51.85%),and reduced chloride levels in 14 cases(51.85%),and reduced glucose levels in 6 cases(22.22%).Positive cerebrospinal fluid culture(CSF)was found in 1 case(3.70%).There were 8 cases(29.63%)with white matter damage,5 cases(18.52%)with meningeal enhancement,3 cases(11.11%)with spinal cord lesions,2 cases(7.41%)with cerebral edema and 2 cases(7.41%)with brain abscess.There were 10 cases(37.04%)with sensory nerve damage in the extremities,4 cases(14.81%)with auditory nerve damage and 2 cases(7.41%)with motor nerve damage.All patients were treated with regular anti-Berg's disease for 6 weeks and were followed up for 1 year(every 3 months)after the treatment.Nineteen patients(70.37%)were cured,7 patients(25.93%)developed sequelae and 1(3.70%)patient died.Conclusions The analysis suggests that NB have a variety of clinical characteristics.CSF examination,imaging examination and neuro electrophysiology detection have contribution to the diagnosis of the NB.
4.Safety and feasibility of enhanced recovery after surgery in perioperative management of pancreatectomy: a Meta analysis

Objective To systematically evaluate the safety and feasibility of enhanced recovery after surgery (ERAS) in perioperative management of pancreatectomy.Methods Literatures were researched using CNKI,Wanfang database,VIP database,PubMed,Cochrane Library,Embase from January 1990 to March 2018 with the key words including "快速康复外科,加速康复外科,胰腺切除术,胰十二指肠切除术,惠普而术,ERAS,enhanced recovery,fast track,pancreatic surgery,pancreatectomy,Whipple,pancreatoduodenectomy,pancreatoduodenal resection".The cohort study about ERAS in elective pancreatic surgery or pancreaticoduodenectomy were received and enrolled.The patients using ERAS in perioperative management and using traditional perioperative management were respectively allocated into the ERAS group and control group.Two reviewers independently screened literatures,extracted data and assessed the risk of bias.Count data were described as odds ratio (OR) and 95% confidence interval (CI).Weighted Mean Difference (WMD) was used as a consolidated statistics for measurement data that were measured using the same tool,and standardized mean difference (SMD) was used as a consolidated statistics for measurement data that were measured using the different tools.The heterogeneity of the studied was analyzed using the I2 test.Results Nineteen retrospective cohort studies were enrolled in the Meta analysis,and total sample size was 3 699 patients,including 1 823 in the ERAS group and 1 876 in the control group.The results of Meta analysis showed that there were statistically significant differences in the time of postoperative nasogastric tube removal,time for postoperative solid diet intake,time of postoperative defecation recovery,incidence of postoperative overall complications,incidence of postoperative delayed gastric emptying,incidence of postoperative intra-abdominal infection,duration of postoperative hospital stay and hospital expenses between ERAS group and control group (WMD=-1.70,-3.61,-0.86,OR =0.65,0.60,0.70,WMD=-4.64,SMD=-0.48,95%CI:-2.97--0.42,-4.70--2.53,-1.01--0.71,0.52-0.81,0.45-0.80,0.54-0.91,-5.91--3.38,-0.77--0.18,P < 0.05).There was no statistically significant difference in the operation time,volume of intraoperative blood loss,incidence of postoperative pancreatic fistula,incidence of postoperative wound infection,readmission rate,reoperation rate and mortality between ERAS group and control group (WMD=-9.73,-14.39,OR=0.85,0.72,1.05,0.81,0.74,95%CI:-34.24-14.78,-116.96-88.17,0.72-1.01,0.46-1.14,0.83-1.32,0.58-1.13,0.53-1.02,P>0.05).The results of subgroup analysis showed that heterogeneity of data was from eastern and western countries.Conclusion ERAS in the perioperative management of pancreatectomy is safe and feasible,it can also promote postoperative recovery of patients and reduce incidence of complications and financial burden.
5.The clinical and electrophysiological research on peripheral nerve injuries in neurobrucellosis

Objective To characterize the clinical manifestations and to evaluate the value of the electrophysio-logical examination in patients with peripheral nerves injuries caused by neurobrucellosis. Methods Electrophysiology examination was conducted in 32 patients with peripheral nerve injuries caused by neurobrucellosis and 32 normal controls who had equivalent age and gender. The results were further statistically analyzed. Results There were sig-nificant difference between patients and healthy control group in distal motor latency (DML), compound motor active potentials (CMAP) amplitude, motor nerve conduction velocity (MCV), sensory nerve action potential latency(SL),senso-ry nerve action potential (SNAP) amplitude and sensory nerve conduction velocity (SCV) ( P﹤0.05). Electrophysiology examination revealed peripheral nerve damage of limbs involving, both sensory nerve and motor nerves, of which sen-sory and motor nerve injuries accounted for 55.47% and 16.80%, respectively. Median and sural nerve injuries were most frequently affected in upper (64) and lower limbs (16). Motor and sensory nerve conduction velocity was delayed in 43 nerves (16.80%) of 256 examined motor nerves and in 142 nerves (55.47%) of 256 examined sensory nerves. The damage of the sensory nerves was more severe than those of the motor nerves and damage of nerves in the upper limbs was more severe than those in the lower limbs. Conclusion Electrophysiologoical examination provides objective basis for the diagnosis of peripheral nerve injuries in neurobrucellosis.
6.Research on acoustic source generating mechanism of magnetoacoustic tomography based on acoustic dipole theory.

Magnetoacoustic tomography (MAT) has some advantages such as high sensitivity and high spatial resolution. The generating mechanism of acoustic source is the research foundation of forward and inverse problems. A coustic signals were respectively simulated by using monopole and dipole radiation theory in the experimental conditions, then the differences between their acoustic pressures were discussed, and furthermore the contrast and validation were conducted by physical experiments in this study. The physical experimental results showed that acoustic waveform of MAT had a certain directivity and therefore they indicated that dipole model showed higher approximation to the real facts than monopole model. It can be well concluded that this research has cardinal significance for the accurate algorithm of MAT.
