1.Application of the Transfer and Discharge Sheet of Medical Record in"National Examination"and DRG Indicator Evaluation of Clinical Departments

Objective To explore the application of the data of the transfer and discharge sheet of medical record combined with the data of medical record home page in the performance appraisal of tertiary public hospitals and DRG main indicator evaluation in clinical departments.Methods The data of the transfer and discharge sheet of medical record and medical record home page of HIS in a tertiary general hospital in Guizhou Province from January to August 2022 were selected,and the diagnosis and treatment data of the transferred patients in different clinical departments during hospitalization were statistically segmented,combined with the data of the first page of medical records,the main indicators of"national examination"and DRG in each department were recalculated.Results 98.00%(49/50)of the clinical departments in the sample hospitals had transfer behavior.Compared with the data statistics of the medical record home page,the number of four-level and minimally invasive surgeries and the number of complications of surgical patients in different departments of the hospital were significantly change by the data statistics of the transfer and discharge sheet of medical record of the department.The increase was mainly in surgery,and the decrease was mainly in internal medicine.There were 37 departments with total weight changes,of which the most increased was the Department of Critical Care Medicine(increase of 39.41%),and the most reduced was the Department of interventional Medicine(decrease by 18.00%).There were 36 departments with changes in CMI.Conclusion Through the data of the transfer and discharge sheet of medical record,the performance evaluation indicators of patients with transfer behavior in different departments can be calculated more accurately and reasonably,which is conducive to objectively reflecting the workload,technical difficulty and medical quality of each department of the hospital.

9.Reflection on blood transfusion in a patient with RhC/E gene deletion

The patient′s ABO blood type and Rh antigen phenotype were identified by monoclonal antibody serum test tube agglutination, and Rh antigen deletion was confirmed by gene sequencing.The ABO blood type and Rh antigen phenotype of the patient were identified using monoclonal antibody serum in vitro agglutination assay, and Rh antigen deletion was confirmed using gene sequencing. The Rh typing saline method showed that the patient was positive for anti D, but negative for anti E, -C, -c, and -e. The saline method for antibody screening showed negative results for cells I to III, positive results for polyamine and anti human globulin tests, positive results for antibody identification cells 1 to 16, and negative results for themselves. Direct anti globulin tests showed negative results. The sequencing results of RhC/E gene showed that exons 9-10 were normal, while exons 1-8 were missing. The patient had a deletion of exons 1-8 of the RhC/E gene, resulting in a loss of Rh antigen E/e and C/c expression. After the first random matching transfusion, the patient produced antibodies targeting E/e and C/c, resulting in an incompatibility between the main and side matching during the second infusion of red blood cell products and the inability to transfuse. In order to solve this situation, first we need to establish a rare blood group bank for Rh C/E gene deletion. Secondly, during the first blood transfusion, a small amount of RH antigen red blood cells should be injected. Stored autologous blood transfusion should also be considered.
10.Identification of a novel FUT1 allele in a Chinese individual featuring para-Bombay phenotype.

OBJECTIVE:
To explore the genetic basis for an individual with a para-Bombay phenotype.
METHODS:
A proband with mismatched forward and reverse serotypes for the ABO blood group was identified. Weakly expressed ABH blood type antigen on the surface of red blood cells was verified by absorption and release test, and the blood group substances in saliva was detected by sialic acid test. Exons 6 and 7 of the ABO gene and exons of the FUT1 and FUT2 genes were subjected to direct sequencing.
RESULTS:
The proband was found to be of O type by forward ABO serotyping and AB type by reverse ABO serotyping, though H and substance A and B were detected in her saliva. DNA sequencing revealed that she has harbored c.35C/T, c.328G/A, and c.504delC compound heterozygous variants of the FUT1 gene. Haploid analysis showed that her FUT1 genotype was h328A/h35T+504delC, which has been uploaded to the NCBI website (No. MW323551).
CONCLUSION
The para-Bombay phenotype of the proband may be attributed to the novel compound heterozygous variants including c.504delC of the FUT1 gene, which may affect its function by altering the activity of FUT1 glycotransferase.
