1.Preparation of rapamycin photosensitive nanodiscs and evaluation of their in vitro antitumor activity

ObjectiveTo prepare photosensitive nanodiscs loaded with rapamycin and evaluate their in vitro antitumor activity against ovarian cancer. MethodsNanodiscs were prepared by the thin-film hydration method, and their particle size, potential and morphology were characterized by dynamic light scattering and transmission electron microscopy. High performance liquid chromatography was established for the determination of rapamycin content, and the specificity, linearity, precision, repeatability, stability, and recovery rate of the method were verified. The singlet oxygen generation and light-controlled drug release properties of nanodiscs under near-infrared light were evaluated respectively by singlet oxygen detection probe and reverse dialysis method. The cytotoxicity of the nanodiscs against human ovarian cancer cells (OVCAR-8) was evaluated by detecting cell metabolic activity. Confocal microscopy and flow cytometry were used to investigate the cellular uptake and endocytosis mechanisms of the nanodiscs. ResultsThe nanodiscs exhibited a hydrodynamic diameter of (63.66±0.27) nm, a zeta potential of (-18.2 ± 2.4) mV, and demonstrated good stability. The results from transmission electron microscopy revealed that the nanodiscs exhibited spherical and short rod-like morphologies. The peak area of rapamycin showed good linearity in the concentration range of 0.2-40 μg/mL (R²=0.999 9), and the relative standard deviations of precision, repeatability and recovery were all less than 5%. Under near-infrared light irradiation, the nanodiscs could effectively produce singlet oxygen. At the same time, the singlet oxygen generated could induce lipid membrane oxidation, which significantly promoted the responsive release of rapamycin (P<0.001). Nanodiscs had no obvious cytotoxicity, but the cytotoxicity to ovarian cancer cells was significantly enhanced after near-infrared irradiation. Cellular uptake experiments confirmed that the nanodiscs mainly entered cells through clathrin-mediated, energy-dependent endocytosis (P<0.001), with drug release triggered by light exposure. ConclusionRapamycin photosensitive nanodiscs were successfully prepared, which showed significant light-controlled release properties and enhanced anti-ovarian cancer activity in vitro.
2.Effects and mechanism of triptolide on cerebral ischemia-reperfusion injury in rats

Objective To investigate the effect of triptolide on cerebral ischemia- reperfusion injury (CIRI) and explore its molecular mechanism. Methods One hundred and forty-four Wistar rats were randomly divided into sham operation group, model group, low, medium, high dose of triptolide group and butylphthalide group, with 24 rats in each group. The CIRI rat model was established by blocking the middle cerebral artery for 2 hours. 3 days before modeling, the rats in each group were ip administration once a day. 24 hours after reperfusion, the neurological deficit score was detected, the rate of cerebral infarction was measured by TTC staining, the blood brain barrier (BBB) permeability was detected by EB penetration test. The pathological changes neurons in the ischemic penumbra cortex were observed by HE and TUNEL staining. The content of inflammatory factors in ischemic cerebral cortex were detected by Elisa method. The expression of TLR4/NF-κB pathway related proteins were detected by Western blot. Results Compared with the model group, the neurological deficit score, cerebral infarction rate and EB content in the triptolide middle, high dose groups and the butylphthalide group were significantly decreased (P<0.05). The pathological changes of cortical neurons in the ischemic penumbra were significantly improved, and the apoptosis rate of neurons was significantly decreased (P<0.05). The content of TNF-α, IL-1β and the expression of TLR4, p-NF-κB, cleaved caspase-3, Bax were significantly decreased, the expression of Bcl-2 was significantly increased, the ratio of p-NF-κB/NF-κB and Bax/Bcl-2 were significantly decreased (P<0.05). The regulatory effect of the high dose triptolide group on various detection indexes were better than that of the butylphthalide group (P<0.05). Conclusion Triptolide could protect the permeability of BBB, improve the neurological deficit and neuropathy in CIRI rats, and reduce the rate of cerebral infarction, its mechanism may be related to the inhibition of TLR4/NF-κB pathway and which mediated inflammatory response and neuronal apoptosis.
3.Qualitative Analysis of Chemical Components in TangNiaoLing Tablets by UHPLC-Q-Exactive-Orbitrap-MS/MS

Objective To determine the chemical composition of TangNiaoLing Tablets by UHPLC-Q-Exactive-Orbitrap-MS/MS.Methods A Waters ACQUITY HSS T3 column(100 mm×2.1 mm,1.8 μm)was used for separation at a total flow rate of 0.2 mL/min.The mobile phase included an aqueous solution of 0.1%formic acid and acetonitrile mixed with 0.1%formic acid was supplied.The injection volume was set at 2 μL and the column oven temperature was 40℃.High-resolution mass spectrometric data were obtained by concurrently scanning the positive and negative ion modes.The identification was accomplished by inferring the empirical fragmentation patterns and comparing it with databases and references.Results 100 different chemical elements,including triterpenes,flavonoids,phenylpropanoids,phenylethanoid glycosides,iridoid glycosides,and phenols,among others were identified from the 50%methanol extract of TangNiaoLing pills.Conclusion The chemical contents of TangNiaoLing tablets were identified and analyzed using the UHPLC-Q-Exactive-Orbitrap-MS/MS method for the first time.This served as a foundation for future research into the tablets' effective components and quality control.

5.Combining respiratory function training with balloon dilatation for the rehabilitation of cricopharyngeal achalasia after radiotherapy for nasopharyngeal carcinoma

Objective:To observe the rehabilitative effect of combining respiratory function training with balloon dilatation for persons with cricopharyngeal achalasia after radiotherapy for nasopharyngeal carcinoma.Methods:A total of 120 nasopharyngeal carcinoma patients with cricopharyngeal achalasia after radiotherapy were randomly divided into an observation group and a control group, each of 60. Both groups were treated with routine functional swallowing training and balloon dilatation, while the observation group was additionally provided with respiratory function training 5 days a week for 8 weeks. Before the treatment and after 4 and 8 weeks of treatment, the swallowing function of both groups was evaluated using video fluoroscopy (VFSS), a functional oral intake scale (FOIS), cricopharyngeal muscle functional status and the M. D. Anderson dysphagia inventory (MDADI).Results:After 4 weeks of treatment the average VFSS, FOIS and MDADI scores of both groups were significantly better than before the treatment, and significant improvement continued over the subsequent four weeks. After 8 weeks of treatment, the average VFSS, FOIS and MDADI scores of the observation group had again improved significantly. Physiology, functioning, and their emotional state were also adjudged to have improved compared with 4 weeks earlier and compared with the control group.Conclusion:Combining respiratory function training with balloon dilatation can improve swallowing and can significantly reduce or delay swallowing disorders among patients with cyclopharyngeal achalasia after radiotherapy.
6.Research progress in molecular mechanism of bone homeostasis imbalance due to bone infection

Bone infection is an osteal disorder with bone damage resulting from infection of bone marrow,cortex,periosteum and surrounding tissues,characterized by a high incidence and severe symptoms. The treatment is often complicated and prolonged. Regardless of the method used,the goal is to reduce bone defect caused by infection,control infection,rebuild impaired tissues,and promote bone healing,so as to restore bone homeostasis. Starting from bone homeostasis,the authors review the research progress in the commonly used drugs and molecular mechanisms that affect bone formation-related pathways caused by bone infection,activate bone resorption-related mechanisms,and regulate bone homeostasis,in order to provide a theoretical basis for reducing bone defect caused by bone infection and promoting bone healing.
7.Comparative study on diagnostic yield and accuracy of double balloon enteroscopy with barium enteroclysis and capsule endoscopy in patients suspected with small bowel tumors

Objective To investigate the diagnostic yield and accuracy of double balloon enteroscopy, barium enteroclysis and capsule endoscopy in patients with suspicion of small bowel tumors. Methods Double balloon enteroscopy were performed in fifty nine patients with suspicion of small bowel tumors.The route of enteroscopy could be either via mouth or via anus.At the same time,34 and 17 out of 59 subjects received either barium enteroclysis or capsule endoscopy.The results of exams were analyzed independently and final diagnosis of each case was compared thereafter.Results Nineteen of 34 patients undergone the enteroclysis were diagnosed as small bowel tumor.The diagnostic yield was 55.9%.The diagnosis was finally confirmed by the enteroscopy in 12 cases,which indicated the accurate rate of enteroclysis was 63.2%(12/19).Double balloon enteroscopy detected tumors in 3 of 15 subjects with negative enteroclysis finding.The diagnostic yield of capsule endosocpy was 47.1%(8/17),and among the 8 cases diagnosis was comfirmed by the enteroscopy in 4 cases.Small bowel tumors were detected in 2 of 9 cases with negative capsule endoscopy findings.Thirty-six cases of small bowel tumor were detected by double balloon enteroscopy via a route(mouth or anus),and 16 patients were diagnosed after both route procedure.No small bowel tumor was found in 7 paitents.The overall diagnostic yield of enteroscopy was 88.1%.The diagnosis were all finally confirmed by pathological examination.No procedure-related complication were observed.Conclusion Double balloon enteroscopy is superior to enteroclysis and capsule endoscopy in diagnostic yield and accuracy for small bowel tumors.
