To evaluate the design and implementation effects of the "Rapid Recognition and Management of Critically Ill Patients" course, and to provide a reference for the improvement of situational simulation teaching in emergency and critical care settings. All medical staff who participated in thiscourse training in June 2025 were enrolled as study subjects. The course adopted a blended teaching method combining online theoretical selfstudy with offline situational simulation. Multi-dimensional evaluation was conducted through theoretical assessments, skill assessments, and satisfaction surveys regarding the course and instructors. A total of 142 trainees were enrolled, including 103 clinical postdoctoral fellows (72.5%), 24 standardized training nurses (16.9%), and 15 standardized training resident physicians (10.6%). The mean theoretical examination score was 93.27±6.95, with statistically significant differences among different trainee categories (η2=0.346, 95% CI: 0.241-1.000, The course achieved preliminary and favorable training outcomes. Theoretical examination scores, skill assessment scores, and course evaluation ratings were mutually independent, reflecting distinct dimensions of competence and learning experience. It is recommended that theoretical and skill assessments be set as independent passing thresholds, and that targeted training be strengthened in the areas of arrhythmia recognition and ventricular fibrillation management.

This study aims to explore the mechanism of Shenling Baizhu San(SLBZS)in the treat-ment of ulcerative colitis(UC)based on systematic network pharmacology and in vivo experimen-tal verification.Systematic network pharmacology was used to predict the core targets and key pathways of the SLBZS in the treatment of UC,it was further validated in UC model of BALB/c mice which were established with DSS[3.5%(w/v)],and SLBZS was given orally for treatment at the same time.Network analysis of SLBZS identified 153 active compounds and 68 compound-re-lated targets correlated with UC.Additionally,5 core active compounds were obtained, such as rob-inin,luteolin,kaempferol,quercetin,denudatin B.Most of the compounds have proven to have phar-macological activities.Based on PPI network and pathway enrichment studies,SLBZS may exert anti-inflammatory,immunological regulation, and participate in the process of apoptosis through regulating IL6,TNF,IL1B,PTGS2,TP53,STAT3,CASP3,IL10,CXCL8,IL2,PPARG,PTPRC,IFNG,MTOR,MMP9,so as to affect cell differentiation,TNF signaling, cancer signaling,IL-17 signaling,NF-Kappa B signaling of Th17 cell. The results of in vivo animal experiments showed that SBP could significantly improve the clinical symptoms and colonic pathological changes of UC mice, regulate the abnormal changes of blood routine indexes, increase the serum inflammatory factors IL-4 and IL-10,and reduce the contents of IL-6,IL-2,IL-1β and TNF-α.These results indi-cates that SBP can treat ulcerative colitis through TNF signaling pathway and NF-Kappa B signa-ling pathway.

This study aims to investigate the effects of Pulsatilla powder(PP)on colonic intestinal flora and tryptophan metabolism in piglets with diarrhea.Twenty-four weaned piglets were ran-domly divided into the normal control group, model group,self-healing group, and PP group. The model of piglet diarrhea was established by the compound factor of external evil+internal injury+epi-demic virus, and treated with PP.During the modeling period, the body weight, mental status and fecal patterns of piglets were monitored daily. After the experiment, the colon tissues were collect-ed for histopathological observation, and the inflammatory factors IL-1β and IL-6 in the colon tis-sue was detected by ELISA,16S rRNA was used to analyze colonic intestinal flora and RT-qPCR was used to determine the expression of genes related to tryptophan metabolism in the colon. The results showed that, compared with the model group,PP significantly improved the pathological status of the colon in piglets with diarrhea, with elevated body mass and a significant increase in goblet cells(P<0.05);and significantly reduced the levels of IL-1β and IL-6 in the colon of diar-rhea piglets(P<0.05).The results of intestinal flora showed that PP increased the abundance of Firmicutes(P<0.01),Spirochaeota(P<0.01)and Lactobacillus(P<0.05),reduced the abun-dance of Bacteroidota(P<0.05)、Prevotella_NK3B31_group(P<0.05)and Clostridium_sensu_stricto 1(P<0.01).Meanwhile,PP significantly reduced the expression level of TPH1 mRNA(P<0.05),increased the expression levels of AhR(P<0.05)and IL-22 mRNA in the co-lon of diarrhea piglets. These results indicates that PP may alleviate diarrhea in piglets by regula-ting colonic intestinal flora and genes related to intestinal tryptophan metabolism. This study pro-vides data support for the subsequent investigation of piglet diarrhea prevention and treatment.

It was a retrospective study. The propensity score matching was applied to divide the type 2 diabetes mellitus (T2DM) patients who have underwent percutaneous coronary intervention (PCI) into two groups: short-term (<4 weeks) sodium-glucose cotransporter 2 inhibitor (SGLT2i) group (213 patients) and control group (213 patients). The occurrence of contrast-induced acute kidney injury (CIAKI) after PCI was compared between the two groups. The results showed that the incidence of CIAKI in the SGLT2i group was not significantly different from the control group [10.8% (23/213) vs. 7.5% (16/213), χ2=1.383, P=0.313]. The incidence of CIAKI in patients with SGLT2i application time <1 week was higher than that in control patients, but the difference was not statistically significant [13.00% (16/123) vs. 7.5% (16/213), χ2=2.734, P=0.122]. Multivariate logistic regression analysis showed that short-term (<4 weeks) use of SGLT2i would not increase the risk of CIAKI after PCI in T2DM patients ( OR=0.507, 95% CI 0.238-1.077, P=0.077). Short-term application of SGLT2i before PCI may not increase the risk of CIAKI, but it is advisable to avoid initiating the application of SGLT2i before PCI as much as possible.

Objective:To evaluate the effect of robot-assisted laparoscopic technology via abdominal approach for patients with primary retroperitoneal tumors. Methods:A retrospective cohort analysis was conducted for the clinical data of 71 patients who underwent robot-assisted laparoscopic resection of primary retroperitoneal tumor via abdominal approach at the Department of Urology of Sir Run Run Shaw Hospital,Zhejiang University School of Medicine from January 2015 to December 2023. There were 35 male and 36 female patients. The age ( M(IQR)) was 56(21) years (range: 21 to 83 years). The median tumor diameter was 46 (31) mm (range: 15 to 134 mm). Postoperative pathology revealed 58 benign and 13 malignant cases. Patients were divided into non-adherent group ( n=47) and adherent group ( n=24) based on whether the tumor was adhered to major organs or vessels. Perioperative and postoperative situation were compared between the two groups. Data comparisons were conducted using independent samples t-test for normally distributed continuous variables, Mann-Whitney U tests for non-normally distributed data, χ2 test or Fisher′s exact test for categorical variables. Kaplan-Meier survival analysis was employed to estimate 3-year recurrence or metastasis rate and 3-year mortality rate. Results:Operative time was 120(60) minutes (range: 45 to 440 minutes), intraoperative blood loss was 50 (80) ml (range: 10 to 2 000 ml). The median change of intraoperative mean arterial pressure was 40 (19) mmHg(1 mmHg=0.133 kPa)(range: 10 to 112 mmHg). Intraoperative blood transfusion was required in 7 cases, whereas 64 cases did not necessitate transfusion. The change in hemoglobin levels before and after surgery was (17.9±13.6) g/L (range:-16 to 53 g/L), and the median change in serum creatinine levels was 2.0 (14.5) μmol/L (range:-71.0 to 100.4 μmol/L). Postoperative fasting duration was 2.0 (1.5) days (range: 1 to 6 days), and the median hospital stay was 10.0 (7.5) days (range: 4 to 24 days). No perioperative mortality occurred in any of the patients. The non-adherent group had shorter operation time, less estimated blood loss, lower blood transfusion rate, smaller delta value of hemoglobin before and after surgery, larger delta value of creatinine before and after surgery, fewer postoperative complications, shorter postoperative fasting time, and shorter length of hospital stay than the adherent group(all P<0.05), while there was no significant difference in mean arterial pressure fluctuation between the two groups ( P>0.05). Follow-up data were available for 69 patients, with a median follow-up duration of 39 (43) months (range: 4 to 88 months). Among these patients, 40 completed the 3-year follow-up. The 3-year recurrence or metastasis rate was 10.14%, and the 3-year mortality rate was 2.90%. Conclusions:Robot-assisted laparoscopic technology via abdominal approach for resection of primary retroperitoneal tumors is safe and feasible. It can also achieve secure surgical outcome for primary retroperitoneal tumors adherent to surrounding organs or vessels, albeit with increased surgical complexity and slower postoperative recovery compared to non-adherent cases.
