2.Clinical efficacy of demineralized dentin matrix particles in immediate implantation for bone defects in posterior region: a 1 to 5-year follow-up study.

OBJECTIVES:
This study aims to evaluate the short- to medium-term clinical efficacy of demineralized dentin matrix (DDM) particles applied during the immediate implantation of alveolar bone defects in the posterior region.
METHODS:
A total of 76 patients with 110 simple taper retentive implants were included in the conducted study and divided into Groups A and B in accordance with the bone grafting materials. Cone beam computed tomography and panoramic radiographs were taken immediately after implant surgery, immediate crown repair, and final follow-up time. The average follow-up time for Groups A and B was recorded. The primary observed clinical indicators were overall survival rate of the implant, bone resorption of the mesial and distal margins of the implant, buccal bone width resorption at the platform level and 1 mm below the platform, and bone height of the implant. Implant complication was a secondary observed clinical indicator.
RESULTS:
During the 1-to-5-year follow-up observation period, the mean follow-up of Group A was 38.2 months while that of Group B was 39.9 months. In Group A, two implants failed, one of which fractured, and implant overall survival rate was 96.4%. Four implants failed in Group B due to peri-implantitis, and implant overall survival rate was 92.6%. No statistically significant difference in implant overall survival rate was found between the two groups (P>0.05). In Group A, the average bone resorption in the mesial and distal margins of the implants was (1.011±2.047) mm and (0.841±2.183) mm, respectively. In Group B, the average bone resorption of the mesial and distal margins of the implants was (1.546±1.778) mm and (1.431±1.909) mm, respectively. No statistically significant difference was noted between the two groups (P>0.05). In Group A, buccal bone width resorption at the platform level and 1 mm below the platform of the implant was (0.782±2.084) mm and (0.681±2.307) mm, respectively. In Group B, buccal bone width resorption at the platform level and 1 mm below the platform of implant was (1.071±1.474) mm and (0.949±1.909) mm, respectively. No statistically significant difference was found between the two groups (P>0.05). In Group A, the buccal bone height of resorption of the implant was (1.044±2.214) mm. In Group B, the buccal bone height of resorption of the implant was (1.075±1.456) mm. No statistically significant difference in bone height was observed between the two groups (P>0.05).
CONCLUSIONS
During the 1-to-5-year follow-up observation period, DDM particles can effectively increase the height and width of alveolar bone, and they can achieve the same effect of maintaining alveolar bone contour and bone augmentation compared with deproteinized inorganic calf bone. DDM particles can be used as a potential new bone grafting material for the treatment of bone defects in clinical practice.

3.Analysis of risk factors for early failure of simple taper retentive implants.

OBJECTIVES:
To explore the related risk factors of early failure of simple taper retentive implants, and to provide theoretical guidance for clinical work.
METHODS:
Collect cases of patients who visited the Department of Stomatology of the Fourth Affiliated Hospital of Nanchang University from January 2021 to June 2024, received simple taper retentive implants, and had complete medical records. Taking the implants as the unit, analyze the influence of patient-related factors (gender, age, smoking history, hypertension history, diabetes history), implant-related factors (implant length, implant diameter, implant surface treatment), and surgical-related factors (implant site, implant timing, simultaneous maxillary sinus floor elevation, simultaneous bone augmentation) on the early failure of implants. Univariate analysis and multivariate analysis were adopted to explore the potential risk factors for early failure of simple taper retentive implants.
RESULTS:
A total of 3,533 simple taper retentive from 1,681 patients were included during the study period. Among them, 53 implants from 49 patients experienced early failure, with an early failure rate of 2.9% at the patient le-vel and 1.5% at the implant level. Multivariate analysis revealed that smoking (OR=2.148, P=0.021), the anterior mandibular region (OR=3.669, P=0.006), and the posterior maxillary region (OR=2.191, P=0.033) were risk factors for early failure of simple taper retentive implants. In the univariate analysis, simultaneous maxillary sinus floor elevation had a higher risk of early failure, but this effects was no longer significant in the multivariate analysis (P>0.05).
CONCLUSIONS
Smoking, the anterior mandibular region, and the posterior maxillary region are risk factors for the early failure of simple taper retentive implants, and could be comprehensively considered in the preoperative treatment plan.

5.Immediate implantation of simple taper retentive implants in chronic apical infected teeth in the posterior region:a 5-year clinical observation

Objective This study aims to evaluate the mid-and long-term clinical efficacies of simple taper retentive implants for immediate placement in chronic apical infected teeth in the posterior region.Methods A total of 36 pa-tients with 41 simple taper retentive implants were included in the study conducted.These patients underwent immediate implant treatment for teeth with chronic apical infection in the posterior region.The soft and hard tissues surrounding the implants were assessed and documented over a period of 5 years following the completion of crown restoration.Re-sults During the 5 years follow-up period,one implant failed with mobility,and the implant survival rate was 97.6%.The mean marginal bone resorption was(-0.74±1.03)mm from the immediate post-implant placement to the immediate post-crown restoration,and the difference in marginal bone heights between the two time points was statistically signifi-cant(P<0.05).The mean marginal bone resorption was(-0.21±0.70)mm from the immediate post-crown restoration to the 5-year post-crown restoration,and the difference in marginal bone height between the two time points was not statisti-cally significant(P>0.05).No significant difference(P>0.05)was observed in bone resorption between mesial and distal,maxillary and mandibular,different tooth posi-tions,and different implant lengths.The soft tissue sur-rounding the implants remained healthy at the 5 years follow-up period,one implant exhibited peri-implantitis,and no mechanical complications were observed.The overall satisfaction of patients was good.Conclusion Immediate place-ment of simple taper retentive implants after thorough debridement of chronic infected alveolar socket in the posterior re-gion can achieve good mid-and long-term clinical outcomes after 5 years of loading.The soft and hard tissues were maintained in a steady state,which expanded the indications for immediate implantation.

7.Changes of intestinal macrophage subsets at different stages in mice of inflammatory bowel disease

Objective:To establish a mice model of inflammatory bowel disease (IBD) induced by dextran sulfate sodium (DSS), and to analyze the changes in intestinal inflammation and macrophage subsets at different stages, so as to find a new target for the treatment of IBD.Methods:Thirty male C57BL/6 mice of 6-8 weeks were randomly divided into control group, activation stage group and resolution stage group.The latter 2 groups were given 25 g/L DSS for 5 consecutive days to establish the IBD model.After 5 days, the mice were given filtered and sterilized water and sacrificed on the 10 th and 15 th day, respectively.Colon inflammation in mice was evaluated, including body weight, disease activity index (DAI) score, changes in colon length, histopathology and histopathological score.Then the expression levels of interleukin (IL)-1β, IL-6, tumor necrosis factor (TNF)-α, transforming growth factor (TGF)-β in colon tissues were detected by quantitative real-time PCR(qPCR). Finally, the changes of intestinal macrophage subsets were detected by flow cytometry. Results:The colon inflammation of mice in the activation stage group was significantly more severe than that in the control group, while the colon inflammation of mice in the resolution stage group was reduced.The colon length of mice in the activation stage group was (5.94±0.40) cm, which was significantly shorter than that in the control group [(7.25±0.29) cm], and the situation was slightly improved in the resolution stage with the colon length of [(6.87±0.95) cm], and the differences were statistically significant (all P<0.05). The mRNA expression levels of pro-inflammatory cytokines IL-1β, IL-6 and TNF-α in the activation stage were 53.40±6.58, 117.69±30.78 and 2.52±0.25, respectively, which were significantly higher than those in the control group (1.00±0.13, 1.00±0.39, 1.00±0.10); the mRNA expression levels of IL-1β, IL-6 and TNF-α in the resolution stage were 2.51±0.13, 5.43±0.51 and 1.73±0.14, respectively, which were significantly lower than those in the activation stages(all P<0.05). The expression level of anti-inflammatory cytokine TGF-β in the resolution stage was 2.41±0.17, which was significantly higher than that in the activation stage (0.94±0.12), and the diffe-rence was statistically significant ( P<0.05). During the progression of IBD, there were 3 groups of macrophages in the lamina propria of intestinal mucosa of mice, of which the number of F4/80 lowCD 64-MHCⅡ - macrophage subset with the lowest maturity increased significantly in the activation stage of IBD, accounting for (10.68±4.62)%, and it decreased and returned to the normal level in the resolution stage, accounting for (4.63±1.06)%, and the difference was statistically significant ( P<0.05). Conclusions:Macrophages play an important role in the progression of IBD, the hindrance of maturation and development may be the main cause of inflammatory injury in the activation stage of IBD, and the transformation of macrophage subsets may become a new target for the treatment of IBD.
8. Surgical selection and metastatic warning of splenic lymph node dissection in advanced gastric cancer radical surgery: a prospective, single-center, randomized controlled trial

Objective:
To investigate the surgical options for splenic lymph node dissection in patients with advanced gastric cancer undergoing radical total gastrectomy, and to evaluate the sentinel effect of No. 4s lymph node on splenic lymph node metastasis.
Methods:
A prospective, single-center, randomized and controlled study was carried out (Trial registration, No.NCT02980861). Enrollment criteria: (1) >18 years old and <65 years old; (2) gastric adenocarcinoma locating in the proximal or corpus; (3) preoperative clinical staging as cT2-4aN0-3M0; (4) D2 radical total gastrectomy feasible judged before operation; (5) physical ability score 0 to 1; (6) I to III of ASA classification. Pregnant or lactating women, patients with severe mental illness or previous history of upper abdominal surgery, those suffered from other malignant tumors in the past 5 years, or heart and lung system diseases judged to affect surgery before operation, those receiving preoperative chemotherapy, radiotherapy or targeted therapies, and distant metastases being found during surgery were excluded. 9.Meta-analysis of the effects of triamcinolone acetonide alone and in combination with 5-fluorouracil for treating keloids

Objective:To compare the efficacy and safety of triamcinolone acetonide (TA) alone and in combination with 5-fluorouracil (5-FU) for treating keloids using meta-analysis.Methods:Databases including PubMed, Embase, and Cochrane Library were retrieved with the search terms of " triamcinolone acetonide, 5-fluorouracil, glucocorticoid, fluorouracil, keloid, scar, TAC, 5-FU, hypertrophic scar " and databases including Chinese Journal Full- Text Database, Chinese Biomedical Database, and Wanfang Data were retrieved with the search terms of "曲安奈德,瘢痕疙瘩, 5-氟尿嘧啶,糖皮质激素,增生性瘢痕" in Chinese to obtain the publicly published randomized controlled trials about the effects of TA alone and in combination with 5-fluorouracil for treating keloids from the establishment of each database to august 2019. The outcome indexes included effective proportion of treatment, incidence proportion of adverse reactions, and recurrence proportion of keloids. RevMan 5.3 and Stata 14.0 statistical software were used to conduct a meta-analysis of eligible studies. Results:A total of 1 326 patients with keloids were included in 14 studies, including 668 patients in TA+ 5-fluorouracil group whose keloids were injected with TA and 5-fluorouracil and 658 patients in TA alone group whose keloids were injected with TA alone. A total of 7 articles achieved 1 to 3 points in modified Jadad score, while 7 articles achieved 4 to 7 points in modified Jadad score. Patients in TA+ 5-fluorouracil group had a higher effective proportion of treatment than that of TA alone group (relative risk=1.28, 95% confidence interval=1.16-1.41, P<0.01). Subgroup analysis showed that the quality of the included literature and ethnic factors might be the source of heterogeneity in effective proportion of treatment. Patients in TA+ 5-fluorouracil group had a lower incidence proportion of adverse reactions than that of TA alone group (relative risk=0.44, 95% confidence interval=0.25-0.75, P<0.01). Patients in TA+ 5-fluorouracil group had a lower recurrence proportion of keloids than that of TA alone group (relative risk=0.25, 95% confidence interval=0.14-0.44, P<0.01). There was no publication bias in incidence proportion of adverse reactions ( P>0.05), while the effective proportion of treatment and recurrence proportion of keloids had publication bias ( P<0.05). Conclusions:TA combined with 5-fluorouracil is more effective than TA alone for treating keloids, with less incidence of adverse reactions and recurrence.
10.Clinical and literature case analysis on levetiracetam-induced thrombocytopenia

Objective:To explore the clinical characteristics of thrombocytopenia caused by levetiracetam (LEV).Methods:The diagnosis and treatment of a patient with thrombocytopenia caused by LEV in Peking University Third Hospital were reported. The main clinical data (gender, age, indications of LEV medication, usage and dosage of LEV, comorbidities, combined medication, platelet count before and after medication, time from LEV medication to onset of thrombocytopenia, clinical management and outcome, etc.) of the case and relevant cases collected from PubMed, Embase, CNKI, and Wanfang database (as of February 1, 2020) were summarized and analyzed.Results:A total of 14 patients were enrolled in the study, including 4 males and 10 females, aged from 4 months to 79 years with a median age of 57 years. There were 5 cases of primary epilepsy and 9 cases of secondary epilepsy, including 4 cases of stroke secondary epilepsy, 4 cases of brain tumor secondary epilepsy, and 1 case of viral encephalitis. Among them, 8 patients had low immune function; 4 patients were treated with LEV alone and 10 were treated with LEV combined with other antiepileptic drugs. The time from the application of LEV to onset of thrombocytopenia was 2-150 days and the median time was 8.5 (3.2, 18.5) days after medication. The severity of thrombocytopenia was defined as grade 1, 2, 3, and 4 in 1, 4, 1, and 8 patients, respectively. After the onset of thrombocytopenia, LEV was discontinued in all the 14 patients. Of them, 9 cases were improved without special intervention, 4 cases were improved after symptomatic treatments (with human immunoglobulin, hormone, and platelet transfusion in 1 case, human immunoglobulin, hormone, and anti-CD20 monoclonal antibody in 1 case, and intravenous platelet transfusion in 2 cases), and a 4-month-old infant died of septic shock and multiple organ failure.Conclusion:LEV-related thrombocytopenia may occur 2-150 days after the first medication and usually in patients with immunodeficiency, which could be improved by drug withdrawal alone or combination with symptomatic treatments as appropriate.
