ObjectiveBased on functional near-infrared spectroscopy (fNIRS), we investigated the brain activity characteristics of gross motor tasks in children with autism spectrum disorder (ASD) and motor dysfunctions (MDs) to provide a theoretical basis for further understanding the mechanism of MDs in children with ASD and designing targeted intervention programs from a central perspective. MethodsAccording to the inclusion and exclusion criteria, 48 children with ASD accompanied by MDs were recruited into the ASD group and 40 children with typically developing (TD) into the TD group. The fNIRS device was used to collect the information of blood oxygen changes in the cortical motor-related brain regions during single-handed bag throwing and tiptoe walking, and the differences in brain activation and functional connectivity between the two groups of children were analyzed from the perspective of brain activation and functional connectivity. ResultsCompared to the TD group, in the object manipulative motor task (one-handed bag throwing), the ASD group showed significantly reduced activation in both left sensorimotor cortex (SMC) and right secondary visual cortex (V2) (P<0.05), whereas the right pre-motor and supplementary motor cortex (PMC&SMA) had significantly higher activation (P<0.01) and showed bilateral brain region activity; in terms of brain functional integration, there was a significant decrease in the strength of brain functional connectivity (P<0.05) and was mainly associated with dorsolateral prefrontal cortex (DLPFC) and V2. In the body stability motor task (tiptoe walking), the ASD group had significantly higher activation in motor-related brain regions such as the DLPFC, SMC, and PMC&SMA (P<0.05) and showed bilateral brain region activity; in terms of brain functional integration, the ASD group had lower strength of brain functional connectivity (P<0.05) and was mainly associated with PMC&SMA and V2. ConclusionChildren with ASD exhibit abnormal brain functional activity characteristics specific to different gross motor tasks in object manipulative and body stability, reflecting insufficient or excessive compensatory activation of local brain regions and impaired cross-regions integration, which may be a potential reason for the poorer gross motor performance of children with ASD, and meanwhile provides data support for further unraveling the mechanisms underlying the occurrence of MDs in the context of ASD and designing targeted intervention programs from a central perspective.

Objective:To evaluate the effectiveness of health education and health promotion for coal-burning pollution-induced endemic fluorosis (coal-burning fluorosis) in Ankang City, Shaanxi Province, and provide reference for further developing health education and health promotion measures. Methods:From 2020 to 2023, one county (district) was selected as the project county in coal-burning fluorosis area of Ankang City each year, and 3 to 5 towns of each project county were selected as the project town. Health education and health promotion activities on coal-burning fluorosis were carried out for department heads, endemic disease propaganda workers of project county, and students in grades 4 to 6 from center primary schools, local residents and migrant workers of all affected villages of project county. Before and after the implementation of health education and health promotion projects, 30 to 45 primary school students and 30 local residents from each project town were selected to conduct a questionnaire survey on the prevention and control knowledge of coal-burning fluorosis every year, and a household survey was conducted to investigate the prevalence and changes of specific behaviors of local residents who open their stoves. Results:From 2020 to 2023, the coverage rate of intervention activities for department heads, endemic disease propaganda workers, and primary school students in the project county was 100%; the coverage rate of intervention activities for local residents in the project county was 94.44% (18 271/19 346), and for migrant workers was 97.86% (8 683/8 873). The awareness rate of prevention and control knowledge of coal-burning fluorosis among primary school students and local residents after health education and health promotion in each year was significantly higher than those before health education and health promotion ( P < 0.001), and showed an increasing trend year by year (χ 2trend = 36.73, 38.47, P < 0.001). From 2020 to 2023, a total of 420 local residents were surveyed, and 110 people used open stoves. The prevalence rate of specific behaviors was 26.19%, and it showed a decreasing trend year by year (χ 2trend = 0.29, P < 0.001). After health education and health promotion, the number of people who changed their behavior during a certain period was 106, and the specific behavior change rate was 96.36% (106/110). Conclusions:The health education and health promotion on coal-burning fluorosis in Ankang City has achieved significant results. In the future, we should continue to increase investment in health education and health promotion, continuously improve the long-term mechanism of endemic disease health education and health promotion, innovate health education models, further improve the effectiveness of health promotion, and consolidate the achievements of prevention and control.

Objective:To construct a prediction model for the risk of sarcopenia in middle-aged and elderly patients with knee osteoarthritis (KOA) based on machine learning, and to provide a basis for carrying out the prevention of sarcopenia in patients with KOA.Methods:Clinical data of KOA patients from three tertiary hospitals in Guangdong Province were collected between December 2023 and September 2024 using a convenience sampling method. The data were randomly split into training and test sets at an 8:2 ratio, with the occurrence of sarcopenia as the outcome variable. Risk prediction models for sarcopenia were constructed using eight machine learning algorithms: logistic regression, K-nearest neighbors, support vector machine, decision tree, neural network, random forest, gradient boosting machine (GBM), and eXtreme gradient boosting. Model performance was evaluated based on metrics including the area under the receiver operating characteristic curve (AUC), accuracy, precision, sensitivity, specificity, and F1 score. The optimal model was selected, and feature importance was visualized using the Shapley Additive exPlanations (SHAP) method. Results:Data from 640 KOA patients were analyzed, 143 males and 497 females, (67.51± 7.72) years, with 136 cases (21.25%) developing sarcopenia. All eight prediction models showed high AUC values, with the GBM model demonstrating the best performance. Its metrics included an AUC of 0.926 (95% CI 0.874 - 0.965), accuracy of 0.852, precision of 0.611, sensitivity of 0.815, specificity of 0.861, and F1 score of 0.698. SHAP analysis identified body mass index, calf circumference, body fat percentage, WOMAC score, and age as the most important predictive features. Conclusions:The GBM-based risk prediction model for sarcopenia in middle- aged and elderly KOA patients demonstrated optimal performance, enabling healthcare professionals to accurately and promptly identify high-risk groups among these patients and to develop effective, evidence-based intervention strategies.

Objective:To evaluate the relationship between sevoflurane preconditioning-induced reduction of cognitive impairment and hippocampal necroptosis after cardiopulmonary bypass (CPB) in rats. Methods:Sixty SPF healthy male Sprague-Dawley rats, aged 6 months, weighing 400-450 g, were divided into 4 groups ( n=15 each) using the random number table method: control group (group C), sevoflurane group (Sev group), CPB group and CPB+ sevoflurane preconditioning group (CPB+ Sev group). The rats were exposed to 0.4% sevoflurane for 2 h in CPB+ Sev group and Sev group. The CPB model was established at 30 min after the end of sevoflurane preconditioning in CPB+ Sev group. The open field test was performed to assess the autonomic movement ability on the 2nd day after CPB. The Morris water maze test was used to assess the cognitive function on the 3rd day after CPB. The hippocampal tissues were removed after the end of the Morris water maze test for determination of the necroptosis rate and cytosolic calcium concentration of hippocampal neuron ([Ca 2+ ] i) (by flow cytometry) and the expression of phosphorylated receptor-interacting protein kinase 1 (p-RIPK1), phosphorylated RIPK3 and phosphorylated mixed-lineage kinase-like domain (p-MLKL) (by Western blot) and for microscopic examination of the ultrastructure of hippocampal neurons (by transmission electron microscopy). Results:There was no statistically significant difference in the parameters of the open field test among the four groups ( P>0.05). Compared with group C, the escape latency was significantly prolonged, the number of crossing the original platform was decreased, the time of staying at the original platform quadrant was shortened, the hippocampal necroptosis rate and [Ca 2+ ] i were increased, the expression of p-RIPK1, p-RIPK3 and p-MLKL was up-regulated ( P<0.05), the organelles of hippocampal neurons swelled, lysosomes broke, and some chromatin in nuclei dissoluted in CPB group. Compared with CPB group, the escape latency was significantly shortened, the number of crossing the original platform was increased, the time of staying at the original platform quadrant was prolonged, the hippocampal necroptosis rate and [Ca 2+ ] i were decreased, the expression of p-RIPK1, p-RIPK3 and p-MLKL was down-regulated ( P<0.05), and the damage to the ultrastructure of hippocampal neurons was sinificantly reduced in CPB+ Sev group ( P<0.05). Conclusions:The mechanism by which sevoflurane preconditioning attenuates cognitive impairment may be related to the inhibition of calcium overload-mediated hippocampal necroptosis in a rat model of CPB.

Journal of China Pharmaceutical University 2025;56(4):498-506
Recent studies have identified a missense mutation in the β1-receptor (ADRB1-A187V) that exerts a pronounced impact on human sleep, with a noted decrease in protein abundance in vivo. The administration of β-blockers is frequently associated with sleep disturbances in clinical settings. In this study, we assessed the influence of various β-blockers on sleep within mouse models. Our findings indicated that β-blockers could induce varying degrees of arousal, sleep disruption, and a decrease in REMS (rapid eye movement sleep). We examined the dose-dependent effects of metoprolol and nebivolol on both sleep and cardiac functionality in both wild-type and Adrb1-A187V mutant mice. Our data suggested that, in contrast to cardiac effects, higher doses of metoprolol are required to have noted impact on sleep. No genotype effect was observed with metoprolol in terms of sleep or cardiac function. In contrast, the mutant mice demonstrated increased sensitivity to nebivolol, which exacerbated sleep fragmentation and impeded the onset of REMS. This study is expected to provide some reference for minimizing the occurrence of sleep disorders and reducing the adverse reactions of drugs to the greatest extent.

Objective:To analyze the imaging features of testicular conventional ultrasonography(US),shear wave elastography(SWE),and contrast-enhanced ultrasonography(CEUS)in subtypes of spermatogenic dysfunction(focal, arrest, and exhausted),and to evaluate the diagnostic efficacy of testicular multimodal ultrasound. Methods:A prospective study enrolled 310 spermatogenic dysfunction patients(focal type group: n=77,arrest type group: n=20,exhaustive type group: n=213)and 30 healthy volunteers(control group)at Shanghai General Hospital between October 2023 and December 2024. All patients underwent preoperative testicular US,SWE,and CEUS examinations, followed by microdissection testicular sperm extraction. Ultrasound parameters were compared among groups, and receiver operating characteristic(ROC)curves were plotted to assess the diagnostic efficacy of multimodal ultrasound for different subtypes. Results:①Testicular volume:The arrest group exhibited significantly greater volume compared to the focal group and the exhausted group(all P<0.05). No statistically significant difference was observed between the arrest group and the normal group( P>0.05). ②Mean Young's modulus(Emean):The arrest group exhibited significantly lower Emean compared with both the focal and exhausted groups(all P<0.05),but showed no significant difference versus the normal group( P>0.05). ③CEUS quantitative parameters:The arrival time(AT)and time to peak(TTP)in the exhausted group were higher than those in the normal group, while the peak intensity(PI)and area under the time-intensity curve(AUC)were lower(all P<0.05). No significant differences were found in AT,TTP,or AUC among the three spermatogenic dysfunction subgroups(all P>0.05). ④CEUS perfusion patterns:The focal group predominantly exhibited a mottled pattern(70.13%,54/77). Both the arrest group(90.00%,18/20)and the normal group(93.33%,28/30)predominantly exhibited an rich pattern. The exhausted group predominantly exhibited a sparse pattern(56.34%,120/213). ⑤ROC curve analysis showed that for differentiating the arrest group from the focal type group and the exhaustive type group, the combination of Emean, testicular volume, and CEUS perfusion pattern yielded an AUC of 0.931,with a sensitivity of 95.00% and specificity of 85.86%. For differentiating the focal type group from the exhaustive type group, the combination of testicular echogenicity,CEUS perfusion pattern, and Emean yielded an AUC of 0.833,with a sensitivity of 81.82% and specificity of 75.12%. Conclusions:Multimodal ultrasonography can comprehensively characterize the imaging features of different subtypes of spermatogenic dysfunction, holding promise as a noninvasive predictive tool.

Objective:To analyze the relationship between the optimal surgical margin value and clinical prognosis of transoral robotic surgery(TORS) in treating human papillomavirus(HPV) -positive oropharyngeal squamous cell carcinoma. Methods:A single-center, prospective, observational cohort study was conducted, enrolling patients with early and moderated stage(≤T3 stage) oropharyngeal carcinoma undergoing TORS between July 2020 and April 2024. The proposed optimal surgical margin cutoff value for TORS was set as 2 mm. The primary objectives were to evaluate the optimal clear margin for TORS and its association with overall survival(OS) and progression-free survival(PFS). Logistic regression was used to analyze correlations between surgical margins and clinical variables, while Cox regression models assessed the impact of surgical margins on OS and PFS. Results:A total of 90 patients(60 males, 66.7%) were included, all had squamous cell carcinoma, with a mean age of 58.0±9.0 years(range: 39-84 years) old. The 1, 2 and 3-year OS rates were 92.3%, 89.9% and 85.0%, respectively, while the 1, 2 and 3-year PFS rates were all 90.1%. For surgical margins ≤2 mm, the 1, 2 and 3-year OS rates were 80.8%, 69.3% and 69.3%, respectively, and PFS rates were 77.9% across three time points. For surgical margins>2 mm, the 1, 2 and 3-year OS rates were 96.5%, 96.5% and 90.6%, respectively, with PFS rates of 94.6%. Logistic regression showed no correlation between surgical margins and tumor type, T/N stage, smoking, alcohol use, or gender(P>0.05). Cox analysis identified surgical margins>2 mm as a significant factor improving PFS(HR=0.14, 95%CI 0.02-0.90, P=0.038). Conclusion:This systematic analysis suggests setting a 2 mm and longer as clear surgical margin for TORS. Margins>2 mm are associated with superior postoperative PFS rate and prolonged PFS time in HPV-positive oropharyngeal carcinoma patients.
