1.Effects of different concentrations of ropivacaine in femoral nerve block on early motor function following total knee arthroplasty.

OBJECTIVE:
To compare the effects of different concentrations of ropivacaine femoral nerve block on postoperative pain and early exercise fllowing total knee arthroplasty(TKA).
METHODS:
A total of 90 patients who underwent primary TKA between September 2022 and February 2023 were consecutively enrolled in this study. The cohort consisted of 34 males and 56 females, with a mean age of (66.66±7.03) years old. According to different concentrations of ropivacaine, patients were divided into 0.1% group, 0.2% group and 0.4% group, with 30 patients in each group. The age, gender, body mass index(BMI), American Society of Aneshesiologists(ASA) grade, operation time, anesthesia time, tourniquet using time, Post Anesthesia care unit(PACU) stay duration, ambulation time, first reaching to Bromage 0 grade time, visual analogue scale(VAS), hospitalization period and postoperative adverse reactions were compared among the three groups.
RESULTS:
All 90 patients were followed up for an average of (31.56±5.62) days, and no postoperative adverse reactions occurred. There were no significant differences among the three groups in terms of age, gender, BMI, ASA classification, operation time, anesthesia time, tourniquet application time, PACU stay duration, and hospitalization period (P>0.05). Significant differences were observed in VAS scores at 1, 2, 4, 6, and 12 hours post-operation among the three groups (P<0.05). Additionally, significant variations were noted in ambulation time and the first reaching to Bromage level 0 time among the three groups (P<0.05). In terms of postoperative pain, the VAS of the 0.1% group at 1, 2, 4, 6, and 12 hours after surgery(1.93±0.52), (2.57±0.77), (3.10±0.71), (3.10±0.71), (3.07±0.45) points were higher than those of the 0.4% group (1.57±0.50), (2.10±0.55), (2.23±0.57), (2.47±0.73), (2.50±0.57) points, and the differences were statistically significant (P<0.05);the VAS of the 0.1% group at 4, 6, and 12 hours after surgery were higher than those of the 0.2% group (2.43±0.57), (2.53±0.57), (2.63±0.56) points, and the differences were statistically significant (P<0.05);there was no statistically significant difference in VAS between the 0.2% group and the 0.4% group (P>0.05). In terms of early postoperative mobility, the time to ambulation time (8.30±2.76) h and the time to achieve the first Bromage grade 0 (6.13±2.18) h were significantly prolonged in the 0.4% group compared to both the 0.1% group (6.93±1.76) h, (4.17±1.18) h and the 0.2% group (6.53±1.59) h, (4.87±1.53) h. No statistically significant differences were observed between the 0.1% and 0.2% groups (P>0.05).
CONCLUSION
0.2% ropivacaine femoral nerve block can effectively reduce postoperative pain after TKA and can perform early exercise earlier.

2.Research on structural parameters of injection pump head flow channel based on energy dissipation analysis

Objective To study the influences of the structural parameters of the pump head flow channel on the energy dissipation of injection pumps and optimize the pump head flow channel structural parameters.Methods Firstly,three structural parameters of the injection pump head flow channel,namely the input chamber diameter,the connecting chamber diameter and the plunger rod chamber diameter,were selected as the experimental factors for orthogonal experimental design,and the total entropy production values of the injection pump head flow channels with different structures were obtained through entropy production analysis;secondly,the effects of the three structural parameters on the total entropy production values were explored with the range method;finally,the Kriging interpolation proxy model analysis and computational fluid dynamics(CFD)numerical analysis of the injection pump head flow channel were carried out to determine the effects of the combinations of the three structural parameters by pairs on the total entropy production values,and the structural parameters of the injection pump head flow channel were optimized with the objective of energy dissipation.Results The minimum total entropy production value was 2 783 970 mW/K,which was 29.8%lower than that in case of the initial structure(3 966 072 mW/K),when the input chamber diameter was 4.8 mm,the connecting chamber diameter was 0.32 mm and the plunger rod chamber diameter was 1.0 mm;the three structural parameters had significant differences in the effects on the total entropy production value,ordered as the plunger rod chamber diameter,the input chamber diameter and the connecting chamber diameter;considering the energy dissipation,the total entropy production value was minimized in case of the minimal input chamber diameter,the maximal connecting chamber diameter and the medium-sized plunger rod chamber diameter.Conclusion The input chamber diameter,the connecting chamber diameter and the plunger rod chamber diameter have differences in the sensitivity to the effects on the energy dissipation of the injection pump head flow channel,and the structural parameters of the head flow path can be optimized accordingly with the goal of minimizing energy dissipation.[Chinese Medical Equipment Journal,2025,46(9):28-32]
3.Construction of a Colorectal Cancer Prognostic Risk Model Based on Disulfide Death Associated Long-Chain Non Coding Ribonucleic Acids

Objective:This study aims to construct a colorectal cancer(CRC)prognosis prediction model based on disulfide death associated long-chain non coding ribonucleic acids(DRLncRNAs),and to explore its potential application in predicting prognosis and guiding personalized treatment.Methods:The data for this study was sourced from the Cancer Genome Atlas(TCGA)database,which includes RNA sequencing data and detailed clinical information of CRC patients.Using co expression network analysis to screen DRLncRNAs.Preliminary screening of DRLncRNAs related to prognosis was conducted through univariate Cox regression analysis,followed by variable screening and dimensionality reduction using LASSO Cox regression analysis.Finally,key DRLncRNAs for constructing a prognostic risk model were identified through multivariate Cox regression analysis.Evaluate the predictive ability of prognostic risk models for survival in CRC patients through Kaplan Meier survival curve analysis.The accuracy of the model was analyzed using receiver operating characteristic(ROC)curves,and the ability of different gene expression profiles to distinguish risk stratification was evaluated using principal component analysis(PCA).Gene ontology(GO)and KEGG enrichment analysis were used to evaluate the enrichment of high and low expression gene sets in different risk groups.Results:Five key DRLncRNAs(AC068580.3,AL161729.4,ZEB1-AS1,AC073896.3,and SNHG16)were successfully screened.In the overall concentration,the area under the curve(AUC)of the model's 1-year,3-year,and 5-year survival predictions are 0.674,0.746,and 0.727,respectively;In the training set,the predicted AUC for 1-year,3-year,and 5-year survival were 0.669,0.762,and 0.756,respectively;In the test set,the predicted AUC for 1-year,3-year,and 5-year survival were 0.666,0.725,and 0.697,respectively.SNHG16 and AC073896.3 are protective factors,and high expression was associated with better prognosis;ZEB1-AS1,AC068580.3,and AL161729.4 were risk factors,and high expression indicates poor prognosis.The Kaplan Meier survival analysis results showed that there was a significant difference in survival between the high-risk and low-risk groups(P<0.05),and the overall survival rate of the high-risk group was significantly lower(P<0.05).The enrichment analysis results showed that the phosphatidylinositol mediated signal transduction,mitochondrial gene expression,and other biological processes related to 5 DRLncRNAs may be involved in the occurrence,development,and poor prognosis of CRC.The results of this study showed that a prognostic risk model based on five DRLncRNAs(AC068580.3,AL161729.4,ZEB1-AS1,AC073896.3,and SNHG16)was successfully established,which showed independent correlation with the overall survival status of CRC patients.In addition,the prognostic risk model constructed based on these 5 DRLncRNAs can effectively predict the prognosis of CRC patients.Conclusion:This study identified five DRLncRNAs,including AC068580.3,AL161729.4,ZEB1-AS1,AC073896.3,and SNHG16,that are associated with the prognosis of CRC patients.The prognostic risk model constructed based on these five DRLncRNAs has a high evaluation efficiency for the prognosis of CRC patients.

The COVID-19 epidemic has spread to the whole world for three years and has had a serious impact on human life, health and economic activities. China's epidemic prevention and control has gone through the following stages: emergency unconventional stage, emergency normalization stage, and the transitional stage from the emergency normalization to the "Category B infectious disease treated as Category B" normalization, and achieved a major and decisive victory. The designated hospitals for prevention and control of COVID-19 epidemic in Tianjin has successfully completed its tasks in all stages of epidemic prevention and control, and has accumulated valuable experience. This article summarizes the experience of constructing a hospital infection prevention and control system during the "Category B infectious disease treated as Category A" period in designated hospital. The experience is summarized as the "Cluster" hospital infection prevention and control system, namely "three rings" outside, middle and inside, "three districts" of green, orange and red, "three things" before, during and after the event, "two-day pre-purification" and "two-director system", and "one zone" management. In emergency situations, we adopt a simplified version of the cluster hospital infection prevention and control system. In emergency situations, a simplified version of the "Cluster" hospital infection prevention and control system can be adopted. This system has the following characteristics: firstly, the system emphasizes the characteristics of "cluster" and the overall management of key measures to avoid any shortcomings. The second, it emphasizes the transformation of infection control concepts to maximize the safety of medical services through infection control. The third, it emphasizes the optimization of the process. The prevention and control measures should be comprehensive and focused, while also preventing excessive use. The measures emphasize the use of the least resources to achieve the best infection control effect. The fourth, it emphasizes the quality control work of infection control, pays attention to the importance of the process, and advocates the concept of "system slimming, process fattening". Fifthly, it emphasizes that the future development depends on artificial intelligence, in order to improve the quality and efficiency of prevention and control to the greatest extent. Sixth, hospitals need to strengthen continuous training and retraining. We utilize diverse training methods, including artificial intelligence, to ensure that infection control policies and procedures are simple. We have established an evaluation and feedback mechanism to ensure that medical personnel are in an emergency state at all times.
7.Research progress on self-care of patients with left ventricular assist devices

With the increase in the number of patients with heart failure and the improvement of medical care,the prevalence of patients with end-stage heart failure is increasing.Due to the limited donor supply for heart transplantation,the implantation of left ventricular assist devices has become a major alternative treatment option for patients with end-stage heart failure.Patients with left ventricular assist device have to face potential heart failure and long-term self-care related to left ventricular assist devices.It is of great significance to comprehensively evaluate the level of self-care behavior of patients with left ventricular assist devices and to formulate targeted self-care guidance programs to improve their prognosis.This article reviewed the concept and connotation,assessment tools,current status,influencing factors and intervention of self-care of patients with left ventricular assist devices,in order to provide theoretical basis and practical guidance for improving the self-care level of patients with left ventricular assist devices,so as to improve the quality of life and improve the prognosis of patients.
8.Comparison of the effects of transumbilical single-incision appendectomy and laparoscopic appendectomy in pediatrics

Objective To compare the effects of transumbilical single-incision appendectomy and laparoscopic appendectomy in the treatment of acute appendicitis in pediatric patients.Methods The clinical data of 162 pediatric patients under 14 years old with acute appendicitis admitted to our department were retrospectively analyzed,and the pediatric patients were divided into the transumbilical single-incision surgery group(n=59)and the laparoscopic group(n=103)according to their surgeries.The completion of the operation was recorded,and the operation time,intraoperative blood loss,gastrointestinal function recovery time,hospitalization cost,hospital stay and complications were compared between the two groups.The postoperative wound healing was observed.The wound infection,abdominal pain,abdominal distension,vomiting and abnormal stool were observed during the 6-month follow-up after operation.Results The operations of pediatric patients in the laparoscopic group were successfully completed without conversion to open surgery.The appendixes of 2 cases in the transumbilical single-incision surgery group were found by transumbilical laparoscopy during the operation due to the difficulty in finding the appendixes.The operation time and hospital stay in the transumbilical single-incision surgery group were shorter than those in the laparoscopic group(P<0.05).There was no significant difference in the hospitalization cost,intraoperative blood loss,gastrointestinal function recovery time,or incidence of complications between the two groups(P>0.05).The incision healing in the transumbilical single-incision surgery group was more beautiful.During the follow-up period,the incision of the pediatric patients in the two groups healed well,and there was no abdominal pain,abdominal distension,vomiting or abnormal stool.Conclusion Compared with the traditional laparoscopic appendectomy,transumbilical single-incision appendectomy has the advantages of simple operation,short operation time and hospital stay,and more beautiful incision healing.
9.Mechanism of Smilax glabra flavonoids amelioration of isoproterenol-induced cardiac hypertrophy and cardiac inflammation in mice

Objective To study the effects and mechanisms of Smilax glabra flavonoids(SGF)on myocardial hypertrophy and cardiac inflammation induced by isoproterenol(ISO)in mice.Methods C57/6J mice were randomly divided into a normal group,ISO model group(1.25 mg/(kg·d)),SGF low-dose group(50 mg/(kg·d)),and SGF high-dose group(100 mg/(kg·d)).The SGF groups were given prophylactic SGF for 7 days before modeling,then subcutaneous ISO injection was given to each group,and echocardiography was performed after continuous injection for 7 days.Serum was separated from orbital blood,and the NT-proBNP and inflammatory factor(IL-1β,IL-6,and TNF-α)contents of the blood were detected.Myocardial specimens were collected,and pathological changes to myocardial tissue were observed.Myocardial ROS levels were detected by DHE staining.The mRNA levels of heart-related hypertrophy genes and changes in the expression of key proteins in the inflammatory pathway of NLRP3/Caspase-1/IL-18 in myocardial tissue were detected.Results SGF prophylactic administration decreased IVSd,IVSs,and EF in echocardiography and increased LVIDs,LVIDd,and LVESV.The IL-1 β,IL-6,TNF-α,and NT-proBNP contents of blood decreased,and the mRNA expression levels of the heart-related hypertrophy genes for ANP,BNP,and β-MHC were subdued.The increase in myocardial ROS levels was also inhibited.The protein expression of NLRP3,Caspase-1(p20),and IL-18,which are key factors in the NLRP3/caspase-l/IL-18 inflammatory pathway,was down-regulated in myocardial tissue.The hypertrophy and disordered arrangement of cardiomyocytes were improved,the increase in fibroblast numbers outside myocardial fibers was reduced,and the infiltration of inflammatory cells and fibrosis of myocardial tissue were alleviated.Conclusions SGF can improve ISO-induced myocardial hypertrophy and cardiac inflammation in mice and may act through the NLRP3/Caspase-1/IL-18 inflammatory pathway.
10.Exosomes from ectoderm mesenchymal stem cells inhibits lipopolysaccharide-induced microglial M1 polarization and promotes survival of H2O2-exposed PC12 cells by suppressing inflammatory response and oxidative stress

Objective To investigate the potential value of exosomes derived from rat ectoderm mesenchymal stem cells(EMSCs-exo)for repairing secondary spinal cord injury.Methods EMSCs-exo were obtained using ultracentrifugation from EMSCs isolated from rat nasal mucosa,identified by transmission electron microscope,nanoparticle tracking analysis(NTA),and Western blotting,and quantified using the BCA method.Neonatal rat microglia purified by differential attachment were induced with 100 μg/L lipopolysaccharide(LPS)and treated with 37.5 or 75 mg/L EMSCs-exo.PC12 cells were exposed to 400 μmol/L H2O2 and treated with EMSCs-exo at 37.5 or 75 mg/L.The protein and mRNA expressions of Arg1 and iNOS in the treated cells were determined with Western blotting and qRT-PCR,and the concentrations of IL-6,IL-10,and IGF-1 in the supernatants were measured with ELISA.The viability and apoptosis of PC12 cells were detected using CCK-8 assay and flow cytometry.Results The isolated rat EMSCs showed high expressions of nestin,CD44,CD105,and vimentin.The obtained EMSCs-exo had a typical cup-shaped structure under transmission electron microscope with an average particle size of 142 nm and positivity for CD63,CD81,and TSG101 but not vimentin.In LPS-treated microglia,EMSCs-exo treatment at 75 mg/L significantly increased Arg1 protein level and lowered iNOS protein expression(P<0.05).EMSCs-exo treatment at 75 mg/L,as compared with the lower concentration at 37.5 mg/L,more strongly increased Arg1 mRNA expression and IGF-1 and IL-10 production and decreased iNOS mRNA expression and IL-6 production in LPS-induced microglia,and more effectively promoted cell survival and decreased apoptosis rate of H2O2-induced PC12 cells(P<0.05).Conclusion EMSCs-exo at 75 mg/L can effectively reduce the proportion of M1 microglia and alleviate neuronal apoptosis under oxidative stress to promote neuronal survival,suggesting its potential in controlling secondary spinal cord injury.
