1.Effects of baicalin on insulin resistance in rats with gestational diabetes mellitus and its mechanism

OBJECTIVE To investigate the effects of baicalin (BC) on insulin resistance in rats with gestational diabetes mellitus (GDM) and its underlying mechanism based on the adenosine monophosphate-activated protein kinase (AMPK)/suppressor of variegation 3-9 homolog 1 (SUV39H1)/histone H3 lysine 9 trimethylation (H3K9me3) axis. METHODS A GDM rat model was established by a combination of a high-fat diet and streptozotocin injection. The successfully modeled rats were divided into the GDM group, BC low-dose group, BC high-dose group, and high-dose of BC+AMPK inhibitor (Compound C) group, with 10 rats in each group. Another 10 pregnant rats fed a normal diet served as the control group. Rats in each group were given corresponding drugs/normal saline intragastrically and/or intraperitoneally, once daily for 2 consecutive weeks. After the last administration, the levels of fasting blood glucose (FBG), pancreatic function indexes [fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA-IR), insulin sensitivity index (ISI)], blood lipid indexes (total cholesterol, triglyceride, low-density lipoprotein cholesterol), liver function indexes (alanine transferase, aspartate transferase, alkaline phosphatase), inflammatory indicators (C-reactive protein, interleukin-1β, interleukin-6), metabolic regulatory protein [complement-C1q/tumor necrosis factor-related protein 3 (CTRP3)], insulin sensitivity related factors [glucose transporter 4 (GLUT4), adiponectin], and oxidative stress indicators [superoxide dismutase (SOD), catalase (CAT), malondialdehyde (MDA)] were measured. Pathological changes in liver tissue were observed, and the expressions of proteins related to the AMPK/SUV39H1/H3K9me3 axis in liver tissue were detected. RESULTS Compared with the GDM group, rats in the BC low- and high-dose groups showed varying degrees of improvement in pathological changes such as disordered cell arrangement, vacuolar degeneration, lipid deposition, and inflammatory cell infiltration in liver tissue. Their FBG and FINS levels, HOMA-IR, the levels of blood lipid indexes, liver function indexes, inflammatory indicators and MDA, and the expressions of SUV39H1 and H3K9me3 were significantly decreased or down-regulated, while metabolic regulatory protein, insulin sensitivity-related factors and AMPK protein phosphorylation levels were significantly increased ( P <0.05). The improvement was more significant in the BC high-dose group ( P <0.05). Compound C could significantly reverse the ameliorative effects of high-dose BC on the above quantitative indicators ( P <0.05). CONCLUSIONS BC can significantly reduce oxidative stress and inflammatory responses, increase serum levels of CTRP3, GLUT4 and adiponectin, thereby improving insulin resistance in GDM rats. These effects may be related to the activation of AMPK and inhibition of SUV39H1-mediated H3K9me3 modification.

This article discusses the revision background and core content of 9621 General Requirements for Phar-maceutical Packaging Materials and Containers in the Chinese Pharmacopoeia 2025 Edition.In combination with the requirements of bundling review of pharmaceutical packaging and drug products,as well as harmonization of inter-national standards,it analyzed the revision philosophy of the new guideline and the impact to industry.The new edition of the guideline 9621 introduces the concept of "pharmaceutical packaging system" for the first time,constructing four suitability evaluation dimensions covering protection,compatibility,safety,and functionality,it also introduces the requirements for the self-stability evaluation of plastic and rubber pharmaceutical packaging materials,and strengthening the concept of full lifecycle management and risk management.Through the"1+4+58" standard system,the basis for determining the key quality attributes of packaging materials for phar-maceutical use and the optimization path of dynamic inspection rules have been clarified.This revision has resolved the contradiction between the old version of standards and current regulatory requirements through a systematic upgrade,promoting the transformation of pharmaceutical packaging materials and containers quality control towards a scientific,international,and full lifecycle management model,proving technical supports in ensuring the safety,effectiveness of drug products from standard perspective,including advancing the industry high-quality development.
3.Research progress of adiponectin in the regulation of cardiovascular system

As an important endocrine organ of human body,adipose tissue secrets a large amount of endocrine fac-tors that regulate a variety of physiological functions,and role of adiponectin in cardiovascular system is especially important.At the cellular and molecular level,adiponectin possesses profound anti-inflammatory,anti-oxidative and anti-apoptotic effects,which can reduce various pathogenic factors of cardiovascular diseases(CVD).In ad-vanced stage of CVD,the expression of adiponectin in adipose tissue as well as its circulating level compensatively increased.Therefore,adiponectin has a protective effect on the cardiovascular system,and increased adiponectin level may suggest severe CVD.In this review article,we systematically introduced the role of adiponectin in CVD and discussed its application prospects as a clinical biomarker.
4.Inhibitory effect of guggulsterone on diethylnitrosamine-induced liver fibrosis in rats and its mechanism

Objective To explore the inhibitory effect of guggulsterone(GS)on diethylnitrosamine(DEN)-induced liver fibrosis in rats and its mechanism.Methods DEN-induced liver fibrosis model was established in SD rats.The successful model rats were randomly divided into model group(n=6),GS group(n=6,50 mg/kg,intraperitoneal injection for 4 weeks),GS+SRI group(n=6,50 mg/kg+30 mg/kg,intraperitoneal injection for 4 weeks),and control group(n=6,without DEN-induced).Rats in the control group and the model group were injected with the same amount of normal saline.The pathological changes of the liver were detected by HE staining.Serum liver function indexes including alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB)and alkaline phosphatase(ALP)were detected by automatic biochemical analyzer.The serum levels of pro-collagen Ⅲ(PC-Ⅲ),collagen Ⅳ(Ⅳ-C),hyaluronidase(HA),laminin(LN),malondialdehyde(MDA),reduced glutathione(GSH),superoxide dismutase(SOD),and catalase(CAT)were detected by ELISA assay.The mRNA and protein expression of TGF-β1,Smad2,and p-Smad3/Smad3 were detected by Real-time PCR and Western blotting.Results Compared with the control group,the model group showed typical pathological changes of liver fibrosis;the serum ALB,GSH,SOD and CAT levels were significantly decreased(P<0.05);the serum levels of ALT,AST,ALP,MDA,PC-Ⅲ,Ⅳ-C,HA,LN and the mRNA expression of TGF-β1,Smad3 and protein expression of TGF-β1,p-Smad3 in liver tissues were significantly increased(P<0.05).Compared with the model group,the pathological changes of liver fibrosis in the GS group were alleviated,and the serum levels of ALB,GSH,SOD and CAT were significantly increased(P<0.05),the serum levels of ALT,AST,ALP,MDA,PC-Ⅲ,Ⅳ-C,HA and LN,the mRNA expression of TGF-β1 and Smad3,and protein expression of TGF-β1 and p-Smad3 in liver tissues were significantly decreased(P<0.05).In addition,after the administration of SRI,TGF-β1 signaling pathway activator,compared with the GS group,the GS+SRI group showed significantly decreased serum ALB,GSH,SOD and CAT levels(P<0.05),but significantly increased serum levels of ALT,AST,ALP,MDA,PC-Ⅲ,Ⅳ-C,HA and LN as well as the mRNA expression of TGF-β1 and Smad3 and protein expression of TGF-β1 and p-Smad3 in liver tissues(P<0.05).Therefore,SRI attenuated the anti-fibrotic effect of GS on rats with liver fibrosis.Conclusion GS has certain inhibitory effect on DEN-induced liver fibrosis in rats,and its mechanism may be related to the reduction of oxidative stress level and the inhibition of the activation of TGF-β1/Smad3 signaling pathway.
5.The improvement effect and influencing factors of microsurgical varicocelectomy on scrotal pain

Objective To investigate the improvement effect of microsurgical varicocelectomy with vein ligation on scrotal pain and the related factors affecting the improvement of postoperative pain.Methods A retrospective analysis was conducted on the clinical data of 110 patients who underwent microsurgical inguinal varicocelectomy in Central Theater Command General Hospital from November 2021 to June 2023.The degree of pain before and after the operation was evaluated by Numerical Rating Scale(NRS).The pain relief rate,the occurrence of postoperative complications and the overall satisfaction of patients were evaluated.The semen and hormone levels of the patients before and after the operation were analyzed.Univariate and multivariate linear regression analyses were performed to investigate the relationship between clinical variables and postoperative 7-day NRS score.Results The average number of ligated veins during the operation was(6.60±3.76);postoperative satisfaction of patients was(4.23±1.08)points;complications occurred in 7 patients after the operation;after 3 months of follow-up,the overall pain relief rate was 92.73%(102/110).Compared with those before the operation,the NRS score of the patients significantly decreased 7 days after the operation,and the levels of total sperm motility,prolactin and testosterone increased 3 months after the operation,while the levels of serum follicle-stimulating hormone(FSH)and luteinizing hormone(LH)decreased,the differences were statistically significant(P<0.05).The univariate and multivariate linear regression analyses showed that the Dubin-Amelar classification of varicocele(P=0.045),operation time(P=0.049),and number of ligated veins(P=0.024)were significantly correlated with the change of postoperative 7-day NRS score.Patients with 1 to 4,5 to 10,and>10 intraoperative ligation veins were analyzed,and pain relief rates 3 months after the operation were 77.78%(28/36),100%(58/58),and 100%(16/16),respectively,with statistically significant difference(χ2=17.732,P<0.001).Conclusion Microsurgical varicocelectomy can effectively relieve scrotal pain and improve semen quality.Moreover,the relief of postoperative pain is related to the Dubin-Amelar classification of varicocele,the sufficient number of ligated spermatic veins,and the surgical experience of the surgeon.

7.Investigation of the safety and efficacy of SRS-Schwab grade 4 osteotomy versus vertebral column resection in the surgical treatment of type II congenital kyphosis

Objective:To compare the efficacy and safety of SRS-Schwab grade 4 osteotomy and vertebral column resection (VCR) in the surgical correction of type II congenital kyphosis (CK).Methods:A retrospective review was conducted on clinical and radiographic data from patients with type II CK who underwent corrective surgery at Nanjing Drum Tower Hospital between June 2008 and December 2021. Patients were divided into two groups based on the osteotomy technique employed: the VCR group (21cases) and the SRS-Schwab grade 4 osteotomy group (19 cases). Clinical parameters, including operative duration, estimated blood loss (EBL), number of segmentation failure levels, number of fused segments, and the use of anterior support cages, were compared between groups. Radiographic parameters included the number of segmentation fusion and measurements of segmental kyphosis (SK) preoperatively, postoperatively, and at the final follow-up. The degree and rate of SK correction were also calculated and compared. Complications such as neurophysiological monitoring events, neurological injury, implant breakage, proximal junctional kyphosis/lordosis (PJK/PJL), sagittal translation (ST), and implant failure were recorded. Additionally, SRS-22 questionnaire scores were assessed preoperatively, postoperatively, and at the latest follow-up.Results:A total of 40 patients were included, with a mean follow-up duration of 48.96±21.31 months (range, 24-96 months). Compared to the SRS-Schwab group, the VCR group had significantly more levels of segmentation defect (3.85±0.79 vs. 2.68±0.54), required more fused segments (8.00±2.23 vs. 5.47±2.04), had longer operative time (358.71±77.06 min vs. 212.52±77.05 min), and greater EBL (963.66±278.49 ml vs. 698.94±222.20 ml), all with statistically significant differences ( P<0.05). No significant differences were observed in local kyphotic angle, correction magnitude, or correction rate between groups postoperatively (P>0.05). Among patients with ≤48 months of follow-up, the SRS-Schwab group showed significantly lower SK (13.00°±16.28° vs. 34.00°±21.37°) and a higher correction rate (82.52%±21.40% vs. 52.84%±24.67%) at final follow-up ( P<0.05). However, no significant differences were observed in patients with >48 months of follow-up ( P>0.05), or in the overall cohort regardless of follow-up duration ( P>0.05). Complication rates were comparable between groups ( P>0.05). One neurophysiological monitoring event, 1 neurological injury, 3 cases of PJK, 1 case of PJL, 2 cases of ST, and 2 implant failures were observed in VCR group. The SRS-Schwab Grade 4 osteotomy group had 1 neurophysiological monitoring event, 1 neurological injury, 4 cases of PJK, 2 cases of ST, and 1 implant failure. The complication rates between the groups were not statistically different ( P>0.05). Both groups showed significant improvements in the self-image domain of the SRS-22 postoperatively and at the final follow-up ( P<0.05). In the function domain, both groups also demonstrated significant improvement at the final follow-up ( P<0.05), while the SRS-Schwab group showed earlier improvement at 3 months postoperatively ( P<0.05). Conclusions:SRS-Schwab grade 4 osteotomy provides comparable correction of segmental kyphosis to VCR but offers the advantages of significantly reduced blood loss and shorter operative time. Both surgical techniques effectively improve patients' quality of life.
8.Comparative effectiveness of small optical zone orthokeratology lenses and repeated low-level red-light therapy for controlling myopia in adolescents

Objective To compare the effect of small optical zone orthokeratology lenses(OK lenses)and repeated low-level red-light(RLRL)therapy in controlling myopia progression for adolescents,and the therapeutic effectiveness of RLRL is evaluated.Methods A retrospective analysis was conducted on the clinical data of 80 adolescent myopic patients in the First Affiliated Hospital of Army Medical University.The patients were divided into the RLRL group and the OK lenses group according to different intervention methods,with 40 cases in each group.Patients in the RLRL group received RLRL therapy combined with single-vision spectacles,and patients in the OK lenses group were treated with OK lenses.The changes of spherical equivalent(SE),axial length and subfoveal choroidal thickness(SFCT)1,3,6,and 12 months after treatment compared with the baseline,and color vision of patients were assessed.Based on the mean baseline axial length of the RLRL group,the patients in this group were subdivided into the short axial group and the long axial group,and the changes of the axial length and SFCT were further analyzed.Results The diopter 1,3,6,and 12 months after treatment in the RLRL group were not significantly different from the baseline(P>0.05).Axial lengths of patients in the RLRL group progressively shortened after treatment and returned close to the baseline 12 months after treatment.In contrast,axial lengths of patients in the OK lens continued to grow within 12 months after treatment;the axial lengths at each time point after treatment of patients in the two groups were significantly different from the baseline(P<0.05).The changes of axial length of patients in the RLRL group at each time point after treatment were significantly smaller than those in the OK lenses group(P<0.05).The SFCT changes of patients in the RLRL group at each time point after treatment were all greater than those in the OK lenses group(P<0.05).The SFCT at each time point after treatment in the RLRL group were significantly different from the baseline(P<0.05),whereas the SFCT at each time point after treatment in the OK lenses group were not significantly different from the baseline(P>0.05).The changes of axial length at each time point after treatment of patients in the long axial group were all greater than those in the short axial group(P<0.05);the SFCT changes 6 months after treatment of patients in the long axial group was greater than that in the short axial group(P<0.05);the SFCT at each time point after treatment of patients in the two groups were signifi-cantly different from the baseline(P<0.05).Color vision tests revealed no abnormities after treatment in the RLRL group and the OK lenses group.Conclusion RLRL therapy is effective in controlling myopia progression and demonstrates superior axial length control compared to orthokeratology lenses.
9.Lipidome atlas of human myometrium reveals distinctive lipid signatures associated with adenomyosis: Combination of high-coverage lipidomics and mass spectrometry imaging.

Adenomyosis is a common gynecological disease characterized by the invasion of endometrial glands and stroma into the myometrium of uterus, the pathological mechanism of which remains unclear yet. Disturbed lipid metabolism extensively affects abnormal cell proliferation and invasion in various diseases. However, the lipidome signature of human myometrium, which could be crucial in the development of adenomyosis, remains unknown. In this study, we generated the first lipidome profiling of human myometrium using a high-coverage and quantitative lipidomics approach based on ultra-performance liquid chromatography (UPLC) coupled with triple quadrupole (QqQ)-mass spectrometry (MS). A total of 317 lipid species were successfully quantified in the myometrial tissues from women with (n = 38) or without (n = 65) adenomyosis who underwent hysterectomy at Peking Union Medical College Hospital (Bejing, China). Up to 83 lipid species showed significant alternations in content between the two groups. These lipid aberrations involved multiple metabolic pathways, and emphasized inflammation, cell migration, and immune dysregulation upon adenomyosis. Moreover, receiver operating characteristic (ROC) curve analysis found that the combination of five lipid species could accurately distinguished the myometrial samples from women with and without adenomyosis with an area under the curve (AUC) of 0.906. Desorption electrospray ionization MS imaging (MSI) further underscored the heterogeneous distributions of these lipid markers in the adenomyosis lesion and adjacent myometrial tissue. Collectively, these results extremely improved our understanding on the molecular basis of adenomyosis, and could shed light on developing potential biomarkers and new therapeutic directions for adenomyosis.
10.Pathogenesis and treatment strategies for infectious keratitis: Exploring antibiotics, antimicrobial peptides, nanotechnology, and emerging therapies.

Infectious keratitis (IK) is a leading cause of blindness worldwide, primarily resulting from improper contact lens use, trauma, and a compromised immune response. The pathogenic microorganisms responsible for IK include bacteria, fungi, viruses, and Acanthamoeba. This review examines standard therapeutic agents for treating IK, including broad-spectrum empiric antibiotics for bacterial keratitis (BK), antifungals such as voriconazole and natamycin for fungal infections, and antiviral nucleoside analogues for viral keratitis (VK). Additionally, this review discusses therapeutic agents, such as polyhexamethylene biguanide (PHMB), for the treatment of Acanthamoeba keratitis (AK). The review also addresses emerging drugs and the challenges associated with their clinical application, including anti-biofilm agents that combat drug resistance and nuclear factor kappa-B (NF-κB) pathway-targeted therapies to mitigate inflammation. Furthermore, methods of Photodynamic Antimicrobial Therapy (PDAT) are explored. This review underscores the importance of integrating novel and traditional therapies to tackle drug resistance and enhance drug delivery, with the goal of advancing treatment strategies for IK.
