Maximizing the value of data element resources is inseparable from the compliant and safe cross-border flow of health and medical data, yet it is constrained by value conflicts with privacy rights protection and its relevant legal issues. These include, first, the current legal system exhibits fragmented characteristics, lacking consistency and structured overall regulation. Second, the specific implementation rules of privacy rights protection in the cross-border flow of health and medical data remain incomplete, and the applicability norms are insufficient. Furthermore, a mutual trust mechanism for the cross-border flow of health and medical data is still lacking. In this regard, China should achieve the following aspects based on the theory of interest balance. From the perspective of balance, China should clarify the value orientation for cross-border flow of health and medical data and privacy rights protection through data classification and graded protection, as well as through purpose identification and filtering mechanisms. At the systemic level, it is necessary to promote separate legislation for the cross-border flow of health and medical data, adopting an overseas equivalent model to delineate the concurrence issue in the extraterritorial application of the Personal Information Protection Law. In terms of applicability level, by reducing the stringency of domestic rules and clarifying the rules of proactive notification and explicit consent, it is vital to adjust and improve the specific legal regulations for privacy rights protection in the cross-border flow of health and medical data. From the collaborative dimension, through signing bilateral or multilateral international treaties and other forms of joint cooperation, it is essential to promote the coordinated sharing of data governance experiences and technologies, thereby establishing international mutual trust mechanisms for the cross-border flow of health and medical data.

Objective To explore the feasibility of applying fractional flow reserve (FFR) into the assessment of carotid stenosis, and analyze the effects from elastic modulus of vessel wall on hemodynamic parameters of carotid artery stenosis model and FFR calculation results. Methods The standard models of carotid bifurcation and stenosis models with different stenosis rates were established by computer-aided design software. Assuming that the vessel wall was linear elastic material and the blood was incompressible Newtonian fluid, the fluid-structure coupling simulation of blood flow in carotid artery stenosis model under the pulsating flow was carried out by finite element analysis, and the relevant hemodynamic parameters were obtained, and the corresponding FFR was calculated. Results When the elastic modulus was fixed, the FFR for narrow part of the model decreased gradually with the increase of the stenosis rate, and the relative difference between the FFR of elastic wall and rigid wall increased with the increase of the stenosis rate; when the stenosis rate was fixed at 70%, the FFR decreased gradually with the increase of elastic modulus. Conclusions The effect of vascular wall elasticity should be considered in the process of functional assessment on carotid stenosis with FFR; the larger stenosis rate will lead to the greater influence from elastic modulus of vessel wall on FFR.

Objective To investigate the risk factors and the influence of intravenous thrombolysis of acute cerebral infarction with cerebral microbleeds(CMBs)by SWI.Methods 1 64 patients with acute cerebral infarction were enrolled in this study. All pa-tients were scanned with routine MRI and SWI.According to the presence of CMBs on SWI,the patients were classified into two groups:CMBs group(73 cases)and non-CMBs group(91 cases).Past history was recorded and risk factors of CMBs were explored. 76 cases patients(including 35 cases of CMBs group and 41 cases of non-CMBs group)were treated by intravenous thrombolysis and rescanned with routine MRI and SWI to compare the changes in the number of CMBs and hemorrhage transformation 24 hours after thrombolysis. Results The difference age, hypertension, lacunar infarction and leukoaraiosis between the two groups were significant (P <0.05).The difference of CMBs and hemorrhagic transformation between the two groups treated by thrombolysis were not sig-nificant(P >0.05).Conclusion Acute cerebral infarction with CMBs are influenced by age, hypertension, lacunar infarction and leu-koaraiosis.Thrombolysis in acute cerebral infarction with CMBs can not augment the incidence of hemorrhagic transformation.
