1.Comparison of routine pulmonary ventilation function and impulse oscillometry in children with controlled and uncontrolled bronchial asthma

Objective:To investigate the difference between routine pulmonary ventilation function test (PFT) and impulse oscillometry (IOS) in children with bronchial asthma (BA) control and children without BA control.Methods:A total of 165 BA children admitted to the Shenzhen Longhua District People′s Hospital from January to December 2023 were selected. 73 children with asthma control test score ≥23 were selected as the control group, and 92 children with asthma control test score <23 were selected as the control group. The difference of indexes between the two groups were statistically compared in terms of PFT [forced expiratory volume in one second (FEV 1), ratio of FEV 1 to forced vital capacity (FVC) (FEV 1/FVC), instantaneous forced expiratory flow at 50% of forced vital capacity (FEF 50), instantaneous forced expiratory flow at 75% of forced vital capacity (FEF 75), maximum mid expiratory flow (MMEF), peak expiratory flow (PEF)], and IOS metrics [respiratory resistance at 5 Hz (R5), respiratory resistance at 20 Hz (R20), difference between R5 and R20 (R5-R20), reactance at 5 Hz (X5), total respiratory impedance at 5 Hz (Z5), resonance frequency (Fres), reactance area (AX)]. The predictive value of PFT and IOS indexes to BA was analyzed. Results:The levels of FEV 1, FEV 1/FVC, FEF 50, FEF 75, PEF and MMEF in PFT index of the control group were significantly higher than those in the non-control group, and the levels of Z5, R5, R20, R5-R20, X5, AX and Fres in IOS index were significantly lower than those in the non-control group. There was statistical significance between the two groups (all P<0.05). The area under the curve (AUC) predicted by PFT indexes was less than 0.7, and the AUC of FEF 50 and MMEF (all 0.633) was the largest. Among the IOS indexes, Z5, R5, R5-R20, X5, AX and Fres predicted the AUC of BA uncontrolled to be >0.7, while AX had the largest predictive value for BA uncontrolled (AUC=0.786, 95% CI: 0.736-0.836), sensitivity was 78.26%, specificity was 64.13%. Conclusions:All indexes of PFT and IOS have good sensitivity in evaluating the degree of BA control in children, and IOS has better clinical value in predicting uncontrolled BA, among which AX has the greatest predictive value for uncontrolled BA.

3.Correlation Study Between Blood Level of Big Endothelin-1 and Cardiac Remodeling in Patients With Hypertrophic Obstructive Cardiomyopathy

Objective:To explore the relationship between big endothelin-1 (big ET-1) and cardiac remodeling in patients with hypertrophic obstructive cardiomyopathy (HOCM).
Methods:A total of 167 consecutive HOCM patients admitted in our hospital from 2015-01 to 2016-05 were enrolled. Blood levels of big ET-1 and NT-proBNP were measured;electrocardiogram(ECG), dynamicECG,echocardiography (UCG) and cardiovascular magnetic resonance (CMR) were examined for relevant statistical analysis.
Results:In all 167 HOCM patients, blood level of Big ET-1 was positively related to NT pro-BNP (r=0.35, P=0.000), left atrial (LA) diameter (r=0.169, P=0.019) and heart rate (r=0.141, P=0.037);negatively related to hemoglobin (r=-0.173, P=0.013) and the ratio of interventricular septum (IVS)/posterior wall of left ventricle (r=-0.165, P=0.017). Based on the finding of positive correlation between Big ET-1 and LA diameter, the patients were divided into 2 groups:Normal LA group, n=74 and Enlarged LA group, n=93. Compared with Normal LA group,Enlarged LA group had the higher blood level of Big ET-1 (P=0.001);increased left ventricular end-diastolic diameter (LVEDD) (P=0.024), thicker IVS (P=0.003), lower LVEF (P=0.001);enlarged LVED volume (P=0.002) and IVS (P=0.002);elevated ratio of atrial fibrillation(AF);more patients with moderate to severe mitral regurgitation (MR) and NYHA functional classIII/IV.
Conclusion: Blood level of Big ET-1 was positively related to NT pro-BNP and LA diameter in HOCM patients;the patients with enlarged LA had increased Big ET-1, more obvious cardiac remodeling and the higher incidence of AF occurrence which implied that Big ET-1 might play the role in cardiac remodeling in HOCM patients.
4.X-ray Imaging of Articulatio Coxae Subluxation in Children with Cerebral Palsy

Objective To analyze the relationship between the ranks of articulatio coxae subluxation diagnosed by X-ray imaging and related factors.Methods The ranks of X-ray imaging and the severity of subluxation of joint,and the relationship with the severity of cerebral palsy(CP)and ages in 21 CP children were analyzed.Results 15 of the 21 CP children(71.4%)had moderate or severity cerebral palsy.Accompany with growing up,it became more severity.The number of children over 4 years old and with articulatio coxae subluxation was 3 times than younger.Conclusion The severity of articulatio coxae subluxation is related with the severity of CP and age.
