1.Correlation between 25-hydroxyvitamin D and cardiac autonomic nervous function in patients with type 2 diabetes mellitus

Objective To investigate the predictive value of bone metabolism parameters on cardiac autonomic nervous system function in patients with type 2 diabetes mellitus(T2DM).Methods A total of 328 patients with T2DM hospitalized in the Department of Endocrinology of Gansu Provincial People's Hospital were enrolled in this study from October 2022 to October 2023.According to the serum 25(OH)D level,all the participants were divided into<10 ng/ml group(n=80),10~20 ng/ml group(n=173),and 20~30 ng/ml group(n=75).Biochemical indicators,bone metabolic parameters,left ventricular mass(LVM)and left ventricular mass index(LVMI)were compared.Time domain indicators ofheart rate variability(HRV)in 24 h holter electrocardiogram,including the global standard deviation of normal sinus RR interval(SDNN),sinus RR interval mean standard deviation(SDANN),and normal continuous sinus RR interval difference root mean square(RMSSD).Meanwhile,adjacent RR interval difference>50 ms as a percentage of the total inter-period(PNN50),HRV triangle index,standard deviation of the difference between the length of the entire adjacent NN interperiod(SDSD),and 24 h holter electrocardiogram HRV time-domain relevant indicators were compared among the three groups.The influence of bone metabolism parameters on cardiac autonomic nervous function and their correlation were analyzed,and the optimal cutting point of cardiac autonomic nervous function was predicted by receiver operating characteristic(ROC)curve.Results SBP,heart rate(HR),FPG,PWV,PTH and β-CTX in groups of 10 ng/ml,10~20 ng/ml and 20~30 ng/ml decreased in turn(P<0.05),while HDL-C,ABI,25(OH)D,Ca2+and PNN50 decreased.Correlation analysis between Spearman and Pearson showed that 25(OH)D was positively correlated with SDNN,HRV triangle index,PNN50 and rMSSD(P<0.01).Logistic regression analysis showed that 25(OH)D,Ca2+and HR were the influencing factors of cardiac autonomic nervous dysfunction in patients with T2DM.The ROC curve analysis showed that the areas under the ROC curve of 25(OH)D,Ca2+and HR were 0.791,0.607 and 0.629,respectively,with sensitivity of 73.4%,53.2%and 38.7%,and specificity of 74.0%,93.6%and 81.4%,respectively.Conclusions 25(OH)D is the influencing factor of cardiac autonomic nervous dysfunction in patients with T2DM,and patients with high degree of deficiency are more prone to cardiac autonomic nervous dysfunction.
2.Analysis of karyotype and copy number variation in 386 fetuses with increased nuchal translucency

Objective To investigate the karyotypes and the correlation of chromosomal abnormalities in the fetuses with increased nu-chal translucency(NT),so as to provide a basis for prenatal genetic counseling.Methods The clinical data of 386 singleton pregnant women with NT≥2.5mm who underwent invasive prenatal diagnosis at the First Affiliated Hospital of Nanjing Medical University from January 2018 to April 2022 were retrospectively analyzed.The fetuses were grouped according to NT thickness(2.5-3.4,3.5-3.9,4.0-4.9,5.0-5.9,and ≥6.0 mm),fetal ultrasound abnormalities(isolated increased NT,non-isolated increased NT),and maternal age(advanced age ≥35 years,non-advanced age<35 years).The chi-square test was used to compare the differences of the incidence of fetal chromosomal abnormalities among various groups.Results Among the 386 fetuses with increased NT,chromosomal abnormalities were detected in 87 cases with an overall detection rate of 22.5%(87/386),including chromosomal numerical abnormalities accounted for 82.8%(72/87)and copy number variations(CNVs)accounted for 17.2%(15/87).The detection rates of chromosomal abnormal-ities and numerical abnormalities increased with NT thickness(P<0.05),while no statistically significant difference of CNV abnormali-ty rates was found(P=0.41).The detection rates of chromosomal abnormalities(36.5%)and CNV abnormalities(14.1%)in the non-isolated increased NT group were significantly higher than those in the isolated increased NT group(18.6%and 1.0%,respective-ly,both P<0.05).The detection rates of chromosomal abnormalities(34.7%)and numerical abnormalities(31.6%)in the fetuses of advanced maternal age mothers with increased NT were significantly higher than those in the non-advanced age group(18.4%and 14.2%,respectively,both P<0.05).However,the difference of CNV abnormality rates between the two groups was not statistically sig-nificant(P=0.62).Conclusion The detection rate of fetal chromosomal abnormalities elevated with increased NT thickness.Ad-vanced maternal age and the presence of other ultrasound abnormalities were the high-risk factors for fetal chromosomal abnormalities.The risks of CNV abnormalities may not be significantly correlated with NT thickness or maternal age but associated with the presence of other ultrasound abnormalities.
3.Hemolytic phenotype analysis of Staphylococcus aureus clinical isolates

Objective:
To investigate the hemolytic phenotypes of Staphylococcus aureus clinical isolates.
Methods:
The hemolytic phenotypes of 105 Staphylococcus aureus isolates were analyzed and summarized using the three-point inoculation method.Real-time fluorescence quantitative PCR was used to measure the mRNA expression levels of four hemolysin genes (hla,hlb,hlc,and hld) ; The VITEK 2 GP639 antimicrobial susceptibility card was used to detect resistance to commonly used antibiotics ; DNA gel electrophoresis was performed to determine the prevalence of the mecA,sea,tst,and pvl genes ; The microtiter plate crystal violet staining method was used to assess biofilm formation ability ; The CCK-8 assay was used to evaluate cytotoxicity against macrophages.
Results:
Seven hemo- lytic phenotypes were identified among the Staphylococcus aureus clinical isolates. Differences were found among Staphylococcus aureus clinical isolates with different hemolytic phenotypes in terms of mRNA expression levels of he- molysin genes,antibiotic resistance,virulence gene prevalence,biofilm formation ability,and cytotoxicity to mouse macrophages (P <0. 05 ) .
Conclusion
Staphylococcus aureus clinical isolates exhibit diverse hemolytic pheno- types,which should be a focus across multiple dimensions,including microbiological testing,clinical treatment, and nosocomial infection prevention and control.
4.Study on the antagonistic activity of Lactococcus garvieae SHAMU-LG6 against Staphylococcus

Objective :
To investigate the antagonistic activity of Lactococcus garvieae SHAMU-LG6 against Staphy- lococcus .
Methods :
VITEK 2 GP identification card , Microflex LT MALDI-TOF mass spectrometer and 16S rDNA amplification sequencing were used to identify the strain species . The antagonistic activity of L. garvieae SHAMU- LG6 against different Staphylococcus was detected by Oxford cup method for bacterial inhibition ; the antimicrobial active components were preliminarily isolated and purified by adsorption on XAD16 nonionic macroporous resin , gradient ethanol elution and rotary evaporation drying.
Results :
L. garvieae SHAMU-LG6 exhibited potent antago- nistic effect against methicillin-resistant Staphylococcus aureus , methicillin-susceptible S. aureus , S. epidermidis , S. saprophyticus , S. lugdunensis , S. hominis , S. capitis and S. warneri , with inhibitory indices of 3 . 3 , 3 . 0 , 4. 3 , 2. 0 , 4. 0 , 3 . 5 , 3 . 8 , and 3 . 5 , respectively. The antimicrobial active components produced by L. garvieae SHAMU-LG6 were mainly present in 70% and 80% ethanol eluates .
Conclusion
L. garvieae SHAMU-LG6 ex- hibits a potent antagonistic effect on Staphylococcus , and the antimicrobial active components produced by it are ex- pected to be a lead compound for the development of novel antimicrobial agents .
5.Antimicrobial activity of garvicin LG6 against Staphylococcus aureus of different hemolytic phenotypes

Objective To explore the antimicrobial activity of the bacteriocin(tentatively named garvicin LG6)se-creted by Lactococcus garvieae(L.garvieae)SHAMU-LG6 against Staphylococcus aureus(S.aureus)of different hemolytic phenotypes.Methods S.aureus isolated from clinical patients in a hospital of Anhui from 2021 to 2023 were collected.The hemolytic phenotypes of S.aureus were detected by three-point inoculation method.S.aureus of different hemolytic phenotypes were further categorized into methicillin-sensitive S.aureus(MSSA)and methi-cillin-resistant S.aureus(MRSA)according to antimicrobial susceptibility testing results.The antagonistic activity of L.garvieae SHAMU-LG6 against S.aureus of different hemolytic phenotypes was assayed by Oxford cup me-thod.The whole-genome sequencing of L.garvieae SHAMU-LG6 was performed.Biosynthetic gene cluster of gar-vicin LG6 was searched by online databases antiSMASH 7.0 and BAGEL4.Through macroporous resin adsorption,ethanol gradient elution,rotary evaporation,and dried material reconstitution,antimicrobial activity of garvicin LG6 crude extract against S.aureus was detected by the inhibitory testing of Oxford cup method.Results L.garvieae SHAMU-LG6 could significantly antagonize MSSA and MRSA of different hemolytic phenotypes.Biosynthetic gene cluster of garvicin LG6 was present on the chromosomal genome of L.garvieae SHAMU-LG6.The antimicrobial activity of garvicin LG6 secreted by a single colony or 6 mL fermentation fluid of L.garvieae SHAMU-LG6 were at least equal to that of antibiotic disc of 30 pg cefoxitin.Conclusion Garvicin LG6 can efficiently kill MSSA and MR-SA of different hemolytic phenotypes,and has the potential to be developed into a novel antimicrobial agent,which has great prospects for clinical application.
6.Path analysis of the effect of economic toxicity on anxiety symptoms in maintenance hemodialysis patients

Objective:To analyse the role of economic toxicity on anxiety symptoms in maintenance hemodialysis (MHD) patients, in order to provide reference for reducing anxiety symptoms and improving mental health literacy in MHD patients.Methods:This was a cross-sectional survey. A convenience sampling method was used to select MHD patients from Hangzhou Second People's Hospital Affiliated to Hangzhou Normal University and Deqing Hospital Affiliated to Hangzhou Normal University as the study subjects. The General Information Questionnaire, Economic Toxicity Composite Rating Scale, Family Resilience Questionnaire, Chronic Disease Management Self-Efficacy Scale, Social Support Rating Scale, and Self-rating Anxiety Scale were applied.The pathway of economic toxicity on anxiety symptoms in MHD patients was analyzed by AMOS26.0.Results:A total of 290 questionnaires were distributed, and 274 valid questionnaires were collected, with an effective response rate of 94% (274/290). Among the patients, there were 153 males and 121 females, aged (62.35 ± 12.57) years. The patients' economic toxicity score was (23.07 ± 5.36) points and anxiety symptom score (54.65 ± 17.31) points. The structural equation model was well fitted with χ2/ df=1.452, root mean square error of approximation was 0.041,goodness-of-fit index was 0.946,comparative fit index was 0.982, incremental fit index was 0.983, Tucker-Lewis index was 0.978. The results of the path analysis showed that economic toxicity in patients with MHD directly predicted anxiety symptoms, with an effect size of 36.4%; self-efficacy, family resilience, and social support played a significant role in the pathway between economic toxicity and anxiety symptoms pathways, with effect sizes of 23.4%, 27.4%, and 12.8%, respectively. Conclusions:Economic toxicity and anxiety symptoms in MHD patients are at risk values, and economic toxicity can directly affect anxiety symptoms, or indirectly through the mediating role of self-efficacy, family resilience, and social support.
