Inflammatory bowel disease (IBD) is an idiopathic intestinal inflammatory condition with chronic and relapsing manifestations and is characterized by a disturbance in the interplay between the intestinal microbiota, the gut, and the brain. The microbiota-gut-brain axis involves interactions among the nervous system, the neuroendocrine system, the gut microbiota, and the host immune system. Increasing published data indicate that psychological stress exacerbates the severity of IBD due to its negative effects on the microbiota-gut-brain axis, including alterations in the stress response of the hypothalamic-pituitary-adrenal (HPA) axis, the balance between the sympathetic nervous system and vagus nerves, the homeostasis of the intestinal flora and metabolites, and normal intestinal immunity and permeability. Although the current evidence is insufficient, psychotropic agents, psychotherapies, and interventions targeting the microbiota-gut-brain axis show the potential to improve symptoms and quality of life in IBD patients. Therefore, further studies that translate recent findings into therapeutic approaches that improve both physical and psychological well-being are needed.

ObjectiveTen Chinese patent medicines for stable angina pectoris with qi stagnation and blood stasis were comprehensively evaluated， with the aim of providing reference for optimizing the drug list of medical institutions and promoting rational drug utilization. MethodAccording to the Expert Consensus on the Selection of Chinese Patent Medicines in Medical Institutions 2022， an evaluation framework of "6+1" dimensions was established for comprehensive evaluation of the drugs based on the indicators in three levels. ResultThe evaluation results showed that Compound Danshen dripping pills， Shexiang Baoxin pills， Di'ao Xinxuekang capsules， and Xuefu Zhuyu capsules were strongly recommended for stable angina pectoris with Qi stagnation and blood stasis， with the scores of 78.5， 76， 72， and 70.8， respectively. Suxiao Jiuxin pills， Xinkeshu Tablets， Guanxin Danshen Dripping pills， and Yindan Xinnaotong soft capsules were weakly recommended， with the scores of 68.5， 65.5， 60.5， and 60.5 respectively. Lemai pills and Yangxin Dawayimixike migao were not recommended for the time being due to their low scores （50.5 and 48.8， respectively）. There is a lack of research on special populations， pharmacokinetic parameters， post-marketing re-evaluation， and economics of the medicines. ConclusionWith the updating of Chinese patent medicines， real-world research data are needed to enhance the evidence-based support. This evaluation can only reflect the comprehensive situation at a certain stage， and dynamic evaluation remains to be carried out to provide support for decision makers.

Objective Some colorectal cancer patients still face high recurrence rates and poor prognoses even after they have undergone the surgical treatment of radical resection.Identifying potential biochemical markers and therapeutic targets for the prognostic evaluation of patients undergoing radical resection of colorectal cancer is crucial for improving their clinical outcomes.Recently,it has been reported that the T cell immunoglobulin and mucin domain protein 3(Tim-3)and its ligand galactose lectin 9(galectin-9)play crucial roles in immune dysfunction caused by various tumors,such as colorectal cancer.However,their expressions,biological functions,and prognostic value in colorectal cancer are still unclear.This study aims to investigate the relationship between Tim-3 and galectin-9 expression levels and the clinicopathological characteristics and prognosis of patients undergoing radical resection of colorectal cancer.Methods A total of 171 patients who underwent radical resection of colorectal cancer at Chengdu Fifth People's Hospital between February 2018 and March 2019 were selected.Immunohistochemistry was performed to assess the expression levels of Tim-3 and galectin-9 in the cancer tissue samples and the paracancerous tissue samples of the patients.The relationship between Tim-3 and galectin-9 expression levels and the baseline clinical parameters of the patients was analyzed accordingly.Kaplan-Meier analysis was performed to assess the association between Tim-3 and galectin-9 expression levels and the relapse-free survival(RFS)and the overall survival(OS)of colorectal cancer patients.Cox regression analysis was conducted to identify factors associated with adverse prognosis in the patients.Results The immunohistochemical results showed that the high expression levels of Tim-3 and galectin-9 were observed in 70.18%(120/171)and 32.16%(55/171),respectively,of the colorectal cancer tissues,whereas the low expression levels were 29.82%(51/171)and 67.84%(116/171),respectively.Furthermore,the expression score of Tim-3 was significantly higher in colorectal cancer tissues than that in the paracancerous tissues,while the expression score of galectin-9 was lower than that in the paracancerous tissues(P＜0.05).Further analysis revealed that the expression of Tim-3 and galectin-9 was associated with the depth of tumor infiltration,vascular infiltration,and clinical staging(P＜0.05).During the follow-up period of 14-63 months,7 out of 171 patients were lost to follow-up.Among the remaining patients,49 and 112 cases presented abnormally low expression of Tim-3 and galectin-9,respectively,whereas 115 and 52 cases presented high expression of Tim-3 and galectin-9,respectively.Kaplan-Meier survival analysis demonstrated that patients with high Tim-3 expression in colorectal cancer tissues had significantly lower RFS and OS than those with low expression did(RFS:log-rank=22.66,P＜0.001;OS:log-rank=19.71,P＜0.001).Conversely,patients with low galectin-9 expression had significantly lower RFS and OS than those with high expression did(RFS:log-rank=19.45,P＜0.001;OS:log-rank=22.24,P＜0.001).Cox multivariate analysis indicated that TNM stage Ⅲ(HR=2.26,95%CI:1.20-5.68),high expression of Tim-3(HR=0.80,95%CI:0.33-0.91),and low expression of galectin-9(HR=1.80,95%CI:1.33-4.70)were independent risk factors affecting RFS and OS in patients(P＜0.05).Conclusion Aberrant expression of Tim-3 and galectin-9 is observed in colorectal cancer tissues.High expression of Tim-3 and low expression of galectin-9 are closely associated with adverse clinico-pathological characteristics and prognosis.They are identified as independent influencing factors that may trigger adverse prognostic events in patients.These findings suggest that Tim-3 and galectin-9 have potential as new therapeutic targets and clinical indicators.

OBJECTIVE To comprehensively evaluate four weekly preparations of glucagon-like peptide-1 receptor agonist （GLP-1RA） marketed in China，and to provide evidence for hospitals to optimize drug catalogs and clinical rational drug use. METHODS Mini health technology assessment method was used to establish detailed evaluation rules according to A Quick Guideline for Drug Evaluation and Selection in Chinese Medical Institutions， and conduct comprehensive evaluation of four GLP- 1RA weekly preparations from aspects of pharmaceutical characteristics， effectiveness， safety， economy and other attributes. RESULTS Mini health technology assessment scores of the four GLP-1RA weekly preparations from high to low were dulaglutide 78.60 points， semaglutide 77.35 points，polyethylene glycol loxenatide 67.40 points， and exenatide microspheres 65.50 points， respectively. Dulaglutide had advantages in reducing blood sugar， arteriosclerotic cardiovascular disease， kidney benefits， and cost- effectiveness. Semaglutide had advantages in reducing blood sugar and weight loss， but its cost-effectiveness was lower than that of dulaglutide. Exenatide microspheres had advantages in the use of children， but its daily average treatment cost is the highest. Polyethylene glycol loxenatide needed further clinical evidence. CONCLUSIONS Four GLP-1RA weekly preparations all have high pharmaceutical comprehensive scores. Dulaglutide and semaglutide may have more comprehensive pharmaceutical value among them， while the use of exenatide microspheres for children is unique.

Objective To evaluate the association between the decrease of the perioperative serum carcinoembryonic antigen (CEA) level during perioperative period and the prognosis in patients with colon cancer after a curative resection.Methods Retrospective analysis was conducted to evaluate the relationship between preoperative serum CEA level and different clinicopathologic features in 605 cases who underwent a curative resection for colon cancer from January 2006 to April 2011.According to the preoperative serum CEA level,the patients were divided into two groups:≤5 ng/ml and ＞5 ng/ml group.The critical value of the CEA decreasing rate in preoperative serum CEA ＞ 5 ng/ml group was calculated,and the relationship between this critical value and survival rate was then analyzed.Univariate and multivariate models were used to detect the risk factors of overall survival rate (OS) and disease free survival (DFS) in preoperative serum CEA ＞ 5 ng/ml patients.Results The preoperative serum CEA levels were significantly associated with lymphatic invasion (x2 =14.122,P＜0.001),T stages (x2 =40.153,P ＜0.001),N stages (x2 =22.721,P ＜0.001) and pathological stages (x2 =38.576,P ＜ 0.001),except for sex (x2 =0.453,P =0.501),age (x2 =0.195,P =0.659) and histological stages (x2 =6.135,P =0.112).The critical values of CEA decreasing rate for OS and DFS were 48.95％ and 50.81％ in preoperative serum CEA ＞5 ng/ml group respectively.There were significant differences of 5-year OS (31.37％ vs.76.63％,x2 =43.235,P ＜ 0.001) and 5-year DFS (27.69％ vs.72.10％,x2 =55.561,P ＜0.001) between patients after operation whose CEA decreasing rate were lower than critical value and those whose were higher.Univariate analysis showed that the decreasing rate of CEA was an influence factor for OS (x2 =43.235,P ＜ 0.001) and DFS (x2 =55.561,P ＜ 0.001) of preoperative serum CEA ＞ 5 ng/ml patients.The N stages and pathological stages were both related to OS (x2 =14.683,P＜0.001;x2 =12.295,P＜0.001) and DFS (x2 =16.212,P＜0.001;x2 =13.704,P＜0.001)respectively.Multivariate model showed that the decreasing rate of CEA level and N stages were both associated withOS (x2=18.885,P＜0.001;x2 =7.523,P＜0.001) and DFS (x2 =19.275,P＜0.001;x2 =6.997,P ＜ 0.001) of preoperative serum CEA ＞ 5 ng/ml patients.Conclusion A high decreasing rate of serum CEA level after operation in colon cancer patients who have high CEA levels before the curative resection can be a protective factor for prognosis,especially when the decreasing rate is higher than the critical value.

Objective To compare the difference of MRI characteristics of pilomyxoid astrocytomas (PMA)and pilocytic astrocy-tomas (PA)in hypothalamus/optic chiasma region.Methods The MR images and clinical materials of 20 PMAs and 20 PAs proved by pathology were reviewed retrospectively.Imaging characteristics and clinical materials were analyzed,including age,gender, size,the signal characteristics of the tumor,growth,enhancement pattern,hydrocephalus and leptomeningeal dissemination.Results There were statistically significant differences in streaky low signal intensity among the high intensity of T2 WI and leptomeningeal dissemination between PMA and PA (P <0.05).However,no statistically significant differences were found in the other character-istics and clinical materials between the two types of tumors.Conclusion PMA and PA in hypothalamus/optic chiasma region are difficult to make differential diagnosis.The streaky low signal intensity among the high intensity of T2 WI and more frequent lepto-meningeal dissemination are helpful differential features of PMA as compared to PA.

Objective To compare the bony fusion in different degrees of vertebral plate hinge fracture after single open-door cervical expansive laminoplasty with centerpiece mini-plate fixation.Methods The clinical data of 79 patients received the unilateral simple open-door cervical expansive laminoplasty with centerpiece mini-plate fixation from January 2010 to December 2012 were retrospectively analyzed.The vertebral plate hinge fracture was divided into incomplete fracture and complete fracture according to thepostoperative CT scan.The complete fracture was divided into four types:type Ⅰ in which there was no displacement;type Ⅱ in which there was mild to moderate displacement;type Ⅲ in which there was complete displacement or separated; type Ⅳ in which there was hinge' s collapse into cervical canal.The hinge fracture healing was observed and the difference of different types of fracture healing rate was compared.Results Follow-up of 12-45 (21.2 ± 5.6) months.CT scan found 395 segments 1 week after the operation,vertebral plate hinge complete fracture accounted for 58.2％ (230/395).And type Ⅰ accounted for 66.1％ (152/230),type Ⅱ accounted for 25.7％(59/230),type Ⅲ accounted for 6.5％(15/230) and type Ⅳ accounted for 1.7％ (4/230).The fusion rate of incomplete fracture at 6 months after the operation was higher than that complete fracture [97.6％(161/165) vs.80.9％(186/230)],there was significant difference (P＜ 0.05).The fracture rate in different types of complete fracture at 3,6 months after the operation had significant difference (P ＜ 0.05).Type Ⅲ fracture had the lowest healing rate.Conclusion Type Ⅰ fracture is the major type for vertebral plate hinge complete fracture,complete fracture get a relatively bad bony fusion and type Ⅲ fracture has the lowest healing rate.

Objective To explore the influence of chemoradiotherapy combined with inhalation of hyperbaric oxygen in short-term therapeutic effectiveness,viability and quality of life of postoperative patients with malignant gliomas.Methods A total of 86 patients with malignant gliomas,admitted to our hospital from January 2009 to December 2013,were chosen in our study; according to the wishes of patients and their families,all patients were divided into two groups:chemoradiotherapy combined with inhalation of hyperbaric oxygen group (n=47) and chemoradiotherapy group (n=39).Chemotherapy regimens were given:temozolomide of 150 mg/m2 body surface area at the first period once daily for a consecutive five days and temozolomide of 200 mg/m2 body surface area since the second period once daily for consecutive five days.Hyperbaric oxygen therapy was given with duration of 60 min at pressure of 2.8 atm.Conventional radiotherapy (6000 cGy/30F) was given in both groups.WHO established solid tumor chemosensitivity criteria was used to evaluate the effective rate of total symptoms,and activity of daily living (ADL) scale and F-36 scale were employed to evaluate the viability and life quality before and three and six months after the intervention.Results The effective rate of total symptoms,ratio of ADL＞60 points in chemoradiotherapy combined with inhalation of hyperbaric oxygen group 3 and 6 months after treatment were 95.74 ％,53.2％ and 72.3％,which were significantly higher than those in the chemoradiotherapy group (58.97 ％,30.8％ and 48.7％,P＜0.05).The scores of all the items of SF-36 scale in chemoradiotherapy combined with inhalation of hyperbaric oxygen group 3 and 6 months after treatment were significantly higher than those in the chemoradiotherapy group (P＜0.05).Conclusion Inhalation of hyperbaric oxygen can significantly improve short-term therapeutic effectiveness,survival duration,viability and quality of life of postoperative patients with malignant gliomas,and obviously improve the prognosis of patients.

Metabonomics approach is a science that systematically studies the regularity of changes of metabolites and reveals the nature of metabolic activities of the lives in the dynamic process of metabolism. In this study, pregnant C57BL/6J mice were randomly divided into two groups with 15 mice in each. The ones in the experimental group were intraperitoneally injected with Dexamethasone and the others in the control group with isotonic Na chloride from 10th to 12th day of gestational period. On 17.5th day, all the mice were executed. The livers were extracted and prepared into aqueous soluble liver tissue extracts. The technology of nuclear magnetic resonance (NMR) was used to detect the endogenous small molecule metabolites. Finally, through the method of principal component analysis (PCA), changes of metabolites ingredients could be determined. The experimental results showed that there was significant difference in PCA scores plot between the two groups. Furthermore, the difference was in line with that of incidence of cleft palate in the embryos between the two groups. Therefore, metabonomics results can be used to reflect the changes of metabolites group interfered by Dexamethasone in the course of pregnancy of C57BL/6J mice and this method opens up a new way for further study of pathogenic mechanism of cleft lip with or without palate.

In order to investigate the effect of T-bet on malignant cells, we selected SGC-7901, a kind of human gastric carcinoma cell line, and used gene clone technique and adeno-associated virus (AAV) packing technology, thus obtaining a recombinant rAAV-eGFP-T-bet and T-bet gene-transfected SGC-7901 cells. Then the function of T-bet gene-infected SGC-7901 cells was researched by detecting the levels of IFN-gamma and T-bet production. The results showed: (1) It was verified that rAAV-T-bet's packing was completed; (2) After SGC-7901 cells was transfected by rAAV-eGFP-T-bet, a green fluorescence was found in about 30%-40% SGC-7901s, and the gene of 1670 bp (T-bet) and 388 bp (IFN-gamma) were generated from SGC-7901s cells; (3) The proteins of IFN-gamma and T-bet secreted by SGC-7901 cells were also detected. These reveal that SGC-7901 cell is efficiently infected by rAAV encoding T-bet, which can induce transfected cells to secret IFN-gamma. It may be useful in the researches on cancer immune therapy of transfecting T-bet gene.
