1.Assessment of the risk of retinal arteriosclerosis by the triglyceride glucose-WHR index

Objective:To investigate the relationship between Triglyceride Glucose-WHR index and retinal arteriosclerosis.Methods:Retrospective longitudinal cohort research method. Using data from the Tongren Health Care Study of Beijing Tongren Hospital from March 2014 to June 2020. The TyG-WHR index was calculated, the restricted cubic spine regression model was used to explore the dose-response relationship between TyG-WHR index and the risk of retinal arteriosclerosis. Cox proportional regression model was implemented to estimate the impact on the risk of retinal arteriosclerosis associated with the different TyG-WHR groups. Receiver operating characteristic were used to explore the value of triglyceride glucose-WHR index for the risk of retinal arteriosclerosis.Results:A total of 8 215 subjects were included, including 3 334 (40.58%) males, 4 881 (59.42%) females;7 689 cases had normal fundus, 296 cases had newly developed retinal arteriosclerosis, and 230 cases had retinal arteriosclerosis. The median age is 45 years old, and the average age is (47.63±13.89) years old. The cumulative incidence rate of new retinal arteriosclerosis was 3.71%(296/7 985), and the cumulative incidence rate of women (3.31%,158/4 767) was lower than that of men (4.29%, 138/3 218) ( χ2=5.105, P<0.001); The cumulative incidence rate of new retinal arteriosclerosis increased with age ( χ2=365.133, P<0.001); The TyG-WHR index was divided into four groups according to the interquartile range, and the Q2- Q4 group had an increased risk of developing new retinal arteriosclerosis compared to Q1 ( χ2=132.887, P<0.001).In the restricted cubic spine regression model, the results showed a non-linear dose-response relationship ( χ2=54.27, P<0.001) between baseline TyG-WHR index and the new-onset of retinal arteriosclerosis. By the multivariate Cox Regression models, three models were constructed in this study. TyG-WHR index was positively correlated with the risk of retinal arteriosclerosis: the HR (95% CI) of model 1-3 was 1.534,1.517 and 1.502.The AUC curve analysis verified that the prediction AUC of the TyG-WHR index for new-onset retinal arteriosclerosis was 0.755, the best prediction value was 7.343, the sensitivity was 90.2%, and the specificity was 75.60%. Conclusion:TyG-WHR index may be an independent risk factor for new-onset retinal arteriosclerosis.

Adaptive immune cells play an important role in tumor immunity.The function of T cells in tumor microenviron-ment has been studied in detail,but B cells needs to be further explored.Due to the different characteristics of producing antibodies,secreting cytokines,and presenting antigens,tumor-infiltrating B cells are divided into two phenotypes:promoting and inhibiting tu-mor proliferation.This study reviews the specific functions of B cells in the tumor microenvironment in order to explore the mechanisms of B cells involved in tumor immunity,which may provide a basis for the precision treatment targeting B cells in tumor.

4.Analysis of iodine nutritional status monitoring results of children aged 8 - 10 and pregnant women in Xining City, Qinghai Province

Objective:To investigate the iodine nutrition status of children aged 8 - 10 and pregnant women in Xining City, Qinghai Province.Methods:From 2019 to 2021, a stratified cluster sampling method was used to divide 7 counties (districts) under the jurisdiction of Xining City, Qinghai Province into 5 sampling areas according to east, west, south, north, and center each year. One township (town, street) was selected from each area. Forty non boarding students aged 8 to 10 from each primary school (half male and half female, age balanced) and 20 pregnant women from each township (town, street) location were selected to collect edible salt samples at home and a random urine sample to measure salt iodine and urinary iodine level. B-ultrasound was used to measure thyroid volume in children and the goiter rate was calculated.Results:A total of 6 534 samples of household edible salt were collected from children and pregnant women, with an average salt iodine concentration of 25.58 mg/kg. The coverage rate of iodized salt was 97.50% (6 371/6 534), and the qualified iodized salt consumption rate was 89.46% (5 845/6 534). A total of 4 362 urine samples were collected from children, with a median urinary iodine level of 183.10 μg/L. The difference between different years was statistically significant ( H = 20.27, P < 0.001). A total of 2 169 urine samples were collected from pregnant women, with a median urinary iodine level of 168.90 μg/L. The difference between different years was statistically significant ( H = 107.09, P < 0.001). A total of 3 336 cases of thyroid gland examination were conducted in children, including 33 cases of thyroid enlargement, with a goiter rate of 0.99%. There was a statistically significant difference between different years (χ 2 = 15.00, P < 0.001). Conclusion:From 2019 to 2021, children aged 8 to 10 and pregnant women in Xining City are at an appropriate level of iodine, and the achievements in prevention and treatment of iodine deficiency disorders still need to be continuously consolidated.

7.Long-distance Tele-robotic-assisted Percutaneous Coronary Intervention: A Case Report of First-in-patient in China

Robotic-assisted percutaneous coronary intervention (R-PCI) is an innovative way of performing percutaneous coronary intervention (PCI) whereby the operator can manipulate coronary intraluminal guidewires and catheter devices by using remotely controlled technology. Performing tele-R-PCI from a remote location via fifth generation network communication technology has never been reported in China; however, if this were possible, the technique could be used to treat many patients with coronary artery disease who would otherwise not have the opportunity of treatment. The case of a 73-year-old male patient with coronary artery disease who underwent successful tele-R-PCI at 800 km from the operators is presented. Performing long-distance tele-R-PCI in patients with coronary artery disease is feasible with predictably successful outcomes when reliable network connectivity and local cardiac catheterization facilities are present.

9.Prevalence and risk factors of hypertensive retinopathy in a health-checkup population

Objective:To investigate the prevalence and risk factors of hypertensive retinopathy (HRP) in a non-diabetic population over 30 years old during routine health examinations.Methods:This was a cross-sectional study of a non-diabetic population over 30 years of age. The study was conducted during routine health examinations at the Tongren Hospital, Beijing, from January to December 2020. Fundus photographs were taken, and data including medical history, height, weight, and blood pressure were collected. Routine laboratory examinations were performed. The study population was divided into hypertension, transient hypertension, and non-hypertension groups. The prevalence of HRP was compared among the three groups. OR and 95% CI of HRP risk factors was estimated by binary logistic regression, adjusted for age and gender. Results:The prevalence of HRP was 4.3% in the non-diabetic population over 30 years old. Adjusted for age, gender, and systolic blood pressure, the prevalence of HRP in hypertension and transient hypertension groups, was both higher than in the non-hypertension group [ OR(95% CI) of 3.11(2.25-4.30) and 1.72(1.21-2.45), respectively]. The proportion of grade 1-2 HRP was higher (76.2%). There was no significant difference in the prevalence of grade 3 HRP among the three groups. Adjusted for age and gender, systolic blood pressure and creatinine clearance rate were independent risk factors for HRP in the hypertension group [ OR(95% CI): 1.22(1.01-1.48) and 1.66(1.12-2.46)] and transient hypertension group [ OR(95% CI): 1.48(1.10-2.06) and 1.95(1.03-3.46)]. SBP and DBP were independent risk factors for HRP in the non-hypertension group [ OR(95% CI): 1.68(1.07-2.63) and 1.61(1.06-2.44)]. Conclusions:There was a high prevalence of HRP among the non-diabetic population over the age of 30 and there was still relatively high risk of grade 3 HRP among the normotensive population.
10.Investigation of the improvement effect of Dihuang yinzi on neurological function of APP/PS1 double transgenic positive mice based on gut-brain axis and its mechanism

OBJECTIVE To investigate the improvement effect of Dihuang yinzi on neurological function of APP/PS1 double transgenic positive mice and its possible mechanism. METHODS APP/PS1 double transgenic positive mice were randomly divided into APP/PS1 positive model group, Dihuang yinzi low-dose, medium-dose and high-dose groups (12, 24, 48 g/kg,by the amount of crude drug), western medicine group (donepezil hydrochloride 0.8 mg/kg); APP/PS1 transgenic negative mice were included in as APP/PS1 negative normal group, with 6 mice in each group. Administration groups were given relevant medicine intragastrically; APP/PS1 positive model group and APP/PS1 negative normal group were given constant volume of normal saline intragastrically, once a day, for consecutive 28 d. After the last medication, the ability of learning and memory, intestinal flora diversity (only medium-dose group of Dihuang yinzi), pathological changes of neural cells in hippocampus, and the expressions of Bcl-2, Bax and brain-derived neurotrophic factor (BDNF) in mice of each group were investigated. RESULTS Compared with APP/PS1 positive model group, escape latency of mice was shortened significantly in Dihuang yinzi medium-dose and high-dose groups, while the times of crossing the platform significantly increased (P<0.05). Compared with APP/PS1 positive model group, Chao1 index of Dihuang yinzi medium-dose group was lower, while Shannon index was higher (P<0.05); OTU abundance of microbial flora was decreased to some extent; dominant flora was Clostridia o_ Clostridia_ vadinBB60_ Group, verruca G_ UCG_ 005. Compared with the APP/PS1 positive model group, the pathological changes of the nerve cells in the hippocampal CA1 area of mice were improved in the medium-dose and high-dose groups of Dihuang yinzi; the protein expressions of Bcl-2 and BDNF in the hippocampal tissue were significantly increased, while the protein expression of Bax was significantly decreased (P<0.05). CONCLUSIONS Dihuang yinzi can enhance the diversity of flora, change the type of dominant flora, and can inhibit the apoptosis of brain neurons, and increase the level of BDNF; its mechanism may be to protect nerve cells by means of gut-brain axis, thereby improving the learning and memory ability of APP/PS1 positive model mice.
