1.Polyphenolic compounds: Alleviating osteoarthritis by regulating inflammation and oxidative stress

Osteoarthritis (OA) is a prevalent degenerative joint disease predominantly affecting the elderly and is characterized by cartilage degradation, synovitis, and subchondral bone sclerosis. Despite its widespread occurrence, no effective pharmacological interventions currently exist to halt or reverse disease progression. Polyphenolic compounds, a diverse class of plant-derived substances, have attracted considerable attention for their potent anti-inflammatory and antioxidant activities. This review summarizes recent advances in understanding the multifaceted roles of polyphenols in OA. Specifically, polyphenols protect chondrocytes and preserve the extracellular matrix by mitigating oxidative stress, suppressing inflammation, regulating autophagy and cholesterol metabolism, and inhibiting programmed cell death pathways, including apoptosis, pyroptosis, and ferroptosis. Furthermore, they exert protective effects on synovial tissue by regulating macrophage polarization and inhibiting pathogenic fibroblast activation, while also contributing to the maintenance of subchondral bone homeostasis. Recent progress in nanotechnology-based delivery systems, designed to overcome the poor solubility and limited bioavailability of polyphenols, is also highlighted. Collectively, this review integrates mechanistic insights with emerging therapeutic strategies, underscoring the potential of polyphenolic compounds as disease-modifying agents for OA.
2.Application of systemic immune-inflammation index and cystatin C in evaluating severity and progno-sis of acute pulmonary embolism

Objective:To investigate the association of systemic immune-inflammation index(SII)and serum cysta-tin C(CysC)with the severity of acute pulmonary embolism(APE),and their predictive value for prognosis.Meth-ods:A total of 181 patients who were first diagnosed with APE in Cangzhou People's Hospital between January 2018 and January 2023 were retrospectively selected.The severity of APE was determined according to risk stratification criteria for pulmonary embolism,and the patients were divided into low-risk group(n=67),middle-risk group(n=81)and high-risk group(n=33).General clinical data and venous blood neutrophils,platelet and lymphocyte counts,CysC and other indicators were collected,and SII was calculated according to the formula.The relevant in-dicators were compared among three groups,and their correlation with the severity of APE was analyzed by Spearman correlation analysis.According to the prognosis,all APE patients were divided into favorable outcome group(n=129)and unfavorable outcome group(n=52).The general clinical data were compared and multivariate Cox regression analysis was used to study the influencing factors of unfavorable outcome in APE patients.The re-ceiver operating characteristic curve(ROC)was drawn to evaluate the predictive value of SII,CysC and their com-bination for the prognosis of APE patients.Nomogram model for prognosis was constructed.Results:Compared with patients in low-risk group,those in the middle-risk group and the high-risk group had significantly higher levels of serum creatinine,CysC and uric acid(P<0.05 or<0.01).The SII in the high-risk group was significant-ly higher than those of middle-risk group and low-risk group(P<0.001 all).Spearman correlation analysis showed that serum creatinine,CysC,uric acid and SII were significant positively correlated with the severity of APE(r=0.356,0.358,0.233,0.353,P<0.01 all).Compared with patients in the favorable outcome group,those in the unfavorable outcome group had significantly higher levels of D-dimer,serum creatinine,CysC,uric acid and SII(P<0.01 all).There was a statistically significant difference in the severity of APE between the two groups(P=0.001).Multivariate Cox regression analysis showed that CysC,SII,and middle or high risk of disease severity were independent risk factors for unfavorable outcome in APE patients(HR=1.001~14.453,P<0.05 or<0.01).ROC curve indicated that the AUC of SII,CysC and their combination in predicting unfavorable outcome of APE patients were 0.815(95%CI 0.749~0.881),0.747(95%CI 0.661~0.832)and 0.878(95%CI,0.821~0.936),respectively.The combined AUC of the two was significantly higher than those of SII and CysC alone(Z=-2.234,-3.500,P<0.05 or<0.01).Based on the above independent risk factors,the AUC values of the 1-year and 3-year unfavorable outcome nomogram models were 92.9 and 88.2,respectively.The calibration prediction curve and the ideal curve fitted well.The decision curve showed that the model had a good net benefit.Conclusion:SII and CysC are significant positively correlated with the severity of APE and they are independent risk factors for unfavor-able outcome of APE,and the combination of the two indicators has a good predictive value for the prognosis of APE.The nomogram constructed has good accuracy and practicability.
3.Advances in role and mechanism of traditional Chinese medicine active ingredients in regulating balance of Th1/Th2 and Th17/Treg immune responses in asthma patients.

Asthma is a chronic inflammatory disease involving multiple inflammatory cells and cytokines. Its pathogenesis is complex, involving various cells and cytokines. Traditional Chinese medicine(TCM) theory suggests that the pathogenesis of asthma is closely related to the dysfunction of internal organs such as the lungs, spleen, and kidneys. In contrast, modern immunological studies have revealed the central role of T helper 1(Th1)/T helper 2(Th2) and T helper 17(Th17)/regulatory T(Treg) cellular immune imbalance in the pathogenesis of asthma. Th1/Th2 imbalance is manifested as hyperfunction of Th2 cells, which promotes the synthesis of immunoglobulin E(IgE) and the activation of eosinophil granulocytes, leading to airway hyperresponsiveness and inflammation.Meanwhile, Th17/Treg imbalance exacerbates the inflammatory response in the airways, further contributing to asthma pathology.Currently, therapeutic strategies for asthma are actively exploring potential targets for regulating the balance of Th1/Th2 and Th17/Treg immune responses. These targets include cytokines, transcription factors, key proteins, and non-coding RNAs. Precisely regulating the expression and function of these targets can effectively modulate the activation and differentiation of immune cells. In recent years,traditional Chinese medicine active ingredients have shown unique potential and prospects in the field of asthma treatment. Based on this, the present study systematically summarizes the efficacy and specific mechanisms of TCM active ingredients in treating asthma by regulating Th1/Th2 and Th17/Treg immune balance through literature review and analysis. These active ingredients, including flavonoids, terpenoids, polysaccharides, alkaloids, and phenolic acids, exert their effects through various mechanisms, such as inhibiting the activation of inflammatory cells, reducing the release of cytokines, and promoting the normal differentiation of immune cells. This study aims to provide a solid foundation for the widespread application and in-depth development of TCM in asthma treatment and to offer new ideas for clinical research and drug development of asthma.

4.Developing a polygenic risk score for pelvic organ prolapse: a combined risk assessment approach in Chinese women.

Pelvic organ prolapse (POP), whose etiology is influenced by genetic and clinical risk factors, considerably impacts women's quality of life. However, the genetic underpinnings in non-European populations and comprehensive risk models integrating genetic and clinical factors remain underexplored. This study constructed the first polygenic risk score (PRS) for POP in the Chinese population by utilizing 20 disease-associated variants from the largest existing genome-wide association study. We analyzed a discovery cohort of 576 cases and 623 controls and a validation cohort of 264 cases and 200 controls. Results showed that the case group exhibited a significantly higher PRS than the control group. Moreover, the odds ratio of the top 10% risk group was 2.6 times higher than that of the bottom 10%. A high PRS was significantly correlated with POP occurrence in women older than 50 years old and in those with one or no childbirths. As far as we know, the integrated prediction model, which combined PRS and clinical risk factors, demonstrated better predictive accuracy than other existing PRS models. This combined risk assessment model serves as a robust tool for POP risk prediction and stratification, thereby offering insights into individualized preventive measures and treatment strategies in future clinical practice.

6.Diagnostic value of multi-slice spiral computed tomography for acute pulmonary embolism

Objective To evaluate the value of multi-slice spiral computed tomography (CT) for diagnosing acute pulmonary embolism, and to provide a reference for the precise diagnosis of acute pulmonary embolism. Methods We enrolled a total of 102 patients pathologically diagnosed with acute pulmonary embolism from January 2019 to October 2023. All the patients underwent CT scanning of central and segmental pulmonary arteries with a GE 64-slice spiral CT scanner. The diagnostic efficacy of multi-slice CT scans for acute pulmonary embolism was evaluated with the pathological results as the gold standard. Results Of the 102 patients pathologically diagnosed with acute pulmonary embolism, multi-slice CT detected 92 cases, with an accuracy of 90.20% (92/102), including 17 cases (18.48%) of embolism in left pulmonary arteries, 31 cases (33.70%) of embolism in right pulmonary arteries, and 44 cases (47.82%) of embolism in both pulmonary arteries. Multi-slice CT visualized a total of 9905 pulmonary artery branches, and 304 of them (3.07%) had embolism, with the highest embolism rate in lobar arteries (43.89%). Conclusion Multi-slice spiral CT has high accuracy for diagnosing acute pulmonary embolism by directly and clearly visualizing embolism in the arteries of the lungs, which deserves clinical promotion.
7.Determination of concentration and pharmacokinetics of 3-demethylcolchicine in rat plasma by HPLC

Objective To establish a method based on high performance liquid chromatography for the determination of 3-demethylcolchicine(3 DMC)in rat plasma and to study its pharmacokinetic behavior in rats.Methods The concentration of 3 DMC in rat plasma was determined by high performance liquid chromatography(HPLC).Plasma samples were precipitated by acetonitrile and supernatant was centrifuged for analysis.The determination conditions were as follows:The chromatography was performed on Alphasil VC-C18 column(4.6 mm × 150.0 mm,5.0 μm)with mobile phase consisted of water-acetonitrile-methanol(79∶16∶5),iso-elution at the flow rate of 0.8 mL·min-1.The detection wavelength was set at 240 nm,with polydacryin as the internal standard.The specificity,standard curve and lower limit of quantitation,precision and recovery,matrix effect and stability of the method were investigated.The rats were randomly divided into low,medium and high dose groups(1,2 and 4 mg·kg-1 3DMC were injected into the tail vein,respectively),and blood was collected from orbital venous plexus.The concentration of 3DMC in plasma samples was measured;the pharmacokinetic parameters were calculated using Drug And Statistics(DAS)2.0.Results 3DMC has a good linear relationship between 0.39 and 25.00 μg·mL-1,and its standard curve is y=5.96 × 10-2x+7.60 × 10-3(r=0.999 5).The lower limit of quantitation was 0.39 μg·mL-1,the intra-day and inter-day relative standard deviations were-5.08%to 0.75%,the recovery was 93.23%to 108.90%,the matrix effect was 94.88%to 104.00%,and the stability relative standard deviation(RSD)was less than 8.72%.All the results met the requirements.Pharmacokinetic parameters of low,medium and high dose groups:Cmax were(1.66±0.24),(4.36±0.78)and(9.73±1.42)mg·L-1,respectively;t1/2 were(0.31±0.02),(0.32±0.11)and(0.48±0.06)h,respectively;AUC0-t were(0.50±0.09),(1.53±0.16)and(2.67±0.11)mg·L-1·h,respectively.Conclusion The established method for the determination of 3DMC concentration in rat plasma is simple,specific and sensitive,and is suitable for the determination of 3DMC concentration and pharmacokinetic study in rat plasma.
8.Chemical constituents and anti-inflammatory activity of Dracocephalum tanguticum.

The chemical constituents of Dracocephalum tanguticum were investigated using normal-and reverse-phase silica gel column chromatography, RP-HPLC, and other separation techniques, combined with experimental methods such as NMR, UV, IR, MS, and ORD, as well as comparison with reported literature data. From the ethanol extract of D. tanguticum, 10 compounds were isolated and identified: dracotangusion C(1),(1R,4S,5S,10R)-(+)-germacrone-1(10)-4-diepoxide(2), 1β,4β,5β-trihydroxy-7(11),9-germacradien-8-one(3), curcumadione(4), β-sitosterol(5), lilacoside(6), diosmetin-7-O-β-D-glucopyranoside(7), diosmin(8), luteolin-7-O-glucoside(9), and luteolin(10). Among these, compound 1 is a novel sesquiterpenoid, while compounds 2-4 represent the first isolation of sesquiterpenoid components from this genus. By measuring the release of NO in the culture medium of lipopolysaccharide(LPS)-induced RAW264.7 macrophages, it was found that at an administration concentration of 20 μmol·L~(-1), all 10 compounds significantly inhibited the LPS-induced release of NO(P < 0.01), exhibiting anti-inflammatory effects.

9.Preparation of Macroporous Strong Cation Exchange Chromatography Medium by Allyl Activation and Its Application in Protein Separation

Strong cation exchange mediums have been widely applied in protein purification.In this work,the strong cation exchange chromatography medium was prepared by coupling small molecular ligands through allyl activation commonly used in agarose-based media.The matrix based on the macroporous microspheres was a copolymer of glycidyl methacrylate and ethylene glycol dimethacrylate.The effects of reaction factors on the ligand density were detailedly investigated.The ligand density in the prepared medium was controlled in the range of 0.126-0.370 mol/L(wet gel).Its adsorption performance was evaluated with lysozyme as a model protein.The static binding capacity was in the range of 37.0-81.0 g/L.The maxium dynamic binding capacity reached 37.7 g/L and the maximum lysozyme recovery was 98.97%.The strong cation exchange medium obtained here showed low non-specific adsorption and high separation efficiency on lysozyme.This strong cation exchange medium showed a great potential in rapid protein purification.
10.Determination of concentration and pharmacokinetics of 3-demethylcolchicine in rat plasma by HPLC
