1.Clinical efficacy of parathyroid autotransplantation into the forearm muscle in thyroid surgery: a retrospective cohort study on functional restoration

Purpose:
This study was conducted to evaluate the clinical efficacy of autologous parathyroid transplantation into the forearm brachioradialis muscle during thyroid surgery and analyze the timing of graft function recovery.
Methods:
A total of 52 patients undergoing thyroid surgery with autologous parathyroid transplantation were selected.Serum parathyroid hormone (PTH) levels were measured at various postoperative intervals, and instances of hypoparathyroidism were recorded.
Results:
Postoperative PTH levels on the transplant side were significantly higher than on the non-transplant side from week 1 to 1 year. Effective graft rates were 1.9% on day 1, 40.4% at week 1, 84.6% at 1 month, and stabilized at around 73% after 1 year. Transient hypoparathyroidism was observed in 17.3% of patients, with no permanent cases.
Conclusion
Autologous parathyroid transplantation into the forearm brachioradialis muscle effectively preserves parathyroid function, with grafts regaining optimal function within 2–3 months and the graft still having good secretory function 1 year after surgery.
2.Combination of CT/MRI LI-RADS With Second-Line Contrast-Enhanced Ultrasound Using Sulfur Hexafluoride or Perfluorobutane for Diagnosing Hepatocellular Carcinoma in High-Risk Patients

Objective:
The CT/MRI Liver Imaging Reporting and Data System (LI-RADS) demonstrates high specificity with relatively limited sensitivity for diagnosing hepatocellular carcinoma (HCC) in high-risk patients. This study aimed to explore the possibility of improving sensitivity by combining CT/MRI LI-RADS v2018 with second-line contrast-enhanced ultrasound (CEUS) LI-RADS v2017 using sulfur hexafluoride (SHF) or perfluorobutane (PFB).
Materials and Methods:
This retrospective analysis of prospectively collected multicenter data included high-risk patients with treatment-naive hepatic observations. The reference standard was pathological confirmation or a composite reference standard (only for benign lesions). Each participant underwent concurrent CT/MRI, SHF-enhanced US, and PFB-enhanced US examinations. The diagnostic performances for HCC of CT/MRI LI-RADS alone and three combination strategies (combining CT/ MRI LI-RADS with either LI-RADS SHF, LI-RADS PFB, or a modified algorithm incorporating the Kupffer-phase findings for PFB [modified PFB]) were evaluated. For the three combination strategies, apart from the CT/MRI LR-5 criteria, HCC was diagnosed if CT/MRI LR-3 or LR-4 observations met the LR-5 criteria using LI-RADS SHF, LI-RADS PFB, or modified PFB.
Results:
In total, 281 participants (237 males; mean age, 55 ± 11 years) with 306 observations (227 HCCs, 40 non-HCC malignancies, and 39 benign lesions) were included. Using LI-RADS SHF, LI-RADS PFB, and modified PFB, 20, 23, and 31 CT/MRI LR-3/4 observations, respectively, were reclassified as LR-5, and all were pathologically confirmed as HCCs. Compared to CT/MRI LI-RADS alone (74%, 95% confidence interval [CI]: 68%–79%), the three combination strategies combining CT/MRI LI-RADS with either LI-RADS SHF, LI-RADS PFB, or modified PFB increased sensitivity (83% [95% CI: 77%–87%], 84% [95% CI: 79%–89%], 88% [95% CI: 83%–92%], respectively; all P < 0.001), while maintaining the specificity at 92% (95% CI: 84%–97%).
Conclusion
The combination of CT/MRI LI-RADS with second-line CEUS using SHF or PFB improved the sensitivity of HCC diagnosis without compromising specificity.
3.Uniportal endoscopic decompression and debridement for infectious diseases of spine with neurological deficits: a retrospective study in China

Methods:
This retrospective study analyzed 32 consecutive IDS patients who underwent UEDD surgery. Clinical features, laboratory data (erythrocyte sedimentation rate and C-reactive protein), and treatment outcomes were analyzed.
Results:
Definite microorganisms were identified in 27 patients (84.3%), with 24 (88.9%) meeting cure criteria. The cure rate was significantly higher in the detected pathogen group compared to the undetected pathogen group (88.9% vs. 80%; χ²=19.36, p<0.0001). Metagenomic next generation sequencing (mNGS) provided faster diagnosis (41.72±6.81 hours) compared to tissue culture (95.74±35.47 hours, p<0.05). The predominant causative pathogen was Mycobacterium tuberculosis, followed by Staphylococcus aureus. Significant improvements were observed in Visual Analog Scale pain scores, from a mean of 7.9 preoperatively to 1.06 at 1 year postoperatively. The Oswestry Disability Index revealed a similar trend, showing significant improvement (p<0.05).
Conclusions
UEDD is a viable alternative to traditional open surgery for managing IDS in high-risk patients. UEDD offers a dual therapeutic-diagnostic advantage during the initial admission phase, enabling simultaneous debridement, neurological decompression, and targeted biopsy in a single intervention. Compared with traditional tissue culture, mNGS enables rapid microbiological diagnosis and extensive pathogen coverage.
4.Analysis of the Medication Patterns and Mechanisms of Traditional Chinese Medicine Patents for Treating Pulmonary Nod-ules Based on Data Mining and Network Pharmacology

OBJECTIVE To analyze the medication patterns of traditional Chinese medicine combinations for the treatment of pulmonary nodules in national patents using data mining and network pharmacology methods,providing a reference for the clinical treat-ment of pulmonary nodules.METHODS Patent data on traditional Chinese medicine compound prescriptions for the treatment of pul-monary nodules were collected from the China National Intellectual Property Administration Patent Inquiry System and the China Na-tional Knowledge Infrastructure(CNKI)patent database.Formula statistics were performed using Excel software.The Ancient and Modern Medical Case Cloud Platform was used to conduct high-frequency analysis of traditional Chinese medicine including frequency,properties and flavors,meridian tropism,efficacy categories,association rules,clustering,complex network analysis to screen out core drugs.Network pharmacology was then used to predict potential targets and pathways in patent prescriptions for the treatment of pulmo-nary nodules.RESULTS A total of 67 valid patents for the treatment of pulmonary nodules were included,involving 276 traditional Chinese medicines,with a cumulative total frequency of 859.The top five traditional Chinese medicines in terms of frequency of use were Glycyrrhiza uralensis Fisch,Astragalus membranaceus,Pinellia ternate,Curcuma zedoary and Hedyotis diffusa.These traditional Chinese medicines were primarily sweet and warm in property,primarily targeting the lung,liver,and spleen meridians,and their main effects were clearing heat,drying dampness and resolving phlegm,and promoting diuresis and reducing swelling.Association a-nalysis revealed that the top drug pairs were Scutellaria baicalensis Georgi-Pinellia ternate,Fagopyrum cymosum-Pinellia ternate,Ra-nunculus ternatus-Curcuma zedoary,Radix Angelicae Sinensis-Radix Paeoniae Alba,and Radix Angelicae Sinensis-Glycyrrhiza ura-lensis Fisch.Cluster analysis identified three drug combinations,and complex network analysis demonstrated that the core drug compo-nents were Scutellaria baicalensis Georgi,Pinellia ternate,Astragalus membranaceus,Curcuma zedoary,Fritillaria thunbergii,Fago-pyrum dibotryis,Hedyotis diffusa and Ranunculus ternatus.Network pharmacology analysis showed that the key targets for the treat-ment of lung nodules with patent prescriptions were GAPDH,IL6,TNF and so on.The core active ingredients were Baicalein,Moslosooflavone,and Norwogonin and so on.The main pathways involved were cancer pathways,lipids and arteriosclerosis,and viral carcinogenesis.CONCLUSION The inclusion of traditional Chinese medicine compound patent in this case is consistent with the eti-ology and pathogenesis of traditional Chinese medicine for treating lung nodules.Commonly used drug pairs and cluster prescriptions re-flect the flexible compatibility of traditional Chinese medicines in the treatment of pulmonary nodules,such as clearing away heat and toxic materials,resolving phlegm and eliminating swelling,regulating qi and strengthening spleen,promoting blood circulation and remo-ving blood stasis.The core drugs exert their effects on pulmonary nodules through multiple components,multiple targets and multiple pathways.
5.Preparation of the Fusion Protein Between Diphtheria Toxin Mutants and the Receptor Binding Domain of Botulinum Neurotoxin Serotype E(EHc)Molecules and the Immunological Effect Evaluation

CRM 197(cross-reacting material 197),a naturally occurring mutant of diphtheria toxin,is a safe and effective vaccine vector and extensively used on developing conjugate or combined vaccines.The mutant loses its enzymatic activity,but fully retains its receptor-binding ability and immunogenicity.In current work,the diphtheria toxin mutant CRM 197 and its fusion proteins with the receptor-binding do-main of botulinum neurotoxin serotype E(EHc)were developed using genetic engineering technology.These recombinant proteins were confirmed by Western blotting and SDS-PAGE.BALB/c mice were im-munized with the CRM197-EHc and EHc-CRM197 fusion proteins,and their immunogenicity was evalua-ted.These two fusion protein molecules,CRM197-EHc and EHc-CRM197,as subunit vaccines,elicited a robust humoral immune response targeting both CRM197 and EHc antigens in the immunized mice.Compared to the mixture of CRM197 and EHc,the mice vaccinated with the fusion proteins(CRM197-EHc and EHc-CRM197)induced higher levels of anti-CRM197 antibodies,and the mice vaccinated with EHc-CRM197 also generated strongest anti-EHc antibodies.Consequently,as a carrier molecule in the fusion protein vaccine,EHc enhances the immunogenicity of CRM197 molecules.Likewise,CRM197 boosts the immunogenicity of EHc in the EHc-CRM197 fusion protein.
6.Bioequivalence of rivaroxabanpian in healthy Chinese subjects

Aim To evaluate the bioequivalence of two oral preparations of rivaroxaban tablets(test preparation T and refe-rence preparation R)in fasting/postprandibular state in healthy Chinese subjects.Methods A randomized,open,single-dose,four-cycle,completely repeated crossover experiment was used in this study.A total of 70 healthy male and female subjects were enrolled,including 38 subjects in the fasting group and 32 sub-jects in the postprandial group.Rivaroxaban tablets(2.5 mg/tablet)were taken orally once per cycle and their reference preparations were tested.The plasma rivaroxaban concentration was determined by LC-MS/MS method.The pharmacokinetic parameters of rivaroxaban tablets were calculated by WinNonlin software,and the parameters were analyzed and processed.Re-sults The PK parameters of rivaroxaban tablets and reference preparations in fasting group were as follows:Cmax was(72.48±17.08)and(66.36±15.64)μg·L-1,respectively.AUC0-t were(383.49±101.06)and(370.43±102.16)h·ng·mL-1,and AUC0-inr were(389.58±102.28)and(375.84±103.01)h·μg·L-,respectively.Main PK parameters of subjects taking rivaroxaban tablets orally after meals:Cmax were(66.48±15.64 and 60.87±13.44)μg·L-1,AUC0-t were(404.44±72.58)and(381.80±79.93)h·μg·L-1,re-spectively.AUC0_inf was(410.88±73.55)and(393.64±69.71)h·μg·L-1,respectively.Under fasting and postmeal conditions,subjects took rivaroxaban test and reference prepara-tion orally,one tablet(2.5 mg/tablet)each time.The geometric mean of the main pharmacokinetic parameters of rivaroxaban in plasma(Cmax,AUC0-t,AUC0-inf)and their corresponding values had a 90%confidence interval ranging from 80.00%to 125.00%.No serious adverse events or unexpected adverse e-vents occurred in both groups.Conclusion Rivaroxaban tablets are bioequivalent and safe in vivo under fasting and postprandial conditions.
7.Safety of colostrum collection in the third trimester and evaluation of postpartum breastfeeding in pregnant women with hyperglycemia during pregnancy

Objective To preliminarily explore the safety of collecting colostrum in the third trimester,and to evaluate postpartum breastfeeding in pregnant women with hyperglycemia during pregnancy.Methods Pregnant women with hyperglycemia during pregnancy who had prenatal examinations in Obstetrics and Gynecology Hospital,Fudan University from Jul to Nov 2022 were prospectively divided into the colostrum collection group(n=52)in the third trimester and the control group(n=55)by randomized controlled grouping method.The t-test,χ2 test,Fisher's exact probability method and rank sum test were used for statistical analysis of the data to compare the delivery outcomes,neonatal outcomes and postpartum breastfeeding status between the two groups.Results There were no significant differences in the gestational weeks at delivery,delivery methods,breastfeeding rates at 42 days postpartum and 4 months postpartum between the two groups of pregnant women with hyperglycemia during pregnancy.There were also no significant differences in the Apgar scores at 1 minute and 5 minutes after birth and the neonatal hospitalization rate.The proportion of formula milk as the first supplementary feeding after delivery and the delayed lactation rate at 3 days postpartum in the colostrum collection group were significantly lower than those in the control group(P<0.05).The exclusive breastfeeding rates at 24 hours postpartum and 3 days postpartum in the colostrum collection group were significantly higher than those in the control group(P<0.05).Conclusion Collecting colostrum in the third trimester among pregnant women with hyperglycemia during pregnancy is safe,and it can reduce the rate of supplementary feeding with formula milk after delivery,and increase the exclusive breastfeeding rates at 24 hours postpartum and 3 days postpartum.
8.Exploration and Revelation on Multi-Level Cross-Domain Collaboration of Disciplines,Institutions and Systems:Analysis Based on Theoretical Teamwork Models in Integrated Cancer Medical Care

Objective To understand the theoretical models research of team collaboration in integrated healthcare systems for cancer treatment,analyze model characteristics and current research status,and provide theoretical references for effective practice in integrated healthcare systems.Methods Relevant studies were retrieved from five domestic and international literature databases,followed by literature screening and a systematic review.Results A total of 15 English-language articles were included,with the majority of first authors from the United States.The most publications were in 2022 and 2023.The studies were categorized into three types:interdisciplinary collaboration,inter-institutional collaboration,and inter-system collaboration.Conclusion Different levels of cross-domain team collaboration theoretical models exhibit common characteristics such as patient-centeredness,adaptability to local conditions,emphasis on patient participation,focus on primary healthcare,emphasis on technological innovation,and a forward-looking perspective.It is recommended to further research the effectiveness and mechanisms of integrated healthcare team collaboration models,considering the dynamic nature of regional characteristics and development stages to refine related studies.
9.ALKBH5 mediated m6A modification of NLRP3 promotes cardiomyocytes pyroptosis in mice with myocardial infarction

Aim To investigate the effects of m6A demethylase ALKBH5 on cardiomyocytes pyroptosis in mice with myocardial infarction(MI).Methods The MI model of left anterior descending coronary artery ligation surgery was established by knocking down ALKBH5 using adeno-associated virus,and the hypox-ia model of mouse cardiomyocytes(HL-1)was estab-lished by knocking down small interfering RNA.The effects of ALKBH5 on the pyroptosis of MI mice and hypoxic HL-1 cells were observed.Subsequently,mechanism studies were conducted at the cellular lev-el,and the binding of ALKBH5 and IGF2BP2 to NL-RP3 mRNA was detected through RNA pull down and RNA immunoprecipitation(RIP)experiments.The MeRIP-qPCR method was used to determine the effects of ALKBH5 on the mRNA m6A level of NLRP3.Acti-nomycin D for RNA stability experiments were conduc-ted to detect the effects of ALKBH5 and IGF2BP2 on the stability of NLRP3 mRNA.Results Knocking down ALKBH5 in vivo and in vitro both inhibited NL-RP3 inflammasome activation and alleviated pyroptosis in MI mice and hypoxic HL-1 cells.Mechanistically,the results showed that NLRP3 mRNA could bind to ALKBH5 protein in HL-1 cells;knocking down ALK-BH5 could increase the m6A level of NLRP3 and re-duce the stability of NLRP3 mRNA;subsequently,it was confirmed that NLRP3 mRNA and IGF2BP2 pro-tein bound to each other;knocking down IGF2BP2 in-creased the mRNA stability of NLRP3.The Rescue ex-periment showed that knocking down IGF2BP2 re-versed the decrease in NLRP3 mRNA expression caused by knocking down ALKBH5.Conclusions ALKBH5 mediated m6A modification of NLRP3 pro-motes cardiomyocytes pyroptosis in mice with myocardi-al infarction.
10.Diagnostic value of MRI in differentiating gastrointestinal stromal tumors and uterine adnexal tumors in female pelvis

Objective To explore the diagnostic value of MRI in differentiating between gastrointestinal stromal tumors and uterine adnexal tumors in female pelvis.Methods Imaging data were retrospectively collected from 42 patients admitted for pelvic mass and diagnosed with gastrointestinal mesenchymal tumor or uterine adnexal tumor by postoperative pathology in some hospital from 2016-2022,including 12 patients with gastrointestinal stromal tumor and 30 patients with uterine adnexal tumor composed of 20 ones with malignant epithelial tumors of the ovary,5 ones with ovarian sex cord-stromal tumors and 5 ones with exophytic uterine fibroids.The tumor blood vessels and normal ovarian structures were observed,and the positive rates of tumor vascular display were calculated;the location distribution of the cystic portions of gastrointestinal stromal tumors and ovarian malignant epithelial tumors was inspected;gastrointestinal stromal tumors and uterine adnexal tumors were compared in terms of conventional MRI signs including cystic or solid mass,morphology,signal characteristics and pelvic lymph nodes,and the accuracy,specificity and sensitivity of MRI for differentiating between gastrointestinal stromal tumors and uterine adnexal tumors were calculated with the postoperative pathological results as the gold standard.SPSS 26.0 software was used for statistical analysis.Results The overall display rate of tumor vascular was 80.9%,and the display rates being 75%,100%,60%and 40%for pelvic gastrointestinal stromal tumors,ovarian malignant epithelial tumors,ovarian sex cord-stromal tumors and exophytic uterine fibroids,respectively.A significant difference was found in the locations of the cystic portions between pelvic gastrointestinal stromal tumors and ovarian malignant epithelial tumors(P<0.001),while no significant differences were observed in other conventional MRI signs between the two groups(P>0.05).MRI demonstrated an overall diagnostic accuracy of 95.2%,with a sensitivity of 100%and a specificity of 83.3%.Conclusion MRI shows clearly the normal ovarian structure,ovarian vascular bundles and tumor MRI signs and has high rates for tumor vascular display,which is of great value for differentiating between gastrointestinal stromal tumors and uterine adnexal tumors in female pelvis.[Chinese Medical Equipment Journal,2025,46(9):60-64]
