Objective:To explore the incidence status, influencing factors and short-term prognosis characteristics of early onset coronary heart disease in women in Wansheng District of Chongqing, and to provide scientific basis for formulating regional prevention and treatment strategies. Methods:This study was a single-center retrospective study,100 coronary heart disease in women from January 2022 to December 2023 at Chongqing Wansheng Economic and Technological Development Zone People's Hospital were prospective selected, and they were divided into early onset group of 40 cases(≤ 65 years old)and late onset group of 60 cases(>65 years old)based on their age of onset.Another 60 healthy women who underwent physical examinations during the same period to exclude coronary heart disease were selected as the control group.Univariate factor and multiple factor logistic regression analysis were used to identify independent risk factors for early onset coronary heart disease in women.Draw receiver operating characteristic(ROC)curve for the subjects, the efficacy of risk factors in predicting early onset coronary heart disease based on the area under the curve(AUC)of ROC curve were evaluated.Patients were followed up for 1 year to observe the occurrence of major adverse cardiovascular events(MACE).Result:Among 100 fcoronary heart disease in women, the early onset group accounted for 40.00%(40/100).Univariate analysis showed that age, hyperlipidemia history, smoking history, hypertension history, family history, diabetes history, total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C)were related to the early onset coronary heart disease.Multivariate analysis showed that, hyperlipidemia history(OR=4.124,95%CI:2.343-7.217),smoking history(OR=3.564),hypertension(OR=3.253),family history(OR=2.981),diabetes history(OR=2.873)were independent risk factors.ROC curve analysis results showed that joint evaluation had the best predictive value, with AUC of 0.829,which was higher than the AUC of individual evaluation for each factor. The incidence of MACE in the early onset group(45.00%)was significantly higher than that in the late onset group(P<0.05).Conclusion:Early onset coronary heart disease in women in Wansheng District of Chongqing is related to the hyperlipidemia history, smoking, hypertension history, family history and diabetes history. The incidence of MACE in early-onset patients followed up for 1 year is higher than that in late-onset patients.

Objective:To establish a health economics evaluation-based adjustment mechanism for the medical insurance fund package in compact county-level medical community. Methods:A health risk assessment system was designed, with weights determined using the Delphi method.Population health risks were evaluated via rapid assessment methods to calculate health risk scores for each county(city, district),which were then used to determine the medical insurance fund package adjustment amount. Results:The county-level health risk evaluation index system encompassed four dimensions:health risk influencing factor, health protection resource, population health risk, and health level.Through information collection and field surveys, health risk scores for counties(cities, districts)in a case prefecture-level city were obtained, serving as a tool for allocating medical consortium package adjustment funds. Conclusion:The health economics evaluation-based medical insurance fund adjustment mechanism enhances the equity of total medical insurance fund allocation, activates the incentive role of medical insurance in healthcare delivery, and transmits intervention pressure to reduce residents'health risks to public management departments at the county level. This approach realizes mutual benefits among medical insurance, residents, and healthcare providers.

Objective To explore the targeted regulatory relationship of miR-330-5p on OY-TES-1 in glioblastoma and the effect of miR-330-5p/OY-TES-1 axis on the migration ability of glioblastoma. Methods Bioinformatics analysis was performed to analyze the expression level of miR-330-5p in patients with glioblastoma and its influence on prognosis and survival of patients. The glioblastoma cells U251 were divided into miR-330-5p minics group,minics-NC group, and miR-330-5p+OY-TES-1 overexpression group(miR-330-5p minics+pcDNA3.1-OY-TES-1).The effect of miR-330-5p on the activity of OY-TES-1 3'UTR region was detected by double luciferase reporter gene experiment. The expression of OY-TES-1 mRNA was detected by qRT-PCR.The effect of miR-330-5p/OY-TES-1 axis on the migration ability of glioblastoma cells was detected by Transwell migration assay. Results The expression of miR-330-5p in glioblastoma tissue was significantly lower than those in non-tumor brain tissue and low-grade glioma tissue(P<0.05).The survival time of glioblastoma patients with high expression of miR-330-5p was significantly longer than that of patients with low expression of miR-330-5p(P<0.05).After overexpression of miR-330-5p,the activity of OY-TES-1 3'UTR region was decreased(P<0.05).Compared with minics-NC group, the expression levels of OY-TES-1 mRNA of U251 and U87MG cells in miR-330-5p minics group were significantly decreased(P<0.01).Compared with minics-NC group, the numbers of migrating cells in miR-330-5p minics group and miR-330-5p+OY-TES-1 overexpression group were significantly decreased(P<0.05).Compared with miR-330-5p minics group, the number of migrating cells in miR-330-5p+OY-TES-1 overexpression group was significantly increased(P<0.01).Conclusion MiR-330-5p targets OY-TES-1 to inhibit the migration of glioblastoma.

Objective:To assess the impact of health insurance packaged payment in medical communities on the economicburden of patients, income and satisfaction of medical staff in counties. Methods:Using sample data from 2018-2022 from the national monitoring counties of medical communities, taking 2020 as the year of implementation of packaged payment,a double difference model was constructed with county population density and county per capita GDP as the control variables to assess the impact of packaged payment on the economic burden of patients, medical staff income and satisfaction in the county. Results:The packaged payment policy reduced the economic burden of patients to a certain extent and had a statistically significant positive effect on medical staff income in 2021,but it did not significantly increase the satisfaction of both supply and demand. Conclusion:The implementation of health insurance packaged payment of the MEC will not increase the economic burden of patients. It has a good pro-poor effect, and the income of medical staff has been improved to some extent, but there is still room for optimisation and improvement of the policy.
