1.Dendrobium officinale polysaccharide on high glucose-induced apoptosis in retinal capillary pericytes

AIM:To investigate the protective effects of Dendrobium officinale polysaccharide(DOP)on high glucose-induced apoptosis in retinal capillary pericytes and its potential mechanism involving mitochondrial function.METHODS:Retinal capillary pericytes were allocated into five groups: normal control(NC), high glucose(HG), and three DOP treatment groups(low, DOP-L; medium, DOP-M; high, DOP-H). Pericyte ultrastructure was analyzed using transmission electron microscopy(TEM). Apoptotic rate was quantified via Annexin V-FITC staining. Mitochondrial transmembrane potential was assessed using the JC-1 probe. Quantitative real-time polymerase chain reaction(qRT-PCR)and Western blot were employed to measure expression levels of cytochrome C(Cyt C), B-cell lymphoma 2(Bcl-2), Bcl-2-associated X protein(Bax), Caspase-9, and Caspase-3, respectively.RESULTS:Compared to the NC group, pericytes exposed to HG exhibited significant mitochondrial damage, elevated apoptotic rate, increased mRNA and protein expression of Cyt C, Bax, Caspase-9, and Caspase-3(all P<0.01), alongside a marked reduction in mitochondrial transmembrane potential and expression of Bcl-2 mRNA and protein(all P<0.01). In contrast, DOP treatment groups(DOP-M,DOP-H)dose-dependently ameliorated mitochondrial damage, reduced apoptotic rate, downregulated Cyt C, Bax, Caspase-9, and Caspase-3 expression, enhanced mitochondrial transmembrane potential, and upregulated Bcl-2 expression relative to the HG group(all P<0.05).CONCLUSION:DOP attenuates high glucose-induced apoptosis and mitochondrial injury in retinal capillary pericytes. The underlying mechanism may involve the restoration of mitochondrial transmembrane potential.
2.Mortality and years of life lost of residents with viral hepatitis among in Pudong New Area of Shanghai in 2003 - 2023

Objective To analyze the characteristics of viral hepatitis mortality and life loss among residents in Pudong New Area from 2003 to 2023, and to provide a basis for related prevention and control work. Methods Viral hepatitis mortality data were obtained from the Pudong New Area mortality monitoring system. The crude mortality rate (CMR), standardized mortality rate (SMR), potential years of life lost (PYLL), average years of life lost (AYLL), and standardized potential years of life lost (SPYLL) were calculated to analyze viral hepatitis deaths. The average annual change (AAPC) and annual percentage change (APC) of the mortality rate were calculated by Joinpoint regression analysis to analyze the trend of mortality. Results The CMR and SMR of viral hepatitis among residents in Pudong New Area from 2003 to 2023 were 3.89/100000 and 1.98/100000, respectively. Both CMR and SMR of viral hepatitis showed a decreasing trend over time (CMR:APC=-5.476, t=-13.581, P<0.001; SMR:APC=- 7.624, t= -21.253, P<0.001). The CMR for males was 4.75/100000 and the SMR for males was 2.65/100000; the CMR for females was 3.04/100000 and the SMR for females was 1.32/100000, with a higher mortality rate for males than for females(ZCME=12.094,P<0.001; ZSMR=-14.718,P<0.001). Deaths were concentrated in the age groups of 45-64 years old and 65 years old and above, accounting for 91.62% of the total deaths. The PYLL of deaths due to viral hepatitis among residents in Pudong New Area from 2003 to 2023 was 26912 person-years, with a PYLLR of 0.45% and an AYLL of 8.88 years per person. Conclusion The mortality rate of viral hepatitis among the residents of Pudong New Area in 2003-2023 shows a decreasing trend over time. The mortality rate of males is higher than that of females, and the deaths of middle-aged and elderly people account for a large proportion of the total deaths. Chronic hepatitis B is the main cause of death.
3.Experience of Seasonal Acupuncture for Treating Allergic Rhinitis Based on the Theory of Latent Pathogen

The seasonal onset or worsening of allergic rhinitis is consistent with the characteristics of not develo-ping, lurking in the body, and recurrent attacks due to the suffering of latent pathogen. Based on the theory of latent pathogen, a seasonal acupuncture treatment approach for allergic rhinitis is proposed. Qi is in the channels in spring, and it is advisable to take the Luo-connecting points and the Ying-spring points and transport points between the muscle interspaces of the body; qi is in the minute collaterals in summer, it is advisable to take the superficial collateral vessels and puncture them for bloodletting; qi is on the skin in autumn, and He-sea points and back-shu points are recommended; qi is in the bone marrow in winter, so it is advisable to take the Jing-well points and Ying-spring points and needle them deeply to the bone.
4.Clinical Efficacy and Mechanism of Bupi Qingfei Prescription in Treating Stable Bronchiectasis

ObjectiveTo explore the clinical efficacy and mechanism of Bupi Qingfei prescription (BPQF) in treating stable bronchiectasis in the patients with syndromes of lung-spleen Qi deficiency and phlegm-heat accumulation in the lungs. MethodsA randomized, double-blind, placebo-controlled trial was conducted. Patients were randomized into BPQF and placebo control (PC) groups. On the basis of conventional Western medicine treatment, the BPQF granules and placebo were respectively administered at 10 g each time, twice a day, for a course of 24 weeks. The TCM symptom scores, Quality of Life Questionnaire for Bronchiectasis (QOL-B) scores, lung function indicators, T lymphocyte subsets, level of inflammatory factors in the sputum, level of neutrophil elastase (NE) in the sputum, and occurrence of adverse reactions were observed before and after treatment in the two groups. ResultsA total of 64 patients completed the study, encompassing 32 in the BPQF group and 32 in the PC group. After treatment, the BPQF group showed decreased TCM symptom scores (P<0.01), increased QOL-B scores (P<0.01), and declined levels of tumor necrosis factor (TNF)-α and NE (P<0.05, P<0.01). The PC group showed decreased TCM symptom (except spleen deficiency) scores (P<0.01), increased the QOL-B health cognition and respiratory symptom domain scores (P<0.05, P<0.01), and a declined TNF-α level (P<0.01). Moreover, the BPQF group had lower TCM symptom (except chest tightness) scores (P<0.05, P<0.01), higher QOL-B (except treatment burden) scores (P<0.05, P<0.01), and lower levels of interleukin-6 and TNF-α (P<0.05) than the PC group. Neither group showed serious adverse reactions during the treatment process. ConclusionBPQF can ameliorate the clinical symptoms of stable bronchiectasis patients who have lung-spleen Qi deficiency or phlegm-heat accumulation in the lungs by regulating the immune balance and inhibiting airway inflammatory responses.
5.Study on the new workflow of PIVAS based on intelligent auxiliary devices

OBJECTIVE To build a new workflow of pharmacy intravenous admixture services (PIVAS), effectively connect intelligent equipment, and promote the intelligent development of PIVAS. METHODS Based on intelligent auxiliary equipment, PIVAS workflow was optimized, and a process-oriented model was established. This model integrated intelligent prescription review (automatic prescription review+manual intervention mode), intelligent labeling, intelligent allocation, intelligent sorting, and finished infusion quality inspection system. Furthermore, an assessment was conducted to examine unreasonable medical order rate of intelligent prescription review, the working efficiency and error rate of intelligent labeling machine and intelligent sorting machine, and the dispensing efficiency and accuracy of intelligent dispensing robot. RESULTS Under the intelligent prescription review mode, the rate of unreasonable medical orders decreased from 0.157% to 0.050% (P<0.05); automatic labeling efficiency reached 21.7 sheets/min, surpassing the manual labeling efficiency of 13.8 sheets/min (P<0.05), and the daily labeling error rate decreased from 6.1‰ to 2.5‰ (P<0.05). Simultaneously operating two dispensing robots significantly improved the efficiency of batch dispensing and reduced the residual amount of liquid medicine (P<0.05); additionally, a quality testing system for finished infusion was established, involving appearance, Tyndall effect, insoluble particles, turbidity, absorbance, pH and osmotic pressure, to ensure the quality of finished infusion and reduce the risk of infusion. CONCLUSIONS The new process of PIVAS connected with intelligent devices in our hospital can improve work efficiency, reduce dispensing errors, ensure the quality of finished infusion, and improve the level of pharmaceutical care.

Oral squamous cell carcinoma (OSCC) is a common head and neck malignancy. Approximately 50% to 60% of patients with OSCC are diagnosed at a locally advanced stage (clinical staging III-IVa). Even with comprehensive and sequential treatment primarily based on surgery, the 5-year overall survival rate remains below 50%, and patients often suffer from postoperative functional impairments such as difficulties with speaking and swallowing. Programmed death receptor-1 (PD-1) inhibitors are increasingly used in the neoadjuvant treatment of locally advanced OSCC and have shown encouraging efficacy. However, clinical practice still faces key challenges, including the definition of indications, optimization of combination regimens, and standards for efficacy evaluation. Based on the latest research advances worldwide and the clinical experience of the expert group, this expert consensus systematically evaluates the application of PD-1 inhibitors in the neoadjuvant treatment of locally advanced OSCC, covering combination strategies, treatment cycles and surgical timing, efficacy assessment, use of biomarkers, management of special populations and immune related adverse events, principles for immunotherapy rechallenge, and function preservation strategies. After multiple rounds of panel discussion and through anonymous voting using the Delphi method, the following consensus statements have been formulated: 1) Neoadjuvant therapy with PD-1 inhibitors can be used preoperatively in patients with locally advanced OSCC. The preferred regimen is a PD-1 inhibitor combined with platinum based chemotherapy, administered for 2-3 cycles. 2) During the efficacy evaluation of neoadjuvant therapy, radiographic assessment should follow the dual criteria of Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 and immune RECIST (iRECIST). After surgery, systematic pathological evaluation of both the primary lesion and regional lymph nodes is required. For combination chemotherapy regimens, PD-L1 expression and combined positive score need not be used as mandatory inclusion or exclusion criteria. 3) For special populations such as the elderly (≥ 70 years), individuals with stable HIV viral load, and carriers of chronic HBV/HCV, PD-1 inhibitors may be used cautiously under the guidance of a multidisciplinary team (MDT), with close monitoring for adverse events. 4) For patients with a poor response to neoadjuvant therapy, continuation of the original treatment regimen is not recommended; the subsequent treatment plan should be adjusted promptly after MDT assessment. Organ transplant recipients and patients with active autoimmune diseases are not recommended to receive neoadjuvant PD-1 inhibitor therapy due to the high risk of immune related activation. Rechallenge is generally not advised for patients who have experienced high risk immune related adverse events such as immune mediated myocarditis, neurotoxicity, or pneumonitis. 5) For patients with a good pathological response, individualized de escalation surgery and function preservation strategies can be explored. This consensus aims to promote the standardized, safe, and precise application of neoadjuvant PD-1 inhibitor strategies in the management of locally advanced OSCC patients.
7.Analysis of natural pregnancy outcome of pump twin after spontaneous cessation of blood flow in the umbilical artery of the acardiac twin with reverse arterial perfusion sequence

Objective:To analyze the relationship between the growth and development of pump twin and the changes of blood flow parameters and the natural pregnancy outcome of pump twin after pontaneous cessation of blood flow in the umbilical artery of the acardiac twin with reverse arterial perfusion sequence(TRAPs)detected by prenatal ultrasound.Methods:From January 2001 to August 2023,a total of 23 cases of TRAPs diagnosed in Hubei Maternal and Child Health Hospital were retrospectively collected and detected by ultrasound follow-up. According to the pregnancy outcome of the pump twin,23 patients were divided into the poor pregnancy outcome group(6 cases)and the good pregnancy outcome group(17 cases). The changes of ultrasound growth indexes and blood flow parameters of the pump twin and acardia twin were compared between the two groups. The hemodynamic parameters with statistically significant differences between the two groups were selected,and the diagnostic efficacy for TRAPs pregnancy outcomes was analyzed using ROC analysis.Results:There were no statistically significant differences in the structural development of acardiac fetus between the two groups,including skin edema,hemicardia,skull structure,trunk structure,upper and lower limbs,cardiothoracic ratio,amniotic fluid,sex and amniotic nature of twins(all P > 0.05). The lower limbs of acardia twin was significantly higher in the poor pregnancy outcome group(6/6)than in the good pregnancy outcome group(7/17),and the difference was statistically significant( P < 0.05). The relevant parameters of ultrasound blood flow were shown in both groups,there were no significant differences in middle cerebral artery pulsation index(PI)and umbilical artery PI between the two groups before natural occlusion of fetal blood flow with TRAPs(all P > 0.05). In the good pregnancy outcome group,the ratio of umbilical cord diameter between pump twin and acardia twin was higher than that in the poor pregnancy outcome group,the difference was statistically significant( P < 0.05). The scatter plot of the ratio of umbilical diameter between the two groups was drawn,and the cut-off value was 2.125 through ROC curve analysis,the area under the curve(AUC)was 0.961,the sensitivity was 94.10%,the specificity was 100%,and the Yoden index was 0.94. Conclusions:The ratio of umbilical cord diameter between pump twin and acardia twin in TRAPs is significant for predicting the pregnancy outcome of pump twin.
8.Preliminary study on the quantitative assessment model of mitral regurgitation in echocardiography based on fully convolutional networks: automatic identification and measurement of regurgitant radius

Objective:To develop an artificial intelligence system using fully convolutional neural networks(FCN)to assist echocardiographers in the quantitative assessment of mitral regurgitation(MR)severity.Methods:From August 2021 to June 2024,echocardiographic images of 441 patients with MR were prospectively collected from Renmin Hospital of Wuhan University and the Central Hospital of Wuhan. After screening,a total of 269 patients(4 917 frames)were included in the study. Of these,3 644 frames(128 patients)of apical four-chamber color Doppler MR flow convergence images from Renmin Hospital of Wuhan University were selected as the training/validation set,while images from 121 patients(813 frames)were used as the internal test set. Additionally,images from 20 patients(460 frames)from the Central Hospital of Wuhan were selected as the external test set. The FCN algorithm was employed to capture features and segment the MR color region on the left atrial side,simultaneously outputting the regurgitant radius(r)for the calculation of the effective regurgitant orifice area and regurgitant volume. The severity of MR was then classified according to the 2017 guidelines of the American Society of Echocardiography. The segmentation and classification performance of the model was evaluated,and the measurement results of the AI system was compared with that of both senior and junior physicians.Results:In the internal test set,the accuracy of r identification for cases classified as Grade Ⅰ to Ⅳ was 0.48,0.81,0.86,and 0.87,respectively. In the external test set,the accuracy of r identification for cases classified as Grade Ⅰ to Ⅳ was 0.60,0.77,0.64,and 0.77,respectively. The average accuracy of MR classification in the internal and external test sets was 0.91 and 0.88,respectively.Conclusions:The FCN model is capable of segmenting the left atrial side regurgitant areas in apical four-chamber heart color Doppler images,aiding physicians in obtaining quantitative assessment parameters for MR,and assisting junior physicians in accurately assessing the severity of MR.
9.Analysis of the medication patterns of famous traditional Chinese medicine practitioners for the treatment of idiopathic membranous nephropathy based on data mining

Objective To explore the medication patterns of famous traditional Chinese medicine practitioners in treating challenging aspects of idiopathic membranous nephropathy(IMN)based on data mining.Methods Medical cases meeting the inclusion and exclusion criteria were collected.Statistical analysis of syndrome patterns,herb frequency,association rules,and cluster analysis were conducted for key challenges,including disease duration ≥ 1 year,renal function impairment,hypoalbuminemia,and hypertriglyceridemia.Results A total of 128 medical cases were included.Spleen-kidney deficiency syndrome was identified as the most common pattern.Based on herb frequency and association rule analysis,Huangqi,Baizhu,Fuling,Danggui,Danshen were the most frequently used in prescription.Cluster analysis revealed core herbal groupings primarily based on classical formulas such as Zhenwu decoction,Shenling Baizhu powder,and Shuilu Erxian dan.Conclusion In the medical cases addressing treatment challenges,the syndrome patterns were predominantly characterized by spleen-kidney deficiency.Core medications included Huangqi,Baizhu,Fuling,Danggui and Danshen.The treatment emphasizes strengthening invigorating spleen and kidney,with variations in medication patterns based on disease duration and complications.
10.Impact of Polygonum cuspidatum and polydatin on lipid deposition in adipose tissue of obese mice

AIM To investigate the effects of Polygonum cuspidatum and polydatin on lipid deposition in adipose tissue of high-fat diet-induced obese mice.METHODS Forty male C57BL/6J mice were randomly assigned to either a control group(10 mice)fed standard chow or a diet-induced obesity(DIO)group(30 mice)fed a high-fat diet for 8 weeks.The successful mouse models were randomly assigned to the model group,the polydatin group(250 mg/kg)and the P.cuspidatum group(4.5 g/kg),with 8 mice in each group,to resume their high-fat diet during the following 8 weeks corresponding drug administration by gavage.Weekly body weight measurements were recorded for all mice.Serum TG,TC and LDL levels were quantified post-treatment.Histopathological assessment of adipose tissue was performed using HE staining.The mRNA expressions of AMPK,SREBP-1c and FAS in adipose tissue were analyzed by RT-qPCR.The protein expressions of p-AMPK,SREBP-1c and FAS in adipose tissue was detected by Western blot.RESULTS Compared to the control group,the model group displayed significantly higher body weight,inguinal fat weight and epididymal fat weight(P<0.05);elevated serum TG,TC and LDL levels(P<0.05);markedly enlarged volumes of inguinal and epididymal adipocytes(P<0.01);reduced p-AMPK protein expression in inguinal adipose tissue(P<0.01);and upregulated mRNA and protein expressions of SREBP-1c and FAS(P<0.05,P<0.01).Compared to the model group,both the P.cuspidatum group and polygonin group exhibited significantly reduced body weight and inguinal fat weight(P<0.05);decreased serum TG and TC levels(P<0.05);reduced inguinal adipocyte size(P<0.01);elevated p-AMPK protein expression in inguinal adipose tissue(P<0.01);and downregulated mRNA and protein expressions of SREBP-1c and FAS(P<0.05,P<0.01).CONCLUSION P.cuspidatum and polydatin significantly increases p-AMPK expression while decreasing SREBP-1c and FAS levels in adipose tissue.This regulatory effect likely contributes to reduction of body weight in obese mice through suppression of lipogenesis.
