OBJECTIVE To evaluate the anticoagulant efficacy and safety of nafamostat mesylate (NM) in the treatment of uremic patients at high risk of bleeding undergoing continuous veno-venous hemofiltration (CVVH) with different methods (pre- dilution and post-dilution). METHODS A total of 130 uremic patients at high risk of bleeding who underwent CVVH treatment in the nephrology department of Chongqing University Three Gorges Hospital from July 2023 to September 2024 were selected. They were divided into pre-dilution group and post-dilution group according to the random number table method, with 65 cases in each group. Both groups of patients received CVVH treatment under NM anticoagulation. The pre-dilution group adopted the pre-dilution replacement method, while the post-dilution group adopted the post-dilution replacement method. The coagulation, pressure, and usage duration of the filter and dialysis circuit venous reservoirs were compared between the two groups. The changes in prothrombin time (PT), prothrombin time-international normalized ratio (PT-INR), activated partial thromboplastin time (APTT), and fibrinogen (FIB) in the peripheral venous blood before the heparin pump and after the filter at 1, 4 and 7 h of CVVH treatment, as well as 20 min after the end of treatment, were compared between the two groups. The single-compartment urea clearance rate (spKt/V), β2-microglobulin (β2-MG) clearance rate and the incidence of adverse reactions were duni2007@foxmail.com compared between the two groups. RESULTS Both the pre-dilution and post-dilution groups had 60 patients who completed the study. The incidence of grade Ⅱ-Ⅲ coagulation of the filter and venous reservoirs, as well as the number of patients with transmembrane and venous pressure alarm intervention in the post- dilution group were significantly higher or more than those in the pre-dilution group (P<0.05), while usage time of the filter and the pipeline in the post-dilution group was significantly shorter than that in the pre-dilution group (P<0.05). The APTT values before the heparin pump as well as PT and APTT values after the filter at 1 h, 4 h, and 7 h of CVVH treatment in the post-dilution group were significantly higher than those in the pre-dilution group (P<0.001). There were no significant differences in PT, PT- INR, APTT and FIB between the two groups of patients 20 min after the end of treatment (P>0.05). The spKt/v and β2-MG clearance rates in the post-dilution group were significantly higher than those in the pre-dilution group (P<0.001). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). CONCLUSIONS When NM is used as an anticoagulant in the CVVH treatment of uremic patients at high risk of bleeding, compared with the pre-dilution treatment method, the post-dilution treatment method has a higher incidence of filter and dialysis tubing venous reservoir, a shorter usage time of the filter and pipeline, and a greater impact on extracorporeal coagulation, but has a higher solute clearance rate. Clinically, different dilution methods can be selected according to the different treatment needs of patients.

Digital and intelligent technologies serve as the core engine driving the inheritance of the essence and the innovation while upholding the fundamentals of traditional Chinese medicine(TCM). Currently, the digital and intelligent transformation of TCM has undergone four developmental stages, exhibiting inherent characteristics such as long-term inevitability, objective standardization, and ecological evolution. By introducing quantitative metrics, digital and intelligent technologies have achieved breakthroughs in TCM knowledge inheritance and innovation, clinical diagnosis and treatment, and herbal medicine supply. The practical applicability of methodological innovations has been empirically validated, though significant disparities exist in technological adaptability and application depth across different fields. Overall, the digital and intelligent transformation of TCM remains in its nascent stage, grappling with multiple structural challenges:weak data foundations, inadequate technological adaptability, incomplete institutional frameworks, shortages of multidisciplinary talent, lagging policies and regulations, and urban-rural digital divide. In order to foster sustainable development and modernization of TCM, this paper establishes a six-dimensional collaborative governance framework of encompassing data, technology, organization, institutions, environment and ethics, which is rooted in data governance and digital governance theories. Future efforts should center on standardization, integration, and ecosystem development to build a data and technology foundation. Focus should be placed on deepening innovation and application of key TCM-specific technologies, while simultaneously strengthening interdisciplinary talent cultivation, improving institutional mechanisms and policy frameworks, and increasing support for rural areas. By adopting a people-centered and technology-empowered approach, we can overcome developmental constraints and unleash the powerful driving force of digital and intelligent technologies for the inheritance of TCM.

OBJECTIVE To observe the clinical efficacy of mycophenolate mofetil (MMF) combined with Budesonide enteric capsules in the treatment of high-risk progressive immunoglobulin A nephropathy (IgAN). METHODS A total of 150 adult patients with high-risk progressive IgAN who attended the Department of Nephrology, Chongqing University Three Gorges Hospital, between August 1, 2024 and March 1, 2025 were enrolled in this study. The control group ( n =94) received MMF combined with glucocorticoid, while the observation group ( n =56) received MMF combined with Budesonide enteric capsules. The 24-hour urine protein (24 h UP), estimated glomerular filtration rate (eGFR), and albumin (ALB) levels of patients in both groups were compared at 1, 2, 3, and 6 months post-treatment. The complete response (CR) rate and overall response rate were calculated for both groups at 1, 2, 3, and 6 months post-treatment. Adverse reactions occurring during treatment were compared between the two groups. RESULTS Compared with before treatment, 24 h UP decreased significantly in both groups at different time points after treatment ( P <0.05), and ALB increased significantly ( P <0.05). However, there was no significant change in eGFR ( P >0.05). The 24 h UP in the observation group at 1, 2, and 3 months after treatment was significantly lower than that of the control group ( P <0.05), while the ALB level was significan tly higher ( P <0.05). However, at 6 months after treatment, there was no statistically significant difference in these two indicators between the two groups ( P >0.05). There was no statistically significant difference in eGFR between the two groups at different time points after treatment ( P >0.05). The overall response rates in the observation group at 1 and 2 months after treatment were significantly higher than those in the control group ( P <0.05). There was no statistically significant difference in the overall response rate at the remaining treatment time points and the CR rate at all time points between the two groups ( P >0.05). Patients in the observation group had significantly lower rates of skin abnormalities, elevated blood glucose, and overall adverse reactions compared with the control group ( P <0.05). CONCLUSIONS Compared with MMF combined with glucocorticoids, MMF combined with Budesonide enteric capsules for the treatment of high-risk progressive IgAN patients can reduce proteinuria and improve serum ALB levels more quickly, significantly increase the early overall response rate, and significantly reduce glucocorticoid-related skin adverse reactions, blood glucose elevation, and the overall incidence of adverse reactions, demonstrating a better short-term benefits.

ObjectiveTo evaluate the predictive value of epicardial adipose tissue (EAT) parameters for microvascular obstruction (MVO) formation in patients with ST segment elevation myocardial infarction (STEMI) using cardiac magnetic resonance quantification. MethodsA total of 139 STEMI patients were included in this study, and various parameters such as EAT thickness, volume, and mass index were measured utilizing cardiac magnetic resonance. All included patients were divided into MVO group and non-MVO group according to whether MVO occurred. Differences in EAT related parameters between two groups were compared and correlation analysis was applied to evaluate the correlation between quantitative indicators of EAT and indicators such as infarct size and ejection fraction. Logistic regression analysis was used to identify the relevant risk factors for MVO formation. Receiver Operating Characteristic (ROC) curve analysis was performed to assess the predictive value of the epicardial adipose tissue (EAT) quality index and other indicators for the occurrence of MVO. ResultsCompared with non MVO group, patients in MVO group presented with higher peak troponin T levels, increase of neutrophil lymphocyte ratio (NLR) and C-reactive protein (CRP), larger infarct size and compromised left ventricular ejection fraction (LVEF) (P<0.05). Total EAT volume, EAT mass index, left atrioventricular EAT volume, left atrioventricular EAT mass index and thickness of EAT in the left atrioventricular groove were significantly higher in patients with MVO. Multivariate Logistic regression analysis demonstrated that NLR, peak troponin T levels and left atrioventricular EAT mass index were independent predictors of MVO. The ROC curve suggested that the left atrioventricular EAT mass index had the highest predictive power for MVO formation in STEMI patients. ConclusionThe parameters of EAT quantified by cardiac magnetic resonance serve as imaging biomarkers for predicting MVO formation in STEMI patients. These metrics enable risk stratification post-myocardial infarction and facilitate early identification of high-risk individuals, thereby supporting personalized therapeutic decision-making.

OBJECTIVE To investigate the expression characteristics and regulatory mechanisms of the circadian clock gene period circadian regulator 1(PER1)and the tumor suppressor gene serine peptidase inhibitor Kazal type 5(SPINK5)in head and neck squamous cell carcinoma(HNSCC),and to elucidate the molecular mechanism by which PER1 regulates SPINK5 transcription via the NF-κB signaling pathway.METHODS Differentially expressed genes in HNSCC were screened using The Cancer Genome Atlas(TCGA)and GSE205155 datasets. The association between SPINK5 expression and patient prognosis was assessed via the GEPIA database.mRNA and protein expression levels of SPINK5 and PER1 in 60 clinical samples were detected by RT-qPCR,immunohistochemistry,and Western blot.PER1 knockdown(using siRNA)and overexpression(via plasmid transfection)were performed in the AMC-HN-8 cell line.Wound healing and colony formation assays were applied to evaluate the effects of PER1,SPINK5,and their interaction on HNSCC cell migration and proliferation.Western blot was utilized to examine the regulatory effect of NF-κB on SPINK5.RESULTS SPINK5 and PER1 were significantly downregulated in HNSCC tissues(all P<0.01),and their low expression was correlated with poor patient prognosis(for SPINK5,HR=0.69,P=0.006 7).A significant positive correlation was observed between PER1 and SPINK5 expression(R2=0.719 2,P=0.001 0).Knockdown and overexpression of PER1 respectively resulted in synchronous alterations in SPINK5 mRNA levels(all P<0.05).PER1 knockdown enhanced cell migration and proliferation(P<0.05),whereas SPINK5 overexpression suppressed these capabilities(P<0.01).Importantly,SPINK5 overexpression reversed the phenotypic changes induced by PER1 knockdown.Mechanistically,PER1 overexpression led to concomitant changes in NF-κB expression, activating the NF-κB pathway and thereby promoting SPINK5 transcription.CONCLUSION PER1 positively regulates SPINK5 transcription via the NF-κB pathway, inhibiting HNSCC cell proliferation and migration. These findings suggest that PER1 and SPINK5 may serve as potential therapeutic targets for HNSCC.

The MXene/MF microspheres were prepared by coating MXene nanosheets on melamine formaldehyde(MF)resin microspheres.Co(NO3)2 was adsorbed on the surface of the microspheres by impregnation, and then calcined at high temperature in an argon atmosphere.MF pyrolysis generated reducing gases such as CO and NH3,reducing Co2+to elemental Co,which was then used as a catalyst for in situ growth of carbon nanotube(CNT)through chemical vapor deposition(CVD),forming MXene-Co-CNT microspheres(MXene-Co-CNT MS).During this process, the pyrolysis of MF microspheres had dual effects. On one hand, the template was sacrificed to produce an internal hollow structure, and on the other hand, the generated gas worked as carbon source to generate CNT,forming an external sea urchin-like structure. Both of them promoted the formation of a novel structure, which combined the advantages of large specific surface area and good conductivity, thus possessing excellent electrocatalytic activity. The MXene-Co-CNT MS was modified on glassy carbon electrode(GCE)and further used in highly sensitive detection of nitroaromatic compounds(NACs).The detection limits of MXene-Co-CNT MS/GCE for 2,4,6-trinitrotoluene(TNT),1,3,5-nitrobenzamide(TNB),2,4-dinitrotoluene(DNT),1,3-dinitrobenzene(DNB),1-Cl-2,4-dinitrotoluene(Cl-DNB),and 4-nitrophenol(4-NP)were 26.84,31.60,35.03,54.14,43.86 and 28.67 nmol/L,respectively.It also had excellent anti-interference ability, and was used to detect NACs in environmental water samples accurately.

In this work, near infrared carbon quantum dots(NIR-CDs)were synthesized by hydrothermal method using biomass material Clausena lansium leaves. The synthesized NIR-CDs emitted maximum fluorescence signal at 677 nm,which was independent of excitation wavelength. The characterization results showed that there were abundant groups on the surface of NIR-CDs.Pd2+could form non-fluorescent compounds with the surface groups of NIR-CDs,resulting in fluorescence quenching(Fluorescence signal was denoted as F0).Because chlorpromazine hydrochloride(CPZ)parent nucleus contained unoxidized S atom,CPZ could form stable colored complex with Pd2+under acidic conditions. In the presence of CPZ,Pd2+dissociated from the surface of NIR-CDs and bonded with CPZ,so that the fluorescence signal could be restored(Fluorescence signal was denoted as F).An"on-off-on"fluorescence sensor was thus constructed. The fluorescence signal recovery value of NIR-CDs(△F=F-F0)showed a good linear relationship with the concentration of CPZ in the range of 5.68-28.43 μg/mL,and the detection limit(3σ)was 0.078 μg/mL.The sensor was applied to determination of CPZ in pharmaceutical preparations, and the recoveries were 94%-106%.The developed fluorescence sensor was expected to be used in quality control of actual pharmaceutical preparations.

Objective To establish a rapid analysis method for cyanide based on a ratiometric fluores-cent probe, providing a quantitative strategy for on-site visual and rapid detection of cyanide. Methods A dual-emission ratiometric fluorescent probe(AuNCs-FL)was constructed by using bovine serum al-bumin(BSA)-stabilized gold nanoclusters(AuNCs,fluorescence emission at 660 nm)as the responsive signal unit and fluorescein(FL,emission at 515 nm)as the internal reference. Results The etching effect of cyanide on AuNCs resulted in fluorescence quenching at 660 nm,while the fluorescence inten-sity of FL at 515 nm remained unchanged, enabling a rapid response analysis of cyanide shift from red to green fluorescence. The developed probe enabled rapid analysis of cyanide within 3 min, with a limit of detection(LOD)of 3.4 mg/L and a visual detection range of 10-100 mg/L.Conclusion The AuNCs-FL fluorescent probe is structurally simple,low-cost,and easy to operate, delivering rapid and accurate results. It also avoids the interference from sulfides encountered in commercial cyanide test kits, making it suitable for the on-site rapid detection of suspected powder samples in cyanide poisoning cases.
