OBJECTIVE To investigate the efficacy and safety of omadacycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneumonia (MUMPP) in children. METHODS A retrospective study was conducted on children aged 1-18 years old with MUMPP who were hospitalized in the Department of Pediatrics, the First Affiliated Hospital of Xinjiang Medical University from January 2022 to June 2025. According to the selection of secondary antibiotics after 72 h of initial treatment with macrolides, they were divided into the omadacycline group and the doxycycline group. Based on conventional treatment, children in the omadacycline group were given intravenous infusion of 2.4 mg/kg (once daily) of omadacycline tosylate, while children in the doxycycline group were given oral doxycycline hydrochloride tablets at 2 mg/kg (twice daily). The efficacy and safety were compared between the two groups of pediatric patients. Univariate analysis and multivariate Logistic regression analysis were performed on clinical efficacy, and subgroup analysis along with multiple sensitivity analyses were conducted to verify the robustness of the conclusions. RESULTS A total of 284 children with MUMPP were included in this study, with 142 in the omadacycline group and 142 in the doxycycline group. In terms of efficacy, although the hospitalization time of children in the omadacycline group was longer than that in the doxycycline group ( P <0.05), the lung lesion absorption rate and clinical efficacy were significantly higher or better than those in the doxycycline group ( P <0.05). The results of multivariate Logistic regression analysis showed that medication (OR=5.300, 95%CI: 2.526-11.123), length of hospital stay (OR=1.348, 95%CI: 1.167-1.556), and medication duration (OR=1.422, 95%CI: 1.169-1.729) were influencing factors of clinical efficacy ( P <0.05). The subgroup analysis results showed that the clinical efficacy of omadacycline was significantly better than that of doxycycline in all subgroups ( P <0.05). The results of multiple sensitivity analysis showed that the regression coefficients B of the four models (gradually adjust variables) before and after inverse probability of treatment weighting were significantly greater than 1 ( P <0.05). In terms of safety, there was no statistically significant difference in the inci dence of adverse drug reactions between the two groups of patients ( χ 2 =0.447, P =0.504). CONCLUSIONS In the case of hospitalization and prolonged medication, the efficacy of omadacycline in treating childhood MUMPP is superior to that of doxycycline, and its safety is good.

In this study, high-performance liquid chromatography coupled with tantrum mass spectrometry (HPLC-MS/MS) technology was employed to determine milrinone nitrogen oxides in milrinone. An ACCHROM XCharge-C18 column (100 mm × 2.1 mm, 5 μm) was used with a mobile phase consisting of A phase (methanol) and B phase (5 mmol/L NH4FA, pH3 adjusted by formic acid). Agilent 6410B triple quadrupole mass spectrometer was used for HPLC-MS/MS analysis. Detection was performed using positive electrospray ionization (ESI+) in multiple reaction monitoring (MRM) mode to analyze the limit of milrinone nitrogen oxides in milrinone, and the quantitative transition for the ion pair was from m/z 228.01 to m/z 181.90. Experimental results showed that the method exhibited good specificity, and that neither blank solvent nor blank samples interfered with the determination of milrinone nitrogen oxides of milrinone. The method demonstrated high sensitivity, with a limit of quantitation (LOQ) of 0.0076 μg/mL and a limit of detection (LOD) of 0.0038 μg/mL. The linear range spanned from 20% to 200% of the LOQ concentration, and a good linear relationship between concentration and peak area was observed within this range. Additionally, the recovery rates were consistently within the range of 80% to 120%, and the RSD for repeatability tests was 12.0%. These results indicated that the precision and accuracy of this method meet the required standards. In summary, the method developed in this study can effectively and accurately determine the content of milrinone nitrogen oxides in milrinone.

ObjectiveTo explore the effects of Bushen Tiaojing Formula (补肾调经方) on the quality of early embryos in repeated controlled ovarian stimulation (COS) and its possible mechanism. MethodsA total of 150 ICR female mice were divided into a normal group, a model group, an inhibitor group, a low-dose and a high-dose Bushen Tiaojing Formula group. Mice in the normal group, inhibitor group and model group were gavaged with distilled water 0.25 ml a day, and mice in the low- and high-dose groups were given 25.6 and 51.2 g/(kg·d) of Bushen Tiaojing Formula, respectively. All groups were gavaged once a day for 13 days as a cycle. Mice in the normal group were injected intraperitoneally with 0.1 ml normal saline on the 11th day of gavage, while mice in the other four groups were used to establish COS models. The inhibitor group was injected with the endoplasmic reticulum stress inhibitor 4-phenylbutyric acid 0.2 ml one hour before modelling. All groups were injected by gavage and intraperitoneal injection for 3 consecutive cycles, with an interval of 4 days between cycles. Immediately after the third intraperitoneal injection, mice were grouped with male mice of the same strain in a ratio of 2∶1, and at 8AM of the second day, the mice were examined, and those with spermatozoa or spermatozoa in the vaginal smears were recorded as pregnant. The mice were executed in the afternoon of the 4th day of pregnancy, and the blastocysts were obtained under an inverted microscope. The morphology of the blastocysts was observed, and the total number of blastocysts and the number of high-quality blastocysts were recorded to calculate the rate of high-quality blastocysts; the apoptosis of blastocyst cells was detected by the Tunel method, and the apoptosis rate was calculated; the endoplasmic reticulum molecular chaperone protein glucose-regulated protein 78 (GRP78), C/EBP homologous protein (CHOP), B-lymphoblastoma 2 (Bcl-2), Bcl-2-associated X protein (Bax), cystathionin 3 (Caspase-3) and cystathionin 12 (Caspase-12) protein levels and their mRNA expression were detected by immunofluorescent assay and RT-qPCR assay, respectively. ResultsIn the normal group, blastocysts had regular morphology, good morphological development and low fragmentation rate; in the model group, blastocysts were poorly developed, cell morphology was irregular, fragmentation rate was high, and there was some stagnation; in the inhibitor group and high-dose, low-dose Bushen Tiaojing Formula groups, cell morphology could be seen to be better developed, with regular morphology and less fragmentation rate. Compared with the normal group, the rate of high-quality blastocysts in the model group reduced, the rate of apoptosis of blastocyst cells increased, the levels of GRP78, CHOP, Caspase-12, Caspase-3, Bax protein and its mRNA expression in blastocysts increased, while the level of Bcl-2 protein and its mRNA reduced (P<0.01). Compared with the model group, the rate of high-quality blastocysts increased, the rate of apoptosis of blastocysts decreased, the levels of GRP78, CHOP, Caspase-12, Caspase-3, Bax protein and their mRNA expression decreased, and the levels of Bcl-2 protein and its mRNA expression increased in the blastocysts in the inhibitor group and the high-dose and low-dose Bushen Tiaojing Formula groups (P<0.01). Compared with the inhibitor group, GRP78, CHOP, Caspase-12, Bax protein levels and their mRNA expressions increased in the high-dose Bushen Tiaojing Formula group, and Caspase-12, Caspase-3, Bax protein levels and their mRNA expressions increased in the low-dose formula group (P<0.05 or P<0.01). ConclusionBushen Tiaojing Formula can inhibit the apoptosis of blastocyst cells caused by repeated controlled ovarian stimulation by improving endoplasmic reticulum stress, which plays a role in improving the quality of early embryos.

In cases where a tracheal injury exceeds half the length of the adult trachea or one-third of the length of the child trachea, it becomes difficult to perform end-to-end anastomosis after tracheal resection due to excessive tension at the anastomosis site. In such cases, tracheal replacement therapy is required. Advances in tissue engineering technology have led to the development of tissue engineering tracheal substitutes, which have promising applications. Hydrogels, which are highly hydrated and possess a good three-dimensional network structure, biocompatibility, low immunogenicity, biodegradability, and modifiability, have had wide applications in the field of tissue engineering. This article provides a review of the characteristics, advantages, disadvantages, and effects of various hydrogels commonly used in tissue engineering trachea in recent years. Additionally, the article discusses and offers prospects for the future application of hydrogels in the field of tissue engineering trachea.

Objective:
To analyze the differences in bile acid profiles during different pregnancy durations of patients with intrahepatic cholestasis of pregnancy (ICP), so as to provide a reference for early prevention and treatment of ICP and optimization of maternal-infant health outcomes.
Methods:
Pregnant women who underwent routine prenatal examinations and delivered at Hangzhou Obstetrics and Gynecology Hospital from 2021 to 2023 were selected as study subjects. According to the ICP guidelines (2020), pregnant women were categorized into normal group, mild ICP group, and moderate/severe ICP group. Age, parity, and gravidity were collected through the obstetric electronic medical record system, liver function indicators and seven bile acid levels were collected through the hospital's laboratory information system. Differences in bile acid profiles across pregnancy durations among the three groups were compared.
Results:
A total of 238 pregnant women were enrolled, including 57 cases (23.95%) in the normal group, 136 cases (57.14%) in the mild ICP group, and 45 cases (18.91%) in the moderate/severe ICP group. There were statistically significant differences between the three groups in total bile acid (TBA), cholic acid (CA), chenodeoxycholic acid (CDCA), glycochenodeoxycholic acid (GCDCA), glycocholic acid (GCA), taurocholic acid (TCA) levels (all P<0.05). Compared with the normal group, CA, GCDCA and GCA, and TCA were higher in the mild and moderate/severe ICP groups; compared with the mild ICP group, GCA was higher in the moderate/severe ICP group (all P<0.05). Significant differences were observed in the levels of GCDCA, GCA, and TCA among three groups pregnant women in the early, mid, and late pregnancy (all P<0.05). Compared with the normal group, mild ICP group had higher GCDCA and GCA in the early and mid pregnancy; moderate/severe ICP group had higher TCA in the early pregnancy and higher GCDCA and GCA in the late pregnancy. Compared with the mild ICP group, mild ICP group had higher TCA in the early pregnancy and the moderate/severe ICP group had higher GCA in the late pregnancy.
Conclusions
GCDCA, GCA, and TCA levels remain higher in ICP patients than in normal pregnant women across all pregnancy durations. Personalized perinatal management plans can be developed based on bile acid profile dynamics to optimize maternal-fetal outcomes.
