Objective To explore conventional MRI and synthetic MRI(syMRI)quantitative parameter histogram features contributing to predicting tumor infiltrating lymphocyte(TIL)level of breast cancer. Methods A total of 114 female patients with solitary breast cancer were retrospectively included and divided into the low-TIL group(L group,0-10%,n=70)and high-TIL group(H group,>10%,n=44).Conventional MRI features of breast cancer were analyzed based on dynamic contrast-enhanced MRI(DCE-MRI),and histogram features of syMRI quantitative parameters(T1,T2 and proton density[PD])images were extracted, including the 10th and 90th percentile(abbreviated to 10th and 90th,respectively),mean,entropy,kurtosis and skewness, which were then compared between groups.Multivariate logistic regression was used to screen the independent predictors for TIL level among histogram features of syMRI quantitative parameters. Results The proportion of Luminal A breast cancer was lower, while the proportion of HER2-positive breast cancer was higher in H group than those in L group(both adjusted P<0.05).H group had a higher proportion of lesions with regular margin than L group(P<0.05).H group exhibited higher value of T1-10th,T1-90th,T1-mean,T2-10th,T2-mean and lower value of PD-skewness compared to those in L group(all P<0.05).T2-mean was identified as an independent predictor of TIL level of breast cancer(P<0.05),and its area under the curve(AUC)of receiver operating characteristic(ROC)was 0.780.Conclusion Tumor margin characteristics on conventional MRI and histogram feature T2-mean based on syMRI contributed to predicting TIL level of breast cancer.

Objective To evaluate the efficacy of modified cardiac rehabilitation combined with Xuefu Zhuyu decoction on patients with heart failure of Qi deficiency and blood stasis type, with a focus on analyzing the improvement of galectin-3(Gal-3),soluble suppression of tumorigenicity 2(sST2)levels,cardiac function, traditional Chinese medicine syndromes, and exercise tolerance. Methods 180 patients with heart failure of Qi deficiency and blood stasis type who attended the Department of Rehabilitation Medicine,Huzhou First People's Hospital from October 2021 to March 2023 were selected and randomly divided into treatment group(modified cardiac rehabilitation+Xuefu Zhuyu decoction)and control group(conventional treatment),with a treatment course of 12 weeks. The changes of Gal-3,sST2,left ventricular ejection fraction(LVEF),New York heart association functional classification(NYHA),N-terminal B-type natriuretic peptide(NT-proBNP),traditional Chinese medicine syndrome score and 6-minute walk test(6MWT)were compared between two groups before and after 12 weeks of treatment, and the safety was evaluated. Results After 12 weeks of treatment,Gal-3 and sST2 levels of patients in treatment group were significantly lower than those in control group;LVEF levels of patients in treatment group were significantly higher than those in control group, and the degree of improvement of cardiac function grading in treatment group was significantly better than that in control group;NT-proBNP in treatment group was significantly lower than that in control group; The 6MWT in treatment group was significantly higher than that in control group; The score of traditional Chinese medicine syndrome in treatment group was lower than that in control group; The difference is statistically significant(P<0.05).After 12 weeks of treatment, the effective rate of treatment group was significantly higher than that of control group(P<0.05);There was no statistically significant difference in adverse reactions and serious adverse events between two groups of patients(P>0.05).Conclusion Modified cardiac rehabilitation combined with Xuefu Zhuyu decoction can significantly improve cardiac function, traditional Chinese symptoms, and exercise tolerance in patients with heart failure of Qi deficiency and blood stasis type, reduce Gal-3 and sST2 levels, and has high clinical application value.

This study summarized the nursing experience of midline catheter insertion in 12 patients with a body mass index≥50,providing clinical references.Key nursing interventions included:pre-insertion patient positioning was adjusted to semi-Fowler's or sitting position based on respiratory status. The puncture sites were chosen at the middle or distal third of the upper arm with elbow flexion. The widened tourniquets and extended-length needles/catheter sheaths were used according to vascular depth and puncture angle. The modified pre-insertion length measurement and real-time ultrasound-guided vascular puncture were applied. The catheter with crisscross elastic bandage was secured post-insertion.All 12 patients successfully underwent catheterization, with devices removed after completing intravenous therapy and blood sampling.

AIM:To investigate the expression of AU-rich element RNA-binding factor 1(AUF1),an RNA-binding protein,in hepatocellular carcinoma(HCC),and to explore its potential role in HCC progression through regula-tion of metabolism-related genes.METHODS:A tissue microarray containing 99 HCC samples and 95 adjacent nontu-morous liver tissues was used to assess AUF1 expression. The associations between AUF1 expression and HCC clinical pa-rameters were analysed using the GEPIA and UALCAN databases. The AUF1 gene was knocked down in human hepato-blastoma HepG2 cells by siRNA,and transcriptomic and TMT quantitative proteomic analyses were performed to identify alterations in metabolism-related genes.RESULTS:The AUF1 expression was significantly elevated in HCC tissues and correlated with a poor prognosis.Knockdown of AUF1 in HepG2 cells resulted in reduced cell viability and increased apop-tosis.Integrative analyses of transcriptomic and proteomic data revealed that AUF1 knockdown in HepG2 cells led to up-regulation of carboxylesterase 3(CES3),fibrinogen gamma chain(FGG)and 4-hydroxy-2-oxoglutarate aldolase 1(HOGA1),and down-regulation of lamin B receptor(LBR),riboflavin kinase(RFK),sterol O-acyltransferase 1(SOAT1)and TWIST neighbor(TWISTNB).Clinical data from GEPIA and UALCAN databases suggested that the expression of these metabolism-related genes in HCC patients exhibited an opposite trend.CONCLUSION:Our findings suggest that AUF1 is highly expressed in HCC,and may contribute to tumor progression and poor prognosis by modulating the expres-sion of a series of metabolism-related genes.

AIM:To investigate the impact of es-ketamine-mediated opioid-free anesthesia(OFA)on postoperative gastrointestinal function in patients undergoing laparoscopic distal gastrectomy for gas-tric cancer.METHODS:A total of 150 pa-tients,scheduled for elective laparoscopic distal gas-trectomy for gastric cancer and meeting the inclu-sion and exclusion criteria, were randomly assigned to either the OFA group or the opioid-based anes-thesia(OBA)group using a random number ta-ble,with 75 patients in each group. The OFA group was administered an anesthesia regimen pri-marily consisting of esketamine, while the OBA group received conventional opioid anesthesia,pri-marily consisting of sufentanil and remifentanil. The primary outcome measure was postoperative flatus time, defined as the interval from the end of sur-gery to the first passage of gas.RESULTS:The OFA group exhibited a shorter postoperative flatus time compared to the OBA group(P<0.01).Intraopera-tive blood loss and norepinephrine consumption were significantly less in the OFA group compared to the OBA group(P<0.05);the postoperative HADS-D score was better in the OFA group than in the OBA group, and both the OFA and OBA groups showed significantly lower postoperative HADS-A and HADS-D scores compared to their preoperative levels(P<0.05);the incidence rate of abdominal distension was significantly lower in the OFA group compared to the OBA group(P<0.05).CONCLUSION:The use of esketamine-mediated opioid-free anesthesia can expedite gastrointestinal function recovery, reduce hospital stay duration, and decrease postoperative adverse reactions in patients undergoing laparo-scopic distal gastrectomy for gastric cancer.
