Verrucous hemangioma is a rare congenital vascular malformation. It presents as multiple erythematous papules, plaques and nodules with a unilateral and linear distribution over the lower extremity. With time, it progressively enlarges, becomes violaceous to bluish-black in color, develops a verrucous surface and may become symptomatic. It clinically behaves like a vascular malformation, but recent findings show immunoreactivity similar to infantile hemangioma; thus, its true nature remains to be elucidated. In the past, it has been reported under various names, thereby concealing its true incidence, especially in Filipinos. We are reporting a rare case of a 19 year-old Filipino man with multiple erythematous to violaceous papules, verrucous plaques, and nodules present since birth and arranged linearly across the left lower extremity. Excision biopsy revealed blood-filled vascular spaces extending to the subcutaneous tissue. CT angiogram revealed no involvement beyond the subcutaneous plane. Patient underwent excision of the lesions followed by split-thickness skin grafting
