1.Analysis of patch test in 186 patients with rosacea

Objective:To analyze patch test results in patients with rosacea, investigate the prevalence of contact sensitization, and identify common allergens, to provide relevant evidence for the diagnosis, treatment, and preventive interventions for rosacea.Methods:A cross-sectional retrospective survey was conducted to analyze the positive contact allergy rate and allergen distribution in 186 rosacea patients who underwent the 60-item China Standard Series Patch Test (CB-1000) at the Department of Dermatology, Peking University First Hospital, from January 2023 to September 2024.Results:Among the 186 patients, 159(85.5%) showed positive reactions in patch tests. The top three allergens with the highest positivity rates were: metal salts, preservatives and fragrances. The top 10 individual allergens with the highest positivity rates were: nickelsulfate hexahydrate 46.8%(87/186), cobaltchloride hexahydrate 29.0%(54/186), hydroperoxides of linalool 15.1%(28/186), carba mix 14.0%(26/186), propolis 9.7%(18/186), fragrance mix Ⅰ 8.6%(16/186), methyl dibromo glutaronitrile 8.1%(15/186), Peru balsam 7.5%(14/186), gold sodium thiosulfate dihydrate 7.5%(14/186), and hydroperoxides of limonene 7.5%(14/186). The overall positivity rate for cosmetics-related allergens was 56.5% (105/186), with the top five allergens being: hydroperoxide of linalool 15.1%(28/186), propolis 9.7%(18/186), fragrance mix Ⅰ 8.6%(16/186), methyl dibromo glutaronitrile 8.1%(15/186), and Peru balsam 7.5%(14/186).Conclusion:The overall positivity rate for patch test allergens in rosacea patients is high. The most common allergen types are metal salts, preservatives, and fragrance agents. Cosmetics-related allergens also show a high positivity rate. Rosacea patients should avoid exposure to these allergens.

In recent years, the wearable device market has been developing rapidly. Wearable devices with personalized health management and chronic disease monitoring are widely used in daily life by virtue of their powerful performance and convenience. However, with the popularization of these devices, skin adverse reactions caused by prolonged wearable wear are gradually increasing, among which allergic contact dermatitis (ACD) is the most common. Common allergens such as acrylates, methacrylates, rosin, chromium and nickel are widely present in adhesives and device components and are the main causes of ACD. Understanding the presence of potential sensitizers in wearable devices can help in diagnostic patch testing in users experiencing skin reactions caused by the device. Future innovations in wearable device materials will focus on the adoption of safer bonding technologies and biocompatible materials to reduce the risk of allergy. The introduction of new materials brings both development opportunities and challenges. Educating users on proper device usage, identifying specific allergens, selecting hypoallergenic materials, and optimizing device maintenance are important for reducing the risk of ACD.

5.The current state of sunscreen development and analysis of its scientific application and safety

Regular and adequate use of broad-spectrum sunscreen has been proven to offer significant protection against acute Ultraviolet-induced photodamage, photoaging, immunosuppression, and the development of skin tumors. However, concerns regarding the safety and standardized use of sunscreens persist, including potential allergenicity and irritability of certain organic sunscreens, the impact of systemic absorption on the endocrine system, the effect on vitamin D synthesis and absorption, and environmental implications. Special caution is advised when using small molecule organic sunscreens and nanoparticle inorganic sunscreens, especially for infants, pregnant women, and areas with damaged skin.
6.Diagnosis and treatment of rosacea in the real world: a survey on the current status

Objective:To analyze the current status of diagnosis and treatment of rosacea in China, and to strengthen the understanding and management of this disease.Methods:A retrospective cross-sectional study was conducted, and patients with rosacea were enrolled from 23 tertiary hospitals in 6 provinces or municipalities in northern and southern China. Clinical characteristics, previous diagnosis and treatment status of these patients were collected through questionnaires. Non-normally distributed continuous data were described by M ( Q1, Q3), and compared using Mann-Whitney U test, while categorical data were compared using chi-square test. Results:Among the 593 patients with rosacea, 164 were males and 429 were females, with a male-to-female ratio of 1∶2.6; 205 patients were from southern China, and 388 from northern China; most patients (349 cases, 58.8%) were aged 20 to 40 years, and the patients from northern China were significantly older than those from southern China (median age: 37 years vs. 30 years, P < 0.001). Multiple-site involvement (371 cases, 62.6%) and coexistence of multiple phenotypes (391 cases, 65.9%) were common, the cheeks (429 cases, 72.3%) and nose (393 cases, 66.3%) were mostly affected, and skin lesions mainly manifested as persistent erythema (354 cases, 59.7%), papulopustules (344 cases, 58.0%), and telangiectasia (282 cases, 47.6%). Involvement of the cheeks was more common in the patients from southern China (160 cases, 78.0%) than in those from northern China (269 cases, 69.3%), but the nose and eyes were less involved in the patients from southern China than in those from northern China (nose: 125 cases [61.0%] vs. 268 cases [69.1%]; eyes: 3 cases [1.5%] vs. 23 cases [5.9%]; both P < 0.05). The prevalence of transient erythema and papulopustules was significantly higher in the patients from southern China (38.0% and 65.4%, respectively) than in those from northern China (20.9% and 54.1% respectively, both P < 0.05), while the patients from northern China more frequently presented with persistent erythema compared with those from southern China (64.9% vs. 49.8%, P < 0.05). The disease duration ( M [ Q1, Q3]) was 12 (4, 30) months among the patients with rosacea, and the time from the onset to diagnosis was 10 (3, 24) months. The disease duration was significantly longer (12 [4, 36] months), and the proportion of patients with disease duration > 5 years was significantly higher (16.4% [63 cases]) in the patients from northern China than in those from southern China (12 [3, 24] months, 9.4% [19 cases], respectively; both P < 0.05). The patients with varied subtypes and severity of rosacea were previously mainly treated with topical antimicrobial agents (71.9%) ; 72.7% of the patients with mild rosacea were treated with systemic drugs; poor patient compliance was observed, and only 40.6% of the patients completed more than 4 consecutive weeks of treatment at a time. Conclusions:Rosacea usually occurred in young and middle-aged people in China, mostly involved the cheeks and nose, and mainly manifested as erythema or papulopustules. Delayed diagnosis, non-standard treatment and poor patient compliance existed in clinical practice.

8.Wendantang Treats Inflammation in Obesity (Syndrome of Phlegm-dampness) by Regulating PI3K/Akt/mTOR Pathway-mediated Adipocyte Autophagy

ObjectiveTo observe the effects of Wendantang on the expression of inflammatory cytokines, autophagy markers, and key molecules of phosphatidylinositol-3-kinase/protein kinase B/mammalian target of rapamycin (PI3K/Akt/mTOR) signaling pathway in the adipocytes of the rat model of obesity (syndrome of phlegm-dampness) and to explore the material basis of inflammation in obesity (syndrome of phlegm-dampness) and the underlying mechanism of Wendantang intervention. MethodA total of 126 SD rats were randomized into 2 groups: 16 rats in the blank group and 110 rats in the modeling group. The blank group was fed with a basic diet while the modeling group with a high-fat diet to establish the animal model of obesity (syndrome of phlegm-dampness) for 8 weeks. After successful modeling, 48 obese rats were selected according to their body mass and randomized into a model control group, an orlistat (ORLI, 32.40 mg·kg-1) group, a rapamycin (RAPA, 2 mg·kg-1) group, and low-, medium-, and high-dose (4.45, 8.90, 17.80 g·kg-1, respectively) Wendantang groups, with 8 rats in each group. In addition, 8 rats were randomly selected from the blank group to be set as the normal control group. The corresponding agents in each group were administrated by gavage and the model and control groups were administrated with equal amounts of distilled water once daily for 6 weeks. The body mass, Lee's index, body fat ratio, and obesity rate were measured or calculated. The expression of UNC51-like kinase-1 (ULK1), Beclin1, human autophagy-related protein 5 (Atg5), p62, and microtubule-associated protein 1 light chain 3 (LC3) Ⅰ/Ⅱ (markers of autophagy in adipocytes) was detected by the immunohistochemical two-step method. Enzyme-linked immunosorbent assay (ELISA) was employed to determine the expression of tumor necrosis factor (TNF)-α, interleukin-6 (IL-6), IL-1β, monocyte chemotactic protein-1 (MCP-1), IL-4, IL-10, IL-13, and transforming growth factor (TGF)-β in adipocytes. Western blot was employed to measure the protein levels of classⅠ-PI3K, phosphatidylinositol triphosphate (PIP3), Akt, mTORC1, ULK1, TSC1, and TSC2 in adipocytes. ResultCompared with the blank group, the modeling group showed increased body mass and Lee's index (P<0.01), the obesity rate >20%, and phlegm-dampness syndrome manifestations such as physical obesity, decreased mobility, decreased appetite, lusterless and tight fur, loose stools, decreased responsiveness to the outside world, and decreased water intake. Compared with the normal control group, the model control group showed increased body mass, Lee's index, body fat ratio, adipocyte autophagy marker expression, pro- and anti-inflammatory cytokine levels (P<0.05, P<0.01), down-regulated protein levels of classⅠ-PI3K, PIP3, Akt, mTORC1, TSC1, and TSC2 (P<0.01), and up-regulated protein level of ULK1 (P<0.01). The intervention groups showed lower body mass, body fat ratio, adipocyte autophagy marker protein expression, and protein levels of TNF-α, IL-6, IL-1β, MCP-1, IL-4, and IL-13 than the model control group (P<0.05, P<0.01). Moreover, the RAPA and Wendantang (medium and high dose) groups showed lowered levels of IL-10 and TGF-β (P<0.01), and the ORLI group showed down-regulated expression of TGF-β (P<0.01). The expression of key molecules of the signaling pathway was up-regulated (P<0.05, P<0.01) while that of ULK1 was down-regulated (P<0.01) in all the intervention groups. Compared with the RAPA group, the Wendantang groups showed up-regulated expression of all autophagy marker proteins in adipocytes (P<0.01). In addition, the low-dose Wendantang group showed elevated levels of inflammatory cytokines (except TNF-α) (P<0.05, P<0.01) and down-regulated expression of all key molecules of the signaling pathway (P<0.05, P<0.01). The levels of inflammatory cytokines (except IL-16, MCP-1, and IL-10) were elevated in the medium-dose Wendantang group (P<0.05, P<0.01). The expression of key molecules except PI3K of the signaling pathway was down-regulated in the medium- and high-dose Wendantang groups (P<0.05, P<0.01). Compared with the ORLI group, low- and medium-dose Wendantang groups showed up-regulated expression of autophagy markers in adipocytes (P<0.01), and the low-dose group showed elevated levels of inflammatory cytokines (IL-6, IL-4, and TGF-β) (P<0.01) and down-regulated expression of all key molecules of the signaling pathway (P<0.01). The medium-dose Wendantang group showed up-regulated expression of IL-4 (P<0.01) and down-regulated expression of key molecules except PI3K of the signaling pathway (P<0.05, P<0.01). The high-dose Wendantang group showed increased body mass, up-regulated expression levels of autophagy markers (ULK1, LC3 Ⅰ/Ⅱ) (P<0.05, P<0.01), down-regulated expression of PIP3, mTORC1, and TSC1 (P<0.05, P<0.01), and lowered levels of Beclin1, Atg5, TNF-α, and IL-13 (P<0.05, P<0.01). ConclusionThe inflammation in obesity (syndrome of phlegm-dampness) is closely associated with the PI3K/Akt/mTOR pathway-mediated adipocyte autophagy. Wendantang can treat the chronic inflammation in obese rats with the syndrome of phlegm-dampness by regulating this signaling pathway and thus improve adipocyte autophagy.

Objective:To investigate the knowledge, attitudes, and practices (KAP) of pulse oximetry among pediatric healthcare providers in China and analyze the factor influencing the KAP.Methods:A self-developed questionnaire was used for an online research on the KAP of 11 849 pediatric healthcare providers from 31 provinces, autonomous regions, and municipalities of China from March 11 to 14, 2022.The factors influencing the KAP of pulse oximetry among pediatric healthcare providers were examined by Logistic regression. Results:The scores of KAP, of pulse oximetry were 5.57±0.96, 11.24±1.25 and 11.19±4.54, respectively.The corresponding scoring rates were 69.61%, 74.95%, and 55.99%, respectively. Logistic regression results showed that the gender and working years of pediatric healthcare providers, the region they were located, and whether their medical institution was equipped with pulse oximeters were the main factors affecting the knowledge score (all P<0.05). Main factors influencing the attitude score of pediatric healthcare providers included their knowledge score, gender, educational background, working years, region, medical institution level, and whether the medical institution was equipped with pulse oximeters (all P<0.05). For the practice score, the main influencing factors were the knowledge score, gender, age, and whether the medi-cal institution was equipped with pulse oximeters (all P<0.05). Conclusions:Chinese pediatric healthcare providers need to further improve their knowledge about and attitudes towards pulse oximetry.Pulse oximeters are evidently under-used.It is urgent to formulate policies or guidelines, strengthen education and training, improve knowledge and attitudes, equip more institutions with pulse oximeters, and popularize their application in medical institutions.
10.Reduction of Target Volume and the Corresponding Dose for the Tumor Regression Field after Induction Chemotherapy in Locoregionally Advanced Nasopharyngeal Carcinoma

PURPOSE: This study aims to investigate the feasibility of contouring target volume according to residual tumor and decreasing the dose to the tumor regression field after induction chemotherapy (IC) in locoregionally advanced nasopharyngeal carcinoma (NPC). MATERIALS AND METHODS: From August 2009 to August 2013, patients with stage III–IVB NPC were treated with IC and concurrent chemoradiotherapy. Gross tumor volume of nasopharynx (GTVnx)–residual and gross tumor volume of cervical lymph node (GTVnd)–residual were contoured according to post-IC residual primary tumor and any N+ disease, respectively. The tumor regression field was included in CTVnx1/CTVnd1 and prescribed a dose of 60 Gy. Outcomes and toxicities of all patients were evaluated. RESULTS: A total of 57 patients were enrolled. At a median follow-up of 68 months, three cases displayed locoregional recurrence and one case showed both distant metastasis and locoregional recurrence. All locoregional recurrences were in the GTVnx-residual/GTVnd-residual and in-field. The 5-year overall, locoregional relapse-free, distant metastasis-free, and progression-free survival rates were 82.2%, 87.7%, 85.8% and 80.3%, respectively. CONCLUSION: After IC, contouring of GTVnx-residual/GTVnd-residual as residual tumor volume and distribution 60 Gy ofradiation dose to the tumorregression field may be feasible and need further investigation.
