1.Effects of whole body vibration training combined with blood flow restriction on motor function and com-munity activity in elderly stroke patients with hemiplegia

Objective To investigate the impact of whole body vibration training combined with intermittent blood flow restriction on the motor function and community activity in elderly stroke patients with hemiplegia.Methods A total of 80 convalescent hemiplegic patients after stroke who were hospitalized in the Rehabilitation Medicine Department of Yichang Central People's Hospital(Xiling Campus)from October 2021 to December 2023 were included and randomly divided into control group(n=25),vibration group(n=28)and combination group(n=27)using the random number table method.All patients received conventional rehabilitation training,on the basis;the vabration group received whole body vibration training,while the combination group received whole body vibration training combined with intermittent blood flow restriction.Before and after 6 weeks of training,the balance function was evaluated with Berg balance scale(BBS),while the gait function was tested with 6-minute walking test(6MWT)and the Community Balance and Mobility scale(CB&M)was used to assess the community activity ability.The community balance and mobility(CB&M)were evaluated in the first and third month after discharge.Results There was no significant difference in BBS,CB&M scores and 6MWT walking distance between the three groups before training(P>0.05).After 6 weeks of training,the three groups showed significant increases in the BBS,CB&Mscores and the walk distance of 6MWT(P<0.001).Furthermore,compared with the control group,the combination group and the vibration group were significantly beter(P<0.05),and there was no signifi-cant difference between the combination group and the vibration group(P>0.05).At the first and third month of follw-up after discharge,the CB&M scores of the three groups were significantly higher than those of the control group(P<0.05),while there was no significant difference between the CB&M scores of the combination group and the vibration group at the first month of follow-up(P>0.05).But the CB&M scores of the combination group was higher than those of the vibration group at the third month of follow-up,and the difference was statistically signifi-cant(P<0.05).Conclusion On the basis of conventional rehabilitation,whole body vibration training combined with intermittent blood flow restriction can significantly enhance balance function,balance confidence,walking ability and mobility in early stage of community life for elderly stroke patients with hemiplegia,potentially facilitat-ing their return to community life.
2.Serum metabolomic characteristics of early papillary thyroid carcinoma and association with lymph node metastasis

OBJECTIVE To compare the metabolomic characteristics of stage T1 papillary thyroid carcinoma(PTC)and nodular goiter(NG),and the relationship between metabolites and lymph node metastasis of PTC.METHODS Serum samples were collected from 60 patients with stage T1 PTC and 30 patients with NG who underwent thyroidectomy at the Department of Otolaryngology Head and Neck Surgery,Civil Aviation General Hospital between September 2021 and April 2022.The PTC group was divided into the N+ group with lymph node metastasis and the N-group without lymph node metastasis according to the presence or absence of lymph node metastasis.The serum metabolites of the N+ and N-groups and the PTC and NG groups were compared and analyzed using an ultra-performance liquid chromatography-mass spectrometry(UPLC-Q-Exactive-MS)coupled platform,and principal component analysis(PCA),partial least squares discriminant analysis(PLS-DA),and orthogonal partial least squares discriminant analysis(OPLS-DA)was performed using SIMCA-P 14.1 software.OPLS-DA modeling,combined with FDR-corrected Mann-Whitney-Wilcoxon test results and metabolite difference multiples in the two groups undergoing comparison,etc.to screen for potential small molecule metabolic markers,and to establish a joint diagnostic model by binary logistic regression analysis.RESULTS There were no significant differential metabolites between the N+ group with lymph node metastasis and the N-group without lymph node metastasis.Seven differential metabolites were found between PCA patients and NG patients,and the five relevant metabolic pathways were the pentose phosphate pathway,pentose and glucuronide interconversion,glycolysis/gluconeogenesis,fructose,and mannose metabolism,and fatty acid biosynthesis.The differential metabolite with an area under the ROC curve>0.9 was D-glyceraldehyde 3-phosphate,and another N-undecanoylglycine,uronic acid,and the area under the ROC curve for three metabolites,N-undecanoylglycine,uric acid,and triiodothyronine glucuronide,was>0.8.CONCLUSION PTC patients differed from NG patients mainly in glucose metabolism and lipid metabolism,and D-glyceraldehyde 3-phosphate could be distinguished from NG patients with the aid of N-undecanoylglycine,uric acid,and triiodothyronine glucuronide,combined with imaging findings.Also,no significant differences in serum metabolites were found in the N+ group compared with the N-group,and the presence or absence of lymph node metastases did not affect serum metabolites in patients with stage T1 PTC.
3.Imaging markers of the recurrent bleeding risk in patients with cerebral amyloid angiopathy

Cerebral amyloid angiopathy (CAA) is a cerebral small vascular disease caused by pathological deposition of β-amyloid protein in cortical and pial arteries and capillaries. It is the main cause of non-traumatic cerebral lobular hemorrhage and has a high risk of recurrent hemorrhage. Studies have shown that specific imaging markers, such as cerebral microbleeds, cortical superficial siderosis, convexal subarachnoid hemorrhage, centrum semiovale-perivascular spaces and the overall burden of cerebral small vascular disease, may be more effective in predicting the risk of bleeding recurrence in patients with CAA. This article reviews the imaging markers used to predict the risk of recurrent bleeding in patients with CAA, in order to provide a new direction for the establishment of a risk assessment system for recurrent bleeding.
4.Research on lightweight model of intelligent-assisted diagnosis of common fundus diseases based on fundus color photography

Objective:To observe the diagnostic value of six classification intelligent auxiliary diagnosis lightweight model for common fundus diseases based on fundus color photography.Methods:A applied research. A dataset of 2 400 color fundus images from Nanjing Medical University Eye Hospital and Zhejiang Mathematical Medical Society Smart Eye Database was collected, which was desensitized and labeled by a fundus specialist. Of these, 400 each were for diabetic retinopathy, glaucoma, retinal vein occlusion, high myopia, age-related macular degeneration, and normal fundus. The parameters obtained from the classical classification models VGGNet16, ResNet50, DenseNet121 and lightweight classification models MobileNet3, ShuffleNet2, GhostNet trained on the ImageNet dataset were migrated to the six-classified common fundus disease intelligent aid diagnostic model using a migration learning approach during training as initialization parameters for training to obtain the latest model. 1 315 color fundus images of clinical patients were used as the test set. Evaluation metrics included sensitivity, specificity, accuracy, F1-Score and agreement of diagnostic tests (Kappa value); comparison of subject working characteristic curves as well as area under the curve values for different models.Result:Compared with the classical classification model, the storage size and number of parameters of the three lightweight classification models were significantly reduced, with ShuffleNetV2 having an average recognition time per sheet 438.08 ms faster than the classical classification model VGGNet16. All 3 lightweight classification models had Accuracy > 80.0%; Kappa values > 70.0% with significant agreement; sensitivity, specificity, and F1-Score for the diagnosis of normal fundus images were ≥ 98.0%; Macro-F1 was 78.2%, 79.4%, and 81.5%, respectively.Conclusion:The intelligent assisted diagnosis of common fundus diseases based on fundus color photography is a lightweight model with high recognition accuracy and speed; the storage size and number of parameters are significantly reduced compared with the classical classification model.
5.Establishment and application of diabetic retinopathy intelligent assisted diagnostic technology evaluation system based on fundus photography

Objective To propose a new evaluation system and evaluate the application value of diabetic retinopathy ( DR) intelligence assisted diagnostic technology based on fundus photography. Methods By using the diagnostic test method,an evaluation system of DR intelligent diagnostic technology based on fundus photography was established. The fundus photographs of 331 diabetic patients (662 eyes) with DR screening were collected in the First Affiliated Hospital of Huzhou University from January 2017 to October 2018. The results of experts ' diagnosis and intelligence assisted diagnosis were compared and evaluated. The evaluation system includes primary evaluation, intermediate evaluation and advanced evaluation. The primary evaluation is the consistency of non-DR ( NDR) in all diabetic patients receiving DR-assisted diagnostic techniques;the intermediate evaluation is the diagnosis consistency of DR lesion degree in patients diagnosed with DR (grade 1-4);the advanced evaluation is the diagnosis consistency of DR classification ( grade 0 -4 ) in all diabetic patients receiving DR-assisted diagnostic techniques. The intermediate evaluation includes two evaluation methods. The main evaluation indicators include sensitivity,specificity and Kappa value. Results Based on experts ' diagnosis, NDR accounted for 22. 7%;mild non-proliferative DR (NPDR),moderate NPDR,and severe NPDR accounted for 19. 9%,18. 7% and 25. 7%,respectively;proliferative DR( PDR) accounted for 13. 0%. Based on intelligence diagnostic system,NDR accounted for 25. 8%;mild NPDR, moderate NPDR and severe NPDR accounted for 19. 7%,19. 3% and 22. 8%,respectively;proliferative DR( PDR) accounted for 12. 4%. Based on evaluation system in the paper,the sensitivity,specificity and Kappa value in primary evaluation were 91. 4%, 84. 7% and 0. 72;the sensitivity, specificity and Kappa value in intermediate evaluation method one were 88. 4%,91. 1% and 0. 79;the sensitivity, specificity and Kappa value in intermediate evaluation method two were 80. 5%,93. 3% and 0. 75;the Kappa value in advanced evaluation was 0. 62. Conclusions The evaluation system can be applied to the evaluation of DR intelligent diagnostic technology,and the evaluation result can be used as the basis for the selection of DR intelligent diagnosis application scene.
6.Diagnostic value of magnetic resonance for preoperative localization of the parathyroid glands in secondary hyperparathyroidism patients

OBJECTIVE To make a research on the diagnostic value of Magnetic resonance image for preoperative localization of the parathyroid glands in secondary hyperparathyroidism patients.METHODS This retrospective study was on 51 patients with secondary hyperparathyroidism who underwent parathyroid gland resection and autoplastic transplantation and were examined with Magnetic resonance imaging 99mTC-MIBI nuclide imaging and Color doppler ultrasound in our hospital from 2010 to 2016.Compare to the gold standard of pathological diagnosis after operation,the sensitivity of various imaging examination on identifying the parathyroid glands were calculated.RESULTS The number of parathyroid glands diagnosed by pathology was 197,including 180 conventional parathyroid glands and 17 eetopic parathyroid glands.The sensitivity of MRI,99mTC-MIBI nuclide imaging and Color doppler ultrasound on identifying the conventional parathyroid glands was 76.67%,51.11% and 44.44% respectively,and the sensitivity on identifying the ectopic parathyroid was 29.41%,0 and 0 respectively.Sensitivity of the Magnetic resonance on identifying the conventional parathyroid and the ectopic parathyroid imagin were better than 99mTC-MIBI nuclide imaging and color doppler(P<0.05).CONCLUSION Magnetic resonance imaging is an effective auxiliary examination on preoperative localization of the parathyroid glands in secondary hyperparathyroidism patients and can be the first choice of the image examination.
7.Expression and Clinical Relations of PTPRD in Esophageal Squamous Cell Carcinoma

Objective To identify the expression of PTPRD in esophageal squamous cell carcinoma (ESCC)and analyze its correlation with pathological features and patient survival.Methods Immunohistochemistry analysis was applied to detect the expression level and location of PTPRD in 236 patients with ESCC.Clinical and pathological features were collected and a 5 years’follow-up after surgery were performed.Results Statistic analysis showed that expression of PTPRD in ESCC was lower than in normal esophageal epithelial cells (22.0% vs 57.2%,P =0.000).The expression of PTPRD was correla-ted to the differentiation grade,depth of tumor invasion and lymph nodes metastasis.The expression of PTPRD was higher in group with well differentiation,less invasion depth and no lymph node metastasis (P =0.013,0.025,0.019).The expres-sion of PTPRD was not correlated to age or gender (P =0.170,0.787).The survival analysis showed that thegroup with more PTPRD expression had better prognosis.Conclusion PTPRD was correlated to progression and prognosis of ESCC.It may be a new potential tumor suppressor gene of ESCC,and its expression level might may a useful marker for predicting prognosis for ESCC patients.
8.Association between time spent on physical exercise, sleep, homework and suspected myopia among students

Objective To investigate the prevalence of suspected myopia among students and to examine the relationship between time spent on physical exercise,sleep,homework and suspected myopia.Methods A total of 8 030 primary and secondary school students from 4th to 12th grades were selected from the National Student Constitution and Health Survey (NSCHS) in Anhui province in 2014.Time spent on exercise,sleep and homework per day were collected using a self-administrated questionnaire.Visual acuity was examined using the Standard Logarithmic Visual Acuity Chart.Results The overall prevalence of suspected myopia was 69.03%.Prevalence rates of suspected myopia appeared higher in girls,in urban students,with the highest in the 16 to 18 year-old groups.Results from the multiple logistic regression analysis showed that the amount of time spent on physical exercise,sleep and homework per day were all significantly associated with suspected myopia.Suspected myopia was associated with longer time on physical exercise among students aged 8 to 12 years (OR=0.80,95%CI:0.64-0.99),and longer sleep time among students in the age groups of 13 to 15 years and 16 to 18 years (OR=0.73,95%CI:0.56-0.94;OR=0.38,95%CI:0.21-0.68,respectively).Longer time spent on homework significantly increased the risk of suspected myopia among students in the age groups of 8 to 12 years and 13 to 15 years (OR=1.41,95%CI:1.11-1.79;OR=1.74,95% CI:1.36-2.23,respectively).Conclusion Suspected myopia appeared common among students.Comprehensive intervention programs focusing on sufficient physical exercise and sleep but less homework might help to prevent myopia among students at different ages.
9.Feasibility of non-contrast enhanced magnetic resonance angiography for diagnosis of renal artery stenosis in elderly patients.

OBJECTIVETo evaluate the diagnostic efficacy of IFIR-FIESTA technique in detecting renal artery stenosis in elderly patients.
METHODSTwenty-seven aged patients underwent both IFIR-FIESTA and 3D CE-MRA examinations. The imaging quality and renal artery stenosis grades were evaluated. Kappa test was used to assess the consistency between the two methods. With CE-MRA as the reference, the diagnostic sensitivity, specificity, accuracy, PPV and NPV for IFIR-FIESTA were calculated in detecting renal artery stenosis.
RESULTSThe images by the two methods were 100% qualified for diagnosis, although the image quality of CE-MRA was significantly better. IFIR-FIESTA and CE-MRA showed excellent consistency in detecting renal artery stenosis. With CE-MRA as the reference, the diagnostic sensitivity, specificity, accuracy, PPV and NPV for IFIR-FIESTA were 97.1%, 100%, 98.1%, 100%, and 95% in detecting renal artery stenosis, respectively.
CONCLUSIONIFIR-FIESTA is feasible as a routine examination for detecting renal artery stenosis in elderly patients.
